第一身体運動に関係する信号、第一心臓活動に関係する信号、応答性信号、認識信号、第二心臓活動に関係する信号、第二身体運動に関係する信号、第二心臓活動に関係するスペクトル解析信号、及び第二身体運動に関係するスペクトル解析信号からなる群から選択される少なくとも2つの身体信号を受信すること、
全身性強直間代性てんかん発作の発生を判定することであって、判定は少なくとも2つの特徴の相関に基づいており、少なくとも1つの特徴は少なくとも2つの身体信号の各々であり、
第一心臓活動信号の特徴が、発作間基準値を上回る患者の心拍数の増加であり、

応答性信号の特徴が、発作間基準値を下回る患者の応答性の減少であり、
認識信号の特徴が、発作間基準値を下回る患者の認識の減少であり、
第二心臓活動信号の特徴が、発作心臓活動基準信号との相関であり、
第二身体運動信号の特徴が、発作身体運動基準信号との相関であり、
第二心臓活動のスペクトル解析信号の特徴が、発作心臓活動スペクトル解析基準信号との相関であり、又は、
第二身体運動のスペクトル解析信号の特徴が、発作身体運動スペクトル解析基準信号との相関であること、及び
てんかん事象の発生の判定に応じて更なる行動を行うこと
を含む方法に関係する。
第二身体運動信号の、発作身体運動基準信号との相関は、発作身体運動テンプレートに対する一致を含み、
第二心臓活動のスペクトル解析信号の、発作心臓活動スペクトル解析基準信号との相関は、発作心臓活動スペクトル解析パターン又はテンプレートに対する一致を含み、又は
第二身体運動のスペクトル解析信号の、発作身体運動スペクトル解析基準信号との相関は、発作身体運動スペクトル解析パターン又はテンプレートに対する一致を含む。信号及びその特徴の側面は、とりわけ強直又は間代性活動の指標に先行する転倒の指標を更に含む身体運動信号を含むことができる。

加速度計信号が、その患者の発作間期間を示す運動間の間隔又は軌道を含む運動パターンであり、

第二身体運動信号の、発作身体運動基準信号との相関は、発作身体運動テンプレートに対する一致を含み、
第二心臓活動のスペクトル解析信号の、発作心臓活動スペクトル解析基準信号との相関は、発作心臓活動スペクトル解析パターン又はテンプレートに対する一致を含み、又は、

(i)認識信号の特徴の少なくとも1つが、基準値を下回る患者の認識減少であるならば、複雑、又は、
(ii)基準値を下回る患者の認識減少がなければ、又は患者の応答性減少があるが、認識が発作間値に留まっているならば、単純。
心臓信号の発作後特徴は、発作状態に関連した値の範囲外の心拍数であり、
身体運動信号の発作後特徴は、発作値範囲外の患者の運動の変化であり、
応答性信号の発作後特徴は、発作基準値を上回る患者の応答性の増加であるが、発作間基準値を下回るままであり、かつ、
認識信号の発作後特徴は、発作基準値を上回る患者の認識の増加であるが、発作間基準値を下回るままである。
心臓信号は、前記患者に関する発作基準値範囲外、かつ発作後状態を示す範囲内の心拍数であり、
加速度計信号は、前記患者に関する発作基準値範囲外、かつ発作後状態を示す範囲内の運動速度、振幅又は単位時間当たりの運動数であり、
加速度計信号は、前記患者に関する発作基準値範囲外、かつ発作後状態を示す範囲内の運動パターン、軌道、又は運動間の間隔であり、
呼吸信号は、その患者に関する発作値範囲外、かつ前記患者に関する発作後範囲内の呼吸速度であり、
応答性信号は、発作範囲及び発作間範囲の両方の外の値への患者の無認識又は認知機能障害の変化であり、そして、

患者からの第一心臓活動に関係する信号、患者からの第一身体運動に関係する信号、患者からの応答性信号、患者からの認識信号、患者の第二心臓活動に関係する信号、及び患者の第二身体運動に関係する信号の少なくとも1つを受信するように設定された、少なくとも1つのセンサと、
少なくとも1つのセンサからの少なくとも1つの信号を受信し、てんかん事象の発生を判定するように設定された検出ユニットと、

前記心臓信号及び前記動力学的信号に基づき、てんかん発作を検出するための検出モジュールと、
を含む医療装置システム。 - 患者の身体信号の2つ以上に基づきてんかん発作を検出するための医療装置であり、
前記患者の心拍信号を検知するための心臓センサと、
前記患者の身体運動信号を検知するための動作センサと、
前記患者の応答性を示す応答性信号を提供する応答性判定ユニットと、
前記患者の認識を示す認識信号を提供する認識判定ユニットと、
実行される時に、てんかん発作を検出するための方法を行うことが可能であるアルゴリズムを有するプログラム記憶装置を含む検出モジュールとを含み、
前記アルゴリズムが、
前記患者の前記心拍信号から心臓特徴を判定し、該心臓特徴が、発作間基準値範囲外の心拍数測定、及び心拍数比基準値を超える、心拍数の短期測定の、心拍数の長期測定に対する比から選択されること、
前記身体運動信号から身体運動特徴を判定すること、
前記心臓特徴と時間的に相関する基準値を下回る、前記患者の前記応答性の減少があるならば、発作を検出すること、
前記身体運動特徴が、1)前記発作間基準値外の姿勢筋緊張の喪失、2)前記発作間基準値を上回る姿勢筋緊張の増加、又は3)前記心臓特徴と相関する転倒を示すならば、前記発作を全身性強直間代性発作として分類すること、
前記患者の前記応答性の減少が、1)前記発作間基準値外の姿勢筋緊張の喪失、2)前記発作間基準値を上回る姿勢筋緊張の増加、又は3)前記転倒を示す前記身体運動特徴に関連しないならば、前記発作を部分発作として分類すること、
前記応答性の減少が、前記発作間基準値を下回る前記認識の減少に関連しないならば、前記発作を単純部分発作として分類すること、及び、
前記応答性の減少が、前記発作間基準値を下回る前記認識の減少に関連するならば、前記発作を複雑部分発作として分類すること
の段階を含む、
医療装置。 - てんかん発作を検出するための医療装置システムであり、
患者の心臓信号を検知するための心臓センサ、及び前記患者の身体運動を検知するための動力学的センサからなる群から選択される少なくとも1つのセンサと、
前記患者の応答性を判定するための応答性検査を行う応答性検査モジュール、及び前記患者の認識を判定するための認識検査を行う認識検査モジュールの少なくとも1つと、
前記てんかん発作を検出するアルゴリズムを有するプログラム記憶装置を含む検出モジュールとを含み、
前記アルゴリズムが、実行される時に、
前記心臓信号及び前記身体運動信号の少なくとも1つを受信すること、
前記少なくとも1つの信号から、第一心臓指数及び第一身体運動指数の少なくとも1つを判定すること、
前記少なくとも1つの指数を少なくとも第一基準値と比較すること、
前記第一基準値を超える前記少なくとも1つの指数に応じて少なくとも1つの検査を誘発し、前記少なくとも1つの誘発された検査が、前記応答性検査及び前記認識検査からなる群から選択されること、
前記1つ以上の誘発された検査の結果に少なくとも部分的に基づき前記てんかん発作の発生を検出すること、及び、
前記検出に応じて更なる行動を行うこと
の段階を含み、
前記更なる行動が、
発作の発生をロギングすること、
前記発作の発現時間をロギングすること、
前記患者、介護士、又は医療介護提供者の少なくとも1人に警告を提供すること、
前記発作を治療するために、治療を提供すること、
前記検出後、前記認識又は前記応答性のような1つ以上の患者パラメータを評価すること、
前記発作の発作重症度指数を判定すること、
前記発作の終了を特定すること、
前記発作の終了時間をロギングすること、及び、
前記患者の発作後障害又は前記発作からの回復を評価すること
の1つ以上を含む、
医療装置システム。 - 前記第一心臓指数及び前記第一身体運動指数の少なくとも1つの前記判定が、
短期心拍数測定、長期心拍数測定、前記短期心拍数測定及び前記長期心拍数測定の比、短期心拍数変動測定、長期心拍数変動測定、及び心拍形態指数から選択される、前記第一心臓指数、及び、
強直性運動の測定、間代性運動の測定、身体運動の方向、前記身体運動の速度、前記身体運動の加速度、前記身体運動の頻度、前記身体運動の力、身体配向測定、身体姿勢指数、筋緊張指数、前記身体運動の配向、前記身体運動の非てんかん指数、転倒、前記転倒の速度、及び前記転倒に続く衝撃力から選択される、前記第一身体運動指数
の少なくとも1つの判定を含み、
少なくとも1つの前記応答性検査の誘発が、
第二心臓指数を判定すること、及び前記第二心臓指数を第二心臓指数基準値と比較することを含む、心臓検査、及び、
第二身体運動指数を判定し、及び前記第二身体運動指数を、第二身体運動指数基準値と比較することを含む、身体運動検査、
の少なくとも1つを更に含む、
請求項22の医療装置システム。 - 前記てんかん発作の前記発生の検出が、
前記患者の前記応答性の測定が、少なくとも所定の差だけ基準応答性レベルと異なると判定すること、及び
前記患者の前記認識の測定が、少なくとも所定の差だけ基準認識レベルと異なると判定すること
の少なくとも1つに少なくとも部分的に基づく、請求項22の医療装置システム。 - 前記第一心臓指数及び前記第一身体運動指数の少なくとも1つの判定が、前記第一心臓指数及び前記第一身体運動指数の両方を判定することを含み、前記誘発が、前記患者の前記第一心臓指数及び前記患者の第一身体運動指数の間の低い相関の判定に少なくとも部分的に基づく、請求項22の医療装置システム。
- 前記第一心臓指数、前記第一身体運動指数、前記応答性、前記認識検査の結果、前記応答性検査の結果の少なくとも1つに基づいて、前記てんかん事象を分類することを更に含む、請求項22の医療装置システム。
- 前記第一心臓指数、前記第一身体運動指数、前記応答性、前記認識検査の結果、前記応答性検査の結果、前記心臓検査の結果、及び前記身体運動検査の結果の少なくとも1つに基づいて、前記てんかん事象を分類することを更に含む、請求項23の医療装置システム。
- 第一の非横臥位の患者において、
a)前記身体運動が、第一の非横臥位からの転倒を含み、(i)前記転倒が、前記応答性の喪失、前記認識の喪失、又は両方に関連し、かつ(ii)前記転倒の後に、全身性身体運動が続くこと
が起こる時、前記てんかん事象が、全身性強直間代性発作として分類される、請求項26または27の医療装置システム。 - 前記全身性身体運動が、律動性身体運動を含む、請求項28の医療装置システム。
- 第一の非横臥位の前記患者において、
(i)前記身体運動が、前記第一の非横臥位からの転倒を含み、前記転倒が、前記応答性の喪失、前記認識の喪失、又は両方に関連し、かつ、
(ii)前記患者が、前記転倒の後の基準値を下回る前記身体運動の有意な低下、前記転倒の後の基準値を下回る筋緊張の有意な低下、又は両方を示すこと
が起こる時、前記てんかん事象が、無緊張発作として分類される、請求項26または27の医療装置システム。 - 前記患者に、
a)基準値を上回る筋緊張の増加、
b)前記応答性の喪失、及び
c)前記全身性律動性運動の欠如
が起こる時、前記事象が、強直性として分類される、請求項26または27の医療装置システム。 - 前記第一身体運動が同期かつ身体の両側に等しい振幅を有するならば、前記てんかん事象が一次性全身性として、かつそうでない場合は二次性全身性として分類される、請求項26または27の医療装置システム。
- 前記患者の心臓活動が認識障害に関連し、かつ転倒又は全身性身体運動に関連しないという発見に基づき、前記てんかん事象が複雑部分発作として分類され、かつ前記患者の前記心臓活動が前記認識障害に関連しないという発見に基づき、前記てんかん事象が単純部分発作として分類される、請求項26または27の医療装置システム。
- 前記てんかん事象の重症度、前記てんかん事象の影響、前記てんかん事象及び少なくとも1つの先行するてんかん事象の間の間隔、時間ウインドウに亘る前記てんかん事象の頻度、前記時間ウインドウに亘る前記てんかん事象の負担、前記時間ウインドウに亘る前記てんかん事象に費やされた時間又は前記てんかん事象のタイプの、1つ以上をロギングすることを更に含む、請求項22の医療装置システム。
- 過去の検出の特異性、前記過去の検出の感度、前記過去の検出速度、前記てんかん事象に関する治療費用、前記てんかん事象に関する患者の治療耐性、前記治療の前記患者の悪影響、及び前記患者の病状の、1つ以上を判定すること、及び、
前記過去の検出の前記特異性が特異性測定を上回った又は下回った、前記過去の検出の前記感度が感度測定を上回った又は下回った、前記過去の前記検出速度が速度測定を上回った又は下回った、前記治療費用が費用測定を下回った又は上回った、前記患者の前記治療耐性が許容可能な耐性を上回った又は下回った、前記悪影響が許容可能なレベルを上回った又は下回った、又は前記患者の病状が第一病状閾値を下回った又は上回った、という1つ以上の判定に基づき、前記第一身体運動、第一心臓活動、応答性レベル、認識レベル、第二心臓活動、第二身体運動、第二心臓活動のスペクトル解析、及び第二身体運動のスペクトル解析の1つ以上に関する基準値の少なくとも1つを適応させることの少なくとも1つを更に含む、
請求項22の医療装置システム。 - プロセッサによって実行される時に、
患者から心臓信号及び身体運動信号の少なくとも1つを受信すること、
前記心臓信号に基づき、第一心臓指数及び前記身体運動信号に基づき、第一身体運動指数の少なくとも1つを判定すること、
前記少なくとも1つの指数を少なくとも第一基準値と比較すること、
前記比較に基づき、前記患者の応答性検査及び前記患者の認識検査の少なくとも1つを誘発すること、
前記少なくとも1つの誘発された検査に少なくとも部分的に基づき、てんかん事象の発生を判定すること、及び
前記てんかん事象の前記発生の前記判定に応じて、更なる行動を行うことを含む方法を行うという命令を記憶し、
前記更なる行動が、
前記発作の発生をロギングすること、
前記発作の発現時間をロギングすること、
前記患者、介護士、又は医療介護提供者の少なくとも1人に警告を提供すること、
前記発作を治療するために、治療を提供すること、
前記検出後、認識又は応答性のような1つ以上の患者パラメータを評価すること、
前記発作の発作重症度指数を判定すること、
前記発作の終了を特定すること、
前記発作の終了時間をロギングすること、及び、
前記患者の発作後障害又は発作からの回復を評価することの1つ以上を含む、
コンピュータ可読記憶装置。 - 患者からの第一心臓活動に関係する信号、前記患者からの第一身体運動に関係する信号、前記患者からの応答性信号、前記患者からの認識信号、前記患者の第二心臓活動に関係する信号、及び前記患者の第二身体運動に関係する信号の少なくとも1つを受信するように設定された、少なくとも1つのセンサと、
前記少なくとも1つのセンサからの前記少なくとも1つの信号を受信し、かつてんかん事象の発生を判定するように設定された検出ユニットと、
前記検出ユニットから前記てんかん事象の前記発生の指標を受信し、前記てんかん事象の前記発生及び/又は前記発生の時間をロギングすること、前記患者、介護士、又は医療介護提供者に警告、アラーム又はアラートを提供すること、前記てんかん事象を防止、中断、及び/又は重症度を低下させるために治療を提供すること、前記てんかん事象中の認識又は応答性のような1つ以上の患者パラメータを評価すること、前記てんかん事象の重症度を評価すること、前記てんかん事象の終了を特定すること、及び前記患者の発作後障害又は前記てんかん事象からの回復を評価することの、少なくとも1つを行うように設定された行動ユニットと
を含むシステム。 - 前記第二心臓活動に関係する前記信号、及び/又は前記第二身体運動に関係する前記信号から少なくとも1つのスペクトル解析信号を発生させるように設定されたスペクトル解析ユニットを更に含み、かつ前記検出ユニットが、前記スペクトル解析ユニットから前記少なくとも1つのスペクトル解析信号を受信するように更に設定される、請求項37のシステム。
