【発明の属する技術分野】
本発明は、超音波診断装置に関する。

【従来の技術】
近年、被検者(被検体、患者)内で発生、反射した信号又は被検者内を透過した信号を受信し、得られた受信信号を視認性、客観性の高い医学的診断用の画像として再構成して表示する医用診断装置が知られている。具体的には、X線CT装置,MRI装置、X線診断装置、超音波診断装置等の医用診断装置として知られており、被検者内部の形態、動体、機能等の画像情報を得ることができる。

上記診断装置による診断では、被検者は診断情報収集のために一定時間拘束され、検査によってはこの拘束時間が数十分、時には1時間を越える場合がある。このような検査を受ける場合、被検者は「検査がいつ終了するのかという不安感、退屈感」、「超音波診断における音響ジェル塗布、心電計装着等の目的が不明なための不安感」、「検査中に発生する音に対する不安感」、「CT装置やMRI装置において、狭い空間に入れられることへの閉所不安感」等の様々な不安感や退屈感を感じることが多い。

このような長時間の検査によって生じる被検者の苦痛を低減する方法として、被検者が検査中に操作者の状況やテレビ放送等を見ることができるようにしたMRI装置が特開平1−249044号公報により知られている。

また、近年、診断装置の高機能化、医学診断手法の複雑化に伴い医用診断装置の操作者に高度な技術と経験が要求されているが、操作者の操作技術、経験によっては検査結果にばらつきが生じ、その結果、検査結果の信頼性が低下する恐れがあるという問題が生じている。このような問題を解決する方法として、予め設定されたワークフロー(検査手順)に従って装置の動作が切替わる超音波診断装置が特開2001−137237号公報に記載されている。

【発明が解決しようとする課題】
上記特開平1−249044号公報に開示された技術はMRI検査においてテレビ放送やビデオを患者に提供することにより患者の退屈感を軽減するものであるが、検査の進行状況に関係なく情報提供が行われていたため適切なタイミングで検査内容の説明や検査中の指示を行うことができなかった。

MRI装置においては検査が装置自体に依存する部分が大きく、検査自体は機械的に一定の流れに基づいて完了するものであり、通常は検査所要時間も検査前に予め予想した範囲から大きく外れることはない。従って、例えば事前におおよその検査所要時間を被検者に伝えておけば、あとは上記公報にあるように閉所、暗所、等への不安感や恐怖感を軽減するためのビデオ等を流すだけで足り、検査の進行状況における各種支持や臨機応変な対応は特別求められるものではない。

これに対し、例えば超音波診断装置では検査が技師等の検査者の手技によるところのものである。すなわち、検査の性格上、検査中の必要に応じて臨機応変に対応する必要がある。つまり、検査の中断や新たな検査の追加、検査中の判断状況によっては本検査から分岐するような別途検査の臨時実施(実施後には本検査に戻る)、等を要することがあるため、検査時間や検査内容などは検査前の予定とは大きく異なってしまうことも少なくない。

従って、被検者は検査がいつ終わるのか、次にどんな検査が行われるのか、など、MRI装置における閉所、音への不安感や恐怖感とは違った不安感を抱くことになる。

一方、このような超音波検査を実施する検査者の側から見ると、このような複雑になりがちな検査手順・検査内容を確実に実施するには高度な技術と経験が要求されることは上記した通りである。

そこで、上記特開2001−137237号公報に開示された内容は、ワークフローに従って装置の動作を切替えるものである。しかしながら、このようなワークフローは検査者に対しては有効であるが、依然として、同公報も患者に対して情報提供を行うことについて述べたものではなかった。

一般的な各種医用診断装置について話を戻せば、被検者は検査中に検査者(操作者、医師)の指示を受けて体位変え、呼吸止め等をする必要があり、従来は検査者が被検者に話しかけることによってこれらの指示を行っていた。しかしながら、会話による説明のみでは説明が不十分になることもあり、又、この説明のために検査者の作業が増えるという問題があった。X線CT装置、MRI装置では、検査者は通常、別室でシステムを操作しているため被検者への説明や指示の作業が大変になる。

本発明は上記問題に鑑みてなされたものであり、被検者の検査に対する不安感及び退屈感の軽減、検査の効率の向上を図ることのできる超音波診断装置を提供することを目的とする。

【課題を解決するための手段】
上記目的を達成するため請求項1にかかる本発明は、「超音波の送受信によって被検者内の状態を表す医用画像を生成する画像生成手段と、提供情報を記憶する提供情報記憶手段と、前記提供情報記憶手段に記憶された提供情報に基づいて実際の提供情報内容を生成する情報生成手段と、前記画像生成手段により生成された医用画像を表示する第1の表示手段と、前記情報生成手段により生成された提供情報内容を表示する第2の表示手段と、検査手順を記憶する検査手順記憶手段と、前記検査手順記憶手段に記憶された検査手順に従って超音波診断装置を制御する制御手段とを更に備え、前記情報生成手段は前記提供情報記憶手段に記憶された提供情報に基づいて実際の提供情報内容を生成し、前記第2の表示手段は前記情報生成手段により生成された提供情報内容を前記検査手順記憶手段に記憶された検査手順に従って表示すると共に、該検査による診断の安全性についての情報をも表示可能であることを特徴とする超音波診断装置。」を提供する。

【発明の実施の形態】
(第1の実施の形態)
図1は第1の実施の形態にかかる超音波診断装置(超音波診断システム)の構成図である。

超音波診断装置本体1は、Bモード処理回路2、ドプラ処理回路3、送受信回路4、制御プロセッサ5、メモリ6、内部記憶装置7、バス8、画像生成回路9、音声処理回路10、カメラ処理回路11、インターフェース12、検査者用モニタ13、検査者用スピーカ・マイク14、検査者用カメラ15、操作パネル16、を備えている。

超音波プローブ22は、超音波と電気信号の変換を行う超音波振動子アレイを有しており被検者に対して超音波の送受波を行う。送受信回路4は、超音波振動子アレイから超音波を送波させるための駆動信号を発生する送信回路と、各超音波振動子により受波された超音波エコー信号に対して遅延加算処理を施して所定の走査ラスタに対応する超音波エコー信号を生成する受信回路を備えている。

Bモード処理回路2は、超音波エコー信号に対して対数変換、包絡線検波等の処理を施す。Bモード処理回路2の出力信号に基づいて生成されたBモード画像は、超音波エコーの信号強度の2次元的な濃度分布像であり、生体内の組織構造、造影剤等を視認できる。

ドプラ処理回路3は、超音波エコー信号に対して直交検波処理、MTI(ムービングターゲットインジケータ)フィルタリング処理、自己相関処理等を施して、被検者内の移動体の速度値、分散値、パワー値を求める。ドプラ処理回路3の出力信号に基づいて生成されたCFM(カラーフローマッピング)像では、速度値、分散値、パワー値に所定の規則に従って色を割当てた2次元的な画像であり、血流、造影剤、生態組織等の移動体の速度、分散、パワー等を視認できる。

ワークフローデータは、超音波診断装置の動作を定義した動作制御項目(以下、アクティビティという。)の配列を記憶したものである。アクティビティには、そのアクティビティの名称、アイコン情報、超音波診断装置の動作が対応付けられており、動作として、Bモード、CFMモード、造影モード等の走査モードの切替え、超音波の送受信条件の変更、超音波診断用の計測プログラムの起動・終了、検査データの記録実行、検査者へのメッセージ表示、被検者への提供情報の変更等を定義することができる。このアクティビティの内容は、装置出荷時に予め記憶しておいても良いし、検査者が独自に作成しても良い。

ワークフロー処理プログラムは、ワークフローデータに基づいて各アクティビティを順次実行し、装置動作の切替えや検査者への作業の指示等を行う。また、内部記憶装置7は、検査中に収集した超音波像のデータ等を記録することもできる。

検査者用モニタ13は、超音波診断装置本体1と一体に構成され、超音波像、アクティビティアイコン、装置設定情報、カメラ撮影像、等の表示をする。被検者用モニタ17は、装置本体から離れた位置に設置可能に構成されており、検査残り時間情報、被検者を精神的に快適にする画像情報、カメラ撮影像、等の表示をするもので、被検者が見やすい位置に配置して使用する。

検査者が超音波診断装置の操作パネル16上にある検査開始ボタンを押すとワークフロー処理プログラムが起動され、ワークフロー処理プログラムは先ず内部記憶装置7に記憶されているワークフローデータに対応する検査名の一覧を生成して検査者用モニタ13にその一覧を表示する(ステップS1)。検査名としては心筋梗塞検査、肝癌検査、胎児発育検査等がある。表1に示すように、各検査名には、それぞれに内容の異なるワークフローデータ及び提供情報が対応付けられており、選択した検査名に応じて、実施するワークフローデータ及び表示する提供情報の内容が変わる。

次にワークフロー処理プログラムは検査名の一覧から検査名を選択できるよう入力待ちの状態になり、これに対して検査者は操作パネル16等を操作して実施する検査名を選択する(ステップS2)。

次にワークフロー処理プログラムは、ステップS2で選択された検査名に対応するワークフローデータを読み出し、ワークフローデータ中に定義されているアクティビティの配列を検査者用モニタ13上にアイコンを用いて表示する(ステップS3)。検査者用モニタ13上には各アクティビティに対応する複数のアイコンが実行順に並んで表示され、検査者はこのアイコンの配列を見ることにより検査の流れを直感的に把握することができる。

尚、上述した例では配列されたアクティビティが順に実行されているが、アクティビティが順次実行されてゆく途中で医師が検査者用モニタ13に表示されている任意のアクティビティアイコンをマウス等の操作で選択することにより実行するアクティビティは任意に変更することができる。この場合も、変更、選択されたアクティビティに対応する動作(被検者への提供情報の提供を含む)が行われる。

上述したフローチャートに従って処理を行うと共に、被検者用モニタ17にはプログレスバー17cを表示し、被検者が検査の残り時間を把握できるようにする。プログレスバー17cは、全体の検査時間を表した枠と、その枠内を検査の進行時間に応じて長さの変わるバーによって構成されている。全体の検査時間は、各ワークフローデータに予め代表的な全体の検査時間を対応付けて記憶しておいたものを用いるか、又は、各アクティビティに代表的な検査時間を対応付けて記憶しておき、ワークフローデータで使用するアクティビティの検査時間の合計を求めたものを用いる。進行時間は、検査での実際の経過時間を、アクティビティの切替等に連動して補正して求める。このようなプログレスバー17cを表示することにより、被検者は、全体の検査の内でどの程度検査が進んでいるか、又、検査終了まで後どの位かかるかということを知ることができる。

なお、検査時間に係る情報の表示については上記の例に制限されることなく、グラフ形態での表示や文字のみによる表示形態、その他通常知られる時間情報表示形態を含め、検査の所要予定時間のみの表示や残り検査時間のみの表示、あるいはこれらの組合せや所要予定時間に対する現在の経過時間の表示などについて、いかなる形態による表示であっても良い。なお、表示はモニタへの視覚表示に限らず、被検者用スピーカ・マイクを通じた音表示であってもよい。

一方、検査者が操作パネル16を操作してカメラ像の表示を指示すると、検査者用カメラ15で撮影された検査者の画像が被検者用モニタ17の小画面17b部分に表示され、被検者用カメラ19で撮影された被検者の画像が検査者用モニタ13の小画面13e部分に表示される。この時、検査者と被検者は、検査者用スピーカ・マイク14と被検者用スピーカ・マイク18を通じて相互に会話できるようになり、相手の表情等を見ながら会話をすることができるようになる。

又、検査者用カメラ15及び被検者用カメラ19で撮影された画像や、検査者用スピーカ・マイク14及び被検者用スピーカ・マイク18で収集された音声を、通信線21を介して検査室外の遠隔した場所にあるコンピュータに送信し、そのコンピュータ上で検査の状況や被検者の状態を他の医師(主治医)等が観察できるようにしても良い。

図4は、ワークフロー処理の一例を示しており、左側にワークフロー中のアクティビティ、中央はアクティビティに対応する検査者及び被検者の作業、右側はアクティビティに対応する被検者への提供情報を示している。

先ずワークフロー処理プログラムは最初のアクティビティ”被検者登録”(ステップS41)を実行し、検査者用モニタ13に被検者登録を行うための画面を表示する。これと共に、ワークフロー処理プログラムは被検者用モニタ17に「ベッド上に横になってください。」という文字のメッセージと横たわり方を表すアニメーション像(提供情報A)を表示し、被検者はこの指示に従ってベッド上に横になる。

被検者がベッド上に寝たことを確認した後に検査者が完了ボタンを押すと、ワークフロー処理プログラムは次のアクティビティである”心電計準備”(ステップS42)を実行し、検査者用モニタに心電計の準備を促すメッセージを表示する。これと共にワークフロー処理プログラムは、心電図をモニタする目的、心電図によって診断可能な疾病の内容、安全性(例えば、皮膚に装着するセンサーは、感電の危険は無い等)等の情報(提供情報B)を被検者用モニタ17及び被検者用スピーカ・マイク18を用いて映像、文字、アニメーション像、音声等で提供する。これを受けながら、被検者は心電計を装着することになる。

心電計の準備が完了した後に検査者が完了ボタンを押すと、ワークフロー処理プログラムは次のアクティビティである”通常超音波診断”(ステップS43)を実行し、アクティビティに対応付けて予め定められている条件に基づいて超音波診断装置のモード切替え(Bモード、CFMモード等の切替え)や送受信条件の切替えを行うと共に、検査者用モニタ13に通常超音波診断を促すメッセージを表示する。検査者は、このメッセージに従い、被検者に対して音響ジェルの塗布などを行った後、検査部位及び検査部位周辺等の超音波像を見て全体的な観察を行う。

この時同時に、ワークフロー処理プログラムは超音波診断の内容及び目的、超音波診断によってどのような疾病が診断可能か、あるいは超音波診断の安全性についての説明(提供情報C)を被検者用モニタ17及び被検者用スピーカ・マイク18から提供する。
