Description
本発明は、体腔内に内視鏡を挿入する際に使用する挿入補助具に関する。
胃、食道等の腫瘍、静脈瘤等消化管内疾患の診断、治療の目的で、内視鏡が広く用いられている。しかし、内視鏡挿入部を、例えば、口腔から食道へ挿入するような場合、患者の苦痛を抑えて挿入するには術者の熟練が必要とされ、さらに、手技によっては、何度も内視鏡を出し入れすることもあって患者への負担は更に大きいものとなる。
そこで、内視鏡挿入部の挿通経路を確保して、内視鏡挿入部を食道内に容易に挿入するために管状の挿入補助具が提案されている（例えば、特許文献１参照。）。

しかしながら、上記従来の挿入補助具では、内視鏡挿入部の外周に外付けのチャンネル、或いは、直接処置具が配された内視鏡挿入部を口腔から体内に挿入して胃内で縫合等の処置を行う際、挿入補助具に挿入した内視鏡挿入部から胃内に空気を送気して胃を膨らまそうとしても、内視鏡や処置具と挿入補助具との間で空気漏れが発生し、胃を十分に膨らませることができない場合があった。
本発明は上記事情に鑑みて成されたものであり、管部を通して管部の先端と基端との間の気密を保つ状態で内視鏡挿入部と処置具との挿抜操作を行うことができ、所定の処置を確実に行って操作者や患者の負担を減らすことができる挿入補助具を提供することを目的とする。

本発明は、上記課題を解決するため、以下の手段を採用する。
本発明に係る挿入補助具は、可撓性を有し、体腔内に挿入される内視鏡挿入部と、処置具及び前記処置具が挿通可能なチャンネルのいずれかとが挿通可能な管部と、該管部内において、前記内視鏡挿入部と、前記処置具及び前記チャンネルのいずれかとを支持する貫通孔を有する気密部材とを備え、前記気密部材は、前記管部の内周と気密に接触して前記管部の内周と摺動可能な第１摺動部、前記内視鏡挿入部の外周と気密に接触して前記内視鏡挿入部の外周と摺動可能な第２摺動部、又は、前記処置具又はチャンネルの外周と気密に接触して前記処置具又はチャンネルの外周と摺動可能な第３摺動部のうち少なくとも一つを有し、前記気密部材が有する前記第１摺動部、前記第２摺動部、又は、前記第３摺動部のうち少なくとも一つは、前記内周又は前記外周に接触する部分を、先端に凸状の曲面を有して突出する凸部として形成され、前記気密部材には、外径が前記管部の内径よりも小さくされた小径部が形成され、前記気密部材の前記小径部の外周を覆うように巻き回された補強バンド部材をさらに備えることを特徴とする。
本発明に係る挿入補助具は、可撓性を有し、体腔内に挿入される内視鏡挿入部と、処置具及び前記処置具が挿通可能なチャンネルのいずれかとが挿通可能な管部と、該管部内において、前記内視鏡挿入部と、前記処置具及び前記チャンネルのいずれかとを支持する貫通孔を有する気密部材とを備え、前記気密部材は、前記管部の内周と気密に接触して前記管部の内周と摺動可能な第１摺動部、前記内視鏡挿入部の外周と気密に接触して前記内視鏡挿入部の外周と摺動可能な第２摺動部、又は、前記処置具又はチャンネルの外周と気密に接触して前記処置具又はチャンネルの外周と摺動可能な第３摺動部のうち少なくとも一つを有し、前記気密部材が有する前記第１摺動部、前記第２摺動部、又は、前記第３摺動部のうち少なくとも一つは、前記内周又は前記外周に接触する部分を、先端に凸状の曲面を有して突出する凸部として形成され、前記気密部材には、外径が前記管部の内径よりも小さくされた小径部が形成され、前記気密部材の前記小径部の外周を覆うように巻き回された補強バンド部材をさらに備えることを特徴とする。
また、上記の挿入補助具において、前記内視鏡挿入部と前記処置具又は前記チャンネルのいずれかとを結束する固定具をさらに備えてもよい。
また、上記の挿入補助具において、前記処置具が挿通された前記チャンネルが前記気密部材の前記貫通孔に挿入されており、前記チャンネルの内周と前記処置具の外周との間に気密構造を有してもよい。
また、上記の挿入補助具において、前記気密部材が有する前記第１摺動部、前記第２摺動部、又は、前記第３摺動部のうち少なくとも一つは、前記管部の軸線方向に沿った少なくとも２箇所に配設されていてもよい。
本発明によれば、管部を通して管部の先端と基端との間の気密を保つ状態で内視鏡挿入部と処置具との挿抜操作を行うことができ、所定の処置を短時間で確実に行って操作者や患者の負担を減らすことができる。
本発明に係る第１の実施形態について、図１から図１１を参照しながら説明する。
本実施形態に係る挿入補助具１は、図１から図３に示すように、体腔内に挿入されて可撓性を有する内視鏡挿入部２と外付けチャンネル（チャンネル）３とが挿通可能な管部５と、管部５内で内視鏡挿入部２及び外付けチャンネル３を支持可能な貫通孔６、７が形成され、内視鏡挿入部２の外周と外付けチャンネル３の外周と管部５の内周とそれぞれに気密かつ移動可能に接して管部５の先端と基端との間の気密を保つ気密部材８と、気密部材８に内視鏡挿入部２と外付けチャンネル３とを挿通後、互いに結束する固定具１０とを備えている。
本実施形態に係る挿入補助具１は、図１から図３に示すように、体腔内に挿入されて可撓性を有する内視鏡挿入部２と外付けチャンネル（チャンネル）３とが挿通可能な管部５と、管部５内で内視鏡挿入部２及び外付けチャンネル３を支持可能な貫通孔６、７が形成され、内視鏡挿入部２の外周と外付けチャンネル３の外周と管部５の内周とそれぞれに気密かつ移動可能に接して管部５の先端と基端との間の気密を保つ気密部材８と、気密部材８に内視鏡挿入部２と外付けチャンネル３とを挿通後、互いに結束する固定具１０とを備えている。
外付けチャンネル３は、図１に示すように、結紮装置（処置具）１１を挿入可能な挿入口１２と、結紮装置１１を挿通可能に覆って内視鏡挿入部２に沿って配され可撓性を有する本体１３とを備えており、挿入口１２の内側にはＯリング（気密構造）１５が配設されて、結紮装置１１の外周と気密に接触可能とされている。
管部５は、図１から図３に示すように、例えば、口腔から胃まで達する長さとして１ｍ、好ましくは６００ｍｍの長さに可撓性を有して形成されており、先端が、体腔内への挿入をしやすくするために先端方向に向かって斜め方向に傾斜して形成され、基端が、内視鏡挿入部２の挿入をしやすくするために基端方向に向かって開口面積が広げられて形成されている。
管部５は、図１から図３に示すように、例えば、口腔から胃まで達する長さとして１ｍ、好ましくは６００ｍｍの長さに可撓性を有して形成されており、先端が、体腔内への挿入をしやすくするために先端方向に向かって斜め方向に傾斜して形成され、基端が、内視鏡挿入部２の挿入をしやすくするために基端方向に向かって開口面積が広げられて形成されている。
気密部材８は、図４に示すように、略円柱状に形成されており、軸方向の貫通孔６、７が、内視鏡挿入部２、外付けチャンネル３の断面形状に対応した形状に形成されてそれぞれ設けられている。ここで、気密部材８の材質はエチレンプロピレンゴムであるが、例えば、天然ゴム、イソプレンゴム、クロロプレンゴム、シリコンゴム、フッ素ゴム、ＳＥＢＳゴム、ＳＩＢＳゴム等、他の弾性体でも構わない。
気密部材８は、中央部の外径が管部５の内径よりも小さくされて管部５の内周から離間するように形成されているが、気密部材８の側面の両端部の外周には、径方向外方に凸状に突出して外形が管部５の内径に対応した形状されて管部５の内周と気密に接触して摺動可能な凸部（摺動部）１６、１７がそれぞれ設けられている。
また、貫通孔６の両端部側には、図５に示すように、径方向内方に突出して内視鏡挿入部２の外周と気密を維持して摺動可能に接触するフランジ状の凸部（摺動部）１８、２０がそれぞれ配設されている。
気密部材８は、中央部の外径が管部５の内径よりも小さくされて管部５の内周から離間するように形成されているが、気密部材８の側面の両端部の外周には、径方向外方に凸状に突出して外形が管部５の内径に対応した形状されて管部５の内周と気密に接触して摺動可能な凸部（摺動部）１６、１７がそれぞれ設けられている。
また、貫通孔６の両端部側には、図５に示すように、径方向内方に突出して内視鏡挿入部２の外周と気密を維持して摺動可能に接触するフランジ状の凸部（摺動部）１８、２０がそれぞれ配設されている。
また、図６（ａ）に示すように、気密部材８の中央部の凸部１６、１７間の側面の外周を覆って巻回可能な補強バンド部材２１が備えられている。
この補強バンド部材２１は、金属製の板部材が気密部材８の中央部の側面と略同一の径で円筒状に曲げられて形成されており、周方向の両端部には、外周方向に折り曲げられた折り曲げ部２２と、内周方向に折り曲げられた折り曲げ部２３とがそれぞれ形成され、図６（ｂ）、図６（ｃ）に示すように、一端側の折り曲げ部２２と他端側の折り曲げ部２３とが係合可能とされている。
この補強バンド部材２１は、金属製の板部材が気密部材８の中央部の側面と略同一の径で円筒状に曲げられて形成されており、周方向の両端部には、外周方向に折り曲げられた折り曲げ部２２と、内周方向に折り曲げられた折り曲げ部２３とがそれぞれ形成され、図６（ｂ）、図６（ｃ）に示すように、一端側の折り曲げ部２２と他端側の折り曲げ部２３とが係合可能とされている。
結紮装置１１は、先端に配設されて生体組織を把持して結紮可能なクリップ２５と、クリップ２５が挿通可能な可撓性を有する導入管２６と、クリップ２５による結紮操作を行う操作部２７とを備えている。
操作部２７は、図７に示すように、操作部本体２８と、操作部本体２８に対して軸方向にスライド自在に装着された第１のスライダ３０と、第１のスライダ３０に対しラチェット機構を介して軸方向にスライド自在な第２のスライダ３１とを備えている。
導入管２６内部には、基端がそれぞれ第１のスライダ３０と第２のスライダ３１とに接続された操作管２６Ａと操作ワイヤ２６Ｂとが互いに進退可能に配されている。
操作部２７は、図７に示すように、操作部本体２８と、操作部本体２８に対して軸方向にスライド自在に装着された第１のスライダ３０と、第１のスライダ３０に対しラチェット機構を介して軸方向にスライド自在な第２のスライダ３１とを備えている。
導入管２６内部には、基端がそれぞれ第１のスライダ３０と第２のスライダ３１とに接続された操作管２６Ａと操作ワイヤ２６Ｂとが互いに進退可能に配されている。
操作部本体２８には、第１のスライダ３０を操作部本体２８へ装着するとともに押すことによってこれを解除するための第１のボタン３２が設けられている。また、第１のスライダ３０には、先端のクリップ２５を回転操作するためのダイヤル３３が配設されている。さらに、第２のスライダには、押すことによって第１のスライダ３０に対して第２のスライダ３１を進退可能にするとともに離すとその位置で固定する第２のボタン３５が配設されている。
操作部２７の各構成品は、例えば、第１のボタン３２は赤、操作部本体２８は黄、第１のスライダ３０は白、ダイヤル３３は黒、第２のスライダ３１は黄、第２のボタン３５は橙、とそれぞれ色分けされている。

次に、本実施形態に係る挿入補助具１の使用方法、及び作用・効果について説明する。
まず、図８に示すように、内視鏡挿入部２を管部５内に挿入して貫通させる。その状態で内視鏡挿入部２のみを口腔から胃まで挿入する。
そして、内視鏡挿入部２をガイドとして管部５をこれに沿って管部５の先端側から患者の口腔内に挿入し、食道を介して先端が胃内に到達するまで管部５を挿入する。
こうして、内視鏡挿入部２を体外へ抜去することによって、図９に示す状態とする。
次に、図１に示すように、内視鏡挿入部２と外付けチャンネル３とを気密部材８の貫通孔６、７にそれぞれ挿通する。このとき、内視鏡挿入部２及び外付けチャンネル３の外周面に、必要に応じてシリコンオイル等の潤滑剤を塗布することによって貫通孔６、７にスムーズに挿入することができる。
気密部材８を内視鏡挿入部２先端からの距離が６００ｍｍ程度の位置に配しておく。そして、気密部材８の凸部１６、１７間の外周に補強バンド部材２１を巻きつけ、折り曲げ部２２を折り曲げ部２３に嵌合させて固定し、結紮装置１１を外付けチャンネル３内に挿入し内視鏡挿入部２及び外付けチャンネル３の先端側を固定具１０で結束する。
まず、図８に示すように、内視鏡挿入部２を管部５内に挿入して貫通させる。その状態で内視鏡挿入部２のみを口腔から胃まで挿入する。
そして、内視鏡挿入部２をガイドとして管部５をこれに沿って管部５の先端側から患者の口腔内に挿入し、食道を介して先端が胃内に到達するまで管部５を挿入する。
こうして、内視鏡挿入部２を体外へ抜去することによって、図９に示す状態とする。
次に、図１に示すように、内視鏡挿入部２と外付けチャンネル３とを気密部材８の貫通孔６、７にそれぞれ挿通する。このとき、内視鏡挿入部２及び外付けチャンネル３の外周面に、必要に応じてシリコンオイル等の潤滑剤を塗布することによって貫通孔６、７にスムーズに挿入することができる。
気密部材８を内視鏡挿入部２先端からの距離が６００ｍｍ程度の位置に配しておく。そして、気密部材８の凸部１６、１７間の外周に補強バンド部材２１を巻きつけ、折り曲げ部２２を折り曲げ部２３に嵌合させて固定し、結紮装置１１を外付けチャンネル３内に挿入し内視鏡挿入部２及び外付けチャンネル３の先端側を固定具１０で結束する。
この管部５に、図１０に示すように、外付けチャンネル３を介して結紮装置１１が装着された内視鏡挿入部２を気密部材８とともに管部５内に挿入し、図１１に示すように、先端を管部５の先端から突出させる。このとき、内視鏡挿入部２、外付けチャンネル３、及び、結紮装置１１の導入管２６がともに可撓性を有しているので、必要に応じて管部５の基端から突出した内視鏡操作部３６や操作部２７を術者が操作しやすい位置に湾曲させておく。
そして、図１２に示すように、所定の部位にて所定の処置を行う。このとき、必要に応じて内視鏡挿入部２に設けられた鉗子チャンネルに別の処置具（例えば、把持鉗子３７）を挿通しておく。
次に、作業領域を確保するため、図示しない送気装置によって内視鏡挿入部２から胃内へ空気を送気して胃を膨らませる。このとき、貫通孔６の凸部１８、２０と内視鏡挿入部２、貫通孔７と外付けチャンネル３、管部５と凸部１６、１７、結紮装置１１の導入管２６とＯリング１５とがそれぞれ気密かつ移動可能に接しているので、管部５内で体腔内と外部とが連通されてしまうのが抑えられる。したがって、送気した空気が外部に逃げずに胃を膨らませることができる。
次に、作業領域を確保するため、図示しない送気装置によって内視鏡挿入部２から胃内へ空気を送気して胃を膨らませる。このとき、貫通孔６の凸部１８、２０と内視鏡挿入部２、貫通孔７と外付けチャンネル３、管部５と凸部１６、１７、結紮装置１１の導入管２６とＯリング１５とがそれぞれ気密かつ移動可能に接しているので、管部５内で体腔内と外部とが連通されてしまうのが抑えられる。したがって、送気した空気が外部に逃げずに胃を膨らませることができる。
そして、処置部位近傍で、内視鏡挿入部２、把持鉗子３７、結紮装置１１、或いは、必要に応じて気密部材８を管部５に対してそれぞれ進退させながら処置を行う。このときも、気密状態が気密部材８で保持されているので、上述と同様に管部５内で体腔内と外部とが連通されてしまうのが抑えられる。したがって、胃を膨らませた状態が維持される。
この状態で結紮処置等を行う。
ここで、結紮装置１１による操作等は、内視鏡挿入部２を操作する術者以外の操作者が術者の指示に従って行われる。その際、様々な操作を短時間で適確に行う必要があるのに対して操作部２７が色分けされているので、予め色に応じた操作を決めておく等、色に意味をもたせておくことにより、色の呼称で操作手順や操作方法を簡略化して操作者に伝達することができる。
なお、色の代わりに操作の順番を示した数字等を付しても同様の効果を得ることができる。
ここで、結紮装置１１による操作等は、内視鏡挿入部２を操作する術者以外の操作者が術者の指示に従って行われる。その際、様々な操作を短時間で適確に行う必要があるのに対して操作部２７が色分けされているので、予め色に応じた操作を決めておく等、色に意味をもたせておくことにより、色の呼称で操作手順や操作方法を簡略化して操作者に伝達することができる。
なお、色の代わりに操作の順番を示した数字等を付しても同様の効果を得ることができる。
この挿入補助具１によれば、管部５内で内視鏡挿入部２と外付けチャンネル３とを気密部材８で支持した際に、管部５内で体腔内と外部とが連通されてしまうのを抑えることができ、管部５内周と気密部材８外周との気密状態を維持しながら気密部材８を管部５に対して進退させることができ、内視鏡挿入部２や外付けチャンネル３も同様に気密部材８に対してそれぞれ進退させることができる。
このとき、気密部材８が凸部１６、１７で管部５内周と気密に接するので、気密部材８と管部５との接触面積が気密部材８の外周全体の面積よりも小さい面積となって摩擦抵抗を小さく抑えることができ、気密部材８を管部５に対して楽に進退操作を行うことができる。
同様に、気密部材８が凸部１８、２０で内視鏡挿入部２と気密に接するので、気密部材８と内視鏡挿入部２との接触面積が貫通孔６内全体の表面積よりも小さい面積となって摩擦抵抗を小さく抑えることができ、内視鏡挿入部２の進退操作を楽に行うことができる。
このとき、気密部材８が凸部１６、１７で管部５内周と気密に接するので、気密部材８と管部５との接触面積が気密部材８の外周全体の面積よりも小さい面積となって摩擦抵抗を小さく抑えることができ、気密部材８を管部５に対して楽に進退操作を行うことができる。
同様に、気密部材８が凸部１８、２０で内視鏡挿入部２と気密に接するので、気密部材８と内視鏡挿入部２との接触面積が貫通孔６内全体の表面積よりも小さい面積となって摩擦抵抗を小さく抑えることができ、内視鏡挿入部２の進退操作を楽に行うことができる。
また、気密部材８の凸部１６、１７間の外周を覆うように補強バンド部材を巻くことによって、気密部材８を管部５内で進退させる際に、凸部１６、１７を中心に気密部材８がよれて変形してしまうのを抑え、管部５と、内視鏡挿入部２、及び、外付けチャンネル３とのそれぞれの気密状態を良好に維持することができる。
さらに、固定具１０によって、内視鏡挿入部２と外付けチャンネル３とを結束することによって、気密部材８のみの場合よりもより強固に両者を束ねることができ、管部５内で安定した進退操作を行うことができる。
また、結紮装置１１の進退を繰り返す必要があっても外付けチャンネル３を管部５に対して静止した状態で結紮装置１１を外付けチャンネル３内で進退させればよいので、外付けチャンネル３と気密部材８との気密状態を良好に維持することができる。
さらに、固定具１０によって、内視鏡挿入部２と外付けチャンネル３とを結束することによって、気密部材８のみの場合よりもより強固に両者を束ねることができ、管部５内で安定した進退操作を行うことができる。
また、結紮装置１１の進退を繰り返す必要があっても外付けチャンネル３を管部５に対して静止した状態で結紮装置１１を外付けチャンネル３内で進退させればよいので、外付けチャンネル３と気密部材８との気密状態を良好に維持することができる。
次に、参考例について図１３及び１４を参照しながら説明する。
なお、上述した第１の実施形態と同様の構成要素には同一符号を付すとともに説明を省略する。
参考例に係る挿入補助具３８が第１の実施形態との異なる点は、気密部材４０が管部５の基端側の内周に固定されている点と、外付けチャンネル３がなく、結紮装置１１が直接気密部材４０に接しているとした点と、貫通孔４１と貫通孔６との間にスリット４０Ａが設けられているとした点である。
なお、上述した第１の実施形態と同様の構成要素には同一符号を付すとともに説明を省略する。
参考例に係る挿入補助具３８が第１の実施形態との異なる点は、気密部材４０が管部５の基端側の内周に固定されている点と、外付けチャンネル３がなく、結紮装置１１が直接気密部材４０に接しているとした点と、貫通孔４１と貫通孔６との間にスリット４０Ａが設けられているとした点である。
また、貫通孔４１が結紮装置１１の導入管２６の断面形状に対応した形状に形成されて導入管２６を支持可能とされており、貫通孔４１の両端部側には、図１４に示すように、径方向内方に突出して導入管２６の外周と気密を維持して摺動可能に接触する凸部（摺動部）４２、４３がそれぞれ配設されている。同様に、貫通孔６の両端部側にも凸部１８、２０が配設されている。したがって、導入管２６の外周と内視鏡挿入部２の外周とがそれぞれ気密部材４０と気密かつ移動可能に接して、管部５の先端と基端との間の気密を保っている。さらに、固定具１０は、気密部材４０に内視鏡挿入部２と導入管２６とを挿通後、互いに結束するものとして備えられている。
次に、本参考例に係る挿入補助具３８の操作方法、及び作用・効果について説明する。
まず、第１の実施形態と同様に、管部５の先端側から患者の口腔内に挿入し、食道を介して先端が胃内に到達するまで管部５を挿入する。
内視鏡挿入部２と導入管２６とを固定具１０で結束した状態で管部５に挿入する。
挿通後は、上記第１の実施形態と同様に、内視鏡挿入部２を介して胃に空気を送気して胃を膨らませる。
まず、第１の実施形態と同様に、管部５の先端側から患者の口腔内に挿入し、食道を介して先端が胃内に到達するまで管部５を挿入する。
内視鏡挿入部２と導入管２６とを固定具１０で結束した状態で管部５に挿入する。
挿通後は、上記第１の実施形態と同様に、内視鏡挿入部２を介して胃に空気を送気して胃を膨らませる。
このとき、貫通孔６の凸部１８、２０と内視鏡挿入部２、貫通孔４１の凸部４２、４３と結紮装置１１の導入管２６とがそれぞれ気密かつ移動可能に接しているので、管部５内で体腔内と外部とが連通されてしまうのが抑えられる。したがって、送気した空気が外部に逃げずに胃を膨らませることができる。
こうして、結紮装置１１等による所定の処置を行う。
この挿入補助具３８によれば、第１の実施形態に係る挿入補助具１と同様の作用・効果を得ることができる。この際、管部５と気密部材４０とが固定されているので、管部５の内周と気密部材４０の外周とをより強固に気密するため、管部５の先端と基端との間の気密をより良好に維持することができる。
こうして、結紮装置１１等による所定の処置を行う。
この挿入補助具３８によれば、第１の実施形態に係る挿入補助具１と同様の作用・効果を得ることができる。この際、管部５と気密部材４０とが固定されているので、管部５の内周と気密部材４０の外周とをより強固に気密するため、管部５の先端と基端との間の気密をより良好に維持することができる。
なお、本発明の技術範囲は上記実施の形態に限定されるものではなく、本発明の趣旨を逸脱しない範囲において種々の変更を加えることが可能である。
例えば、上記実施形態では、内視鏡挿入部２と外付けチャンネル３とが気密部材８に、内視鏡挿入部２と結紮装置１１の導入管２６とが気密部材４０に、或いは、気密部材８が管部５に対して気密かつ移動可能に接しているとしているが、これらについても少なくとも何れか一つが気密部材８又は気密部材４０に固定されていても構わない。
また、第１の実施形態において、気密部材の貫通孔を導入管２６の形状に応じて形成してこれと気密かつ移動可能に接するとして、結紮装置１１を外付けチャンネル３を介さずに気密部材に挿通させても構わない。
例えば、上記実施形態では、内視鏡挿入部２と外付けチャンネル３とが気密部材８に、内視鏡挿入部２と結紮装置１１の導入管２６とが気密部材４０に、或いは、気密部材８が管部５に対して気密かつ移動可能に接しているとしているが、これらについても少なくとも何れか一つが気密部材８又は気密部材４０に固定されていても構わない。
また、第１の実施形態において、気密部材の貫通孔を導入管２６の形状に応じて形成してこれと気密かつ移動可能に接するとして、結紮装置１１を外付けチャンネル３を介さずに気密部材に挿通させても構わない。
さらに、凸部１６、１７が、気密部材８の両端の外周に配設されているが、図１５（ａ）に示すように、気密部材４５の中央部の外周に一つのみ凸部４６が設けられていても同様の作用・効果を得ることができる。また、図１５（ｂ）に示すように、凸部がなく、気密部材４７の外周面全体が管部５の内周に気密かつ摺動可能に接しているとしても、摺動抵抗が上記実施形態に比べて大きくなるが、気密状態の保持については同様の作用・効果を得ることができる。
また、摺動部は凸部１６、１７、１８、２０、４２、４３に限らず、それぞれ顎状に形成されているものでも他の形状でも構わない。

Claims (4)
- 可撓性を有し、体腔内に挿入される内視鏡挿入部と、処置具及び前記処置具が挿通可能なチャンネルのいずれかとが挿通可能な管部と、
該管部内において、前記内視鏡挿入部と、前記処置具及び前記チャンネルのいずれかとを支持する貫通孔を有する気密部材とを備え、
前記気密部材は、前記管部の内周と気密に接触して前記管部の内周と摺動可能な第１摺動部、前記内視鏡挿入部の外周と気密に接触して前記内視鏡挿入部の外周と摺動可能な第２摺動部、又は、前記処置具又はチャンネルの外周と気密に接触して前記処置具又はチャンネルの外周と摺動可能な第３摺動部のうち少なくとも一つを有し、
前記気密部材が有する前記第１摺動部、前記第２摺動部、又は、前記第３摺動部のうち少なくとも一つは、前記内周又は前記外周に接触する部分を、先端に凸状の曲面を有して突出する凸部として形成され、
前記気密部材には、外径が前記管部の内径よりも小さくされた小径部が形成され、
前記気密部材の前記小径部の外周を覆うように巻き回された補強バンド部材をさらに備えることを特徴とする挿入補助具。 - 前記内視鏡挿入部と前記処置具又は前記チャンネルのいずれかとを結束する固定具をさらに備えることを特徴とする請求項１に記載の挿入補助具。
- 前記処置具が挿通された前記チャンネルが前記気密部材の前記貫通孔に挿入されており、前記チャンネルの内周と前記処置具の外周との間に気密構造を有することを特徴とする請求項１に記載の挿入補助具。
- 前記気密部材が有する前記第１摺動部、前記第２摺動部、又は、前記第３摺動部のうち少なくとも一つは、前記管部の軸線方向に沿った少なくとも２箇所に配設されていることを特徴とする請求項１に記載の挿入補助具。
