【発明の属する技術分野】
本発明は、血管、尿道、腹腔等の生体内腔あるいは管腔に挿入部を挿入し、あるいは外科手術的に生体組織に押し当て部分を押し当て、または体表に押し当て部分を押し当てた後に、挿入部や押し当て部分に設置された出射部から、レーザ光、マイクロ波、ラジオ波、超音波等のエネルギーを、前立腺組織等の生体組織に照射して加熱治療を行う加熱治療装置に関する。

【従来の技術】
生体の体腔を利用しあるいは生体に小切開を施すことによって生体内に挿入される長尺状の挿入部を用い、その生体の病変部位にレーザ光、マイクロ波、ラジオ波、超音波等のエネルギーを照射して、その病変部位の組織を加温、変性、壊死、凝固、焼灼あるいは蒸散させて消滅させることにより、病変部位を加熱治療する加熱治療装置が知られている。

例えば、特開平11−333005号公報には、生体組織の表層またはその近傍に位置する病変部位に、レーザ光源装置から供給されたレーザ光を、挿入部の先端近傍に設置されたレーザ反射面であるミラーで反射させて照射するように構成された側射式の加熱治療装置が示されている。

ここで、加熱治療装置における治療条件の設定は、一般に、術者自身が経験により判断して、レーザ光等のエネルギーの出力やエネルギーの照射時間などの治療条件を個々に入力することにより行われている。

ところが、上記のレーザ光をミラーで反射させて病変部位に照射する加熱治療装置においては、レーザ照射中におけるミラーの状態、特に温度の状態を知ることが不可能であった。レーザ出力値を十分に大きく、あるいは照射時間を十分に長く術者が選択した場合で、且つこれらに組み合わされる条件としてミラー周辺を冷却する冷媒の冷却能力が比較的少ない場合にはミラーの温度が上昇する可能性があり、ミラーの温度を実測値として把握することは重要である。

すなわち、ミラーがそのベースとなる基材に対して接着固定等の方法で固定されている場合には、熱作用で接着剤が劣化し、ミラー自身が浮き上がったり、またははがれるなどの可能性がある。また、ミラーと基材が熱膨脹係数において大きく異なる材質の組み合わせで構成されている場合も同様の可能性がある。さらには、基材が特に熱膨脹性に富んだ材質で構成される場合には、基材の熱膨脹により、ミラーを摺動可能に案内するレール手段との摺動抵抗が増加し、ミラーの滑らかな動きが阻害される可能性がある。

また、上記公報には、長尺の挿入部を例えば尿道に挿入した後、レーザ反射面であるミラーを備えた出射部を挿入部内で長手方向に往復運動させながら、レーザ光の出射角度、つまりミラーの角度を変化させることにより、生体組織内の深部に位置する目的部位にレーザ光を集中させるようにした技術が提案されている。これにより、目的部位のみが所望の温度に加熱治療され、目的部位以外の部位は低い温度に保持される。しかしながら、上記公報に記載された加熱治療装置においては、連続的に移動する出射部によるレーザ光の出射動作自体を検出することができなかった。このため、加熱治療される生体組織に対して、レーザ光が適切に移動しながら照射されていることを直接確認することが困難であった。

【発明が解決しようとする課題】
本発明は、上記した従来技術の問題点に鑑みてなされたものであって、本発明の目的は、エネルギーを、目的とする病変部位に向けて適確に印加することにより、良好な治療効果を得ることができる加熱治療装置を提供することにある。

【課題を解決するための手段】
本発明の目的は、下記する手段により達成される。

(1) 生体組織にエネルギーを印加して加熱治療を行う加熱治療装置であって、治療のためのエネルギーを供給するエネルギー供給手段と、前記エネルギー供給手段に接続され、当該エネルギー供給手段から供給されるエネルギーを反射するエネルギー反射手段を備えた、エネルギー出力手段と、前記エネルギー反射手段を往復直線運動させることにより当該エネルギー反射手段の位置を直線方向に変化させるとともに当該位置の変化に伴って当該エネルギー反射手段の角度を変化させる駆動手段と、前記エネルギー反射手段の往復直線運動を検出する検出手段と、前記検出手段の検出結果を用いて、前記エネルギー供給手段の動作状態を制御するエネルギー制御手段と、を有することを特徴とする加熱治療装置。

【発明の実施の形態】
以下、添付図面を参照して本発明の好適な実施の形態を詳細に説明する。

図1は、本発明の第1実施形態に係る加熱治療装置に適用されるレーザ照射装置の側面図、図2は、レーザ照射装置の先端部の断面図である。

本実施形態の加熱治療装置は、レーザ光を生体組織に照射する、側射式のレーザ照射装置1を有している。この加熱治療装置は、生体内にレーザ照射装置1の長尺状の挿入部121を挿入し、この挿入部121に設置されたレーザ出射部122から、レーザ光を生体組織1001(図6参照)に向けて照射して加熱治療を行うものであり、例えば、前立腺肥大症や、各種の癌等の腫瘍の治療に用いられる。

図1および図2に示すように、エネルギー出力手段としてのレーザ照射装置1は、長尺状の挿入部121と、レーザ光を照射するためのレーザ出射部122と、レーザ出射部122を内包しかつ挿入部121の先端部に連接されるハウジング124とを有している。

レーザ出射部122には、一本のアーム128が接続されている。アーム128は、ハウジング124内のレーザ出射部122を支持する。アーム128を挿入部121の軸方向に移動させることにより、レーザ出射部122は軸方向に移動される。

レーザ出射部122は、片面に形成された、レーザ光を反射する平滑なレーザ反射面(ミラー)123を有している。このレーザ出射部122は、例えば樹脂、ガラス、金属、あるいはこれらの複合材料から形成されている。具体的には、例えば、金属を基材として表面を鏡面に研磨したもの、樹脂や金属を基材として金属等の薄膜を蒸着等により形成して鏡面としたもの、ガラス製の鏡等の反射材を樹脂や金属等の基材に接着したもの、等が挙げられる。本実施形態では特に、レーザ出射部122の温度を検出するミラー温度センサ111が設けられている。ミラー温度センサ111としては、例えばサーミスタ、熱電対、白金測温抵抗体などが挙げられるが、これらに限定されるものではない。

このミラー温度センサ111は、好ましくは、レーザ反射面123の裏面に設置される。これにより、ミラー温度センサ111は、レーザ光を直接受けることが防止されるので、温度の検出精度が良好に保たれるとともに、損傷からも保護される。また、レーザ光の導光効率を低下させることもない。なお、ミラー温度センサ111の設置位置は、レーザ反射面123の裏面に限られるものではなく、レーザ出射部122のうちのレーザ反射面123を除く領域であれば設置可能である。ミラー温度センサ111からの信号は、ミラー温度センサ信号リード線112により送られる。

ハウジング124は、レーザ光照射用の窓部127を有するステンレス鋼などの硬質の管状体からなり、レーザ光透過性のカバー部材125によって覆われている。ハウジング124は、レーザ出射部122から出射されるレーザ光の出射角度を変更するために、レーザ出射部122の両側に突出した突起131(図3参照)と係合するための一対の溝129が設けられた内壁を有する。レーザ出射部122のガイドとして機能する溝129は、レーザ出射部122をはさんで両側に配置され、挿入部121の軸方向に対し傾斜している。なお、ハウジング124の先端部は、キャップ126により密封されている。

レーザ光を導くため、光ファイバ101が、挿入部121の内部に配置されている。なお、光ファイバ101の先端に、レンズを設けてもよい。このレンズは、レーザ光の照射角を小さくするための光学素子である。光ファイバ101は、レーザ光源装置3で発生させられたレーザ光を伝達する。緩衝装置132は、ループを形成している光ファイバを収容しており、光ファイバの運動を、吸収する。

レーザ照射装置1には、生体組織表層の観察を行う観測装置8が付設されている。この観測装置8は、レーザ照射装置1に対して脱着自在の内視鏡801を有している。内視鏡801は、レーザ照射装置1の基端部から先端部に向かい、挿入されている。内視鏡801の基端側には、カメラヘッド803が取り付けられており、カメラ信号リード804を通じて画像を送ることができる。また、内視鏡801の基端側にはライトガイド802が接続されており、レーザ光が照射される表層の照明が可能となっている。なお、カメラヘッド803を接続しなくても、内視鏡801に装着されている接眼レンズを介して直接目視することも可能である。

アーム128は、ハウジング124内で左右に分岐してレーザ出射部122を支持しているため、レーザ出射部122の表面にレーザ光が当たることが妨げられることはない。レーザ出射部122は、一辺に、支持部130が設けられ、他辺に、一対の突起131が設けられている。支持部130は、アーム128に回動自在に取付けられており、レーザ出射部122の照射角度の変更に対応可能とされている。突起131は、前述したように、ハウジング124の内壁に配置される溝129と係合する。

アーム128は、レーザ照射装置1の基端部に配置される駆動ユニット4に連結されている。なお、駆動ユニット4をレーザ照射装置1の外部に設置し、アーム128をドライブシャフトを介して駆動ユニット4と接続するように構成してもよい。この場合、ドライブシャフトとしては、金属ワイヤ等を使用することができる。

駆動ユニット4は、モータ401を有している(図13参照)。モータ401としては、例えば、インダクションモータ、サーボモータ、ステッピングモータ等を使用することができる。駆動ユニット4は、モータ401の回転運動を図示しないカム機構ないしリンク機構により往復直線運動に変換してアーム128に伝達することによって、レーザ出射部122を、挿入部121の軸方向に、往復運動させる。そして、レーザ出射部122は、アーム128と溝129との連動に基づき、軸方向の位置に伴って、傾斜角度が変化する。

本実施形態では特に、駆動ユニット4は、レーザ照射装置1に対して、着脱可能に構成されている。具体的には、例えば、モータ401には、モータ401の駆動力を伝達するための駆動力伝達部402が連結されており、一方、レーザ照射装置1のアーム128には、駆動力伝達部402が着脱可能な駆動力受容部135が、支持部134を介して設けられている(図13参照)。なお、アーム128を省略し、光ファイバ101の先端近傍に固着された固定部材にレーザ出射部122を回動可能に取り付け、光ファイバ101自身を往復運動させることにより、レーザ出射部122の位置および角度を変化させる構成とすることもできる。この場合、駆動力受容部135に連設された支持部134は、往復移動される光ファイバ101に連結される。

このように、駆動ユニット4とレーザ照射装置1とを互いに着脱可能に構成すれば、生体内に挿入されるレーザ照射装置1は治療ごとに廃棄することができる一方で、駆動ユニット4は繰り返し使用するという使い方が可能となり、ランニングコストの低減が図られている。また、駆動ユニット4は、当該駆動ユニット4がレーザ照射装置1に接続されていることを検知する接続検知手段として、例えばマイクロスイッチ403を備えている。

図4に示すように、位置P2における、アーム128と非平行な溝129との間の距離は、位置P1に比べ、短い。したがって、レーザ出射部122の支持部130が、位置P1から位置P2に、移動する場合、レーザ出射部122の突起131が、溝129に沿ってスライドし、レーザ出射部122の傾斜角度が、調整される。つまり、レーザ出射部122の挿入部121の軸に対する傾斜角度が、小さくなる。同様に、レーザ出射部122の支持部130が、位置P2から位置P3に、移動する場合、レーザ出射部122の挿入部121の軸に対する傾斜角度が、更に小さくなる。一方、位置P1〜位置P3において、レーザ出射部122によって反射されるレーザ光は、病変部位、つまり目標とする加熱部位であるターゲット部位1101内部のターゲットポイント1102に集中する。

つまり、レーザ光は、ターゲットポイント1102にのみに、連続的に照射され、表層等の他の生体組織には、間欠的に照射される。したがって、ターゲットポイント1102は、照射されたレーザ光により、加熱され、所望温度に達する。一方、表層等の他の生体組織は、レーザ光を受光する時間が短いため、発生する熱量も少なく、ほとんど加熱されない。

ルーメン144,145は、それぞれ、冷却水入口コネクタ103、冷却水出口コネクタ104を介し、給水チューブ272、排水チューブ273に接続されている(図1参照)。冷却水を循環させることにより、冷却能率の向上が図られる。冷却水の温度は、レーザ光の照射によるレーザ出射部122や生体組織の照射表面の損傷を低減できれば特に限定されないが、好ましくは0〜37℃、より好ましくは凍傷の虞れが少なく、かつ冷却効果が高い8〜25℃である。冷却水が逆流することを防ぐために、ルーメン141〜143に、それぞれ、逆止弁を設けることが好ましい。冷却用の冷媒としては、滅菌された液体、例えば精製水や生理食塩水を使用することが好ましい。

挿入部121の先端部が、体腔1002内に挿入され、レーザ出射部122が収容されているハウジング124を、病変部位、つまり目標とする加熱部位であるターゲット部位1101の近傍の表層に密着させる。この際、内視鏡801によって、ハウジング124の位置を、直接確認することが望ましい。なお、挿入部121の長手方向に関する、ターゲットポイント1102の位置は、レーザ照射装置1全体を挿入部121の長手方向に移動させることによって、調整される。また、挿入部121の周方向に関する、ターゲットポイント1102の位置は、レーザ照射装置1全体を回転させることによって、調整される。

レーザ光の照射に際しては、レーザ出射部122は、0.1〜10Hz好ましくは1〜6Hzの周期で、照射角度を変化させながら、軸方向に往復運動させられる。こうしてレーザ光の光路は、連続的に変更されるが、全て、ターゲットポイント1102で交差する。

これにより、生体組織1001内部のターゲットポイント1102およびその近傍は、照射されたレーザ光により、加熱され、所望温度に達する。こうして表層部分の温度上昇を抑えながら、所望のターゲット部位1101内の温度だけを高めることができる。

なお、生体組織1001に照射されるレーザ光は、発散光、平行光あるいは収束光を用いることができる。レーザ光を収束光とするためには、レーザ光の光路の途中に、レーザ光を収束光にする光学系を設ける。また、使用されるレーザ光は、生体深達性を有するものであれば、特に限定されない。しかし、レーザ光の波長は、750〜1300nmまたは1600〜1800nm程度が特に優れた生体深達性を有するため好ましい。なお、上記波長範囲のレーザ光を発生させるレーザ光源装置としては、例えば、He−Neレーザ等の気体レーザ、Nd−YAGレーザ等の固体レーザ、GaAlAsレーザ等の半導体レーザ等が挙げられる。

また、レーザ照射装置1の挿入部の直径、すなわち挿入部121の外径は、体腔1002内に挿入可能であれば、特に限定されない。しかし、挿入部121の外径は、2〜20mm程度が好ましく、3〜8mm程度がより好ましい。

図7は、加熱治療装置を前立腺の治療に使用した例を示す断面図である。レーザ照射装置1の挿入部121は、尿道1003に挿入され、レーザ出射部が設置されている挿入部121の先端近傍が、前立腺1004近傍の表層に密着させられる。図中符号1005は膀胱を示している。尿道壁の温度を検出する尿道温度センサ113a,113bが、挿入部121の先端近傍の内部に配置されている。

また、本実施形態の加熱治療装置は、直腸プローブ5を有している。直腸プローブ5は、肛門1007から直腸1006に挿入される挿入部501と、操作者によって握られるグリップ502とを備える。直腸プローブ5の挿入部501には、直腸壁の温度を検出する複数の直腸温度センサ503a〜503eが設けられており、その検出値が、センサ信号リード504を通って送信されるようになっている。このように、直腸温度センサ503a〜503eは、生体組織に刺入されることなく、尿道1003から見て前立腺1004の深部に配置される。

したがって、加熱治療装置は、尿道壁の温度および直腸壁の温度の検出結果を用いて、加熱治療を実施することができる。これにより、前立腺の近傍に存在する尿道や直腸の正常組織を必要以上に加熱することを防止することが可能となる。ここで、尿道温度センサ113a,113bに用いる温度センサとしては、サーミスタ、熱電対、白金測温抵抗体などが挙げられるが、好ましくは素子が小さく、レーザ照射への影響が小さい熱電対である。また、直腸温度センサ503a〜503eに用いる温度センサとしては、同様にサーミスタ、熱電対、白金測温抵抗体などが挙げられるが、好ましくは安価なサーミスタである。

前記検出手段の検出結果を用いて、前記エネルギー供給手段の動作状態を制御するエネルギー制御手段と
