Description
先に述べた通り、SBMAにおいては患者の殆どは男性であり、女性では遺伝子異常を有する保因者であっても症状が殆どみられないという、性差が著明である。しかし、これまで報告された、切断されたAR遺伝子を導入したトランスジェニックマウスにおいては、このような症状の性差はみられていない(Adachi et al., 2001, Abel et al., 2001)。症状の性差が認められない理由は、これらのマウスにおける導入遺伝子が全長のものではなく、リガンド(テストステロン)の結合部位を有していないためと考えられる。全長のヒトARを導入したSBMAのTgモデルは過去に1つだけ報告があり、運動障害を呈するとされているが、症状の性差などは報告されていない(Morrison et al., 2000)。
次にこのTgマウスを用いて種々の実験を行ったところ、雄において去勢を行って生体内のテストステロン量を減少させることによりSBMA様の症状及び病理所見が著しく改善することが判明し、これとは逆に雌においてテストステロンの投与を行えばSBMA様の症状等の著しい悪化が引き起こされることが判明した。一方、下垂体の黄体形成ホルモン放出ホルモン(LHRH)受容体を減少させ、下垂体からの性腺刺激ホルモン(LHやFSHなど)の分泌を抑制し、それによってテストステロン分泌を抑制する作用を有するLHRHアナログを雄Tgマウスに投与してその効果を観察したところ、去勢した場合と同様に症状の著明な改善が認められた。これらの事実から、生体内のテストステロン濃度を低下させることがSBMAの治療に有効な手段となるとの知見が得られた。
本発明は以上の検討の結果に基づいて完成されたものであって、次の各構成を提供する。
[1] 症状又は病理所見についての以下の(1)〜(5)の特徴を備える非ヒト動物、
(1)進行性筋萎縮が認められる、
(2)筋力低下が認められる、
(3)抗ポリグルタミン抗体を用いた免疫染色によって核のびまん性染色及び核内封入体が認められる、
(4)抗アンドロゲン受容体抗体を用いた免疫染色によって核のびまん性染色及び核内封入体が認められる、及び
(5)神経原性変化が認められる。
[2] 雌である場合は前記(1)〜(5)が認められないか、又は雄である場合に比較して軽症ないし軽度である、[1]に記載の非ヒト動物。
[3] 前記非ヒト動物が齧歯目動物である、ことを特徴とする[1]又は[2]に記載の非ヒト動物。
[4] 前記非ヒト動物がマウスである、ことを特徴とする[1]又は[2]に記載の非ヒト動物。

症状としては、(1)進行性筋萎縮、及び(2)筋力低下である。典型的にはこれらの症状の結果として或はこれらの症状に関連して進行性の運動障害、体格の縮小、寿命短縮、活動低下などの症状も認められる。ここで進行性筋萎縮の有無やその程度については、例えば後脚などの特定の部位を肉眼で観察して経時的な変化を調べることにより確認することができる。一方、筋力低下の有無やその程度については、例えば運動障害の有無やその程度から或は活動低下の有無やその程度から間接的に確認することができる。尚、運動障害の有無やその程度は例えばRotarod taskテスト(Crawley JN. What’s wrong with my mouse. Wiley-Liss, New York, Pp48-52.)によって、また活動低下の有無やその程度は例えばCage activityテスト(Crawley JN. What’s wrong with my mouse. Wiley-Liss, New York, Pp48-52.)によってそれぞれ確認することができる。その他の症状については、体格の縮小であれば肉眼的観察又は体重の測定などによって、寿命短縮であれば累積生存数を調べることなどよってそれぞれ確認することができる。
本発明の非ヒト動物では、典型的には初期症状として体幹及び後脚の筋萎縮が観察される。また、典型的にはこの初期症状に続いて体重減少や運動機能低下が認められる。
神経原性変化は筋組織のHE(ヘマトキシリン−エオシン)染色像を観察すること等によって確認することができる。ここでの神経原性変化は、典型的には群集萎縮と小角化繊維の発生である。
本発明の非ヒト動物では、上述の症状の場合と同様に病理所見についても性によって顕著に異なる。即ち上掲の各病理所見は雄の場合に顕著に認められ、典型的には雌では特に神経原性変化が認められない。
マイクロインジェクション法では、まず交尾が確認された雌マウスの卵管より受精卵を採取し、そして培養した後にその前核に所望のDNAコンストラクトの注入を行う。DNAコンストラクトとしては多数のCAGリピートを含むヒトアンドロゲン受容体(以下、「AR」ともいう)遺伝子の全長をコードする配列(以下、「導入遺伝子」という)を含むものが使用される。ここでCAGリピートの繰返し数は作製されるトランスジェニックマウスがSBMAの特徴を忠実に再現できるのに十分である必要があり、一般にSBMA患者におけるCAGリピートの繰返し数が40以上であることを考慮すれば、少なくとも40、好ましくは70以上、更に好ましくは80以上、更に更に好ましくは90以上にすることが好ましい。使用するDNAコンストラクトには導入遺伝子の効率的な発現を可能とするプロモーター配列が含まれていることが好ましい。このようなプロモーターとしては例えばチキンβ−アクチンプロモータ、プリオン蛋白プロモーター、ヒトARプロモーター、ニューロフィラメントL鎖プロモーター、L7プロモーター、サイトメガロウイルスプロモーターなどを用いることができる。
注入操作を終了した受精卵を偽妊娠マウスの卵管に移植し、移植後のマウスを所定期間飼育して仔マウス(F0)を得る。仔マウスの染色体に導入遺伝子が適切に組込まれていることを確認するために、仔マウスの尾などからDNAを抽出し、導入遺伝子に特異的なプライマーを用いたPCR法や導入遺伝子に特異的なプローブを用いたドットハイブリダイゼーション法等を行う。
本発明のスクリーニング方法は、(a)上記本発明の非ヒト動物に被験物質を投与する工程と、(b)投与後の前記非ヒト動物において、ポリグルタミン病に特徴的な症状又は病理所見が改善されるか否かを調べる工程とを含む。この(b)の工程において検査対象となる症状等の改善が認められれば被験物質はポリグルタミン病の治療剤の有力な候補であると判定される。
尚、本明細書において「ポリグルタミン病治療剤」とは、ポリグルタミン病を発症している患者に対してその症状の改善などを目的として使用される薬剤はもとより、ポリグルタミン病を発症するおそれのある者に対して予防的に使用される薬剤をも含む用語として用いられる。また本明細書において「ポリグルタミン病」とは、遺伝子のコード領域においてCAGリピートの伸長が認められる疾患の総称であって、ハンチントン病、脊髄小脳変性症、球脊髄性筋萎縮症などがこれに分類される。ポリグルタミン病の患者には表現促進現象(anticipation)やCAGリピート数のばらつき(体細胞モザイク)、主に神経組織が選択的に障害されるという共通の病態が観察される。
被験物質としてはペプチド、非ペプチド性低分子化合物、タンパク質、糖タンパク質、脂質、糖脂質、糖等を用いることができる。これらは自然界に存在する天然のものであっても或は合成したものであってもよい。その他、ヒト細胞又は非ヒト動物細胞等の抽出液又は培養上清などを被験物質として用いてもよい。
ポリグルタミン病の中でも球脊髄性筋萎縮症の治療薬をスクリーニングする場合には、テストステロンの分泌を抑制する作用を有する化合物の中から被験物質を選択することが好ましい。効率的なスクリーニングが可能となるからである。このような化合物としては、例えば下垂体からの性腺刺激ホルモンの分泌を抑制する作用を有するもの(例えば下垂体に作用して黄体形成ホルモン放出ホルモン受容体を減少させる作用を有する化合物)を用いることができる。さらに具体的な被験物質としては、黄体形成ホルモン放出ホルモンのアナログ又はその誘導体を例示することができる。
尚、LHRHアナログにその作用を損なわない程度に(作用を高める場合を含む)改変を施した各種誘導体を本発明の有効成分として用いることもできる。
このように製剤化した本発明の治療様薬剤はその形態に応じて、経口投与又は非経口投与(静脈内、動脈内、皮下、筋肉、腹腔内注射など)によって患者に適用することができる。
(導入遺伝子(transgene))
チキンβ−アクチンプロモーター調節下に繰返し数24のCAGリピート(24CAG)又は繰返し数97のCAGリピート(97CAG)を有するヒトAR遺伝子(順に配列番号1、及び配列番号2)を組み込んだ導入用DNAコンストラクト(図1Aを参照)を以下のように作製した。pCAGGSベクター(Niwa et al., 1991) を制限酵素HindIII で処理し、fill inの後に接合することで、新たにNheI 処理可能部位を作製し (pCAGGS-NheI)、24CAGないし97CAGを有する全長のヒトAR遺伝子 (Kobayashi et al., 1998) をこのpCAGGS-NheIにサブクローニングした。5.3kb及び5.5kb挿入配列において、直接DNAシークエンス法によって24CAGリピート及び97CAGリピートを確認した。
プラスミドをSalI-NheI で処理し、AR断片を切り出した。この断片をBDF1受精卵(日本エスエルシー、静岡、日本)に注入(microinjection)し、3匹のAR-24Qおよび5匹のAR-97Qマウスを得た。これらF0マウスをC57BL/6J(日本エスエルシー、静岡、日本)に交配することで維持を行った。導入遺伝子の存在はマウス尾組織から抽出したDNAをPCRにかけてスクリーニングした。この際用いたプライマーは5'-CTTCTGGCGTGTGACCGGCG-3'(配列番号3)および5'-TGAGCTTGGCTGAATCTTCC-3'(配列番号4)であり、CAG繰り返し数の確認には5'-CCAGAGCGTGCGCGAAGTG-3'(配列番号5)と5'-TGTGAAGGTTGCTGTTCCTC-3'(配列番号6)のプライマーを用いた。それぞれの系における導入遺伝子のコピー数は、制限酵素SacIIを用いたサザンブロットにより輝度を解析することで算定した。CAGリピート数の決定にあたっては、テキサスレッド標識プライマーで増幅したPCR産物を、6%ポリアクリルアミドゲルで電気泳動し、5500DNAシークエンサー(日立、日本)で解析した。
マウスのrotarod taskにはEconomex Rotarod (Colombus Instruments、アメリカ)を用い、毎週12時間ごとの明暗サイクルの明時に、以前の報告と同様に行った (Adachi et al., 2001)。試行は3回行い、各々のマウスが回転バーにもっとも長く乗ることのできた時間を記録した。マウスがバーから落下した時ないし180秒に達した時にタイマーを停止し、その時間を記録した。
マウスの1日の行動量であるcage activityは、マウスを透明なケージ(16 x 30 x 14 cm) に入れ、AB system (ニューロサイエンス、日本)を用い、赤外線センサーを使って測定した。測定は10分間隔で24時間行った。
雄AR-97Qマウスと正常雄マウスの去勢およびsham operationは、4週齢のマウスに対し、ケタミン−キシラジン麻酔下(50 mg/kgケタミンおよび10 mg/kgキシラジンを腹腔内注射)で、経腹腔的アプローチで行った。 雌AR-97Qマウスと雌正常マウスには、20μgのエナン酸テストステロンを、20μlのsesame oilに溶解し皮下注射した。投与は4週齢から開始し、解析終了まで毎週施行した。対照群には、同量のsesame oilのみを投与した。
血清テストステロン測定には、Coat-A-Count Total Testosterone radioimmunoassay (Diagnostic Products Corporation、アメリカ)を用いた。
ケタミン-キシラジン麻酔下でマウスより臓器を摘出し、ドライアイスアセトンにて凍結した。凍結組織からTrizol (Life Technologies/Gibco BRL、アメリカ)を用いtotal RNAを抽出した。このRNAにSUPERSCRIPT II逆転写酵素 (Life Technologies/Gibco BRL)処理を行った。導入遺伝子に特異的なプライマーである5'-TTCCACACCCAGTGAAGC-3'(配列番号7)および5'-GGCATTGGCCACACCAAGCC-3'(配列番号8)を用いてRT-PCRを行い、アガロースゲル電気泳動を行った。バンドの輝度を、別の反応系で増幅したβアクチンのmRNAレベルと比較した。
蛋白の抽出にあたっては、凍結組織(湿重量0.1 g)を1000μlの50 mM Tris pH 8.0、150 mM NaCl、1% NP-40、0.5% deoxycholate、0.1% SDSおよび1 mM 2-mercaptoethanolの溶液で1 mM PMSFとaprotinine 6μg/mlとともにhomogenateし、4℃で2500g、15分間遠心した。神経組織では200μg 、筋は80μgのサンプルを、5-20% SDS-PAGEゲルで泳動し、25 mM Tris、192 mM glycine、0.1% SDSおよび10% methanol溶液で、Hybond-P(Amersham Pharmacia Biotechイギリス)にtransferした。1次抗体であるラビット抗AR抗体N-20(1:1000) (Santa Cruz Biotechnology、アメリカ)に反応させ、2次光体に反応させた後、ECL+plus kit (Amersham Pharmacia Biotech)にて発色、現像した。核および細胞質分画はNE-PER nuclear and cytoplasmic extraction reagents (Pierceアリカ)を用い分離、抽出した。
ケタミン−キシラジン麻酔下のマウス左心室から、20 mlの4% paraformaldehyde-リン酸バッファー(pH 7.4)を環流し、摘出した臓器を10%ホルマリン-リン酸バッファーで固定し、その後パラフィン包埋した。4-μm厚の切片を切り出し、脱パラフィン処理し、アルコールにて脱水後、ギ酸にて室温5分間処理した。1次抗体は1C2 (1:10000) (Chemicon、アメリカ)を用い、以前の報告 (Adachi et al., 2001) と同様に染色した。即ちギ酸にて処理後、5%ウマ血清を室温にて30分間反応させ、1次抗体1C2(1:10000)(Chemicon、アメリカ)を4℃、オーバーナイトで反応させたのち、ABC法にて免疫染色を行った。ABC法による染色はビオチン化2次抗体(Vector Laboratories、アメリカ)を4℃、オーバーナイトで反応させ、0.02M PBSで洗浄後、HRP標識ストレプトアビジンで反応させ、DABを用いて発色した。
電子顕微鏡標本もパラフィン固定のものを使用し、1C2 (1:10000) (Chemicon)で同様に(Adachi et al., 2001)染色した。
マウス腓腹筋の6μm厚凍結切片を作製し、ヘマトキシリン・エオジン(H&E)染色を行った。脊髄運動ニューロンの数および形態の解析には、各々の群のマウス3匹ずつから、5μm厚の連続20切片を第5腰髄(#7-8、13週齢)から切り出した。1つおきの切片をNissl染色し、前角にある核小体の明瞭なニューロンを、Luzex FS image analyzer (Nireco、日本)を用い、以前の報告 (Terao et al., 1996) の通り解析した。即ち、計10切片における全ニューロンの断面積を測定した。前根有髄線維の直径はトルイジンブルー染色標本を用い、以前の報告 (Terao et al., 1996) の通り解析した。即ち、各線維の断面積を測定し、それと同じ面積の円の直径を計算した。
統計処理にはunpaired t-testを用い、p値0.05未満を有意とした。
サイトメガロウイルスエンハンサーおよびチキンβ-アクチンプロモーターの調節下に、繰返し数が24又は97のCAGリピート(24CAGリピート、97CAGリピート)を有するヒトアンドロゲン受容体(AR)を発現するTgマウスの作製を試み(図1A)、24CAGリピートを使用したマウス(AR-24Q)3系統と、97CAGリピートを使用したマウス(AR-97Q)5系統とを得た。導入遺伝子のコピー数はAR-24Qでは1〜5、AR-97Qでは1〜3であった(図8)。導入したヒトAR遺伝子におけるCAGの繰り返し数は、PCRおよびポリアクリルアミドゲル電気泳動により確認した。世代間のCAG繰り返し数の変動は明らかでなかった。AR-24Qのマウスはいずれも無症状であったが、AR-97Qの5系統の内3系統では(#2-6、#4-6、#7-8)、進行性の運動障害が認められた。運動障害がみられた3系統のマウスはいずれも体格の縮小、寿命短縮、進行性筋萎縮、筋力低下および活動低下を示した。これらすべての症状は雄のAR-97Qマウスにおいて重篤かつ急速に進行したが、雌では症状が認められないか、あっても雄より遥かに軽症であった(図1B、C、D、E)。初発症状は体幹および後脚の筋萎縮であり、それに続いて体重減少やrotarodによる運動機能低下が明らかとなった。rotarodにおける運動障害が発症するのは雄では8から9週齢であったが、雌では15週齢ないしそれ以降であった(図8)。運動障害のあるマウスは活動性が低下し、後脚を引きずって歩行した。前脚の障害は後脚の萎縮が高度となった後に認められた。雄では雌にくらべ運動機能は急速かつより早期に障害され、寿命もより短縮していた。AR-97Qの全3系統のマウスにおいて、寿命の中央値は雄では66から135日齢であったが、雌では210日齢においても10から30%程度のマウスが死亡したにすぎなかった。死因は低栄養および脱水による悪液質と考えられた。
症状や病理所見の性差が、男性ホルモンであるテストステロン分泌の性差によるものと仮説をたて、まず雄のAR-97Qマウスに去勢を施行した。去勢された雄AR-97Qマウスでは症状、病理所見および変異蛋白の核内局在について、対照群(sham operation群)に比べ劇的な改善が認められた。すなわち、対照群では著しい体重減少を認めたが、去勢されたAR-97Qマウスの体重は去勢された正常マウスと同等で、体重減少はほとんど認められなかった(図3A)。rotarodやcage activityによる運動機能評価では、去勢マウスにおける運動機能低下は対照群に比べはるかに軽度であり、運動機能障害は去勢マウスではほとんどみられなかった(図3B、C)。去勢マウスにおける運動機能は雌のTgマウスとほぼ同様であった。去勢マウスでは寿命も著しく延長し(図3D)、筋萎縮やそれに伴う体格の縮小も著明に改善した(図3E)。マウスの足跡を観察し歩行機能を評価したところ、対照群は後脚をひきずって歩くのに対し、去勢マウスでは正常な歩行がみられた(図3F)。N-20によるウエスタンブロットでは、スタッキングゲルに留まる変異AR蛋白は、対照群に比べ去勢マウスにおいて著明に減少し(図4A)、核分画における変異ARもまた、去勢マウスにおいて著明に減少した(図4B)。1C2免疫染色における核のびまん性染色や核内封入体は、去勢により劇的に改善した(図4C)。これらの結果は、去勢により変異ARの核内移行を抑制したことを意味する。血清テストステロン値は去勢マウスでは測定感度以下であったが、対照群では27.7±1.2 ng/dl(#7-8, n=4)であり、去勢によりテストステロンが著明に低下することが示された。
次に、テストステロン分泌抑制による本マウスの治療を薬剤で再現する目的で、LHRHアナログであるLeuprorelinを雄AR-97Qマウスに投与する実験を行った。Leuprorelinの投与は次のように行った。Leuprorelin acetate 100μgをD-マンニトールを含む溶液に懸濁し、2週間ごとに皮下注射した。投与は5週齢から開始し、解析終了まで行った。対照群には懸濁液のみを投与した。Leuprorelinを投与した雄AR-97Qマウスでは、去勢マウスと同様に、症状の著明な改善が認められた。すなわち、rotarodやcage activityによる運動機能評価では、Leuprorelin投与群における運動機能低下は対照群に比べはるかに軽度であり、運動機能障害はLeuprorelin投与群ではほとんどみられなかった(図5A、B、C、D)。Leuprorelin投与群では体格の縮小も著しく改善し(図5E)、歩行障害も改善した(図5F)。Leuprorelin投与群の血清テストステロン値は測定感度以下であった(#4-6, n=4)。
次に雄AR-97Qマウスにおける去勢の治療的効果が、テストステロンの分泌抑制であることを明らかにするため、元来症状の乏しい雌AR-97Q マウスにテストステロンを投与する実験を行った。テストステロンを投与した雌AR-97Qマウスでは症状、病理所見および変異蛋白の核内局在が、対照群(sesame oil投与群)に比べ劇的に悪化した。すなわち、対照群では体重減少はほとんど認めなかったが、テストステロン投与群の体重は著明に減少した(図6A)。rotarodやcage activityによる運動機能評価では、テストステロン投与群における運動機能低下は対照群に比べ著しく高度であった(図6B、C)。テストステロン投与群における運動機能は雄のAR-97Qマウスとほぼ同様であった。テストステロン投与群では寿命も著しく短縮し(図6D)、筋萎縮やそれに伴う体格の縮小も著明に増悪した(図6E)。足跡の観察では、対照群はほぼ正常であるのに対し、テストステロン投与群では後脚をひきずる歩行障害が明らかであった(図6F)。N-20によるウエスタンブロットでは、スタッキングゲルに留まる変異AR蛋白は、対照群に比べテストステロン投与群において著明に増加し(図7A)、核分画における変異ARもまた同様であった(図7B)。対照群ではほとんど認められなかった核のびまん性染色や核内封入体は、テストステロン投与により著明に増加した(図7C)。これらの結果は、テストステロン投与により変異ARの核内移行が促進したことを意味する。血清テストステロン値は対照群では測定感度以下であったが、テストステロン投与群では158.0±70.7 ng/dl (#2-6, n=3) ないし305.3±182.3 ng/dl (#7-8, n=4) であった。
SBMAと異なり他のポリグルタミン病では原因蛋白の特異的リガンドは発見されていないが、上記の実施例におけるTgマウスの去勢による劇的な治療効果は、変異蛋白の核内移行を抑制することでポリグルタミン病が治療され得ることを示唆している。

(4)抗アンドロゲン受容体抗体を用いた免疫染色によって核のびまん性染色及び核内封入体が認められる、
(5)神経原性変化が認められる、
(6)進行性運動障害が認められる、
(7)体格の縮小が認められる、
(8)寿命短縮が認められる、及び
(9)活動低下が認められる。
12.約8〜9周齢で運動障害が発症する、11.に記載の非ヒト動物。
13.雌である場合は前記(1)〜(9)が認められないか、又は雄である場合に比較して軽症ないし軽度である、11.又は12.記載の非ヒト動物。
14.前記非ヒト動物が齧歯目動物である、ことを特徴とする11.〜13.ずれか記載の非ヒト動物。
15.前記非ヒト動物がマウスである、ことを特徴とする11.〜13.いずれか記載の非ヒト動物。
(i)テストステロンの分泌を抑制する作用を有する化合物を有効成分として含む薬剤を生体に投与するステップ。
22.前記化合物が、下垂体からの性腺刺激ホルモンの分泌を抑制する作用を有する、21.に記載の治療方法。
23.前記化合物が、下垂体に作用して黄体形成ホルモン放出ホルモン受容体を減少させる作用を有する、21.に記載の治療方法。
24.前記化合物が黄体形成ホルモン放出ホルモンのアナログである、21.に記載の治療方法。
25.前記化合物がリュープロレリン又はその誘導体である、21.に記載の治療方法。
32.球脊髄性筋萎縮症治療剤を製造するための、下垂体からの性腺刺激ホルモンの分泌を抑制する作用を有する化合物の使用。
33.球脊髄性筋萎縮症治療剤を製造するための、下垂体に作用して黄体形成ホルモン放出ホルモン受容体を減少させる作用を有する化合物の使用。
34.球脊髄性筋萎縮症治療剤を製造するための、黄体形成ホルモン放出ホルモンのアナログの使用。
35.球脊髄性筋萎縮症治療剤を製造するための、リュープロレリン又はその誘導体の使用。
一方、本発明の治療薬は球脊髄性筋萎縮症の発症メカニズムに基づいて構成されたものであり、かつ球脊髄性筋萎縮症の病態を忠実に再現するモデル動物においてその効果が実証されたものであるから、球脊髄性筋萎縮症の治療薬として極めて有効であると考えられる。
Claims (3)
- 黄体形成ホルモン放出ホルモンのアナログであって、テストステロンの分泌を抑制する作用を有する化合物を有効成分として含んでなる球脊髄性筋萎縮症治療剤。
- リュープロレリンを有効成分として含んでなる球脊髄性筋萎縮症治療剤。
- ゴセレリン、ブセレリン、又はナファレリンを有効成分として含んでなる球脊髄性筋萎縮症治療剤。
