開心術等において使用される人工肺として、主に血液中に直接酸素等のガスを吹き込んで血液の酸素加をはかる気泡型人工肺、人体と同様に膜を介して間接的に血液の酸素加をはかる膜型人工肺等があり、近年では、血液損傷が少ない利点を有する膜型人工肺が広く使用されている。この膜型人工肺の基本構造は、ガス流出口、ガス流入口、血液流出口および血液流入口を有したハウジング内に、中空糸集束体の開口端両端部を固定するものであり、該中空糸膜の片面に酸素を、他面に血液を供給し、膜面を介して酸素と血液中の二酸化炭素のガス交換を行なうものである。

しかしながら、このような人工肺においては、室温が血温より低い場合、人工肺内で加温されたガスが、中空糸膜のガス流出口側開口端から排出されると、外気との温度差によって、ガス中の水蒸気が、中空糸内部或いは中空糸膜開口端に結露するという、いわゆるウエットラング現象が発生してしまう。それによりガスの流れが阻害され、ガス交換の性能の低下を引き起こす恐れがある。また、中空糸膜が多孔質膜の場合、前記中空糸内腔に生じる結露により血液−ガス界面の表面張力が低下するため、血漿の漏出が発生し、やはり性能低下を引き起こす恐れがある。

そのため、上記の課題を解決するものとして、過去に様々な発明が開示されてきた。例えば、特開昭61−128978号公報には、膜集束体のガス導出側集束端部に温度保持手段として、発砲ポリスチロール等の断熱材を設けた膜型人工肺が開示されている。特開昭62−82969号公報には、ガス排出ポートに連通する吸引手段と、ハウジングに大気開放バルブを設けた人工肺装置が開示されている。特開平8−141073号公報には、加温装置をガス導出側ポートを覆うように装着し、電力により加温する一方、発熱した熱を断熱材により断熱する中空糸膜型人工肺が開示されている。特開平8−141074号公報には、ガス流出部10をほぼ被包する熱媒体流通部を有しており、熱媒体流通部に、熱交換器に使用した温水あるいは冷水を循環させて、ガス流出部の温度調整をする人工肺または熱交換器付き人工肺が開示されている。

(2)また、前記の血漿漏出の発生による中空糸膜の性能低下の問題は、中空糸膜のガス流出口側の開口端のみで生じる問題ではなく、中空糸内腔を流れる血液あるいはガスの流速が比較的遅い場合、例えば、補助循環として使用する場合や人工肺が小児用である場合は、中空膜全体で生じる問題である。したがって、このような問題を起こさないようにするために、予め血温と同じ或いはそれ以上の温度に調製されたガスを送風する必要があり、上記の装置では、独立したガス用加温器や、ガス温を血温と同温あるいはそれ以上に調製するためのコントローラーが必要である。
