【発明の属する技術分野】
本発明は、例えば、内視鏡下で腹腔内の診断や手術を行う際に、その腹腔を膨らませるために使用する気腹針に係り、特に外套管付き気腹針に関する。

【従来の技術】
一般に、気腹針は、診断や処置を行う患者の腹腔内にガスを送り込んで、その腹腔を膨らませる目的で、患者の腹部に刺通されるものである。特に、外套管付き気腹針については、独公開特許第 28 35 812号公報において、開示されている。

前記先行例の外套管付き気腹針は、 気腹針に外套管を被覆させたものである。術者は、まず外套管を被覆させた状態の気腹針基端部に気腹装置からの送気チューブを接続し、これを患者の腹部に刺通する。そして、気腹装置からガスを送り込み、腹腔内に診断や処置に必要な空間を確保した後、気腹針のコックを閉じる。次に、気腹針から送気チューブをはずし、外套管側の送気口へ接続し、外套管側のコックを開け、再びガスを送りながら、外套管から気腹針のみを抜き取り、この気腹針の代わりに、外套管内に内視鏡を挿入するというものである。

【発明が解決しようとする課題】
前述した独公開特許第 28 35 812号公報に記載された外套管付き気腹針は、緊急時に、気腹してから内視鏡を挿入するために、新たなトラカールを刺通するという手間を省略するというものである。したがって、前述したように、気腹した後、気腹針のコックを閉じて送気チューブの先端を気腹針から、外套管へ接続し直し、再びコックの操作を行うという手順は、一刻を争う術者にとっては、非常に煩わしい操作であり、その結果、操作手順を間違えたり、充分に気腹ができなくなる虞があった。

本発明は、以上の問題に着目してなされたもので、送気チューブを気腹針から外套管へ接続し直す必要のない外套管付き気腹針を提供することを目的とする。

【課題を解決するための手段】
本発明は、内視鏡と気腹針とを交換自在に挿入可能な外套管を有した外套管付き気腹針であって、前記気腹針が挿入された際には、気腹ガスが漏れない程度に前記気腹針の外表面と勘合し、前記内視鏡が挿入された際には、前記内視鏡外表面との間に気腹ガスが通過できる程度の隙間を生じる内径部と、前記内視鏡が挿入された際に気腹ガスが漏れることを防止するシール部材を備えた外套管本体と、を有する前記外套管と、前記外套管本体に設けられ、前記外套管本体の前記内径部と連通する通路孔を有する送気口金と、前記気腹針に設けられ、前記送気口金の前記通路孔と連通可能なガス通路と、を具備し、前記外套管に対して前記気腹針と前記内視鏡とを入れ替える際に送気チューブを接続しなおす手間を省略したことを特徴とする外套管付き気腹針である。

【発明の実施の形態】
<第1実施形態>
以下に、図を参照して本発明の実施の形態について説明する。

(構成)
図1は本発明の第1実施形態に係り、第1実施形態の外套管付き気腹針の縦断面を示す説明図である。

図1に示すように第1実施形態の外套管付き気腹針は、外套管1と気腹針2から構成される。外套管1の内部には、気腹針2または図示しない内視鏡が挿入される。そして、外套管1に気腹針2を装着した状態で、患者の腹腔へ気腹針2と共に、外套管1の挿入パイプ3を刺通し、その後腹腔内に気腹ガスを流入する。腹腔内の観察を行う際は、外套管1のみを腹腔に刺入したままの状態で残し、気腹針2を引き抜き、その外套管1内に気腹針2の代わりに内視鏡を挿入することで、腹腔内の観察を行うことができる。

前記外套管1の構成を具体的に説明する。外套管1は、金属材料または樹脂材料からなる挿入パイプ3の基端部に、外套管本体4を固着したものである。この挿入パイプ3と外套管本体4は各々筒状で、その内径は略同一となるように設けてある。

外套管本体4の側方には、気腹ガスを送気するための送気口金部5が突出して設けられており、この送気口金部5の内部を連通してガスの通路孔6が設けてあり、その先端は、外套管本体4の内径部に連通している。

また、外套管本体4の手元側端部には、内径部に向かって凹状のルアーテーパー状の接続口金部7が設けてあり、そのテーパ部の一部に気腹針2の長軸周りの回転を規制する回転規制溝8が、長軸方向に設けてある。

さらに、外套管本体4の基端部外側には、気腹針2を接続固定する螺子管16と係合するための係合凸部9が設けてある。

一方、気腹針2は、筒状の穿刺針10と、この穿刺針10に挿入される送気パイプ11とからなる。穿刺針10は、気腹針2を体腔へ穿刺するためのものであり、その外径が挿入パイプ3および外套管本体4の内径に、挿脱可能で、かつガスが漏れない程度に嵌合するように設定されている。そして、穿刺針10の先端部は、穿刺し易いように鋭角な斜面となっており、基端部の外周面には、送気パイプ11の回転を規制する細長状の溝である位置決めスリット12が設けてある。さらに、穿刺針10の基端部は、気腹針本体13の先端部と固着されている。

気腹針本体13の先端部は、外套管1の凹状ルアーテーパ部の接続口金部7と当接する凸状のルアーテーパ部を有する接続部14を有し、その接続部14先端には、外套管本体4の回転規制溝8と係合する突起15が設けてある。気腹針2を外套管1に装着すると、回転規制溝8と突起15が係合して、回転の規制を行う。さらに、気腹針本体13の太径部には、外套管1と気腹針2を確実に固定するための螺子管16が、気腹針本体13に対して軸方向には移動できないが、軸周りには回転可能なように取り付けられている。

螺子管16の先端側内側には、外套管本体4の係合凸部9と係合するための螺旋溝が形成されており、螺子管16を回転させることにより、外套管本体4の接続口金部7と気腹針本体13の接続部14は密着されて、ガスが通過しないように確実に固定される。

送気パイプ11は、体腔内へ充分な分量のガスを送ことができる管路で、その外径は穿刺針10の内径と、長軸方向に摺動可能で、且つガスが漏れない程度に嵌合するように設定されている。先端部には、体組織を損傷することがないように、先端が鈍に丸められたチップ17が固着されている。また、送気パイプ11の基端側には、送気パイプ11内のガスが外部に漏れるのを防止する栓18が固着されている。

栓18の基端側には、送気パイプ11の外径より部分的に突出した突起19が設けてあり、この突起19は、穿刺針10の位置決めスリット12のスリット幅と係合して、軸周りには回転規制されるが、スリットの長さ方向の範囲内で移動が可能である。

穿刺針10の内部の、送気パイプ11と気腹針本体13の間には、バネ20が間挿され、位置決めスリット12の先端部へ突起19が当接するように付勢している。

(作用)
先ず、外套管1に気腹針2を、回転規制溝8と突起15の位置を合わせながら挿入し、螺子管16を回転させて、外套管本体4と気腹針本体13をガスが漏れないように確実に固定する。(図1)
次に、送気口金部5に、気腹ガスを送る図示しないチューブを接続する。

第1実施形態の外套管付き気腹針を体腔内に穿刺する際は、先ず、送気パイプ11の先端のチップ17が体表面に接触する。さらに、バネ20の弾性力に抗して体表面に押しつけると、バネ20は変形して、送気パイプ11は穿刺針10内に押し込まれ、穿刺針10の先端鋭利部が体表面に接触する。さらに、外套管付き気腹針を体表面に押し込むと、穿刺針10先端部と共に、外套管1の挿入パイプ3先端が体腔内に刺通する。このとき、チップ17先端は、開放されるため、バネ20の付勢力により穿刺針10先端よりも先端側に飛び出し、体内組織を穿刺針10の先端鋭利部から保護する。

この状態で、気腹ガスを送ると、ガスは、送気口金部5の通路孔6、通気孔22、23、送気パイプ11の内部空間、及び通気孔21を順次通過して、体腔内に送られる。

次に、螺子管16を逆回転させて、外套管1を体壁に残したまま、気腹針2のみを抜去する。このとき、必要に応じて外套管本体4の基端部開口を指等で塞ぎ、ガス漏れを防ぐ。そして、外套管1の内径部に、図示しない内視鏡を挿入して、観察、処置等を行う。観察、処置等の途中で、気腹ガスが不足してきた場合は、さらに、通路孔6からガスを送り込むと、挿入パイプ3の内径と図示しない内視鏡の挿入部外径との隙間を通して、体腔内にガスが送り込まれる。
なお、外套管本体4の基端部には、図示しない周知のOリング等装着して、外套管本体4の内径部と内視鏡の外径部の間をシールして、内視鏡装着後のガス漏れを防止している。

内視鏡による観察、処置等が終了した場合は、内視鏡と共に外套管1も抜去する。

螺子管16を回転して外套管1に気腹針2を取り付ける際に、気腹針本体13を押さえなくても、回転規制溝8と突起15が係合しているために回転しない。

また、チップ17が穿刺針10の先端をカバーしてしているので、穿刺するとき以外は、穿刺針10先端が露出することがないので、安全である。
さらに、穿刺の過程で、まだ穿刺針10が体壁の途中にあるときは、送気パイプ11は穿刺針10の内部に収まっているので、通気孔21は穿刺針10の内部で、かつ通気孔23が、通気孔22の基端部側に移動する。したがって、ガスを通路孔6から送気しても、ガスは遮断されて穿刺針10の先端まで送られることはないので、体壁中にガスが溜まって、気腫ができるという事故を防止できる。

<第2実施形態>
(構成)
図2は本発明の第2実施形態に係り、第2実施形態の外套管付き気腹針の主要部の縦断面を示す説明図である。

第2実施形態は、外套管本体4の回転規制溝8と気腹針本体13の突起15の回転止め機構、及び栓18の突起19と穿刺針10の位置決めスリット12の回転止め機構とを不要とするため、外套管本体4の内部と通路孔6との間にガス周り部24を設け、通気孔22、23の数を複数にした構成である点を除くと、第1実施形態と同じである。第1実施形態と同様の構成及び作用については、同じ符号を付して説明を省略する。

外套管1に気腹針を取り付けた後、送気パイプ32のチップ33の先端部を体表面に軽く接触させる。次に、外套管1の穿刺針10を、板バネ34の付勢力に抗して、チップ33の偏心部が穿刺針10の先端鋭利部から離れる方向Bに移動させた後、バネ20の弾性力に抗して体表面に押しつけると、バネ20は変形して、穿刺針10が体表面に接触する。さらに、押し込むと、穿刺針10は体壁35に突き刺さる。(図6(b)) さらに押し込むと穿刺針10は、体壁35を刺通する。このとき、チップ33先端は、開放されるため、バネ20の付勢力により穿刺針10よりも先端側に飛び出すと共に、板バネ34の付勢力により、チップ33の偏心部が、穿刺針10の鋭利部の先端部を覆う位置に移動して、静止する。(図6(a))
