Description

The present invention relates to a submucosal detachment treatment tool and a submucosal detachment treatment device system for excising a lesion in the digestive tract.

As a general treatment method for gastrointestinal lesions, endoscopic mucosal resection (EMR) in which a lesion is removed using an endoscope is conventionally known.
Among them, as an “incision / peeling method”, a method is described in which a normal mucous membrane outside the lesion is completely incised using a high-frequency incision tool such as a high-frequency knife, and then the submucosal layer is exfoliated and excised (see FIG. For example, refer nonpatent literature 1.).
In addition, what is used for another treatment as another form of such a high frequency knife is proposed (for example, refer patent document 1).
Hiroyuki Ono and three others, "EMR Tips Using IT Knives for Early Cancer", Gastrointestinal Endoscopy, Gastrointestinal Endoscopy Editorial Committee, Tokyo Medical Co., Ltd., November 2002, Volume 14 No. 11, p. 1737-1740 US 2001/0049497 A1

However, the following problems remain in the conventional treatment tool.
That is, when performing incision / exfoliation by the “incision / exfoliation method” using a conventional treatment instrument, a large number of blood vessels run in the submucosal layer. Special considerations such as changing the output setting of the apparatus as needed are necessary, and there is a problem that the procedure time becomes long.
In addition, incision and detachment with a high-frequency knife, in order to achieve an appropriate sharpness while preventing thermal damage due to high-frequency energy to the incision target part, appropriate strength, angle, knife moving speed, etc. are applied. There was a problem that the technique was difficult to control.

In addition, when incising a lesion that is bulged by a high-frequency knife, the high-frequency knife always comes into contact with the surface of the digestive tract (the surface of the non-bulged portion around the lesion) at a certain angle. It was something that would end up. That is, in a limited space in the digestive tract, it was impossible to incise around the swollen lesion by moving the high-frequency knife parallel to the surface of the digestive tract. Therefore, when the high-frequency knife is cut into the submucosa, the tip of the high-frequency knife should not be inserted into the intrinsic muscle layer (a layer that does not bulge even when applied locally). It was necessary to position the robot at a predetermined position (predetermined depth), and advanced technique and concentration were required, which caused a heavy burden on the operator.

The present invention has been made in view of the above-mentioned problems of the prior art, and the purpose thereof is to easily perform a procedure in a short time when excising a lesion of the digestive tract, and to improve the skill of the operator. It is intended to provide a submucosal layer peeling treatment device and a submucosal layer peeling treatment device system capable of performing a safe procedure without being influenced by the above.

In order to achieve the above object, the present invention provides the following means.
The submucosal detachment treatment tool of the present invention is a submucosal detachment treatment tool for locally detaching the submucosa and excising the lesioned part of the digestive tract, and is a flexible treatment inserted into the submucosa. An instrument body, a first conduit formed from the proximal end side to the distal end side of the treatment instrument body, and a distal end side of the treatment instrument body. An inflatable portion that inflates when supplied, a wire inserted into the first conduit with the distal end fixed, and fixed to the proximal end of the wire, with respect to the treatment instrument body And an operation section that pushes and pulls the wire, and when the operation section is pushed and pulled, at least a distal end portion of the treatment instrument body is curved in a predetermined direction.

In the submucosal layer exfoliation treatment instrument according to the present invention, it is inserted into the first duct by the operation unit in a state where the distal end of the treatment instrument main body is brought close to a lesion part that has been swollen in advance by submucosal local injection or the like. By pushing and pulling the wire, the distal end portion of the treatment instrument body provided with the inflating portion can be bent at an arbitrary angle in a predetermined direction. That is, when the wire whose distal end side is fixed with respect to the treatment instrument body is pushed and pulled, a compressive force or a tensile force is transmitted to the flexible treatment instrument body via the wire, and the treatment instrument body is moved in a predetermined direction ( For example, it bends in a vertical direction or a horizontal direction) and curves. Thereby, even if it is the space in a limited digestive tract, the curve degree of the front end side of a treatment tool main body can be adjusted so that it may become parallel to the surface of a digestive tract.

Then, the distal end side of the treatment tool main body is moved in a direction parallel to the surface of the digestive tract, and inserted into the submucosa layer of the lesioned part where the distal end is swollen. After the insertion, the fluid is supplied from the fluid supply source or the like to the inflating part via the first conduit, and the inflating part is inflated in the submucosa. Thereby, the submucosa can be easily peeled off. Moreover, after inserting an expansion | swelling part in a different site | part, by expanding an expansion | swelling part as mentioned above, the mucosa lower layer of a wide range can be peeled one after another easily.

Thus, the submucosal layer can be peeled without using a conventional high-frequency knife, and the procedure can be facilitated to shorten the procedure time. At this time, even if there is bleeding in the lesioned part, the bleeding part can be compressed by inflating the expanding part, and the bleeding part can be rapidly stopped. Can be prevented in advance.
In particular, by curving the distal end portion of the treatment instrument body, the distal end portion can be moved parallel to the surface of the gastrointestinal tract and inserted into the submucosal layer. There is no possibility that the distal end side of the treatment instrument body will be inserted. Therefore, it is not necessary to pay extreme attention to the insertion depth of the distal end of the treatment instrument body. Therefore, a safe procedure can be performed without being influenced by the skill of the surgeon, and the burden on the surgeon can be reduced as much as possible.
Furthermore, since the first conduit can be used as a common conduit for the conduit that supplies fluid to the inflating portion and the conduit through which the wire is inserted, the diameter of the treatment instrument body can be reduced.

Further, in the submucosal layer peeling treatment tool, it is more preferable that a distal end side of the treatment tool body is provided with a flexible portion having more flexibility than other portions .

In the submucosal layer exfoliation treatment instrument according to the present invention, the flexible part is provided on the distal end side of the treatment instrument body. Therefore, when the operation part is pushed and pulled, the treatment instrument body is more actively provided with the expansion part. It can be bent at the tip side. Thereby, even if the protrusion amount from the endoscope tip is small, the bending can be performed reliably, and the procedure can be reliably performed even when the procedure space in the digestive tract is narrow.

In the submucosal layer peeling treatment tool, at least the distal end side of the treatment tool body includes a bending assisting member having a cross-sectional characteristic that curves in a predetermined direction, and the constant direction and the bending direction are the same. It is more preferable that the direction is as follows .

In the submucosal layer peeling treatment tool according to the present invention, the bending aid member is provided at least on the distal end side of the treatment tool body. For example, when the endoscope is inserted through the channel of the endoscope, the bending condition of the endoscope is reduced. The channel is inserted through the channel so that the direction of the bending assisting member automatically follows (the bending direction). And finally, it protrudes to the outside in a state where the bending assisting member is aligned in the bending direction on the distal end side of the endoscope. That is, the bending direction on the endoscope distal end side coincides with a predetermined constant direction of the bending assisting member. That is, when the submucosal layer peeling treatment tool protrudes from the distal end of the endoscope, the bending direction of the endoscope automatically matches the bending direction of the endoscope.
Thereby, the surgeon can quickly perform the bending operation of the distal end portion of the treatment instrument body in the same direction as the bending direction of the endoscope without adjusting the bending direction. Therefore, the labor for adjusting the bending direction can be saved, and the procedure time for peeling the submucosal layer can be further shortened.

Further, in the submucosal layer peeling treatment tool, when the operation unit is capable of rotating operation, and includes a rotation transmission unit that transmits a rotational force from the operation unit to the wire, the rotation of the operation unit is performed. More preferably, at least the distal end portion of the treatment instrument body rotates about the axis of the treatment instrument body .

In the submucosal layer exfoliation treatment instrument according to the present invention, at the time of performing a bending operation, by rotating the operation part, a rotational force (rotational torque) is transmitted to the wire via the rotation transmission part, so that at least the treatment tool body Can be rotated around the axis. Therefore, the bending direction can be controlled with higher accuracy, and an accurate procedure can be performed.

In the submucosal layer peeling treatment instrument, an insertion path through which the high-frequency incision instrument is inserted is formed from the proximal end side to the distal end side of the treatment instrument body so as to follow the first duct. More preferably.

In the submucosal layer exfoliation treatment instrument according to the present invention, a high-frequency incision instrument can be inserted through the insertion passage in advance, so that the treatment instrument body can be inserted into the submucosa layer when necessary without replacing the treatment instrument. In addition, a high-frequency incision tool can be used promptly for opening a hole to be an insertion hole in the mucous membrane in the vicinity of the lesion. Therefore, the procedure time can be shortened and more various procedures can be performed.

In the submucosal layer peeling treatment tool, it is more preferable that an indicator portion indicating the amount of insertion into the submucosal layer is provided on the treatment tool main body on the proximal side of the expansion portion .

In the submucosal layer peeling treatment tool according to the present invention, by confirming the indicator portion, the position of the inflated portion inserted into the submucosa layer can be accurately grasped, and the desired portion of the submucosal layer can be peeled more accurately. can do.

Also, the submucosal layer peeling treatment instrument system according to the present invention includes an endoscope having an insertion portion provided with a first channel, and a submucosal layer peeling treatment according to any one of the above that is inserted through the first channel. It is characterized by having tools .

In the submucosal layer peeling treatment instrument system according to the present invention, the submucosal layer peeling treatment tool of the present invention can be inserted into the first channel, so that the submucosal layer peeling treatment tool can be surely inserted into the lesion in the digestive tract via the endoscope. Can be peeled off and the submucosa can be peeled off, facilitating the procedure and reducing the procedure time. Further, a safe procedure can be performed without being affected by the skill of the operator, and the burden on the operator can be reduced as much as possible.

In the submucosal layer peeling treatment instrument system, the second channel provided in the insertion portion along the first channel and the second channel are inserted to inject liquid into the submucosal layer. More preferably, a submucosal injection needle is provided .

In the submucosal stripping treatment instrument system according to the present invention, since the submucosal injection needle can be inserted into the digestive tract via the second channel simultaneously with the submucosal stripping treatment instrument, a liquid is injected into the submucosal layer to cause lesions. After the bulging is performed, it is possible to promptly shift to the peeling treatment without replacing the treatment tool. Therefore, it is possible to shorten the time required for the exfoliation of the lesioned part to peel the submucosa.

In the submucosal layer peeling treatment instrument system, a second channel provided in the insertion portion along the first channel, and a grasping forceps that is inserted into the second channel and grips the lesioned portion. And more preferably.

In the submucosal layer peeling treatment device system according to the present invention, the grasping forceps can be inserted into the digestive tract through the second channel simultaneously with the submucosal layer peeling treatment device, so that the mucosal layer can be peeled while the lesion is gripped. Therefore, the expanded part can be accurately inserted into the submucosal layer without being affected by the already peeled lesion, or the submucosal layer can be securely peeled while firmly grasping so that the lesion does not escape. be able to. Therefore, a more accurate procedure can be performed.

According to the submucosal layer peeling treatment tool according to the present invention, the submucosal layer can be peeled without using a conventional high-frequency knife, and the procedure can be facilitated to shorten the procedure time. Further, a safe procedure can be performed without being affected by the skill of the operator, and the burden on the operator can be reduced as much as possible.

Moreover, according to the submucosal layer peeling treatment instrument system according to the present invention, the submucosal layer can be peeled by reliably guiding the submucosal layer peeling treatment tool to the lesioned part in the digestive tract via the endoscope.

The operation wire 14 is fixed to the proximal end side of the operation wire 13 and pushes and pulls the operation wire 13 with respect to the treatment instrument body 10, and is provided at least on the distal end side of the treatment instrument body 10, and is a predetermined constant. A plate member (curvature assisting member) 15 having a cross-sectional characteristic curved in the direction; An insertion path 17 is formed from the proximal end side to the distal end side of the treatment instrument main body 10 along the pipe line 11, and an insertion path 17 through which a high frequency knife (high frequency cutting tool) 16 is inserted. When the pulling operation is performed, at least the distal end portion of the treatment instrument body 10 can be bent in a predetermined direction.

As shown in FIG. 2, the treatment instrument body 10 is inserted into the treatment instrument channel 2 and is a long tubular body extending along the axis L, and a connecting portion 10a is provided on the proximal end side thereof. The operation unit main body 20 of the operation unit 14 and the operation tube 21 of the high-frequency knife 16 are connected to the connection unit 10a so as to be divided into two forks.
Moreover, as shown in FIG.4 and FIG.5, the most distal part 10b of the treatment tool main body 10 is formed in the taper shape which an outer diameter reduces gradually toward the most distal part. The balloon 12 is provided on the proximal end side of the most distal portion 10b so as to cover the outer periphery of the treatment instrument main body 10. In addition, a plurality of communication holes 22 that communicate the conduit 11 and the balloon 12 are formed along a shaft core L in a part of the treatment instrument main body 10 covered with the balloon 12. In addition, a sealing portion 23 that seals the pipe 11 is provided at the tip of the pipe 11, and a plurality of fluids supplied into the pipe 11 do not leak outside from the tip of the pipe 11. It is supplied into the balloon 12 through the communication hole 22.

In addition, an operation wire fixing portion 24 that fixes the distal end side of the operation wire 13 inserted into the conduit 11 is attached to the sealing portion 23. As shown in FIG. 3, the pipe line 11 is formed at a position shifted from the axis L (position slightly away from the center).
As shown in FIG. 2, the operation unit 14 includes an operation unit main body 20 and a slide unit 25 that can move back and forth (slide) with respect to the operation unit main body 20. And the said pipe line 11 is formed to the base end side of the operation part main body 20 through the connection part 10a. The proximal end side of the operation wire 13 is connected to the slide portion 25. When the operation portion 14 is pushed and pulled, that is, when the slide portion 25 is moved forward and backward, the distal end portion of the treatment instrument body 10 including the entire balloon 12 is curved. This will be described in detail later.

Further, a finger ring 20a through which a thumb can be inserted is attached to the proximal end side of the operation unit body 20, and two finger hook holes 25a through which an index finger and a middle finger can be inserted are formed in the slide unit 25. Thus, the surgeon can easily perform the bending operation of the treatment instrument body 10 by operating the operation unit 14 with one hand.
Further, in the vicinity of the connection portion 10 a of the operation portion main body 20, an injection port 26 is provided which is connected to the pipeline 11 and to which a syringe S that is a fluid supply source that supplies a fluid for inflating the balloon 12 is connected.

Moreover, as shown in FIG. 4, since the some communication hole 22 is formed in the part at the front end side of the treatment tool main body 10 as mentioned above, it has a softness | flexibility rather than another part. . In other words, when the operation unit 14 is pushed and pulled, it is easier to bend around the area where the plurality of communication holes 22 are formed. That is, these communication holes 22 function as a flexible portion that makes it easy to positively bend the treatment instrument body 10. Since the plurality of communication holes 22 are formed, the distal end portion of the treatment instrument body 10 is curved over the entire balloon 12 as described above.
That is, as shown in FIG. 6A, when the slide portion 25 is pulled toward the proximal end side of the operation portion main body 20, the treatment including the entire balloon 12 toward the plurality of communication holes 22 (UP direction). When the distal end portion of the tool body 10 is curved and the slide portion 25 is pushed into the distal end side of the operation portion main body 20 as shown in FIG. 6B, the opposite side of the plurality of communication holes 22 (DOWN direction) ), The distal end portion of the treatment instrument body 10 including the entire balloon 12 is curved.

As shown in FIGS. 3 and 4, the plate member 15 is made of a stainless material and has an elongated shape with a short width. The plate member 15 has a cross-sectional characteristic that is curved in the thickness direction (a constant direction) in advance, and the thickness direction is at a position opposite to the pipe line 11 with the axis L sandwiched between the axis L and the operation wire 13. Are provided so as to coincide with the direction connecting the two. As a result, the direction in which the plate member 15 bends coincides with the direction in which the plate member 15 bends when the operation unit 14 is pushed and pulled.
In the present embodiment, the plate member 15 is pushed into the plate member conduit 27 formed in the treatment instrument body 10 from the distal end side, and the plate member tube is caused by frictional force with the plate member conduit 27. It is held in the path 27. The front end of the plate member conduit 27 is sealed by a sealing portion 28.

As shown in FIGS. 2 and 5, the high-frequency knife 16 includes an operation tube 21 formed of an insulating material, a needle-like high-frequency knife 29 arranged to be able to advance and retreat inside the operation tube 21, and an operation tube. 21 and a knife operation unit 30 connected to the base end side. As described above, the operation tube 21 is connected to the connection portion 10a, and one side is inserted into the insertion passage 17 and is provided to the vicinity of the most distal end portion 10b of the treatment instrument body 10, and the other side is provided outside the connection portion 10a. It is extended.
The knife operation unit 30 is connected to the knife operation unit main body 31 connected to the other end side of the operation tube 21 and the knife slide unit 32 connected to the proximal end side of the high-frequency knife 29 and capable of moving forward and backward with respect to the knife operation unit main body 31. And. Similarly to the operation unit 14 described above, a finger hook ring 31 a into which a thumb can be inserted is attached to the proximal end side of the knife operation unit main body 31, and an index finger and a middle finger can be inserted into the knife slide unit 32. Two finger hooking holes 32a are formed. As a result, the operator can easily operate the knife operation unit 30 with one hand.
Further, an electrical connection portion 33 that is electrically connected to the high frequency knife 29 is provided between the two finger hooking holes 32 a of the knife slide portion 32, and a high frequency power source (not shown) that applies a high frequency to the high frequency knife 29 is provided. It can be connected electrically.

Further, as shown in FIG. 7, the treatment instrument main body 10 on the proximal end side with respect to the balloon 12 is provided with an indicator portion 34 that indicates the amount of insertion into the submucosa W. The indicator portion 34 is marked so as to be displayed at an interval of 5 mm from a position 15 mm away from the most distal portion 10 b toward the proximal end side of the treatment instrument body 10.

As shown in FIG. 8, the submucosal local injection needle 6 is formed in a hollow shape by being attached to an outer tube 40, an inner tube 41 that can be advanced and retracted in the outer tube 40, and a tip of the inner tube 41. A needle portion 42 and a needle operation portion 43 attached to the proximal end side of the outer tube 40.
The distal end of the outer tube 40 is formed in a jaw shape protruding radially inward, and only the needle portion 42 of the inner tube 41 is inserted therethrough. The needle operation section 43 is attached to the proximal end side of the inner tube 41 and the needle operation section main body 44 attached to the proximal end side inner peripheral surface of the outer tube 40, and is a liquid for local injection (local injection liquid). And a base 45 for injecting the liquid. The needle portion 42 can be protruded outward from the distal end of the outer tube 40 by moving the cap 45 forward and backward with respect to the needle operation portion main body 44.

A submucosal layer peeling method for resecting a lesion X formed in the digestive tract from the submucosal layer W using the endoscope treatment instrument system 1 and the peeling balloon 5 configured as described above will be described below.
The submucosal layer peeling method of the present embodiment includes a bulging step for bulging the submucosal layer W around the lesion X, a drilling step for opening the insertion hole H in the mucosa N near the bulged lesion X, and a treatment for the insertion hole H. An insertion step of inserting only the most distal end portion 10b of the instrument body 10, and an adjustment step of curving at least the distal end portion of the treatment instrument body 10 and adjusting the bending angle so that the distal end portion is parallel to the surface of the digestive tract Then, the treatment instrument body 10 is moved in a direction parallel to the surface of the digestive tract and pushed into the submucosal layer W from the insertion hole H, and the distal end position of the treatment instrument body 10 is aligned with a predetermined position of the submucosa layer W. An adjustment step, a peeling step for supplying the fluid to the balloon 12 to inflate and peeling the submucosal layer W, and an incision step for incising the mucosa N around the insertion hole H are provided.
Each of these steps will be described in detail below.

First, a swelling step is performed. That is, the endoscope 4 in which the submucosal injection needle 6 shown in FIG. 8 is inserted into the treatment instrument channel 2 is inserted into the digestive tract, and the distal end of the insertion portion 3 is positioned in the vicinity of the lesion X. At this position, the outer tube 40 of the submucosal injection needle 6 is protruded from the distal end of the insertion portion 3 of the endoscope 4, and the base 45 is moved toward the distal end side with respect to the needle operation portion main body 44, so that FIG. As shown to (a), the needle part 42 is protruded from the front-end | tip of the outer tube 40. FIG. Then, as shown in FIG. 9, the needle portion 42 is punctured from the mucosa N in the vicinity of the lesion X into the submucosa W. After puncturing, a local injection solution is injected into the submucosal layer W through the inner tube 41 and the needle portion 42 to bulge the lesion X.
After swelling of the lesioned part X, the base 45 is moved to the proximal end side so that the needle part 42 is immersed in the inner tube 41 and the submucosal injection needle 6 is removed from the treatment instrument channel 2. Note that the endoscope 4 is not moved and is maintained at the same position.

After the bulging step, a drilling step is performed. That is, the treatment instrument body 10 of the peeling balloon 5 shown in FIG. 2 is inserted into the treatment instrument channel 2 and the distal end of the treatment instrument body 10 is projected from the distal end of the insertion portion 3.
Here, since the plate member 15 is provided at the distal end side of the treatment instrument body 10, the treatment instrument body 10 automatically follows the bending angle of the insertion portion 3 when moving in the treatment instrument channel 2. Move while. And when it protrudes from the front-end | tip of the insertion part 3, as shown in FIG.10 and FIG.11, the bending direction (up-down direction) of the insertion part 3 and the thickness direction ((alpha) direction) of the board member 15 correspond. Here, since the thickness direction (α direction) of the plate member 15 is provided so as to coincide with the bending direction of the peeling balloon 5, as a result, the peeling balloon 5 is passed through the treatment instrument channel 2, so that In addition, the bending direction of the treatment instrument body 10 can be aligned in the same direction as the bending direction of the insertion portion 3. That is, the treatment instrument body 10 is in a state where it is difficult to bend in the β direction.
In the present embodiment, description will be made assuming that the side on which the operation wire 13 and the plurality of communication holes 22 are formed is located on the upper position side of the insertion portion 3.

After the peeling balloon 5 is protruded, the knife slide portion 32 is moved to the distal end side with respect to the knife operation portion main body 31, and the high-frequency knife 29 is protruded from the operation tube 21 and the most distal portion 10b. In this state, while applying a high frequency from a high frequency power source connected to the electrical connection portion 33, as shown in FIG. 12, the high frequency knife 29 is advanced to insert the insertion hole of a predetermined size into the mucosa N in the vicinity of the lesion X. Open H.
After the insertion hole H is opened, the application of high frequency is stopped, and the slide portion 25 is moved to the proximal end side so that the high frequency knife 29 is immersed in the operation tube 21.

After the drilling step, an insertion step is performed. That is, as shown in FIG. 13, only the most distal end portion 10 b of the treatment instrument body 10 is inserted into the insertion hole H. At the stage of this insertion step, the distal end side of the treatment instrument main body 10 is in a state in which an angle θ is attached to the surface of the digestive tract as in the conventional case.

After this insertion step, an adjustment step is performed. That is, the operating portion 13 is pulled by moving the slide portion 25 to the proximal end side with respect to the operating portion main body 20. At this time, since the distal end of the operation wire 13 is fixed by the operation wire fixing portion 24, the tensile force transmitted to the operation wire 13 by the pulling operation is transmitted to the treatment instrument body 10. Thereby, a compressive force acts on the treatment instrument main body 10 on the side close to the operation wire 13 and a tensile force acts on the opposite side of the operation wire 13 with the shaft core L interposed therebetween. As a result, as shown in FIG. 14, the treatment instrument body 10 bends and curves toward the operation wire 13 side, that is, the upper side of the insertion portion 3. In particular, since a plurality of communication holes 22 serving as a flexible portion are formed, it is curved around a region where the communication holes 22 are formed. Therefore, the distal end portion of the treatment instrument body 10 is curved over the entire balloon 12.

When the operation wire 13 is pushed in by moving the slide portion 25 to the distal end side with respect to the operation portion main body 20, the force acting on the treatment instrument main body 10 works in reverse, and the entire balloon 12 is included. The distal end portion of the treatment instrument body 10 bends and curves toward the lower side of the insertion portion 3.

By the adjustment step, even in a limited space inside the digestive tract, the bending state can be adjusted so that the distal end side of the treatment instrument body 10 is parallel to the surface of the digestive tract. In particular, it is easy to operate the plate member 15 because the bending operation can be quickly performed in the same direction as the bending direction of the insertion portion 3 of the endoscope 4 without adjusting the bending direction.

Then, after the adjustment step, a length adjustment step is performed. That is, as shown in FIG. 15, the treatment instrument body 10 is moved in a direction parallel to the surface of the digestive tract and pushed into the submucosa W from the insertion hole H, and the distal end position of the treatment instrument body 10 is set to a predetermined level of the submucosa W. Align to position. At this time, since the indicator portions 34 are marked at intervals of 5 mm from the proximal end side of the balloon 12 on the treatment instrument body 10, the alignment can be performed with high accuracy.

A peeling step is performed after the adjustment step. That is, the fluid is supplied from the inlet 26 into the pipe line 11 using the syringe S. The supplied fluid passes through the inside of the pipe line 11 and is supplied into the balloon 12 from the plurality of communication holes 22. Thereby, as shown in FIG. 16, the balloon 12 can be inflated, and the submucosal layer W can be peeled off by the pressure at the time of inflation.
After the submucosal layer W is peeled off, the fluid is once discharged from the injection port 26 and the balloon 12 is contracted to the original state. As a result, as shown in FIG. 17, a cavity H <b> 1 in which the submucosa W is peeled is formed in the submucosa W.

An incision step is performed after the peeling step. That is, the treatment instrument body 10 is pulled back from the submucosa W to the outside of the insertion hole H, and again, as shown in FIG. 18, the high-frequency knife 29 is protruded from the most distal portion 10b of the treatment instrument body 10 in the same manner as the drilling step. To insert into the insertion hole H. In this state, while applying a high frequency again to the high frequency knife 29, the distal end of the treatment instrument body 10 is moved along the periphery of the lesion X, and the mucosa N around the insertion hole H is moved to the high frequency knife as shown in FIG. Incision by 29. Then, after cutting a certain amount of width, the application of high frequency is stopped and the high frequency knife 29 is accommodated in the operation tube 21.

Next, the length adjustment step is repeated in order to perform peeling again. That is, as shown in FIG. 20, the treatment instrument main body 10 is inserted again while confirming the indicator portion 34 in the submucosa W from the insertion hole H, and the tip is positioned at a predetermined position. Subsequently, the peeling step is repeated again, and as shown in FIG. 21, the balloon 12 is inflated, and the submucosal layer W that has not been peeled off in the first peeling step is peeled off to form the next cavity H1. Thereafter, a second incision step is performed as shown in FIG.
In this way, the length adjustment step, the peeling step, and the incision step are repeated in a timely manner according to the size of the lesion X, so that the lesion X including the submucosa W is separated from the digestive tract as shown in FIG. And can be excised.

As described above, according to the endoscope treatment tool system 1 and the peeling balloon 5 of the present embodiment, the balloon 12 is inserted into different positions of the submucosal layer W without using the conventional high-frequency knife 16 frequently. The submucosa W can be peeled one after another, and the submucosa W can be reliably peeled over a wide range, so that the procedure can be facilitated and the procedure time can be shortened. At this time, even if there is bleeding in the lesioned part X, the bleeding part can be compressed by inflating the balloon 12, the bleeding part can be rapidly stopped, and the submucosa W of the lesioned part X can be stopped. The incidental illness which accompanies can be prevented beforehand.

In particular, the distal end portion of the treatment instrument main body 10 including the entire balloon 12 can be curved so that the distal end portion can be parallel to the surface of the digestive tract, and the balloon 12 can be inserted while maintaining this state. There is no possibility that the distal end side of the treatment instrument main body 10 is inserted into the intrinsic muscle layer or the like under the submucosa W as in the prior art. Therefore, it is not necessary to pay extreme attention to the insertion depth of the distal end of the treatment instrument body 10. Therefore, a safe procedure can be performed without being affected by the skill of the operator, and the burden on the operator can be reduced as much as possible.

In addition, since the plurality of communication holes 22 functioning as flexible portions are formed on the distal end side of the treatment instrument body 10, the treatment instrument body 10 can be more actively curved at the balloon 12 portion. Thereby, even if the amount of projection of the endoscope 4 from the treatment instrument channel 2 is small, the bending operation can be performed reliably, and even if the procedure space in the digestive tract is narrow, a reliable procedure is possible. It can be performed.
Further, since the conduit 11 can be used as a common conduit for the conduit for supplying fluid to the balloon 12 and the conduit through which the operation wire 13 is inserted, the diameter of the treatment instrument body 10 can be reduced. it can. Furthermore, since a plurality of communication holes 22 are formed, the function as a flexible portion is naturally, and in addition to that, the fluid can be efficiently supplied to the balloon 12, so that the expansion time can be shortened.

Further, when the peeling balloon 5 protrudes from the distal end of the insertion portion 3 of the endoscope 4 by the plate member 15, the bending direction of the endoscope 4 automatically matches the bending direction of itself. Thereby, the surgeon can quickly perform a bending operation in the same direction as the bending direction of the endoscope 4 without adjusting the bending direction. Therefore, the labor for adjusting the bending direction can be saved, and the time required for the peeling of the submucosa W can be further shortened.

Moreover, since the insertion path 17 for inserting the high-frequency knife 16 is provided, the high-frequency knife 16 can be used promptly when necessary without replacing the treatment instrument. Therefore, the procedure time can be shortened from this, and more various procedures can be performed.
Furthermore, since the indicator portion 34 is marked, the position of the balloon 12 inserted into the submucosa W can be accurately grasped, and a desired portion of the submucosa W can be more accurately peeled off.

Next, a second embodiment of the submucosal layer peeling treatment tool according to the present invention will be described with reference to FIGS. In the second embodiment, the same components as those in the first embodiment are denoted by the same reference numerals, and the description thereof is omitted.
The difference between the second embodiment and the first embodiment is that the peeling balloon 5 of the first embodiment curves the distal end portion of the treatment instrument body 10 including the balloon 12 by pushing and pulling the operation portion 14. However, the peeling balloon (submucosal layer peeling treatment tool) 50 of the second embodiment rotates at least the operation portion 14 in addition to the bending operation, so that at least the distal end portion of the treatment tool main body 10 is placed on the treatment tool main body 10. This is a point that can be rotated around the axis L.
Further, the peeling balloon 5 of the first embodiment includes the plate member 15 so that the bending direction automatically follows according to the angle of the insertion portion 3 of the endoscope 4, but the peeling balloon of the second embodiment. The balloon 50 does not include the plate member 15.

That is, as shown in FIGS. 24 to 26, the peeling balloon 50 of the present embodiment is provided with the conduit 11 and the insertion passage 17 in the treatment instrument main body 10 with the shaft core L interposed therebetween. The operation unit body 20 is rotatably connected to the connection unit 10a. Thereby, the slide part 25 attached to the operation part main body 20 so that advancement / retraction is possible can also be rotated similarly. In addition, the connection portion 10 a includes a rotation transmission unit (not shown) that transmits the rotational torque (rotational force) from the operation unit 14 to the operation wire 13, and reliably transmits the rotation torque due to the rotation operation to the operation wire 13. It is supposed to be.
Note that it is preferable to use a wire with good torque transmission as the operation wire 13 of the present embodiment.

A submucosal layer peeling method for resecting a lesion X formed inside the digestive tract from the submucosa layer W using the peeling balloon 50 configured as described above will be described below.
In the submucosal layer peeling method of this embodiment, after performing the drilling step as in the first embodiment, the distal end of the treatment instrument body 10 is positioned in the vicinity of the insertion hole H, and at least the distal end portion of the treatment instrument body 10 is positioned. An adjustment step for bending and adjusting the bending angle so that the distal end portion is parallel to the surface of the digestive tract, and after the adjustment step, the treatment instrument body 10 is moved in a direction parallel to the surface of the digestive tract to insert the insertion hole H And a length adjusting step of aligning the distal end position of the treatment instrument body 10 with a predetermined position of the submucosa W. Thereafter, a peeling step and an incision step are performed as in the first embodiment.
Further, at the time of the adjustment step, a rotation step of rotating at least the distal end portion of the treatment instrument body 10 around the axis L of the treatment instrument body 10 is performed. Each of these steps will be described in detail below.

First, similarly to the first embodiment, the drilling step is performed.
After the drilling step, an adjustment step is performed. That is, as shown in FIG. 27, with the distal end of the treatment instrument main body 10 positioned in the vicinity of the insertion hole H, the slide portion 25 is moved to the proximal end side with respect to the operation portion main body 20 to move the operation wire 13. By pulling, the distal end portion of the treatment instrument body 10 including the balloon 12 is bent. Simultaneously with this bending operation, the operation section body 20 and the slide section 25 are rotated, and the distal end portion of the treatment instrument body 10 including the balloon 12 is rotated around the axis L. The bending operation and the rotation operation are combined to be finely adjusted so that the distal end portion of the treatment instrument body 10 is bent upward and parallel to the surface of the digestive tract.
Thus, even if the plate member 15 of the first embodiment is not provided, the rotation operation can be performed, so that the fine adjustment of the bending direction can be easily and reliably performed.

After this adjustment step is completed, a length adjustment step is performed. That is, the treatment instrument body 10 is moved in a direction parallel to the surface of the digestive tract and pushed into the submucosa W from the insertion hole H, and the distal end position of the treatment instrument body 10 is aligned with a predetermined position of the submucosa W. Thereafter, similarly to the first embodiment, the lesioned part X can be excised by repeating the peeling step and the incision step according to the size of the lesioned part X.

The technical scope of the present invention is not limited to the above embodiment, and various modifications can be made without departing from the spirit of the present invention.

For example, in the first embodiment, the bending angle is adjusted after inserting only the most distal portion 10b of the treatment instrument body 10 into the insertion hole H. However, after adjusting the bending angle first as in the second embodiment, You may insert the front-end | tip part of the treatment tool main body 10 in the submucosa W.
In each of the above embodiments, the pipe 11 is used as a common pipe for supplying a fluid to the balloon 12 and a pipe through which the operation wire 13 is inserted. It is also possible to provide a dedicated pipeline.

In each of the above embodiments, the plurality of communication holes 22 function as flexible portions. However, the communication holes 22 may be formed in one place, and the flexible portions may be provided separately from the communication holes 22. For example, a bellows-like flexible part may be provided over the entire circumference of the treatment instrument body 10.
Alternatively, two operation wires 13 may be provided and bent in two orthogonal directions (vertical direction and horizontal direction).

In the first embodiment, the submucosal layer peeling treatment instrument system 1 is configured by the endoscope 4 having one treatment instrument channel 2. However, the present invention is not limited to this case. For example, as shown in FIG. In addition to the treatment instrument channel 2, an endoscope 62 having a treatment instrument channel 61 (second channel) provided in the insertion section 3 along the treatment instrument channel 2, the peeling balloon 5 of the first embodiment, The submucosa peeling treatment instrument system 60 may be configured with the submucosal local injection needle 6 that is inserted into the treatment instrument channel 61 and injects a liquid into the submucosa W.
According to the submucosal layer exfoliation treatment instrument system 60 configured in this way, the exfoliation balloon 5 and the submucosal injection needle 6 can be inserted into the digestive tract together. After performing the procedure, it is possible to promptly shift to the peeling treatment without replacing the treatment tool. Therefore, it is possible to shorten the time taken for the exfoliation of the lesion X to peel the submucosa W.

Further, instead of the submucosal local injection needle 6 described above, a submucosal detachment treatment instrument system including grasping forceps that are inserted into the treatment instrument channel 61 and grasp the lesion X may be configured.
In this case, since grasping forceps can be inserted into the digestive tract together with the separation balloon 5 through the treatment instrument channel 61, mucosal separation can be performed while the lesioned part X is grasped. Therefore, without being affected by the already peeled lesion X, the balloon 12 is accurately inserted into the submucosa W, or securely peeled off while firmly grasping so that the lesion X does not escape. Can be done. Therefore, a more accurate procedure can be performed.
