Description

【発明の詳細な説明】 〔産業上の利用分野〕 本発明は、人工股関節を構成するための人工寛骨臼に
関するものである。 〔従来の技術〕 交通事故などによる外傷や慢性関節リウマチ、変形性
関節症、腫瘍などの骨の変性に基づく関節部の病変によ
り機能障害を起こし、疼痛が生じ、保存療法では、治
癒、回復の見込が薄いような場合、関節部分を切除し、
人工関節に補綴する人工股関節全置換形成術が行われて
いる。 この際に用いられる寛骨臼の形成術は、第3図に示す
ように骨頭球Kを可動自在に受容するポリエチレン製の
ソケットPを寛骨Bに重合有機性の骨セメントFで固定
する方法や特公昭61−28570のような寛骨臼に圧入し、
骨の成長による自然固定が行われていた。 〔従来技術及びその問題点〕 上記の如く、有機性の骨セメントにより、固定を行う
場合には、重合により骨セメントが硬化する際に熱が発
生し生体に悪影響を及ぼしたり、骨セメントと海綿状骨
との間に緩みが発生し、そのため再度の手術により再置
換することが必要となることもある。再置換の場合は、
前の骨セメントを除去することが難しく再手術に時間を
要することが多い。 また、骨セメントを用いないで固定する場合には初期
固定が充分でなく、骨が成長し自然固定されるまで患者
が離床し活動するのに時間を要するなどの不都合があっ
た。 〔問題点を解決するための手段〕 上記の如き問題点を解消するため、本発明ではセメン
トを使用せず、寛骨に補綴装置を挿入後、臼蓋ソケット
に設けたスリット入り円筒状の突起を全周方向に拡張せ
しめ、寛骨側に設けた突起用穴の壁面に圧接させること
により初期固定を行うことを特徴とする。 〔実施例〕 第1図には本発明実施例に係る人工寛骨臼1を示し、
この人工寛骨臼1はほぼカップ状の半球体2を成し、不

るように構成され、ネジ5を螺入することによってテー
パコーン4が押し込まれ、固定突起3を図中点線で示す
ように拡大するようになっている。 なお、固定突起3を拡大させるための機構としては第
2図に示すように内部方向のテーパ面3a′をもった孔3c
を固定突起3内にを形成し、孔3c中にテーパコーン4を
介装し、ネジ5でもって引き寄せて拡大せしめるように
したものであってもよい。 また、上記実施例では固定突起3が半球体2の頂部に
一個形成したものをあげたが、それに限らず複数個形成
したものであってもよく、一方、半球体2の表面すなわ
ち、寛骨と当接する部位には溝2や多数の小突起あるい
は小孔を多数形成するなど、骨が成長したとき強固かつ
安定的に寛骨に固定できるようにしてある。 このように構成された人工寛骨臼は第3図に示したと
同様に寛骨Kに装着されるが、この場合、半球体2に形
成した固定突起3を挿入する穴をも寛骨にあけておきネ
ジ5を螺入し固定突起3を拡大して寛骨に対し、半球体
2を機械的に固定して後、半球体2の内側に摺動部材S
を嵌入して股関節を構成する人工寛骨臼を装着する手術
を完了することができる。 〔発明の効果〕 叙上のような本発明によれば、人工股関節全置換形成
術の際に、有害な重合有機性の骨セメントを使用せず、
寛骨臼に設置することが出来る。 また固定突起の外周面が突起用穴の壁面にほぼ全面的
に圧接して応力が分散されるので、骨量の多い部位を選
択して固定突起を配置すれば骨に損傷を与えたり、緩み
が生じたりする恐れがなく、さらに、上記固定突起は、
例えばテーパーコーンの上げ下げにより外周面の開き具
合を調整して前記穴壁面に対する押圧力を調整できるの
で、人工寛骨臼設置の際に押圧力が過大にならないよう
にできるなど、固定力と信頼性を兼ね備えたものであ
