本出願は、「ANTI−CTLA−4 ANTIBODY AND CpG−MOTIF−CONTAINING SYNTHETIC OLIGODEOXYNUCLEOTIDE COMBINATION THERAPY FOR CANCER TREATMENT」という題がついている、2005年7月7日に出願された、本明細書中に参考文献としてその全内容が援用されている米国仮出願第60/697082号に対する優先権を主張する。
本発明は、CpGオリゴヌクレオチドと組み合わせた抗CTLA−4抗体の、癌の処置のための使用に関する。
新しい治療レジメンの開発(特に、現在の化学療法の細胞傷害の副作用を減少させるが、患者の免疫系の抗腫瘍活性を増大させるかまたは増強する能力を有するレジメン)が、必要である。本発明は、このようなレジメンを提供する。

当業者は、いったん、本明細書中に開示されている教示を提供されれば、本発明が、癌を処置するための、抗CTLA−4抗体を組み合わせたCpG ODNの治療、あるいは逐次的に(上記または下記の)外科手術、放射線療法、または両方を組み合わせたこの治療を含むことを認める。すなわち、いったん、本明細書中に提供されている教示を与えられれば、当業者により理解されるように、様々な処置は、抗CTLA−4抗体−CpG ODN PF3512676組み合わせ治療と組み合わせられ得る。
本明細書中に提供されている開示(抗CTLA−4抗体を患者に投与することの免疫増強効果、およびCpG ODN PF3512676と組み合わせてこのような抗体を共投与することの組み合わせた相加効果または相乗効果を含む)に基づいて、本発明は、多数の組み合わせ治療(ここで、この抗体−CpG ODN PF3512676は、患者に、少なくとも1つの他の治療剤と組み合わせて投与され、それによって、治療上の利益を提供する)を含むことが、当業者により認められる。本明細書中に提供されている教示が与えられれば、多くのこのような組み合わせは、当業者にとって容易に明らかであるが、いくつかの組み合わせは、現在議論されている。しかしながら、本発明は決して、単に説明の目的のために本明細書中に示されているこれらの組み合わせに限定されない。

放射線療法は、CpG ODN PF3512676/抗CTLA−4抗体の組み合わせ治療と共に共投与され得る。放射線療法は、乳癌の処置のための周知の放射線療法の方法に従って投与される。放射線療法の用量およびレジメンは、当業者により容易に決定され得、そして疾患の段階および当該分野において周知の他の因子に基づいている。
本発明はまた、第一成分および第二成分に加えて、1つ以上のこれらの成分と同時または連続してのいずれかでの、他の治療剤の投与を含む。このような治療剤としては、鎮痛薬、癌ワクチン、抗血管剤、抗増殖剤、制吐剤、および止痢剤が挙げられる。好ましい制吐剤としては、塩酸オンダンセトロン、塩酸グラニセトロン、およびメトクロプラミドが挙げられる。好ましい止痢剤としては、ジフェノキシラートおよびアトロピン(LOMOTIL)、ロペラミド(IMMODIUM)、およびオクトレオチド(SANDOSTATIN)が挙げられる。
本明細書中に開示されている抗体−CpG ODN PF3512676治療の組み合わせは、癌に冒されている患者に対する治療上の利益を提供するために、幹細胞移植と組み合わせられ得る。幹細胞移植は、当該分野において公知の方法に従って行われ得る。いくつかのこのような方法は、本明細書により本明細書中に参考文献として援用されている、Appelbaum in Harrison’s Principles of Internal Medicine,第14章,Braunwaldら編集,第15版,McGraw−Hill Professional(2001)において記載されている。従って、本発明の方法は、幹細胞移植を受けている哺乳動物における癌の処置に関する。この方法は、ある量のこのヒト抗CTLA−4抗体を、CpG ODN PF3512676と組み合わせて、哺乳動物に投与する工程を包含し、この抗体−CpG ODN PF3512676治療の組み合わせが、幹細胞移植とのさらなる組み合わせにおいて癌を処置する際に有効である。
投薬レジメンは、最適の所望の反応を提供するように調節され得る。例えば、単一のボーラスが投与されてもよく、いくつかの分割された用量が時間をかけて投与されてもよく、または用量が、治療状態の必要性により示されている通りに、比例して減少もしくは増加されてもよい。投与の容易さおよび投薬の均一性のために、投薬単位形態において、非経口的組成物を処方することは、特に有利である。本明細書中に用いられている、投薬単位形態は、哺乳動物被験体が処置されるのに適した1まとまりの投薬としての物理的に別々の単位を指し;各単位は、必要とされる薬学的キャリアと共に、所望の治療効果を生じるように算出された予め決定された量の活性化合物を含む。本発明の投薬単位形態に対する詳細は、(a)上記抗体の特有の性質および達成される特定の治療効果または予防効果、ならびに(b)個体における感受性の処置のためのこのような活性化合物を混ぜ合わせることに関する当該分野における固有の限界により規定され、そして直接的にこれらに依存している。
CpG ODN PF3512676は、当該分野において周知の標準の投与レジメンに従って、投与され得る。粘膜送達または局所送達のためのCpG ODN PF3512676の被験体用量は、典型的に、投与1回当り約1μg〜100mgの範囲にわたり、そしてこの被験体用量は、適用に依存して、毎日、週1回、または月1回およびその間の任意の他の期間で与えられ得る。特に、粘膜用量または局所用量は、投与1回当り約100μg〜50mgの範囲にわたり、最も典型的に、約1〜10mgの範囲にわたり、2〜4回の投与は、何日間かまたは何週間の期間をあけられる。
本発明に従って投与される抗体の治療的に有効な量についての、例示的な、非限定的範囲は、少なくとも約0.1mg/kg、少なくとも約0.3mg/kg、少なくとも約1mg/kg、少なくとも約5mg/kg、少なくとも約6mg/kg、少なくとも約10mg/kg、少なくとも約15mg/kg、少なくとも約20mg/kg、少なくとも約30mg/kg、または少なくとも約50mg/kgである。例えば、抗体の治療的に有効な量は、約0.1〜30mg/kg、または例えば約0.3〜25mg/kg、または例えば約1〜20mg/kg、または例えば約3〜20mg/kg、または例えば約5〜20mg/kg、または例えば約10〜20mg/kg、または約3〜15mg/kg、または約5〜15mg/kg、または約10〜15mg/kgの範囲にわたり得る。

本発明はまた、癌を処置するために有効な量(例えば、少なくとも1mg/kg、少なくとも3mg/kg、少なくとも5mg/kg、少なくとも10mg/kg、少なくとも15mg/kg、または少なくとも20mg/kg)のヒト抗CTLA−4抗体および治療的に有用な量のCpG ODN PF3512676を含む、製品(例えば、i.v.投与のために適合された投薬形態)に関する。特定の実施形態において、上記製品は、ヒト抗CTLA−4抗体、CpG ODN PF3512676、ならびにラベルおよび/または癌を処置することについての使用説明書を含む容器を含む。
本発明は、癌の処置のための様々なキットを含む。上記キットは、治療的に有効な量の本発明のヒト抗CTLA−4抗体および治療的に有効な量のCpG ODN PF3512676を、本発明の方法を実施するためのアプリケーターおよびこの組み合わせの使用を記載する説明書と共に、含む。例示的なキットは下記に記載されているが、他の有用なキットの内容は、本開示に照らして当業者にとって明らかである。これらのキットのうちの各々は、本発明内に含まれている。
存在する場合、外科手術/放射線療法に従って、従来のCTスキャンにより、またはスパイラルCTスキャンにより正確に二次元で測定し得る、少なくとも1つの病巣を有する転移性乳癌を有する患者に、確立されたプロトコールでCpG ODN PF3512676を与える。簡単に言うと、CpG ODN PF3512676を、皮下またはIVで、0.02または20mg/kgの用量で、最も好ましくは、SCとして約0.2mg/kgおよびIVとして2mg/kgで投与する。

Claims (18)
- 癌の処置を必要とする患者における、癌の処置のための方法であって、該方法は、該患者に、治療的に有効な量の抗CTLA−4抗体またはその抗原結合部分を、治療的に有効な量のCpG ODN PF3512676と組み合わせて投与する工程を包含する、方法。
- 請求項1に記載の方法であって、ここで、前記CpG ODN PF3512676は、毎日、2日毎に、週2回、または週1回投与される、方法。
- 請求項1または2に記載の方法であって、ここで、前記処置は、ネオアジュバント療法、アジュバント療法、第一選択療法、第二選択療法、および第三選択療法からなる群から選択される治療である、方法。
- 請求項1、2または3に記載の方法であって、ここで、前記治療的に有効な量の前記ヒト抗CTLA−4抗体は、約0.1mg/kg〜50mg/kgの範囲にわたる、方法。
- 請求項4に記載の方法であって、ここで、前記治療的に有効な量の前記ヒト抗CTLA−4抗体は、約0.3mg/kg〜20mg/kgの範囲にわたる、方法。
- 請求項5に記載の方法であって、ここで、前記治療的に有効な量の前記ヒト抗CTLA−4抗体は、少なくとも1mg/kg、少なくとも3mg/kg、少なくとも6mg/kg、少なくとも10mg/kg、および少なくとも15mg/kgからなる群から選択される、方法。
- 請求項1〜5または6に記載の方法であって、ここで、前記癌は、乳癌、前立腺癌、卵巣癌、膵臓癌、メラノーマ、肺癌、急性骨髄性白血病、結腸直腸癌、腎細胞癌、皮膚T細胞リンパ腫、非ホジキンリンパ腫、胃癌、頭頸部癌、肝臓癌、子宮頸癌、脳の癌、および肉腫からなる群から選択される、方法。
- 請求項1〜6または7に記載の方法であって、ここで、前記抗CTLA−4抗体またはその抗原結合部分は、以下:
(a)約10−8以上のCTLA−4結合親和性を有しかつCTLA−4とB7−1との間の結合、およびCTLA−4とB7−2との間の結合を阻害する、ヒト抗体;
(b)4.1.1、4.8.1、4.10.2、4.13.1、4.14.3、6.1.1、11.2.1、11.6.1、11.7.1、12.3.1.1、12.9.1.1、および10D1からなる群から選択される抗体に由来するCDR配列に対応する少なくとも1つのヒトCDR配列を含むアミノ酸配列を有する、ヒト抗体;
(c)4.1.1、4.8.1、4.10.2、4.13.1、4.14.3、6.1.1、11.2.1、11.6.1、11.7.1.、12.3.1.1、12.9.1.1、および10D1からなる群から選択される抗体の重鎖および軽鎖のアミノ酸配列を有する、ヒト抗体;
(d)CTLA−4との結合を、4.1.1、4.8.1、4.10.2、4.13.1、4.14.3、6.1.1、11.2.1、11.6.1、11.7.1、12.3.1.1、12.9.1.1、および10D1からなる群から選択される抗体のアミノ酸配列を有する少なくとも1つの抗体と競合する、抗体またはその抗原結合部分;ならびに
(e)CTLA−4との結合を、4.1.1、4.8.1、4.10.2、4.13.1、4.14.3、6.1.1、11.2.1、11.6.1、11.7.1、12.3.1.1、12.9.1.1、および10D1からなる群から選択される抗体のアミノ酸を有する少なくとも1つの抗体と交差競合する、抗体またはその抗原結合部分
からなる群から選択される少なくとも1つの抗体である、方法。 - 請求項1〜7または8に記載の方法であって、ここで、前記抗体は、4.1.1、4.13.1、11.2.1、および10D1からなる群から選択される抗体のアミノ酸配列を有するヒト抗体である、方法。
- 請求項9に記載の方法であって、ここで、前記抗体またはその抗原結合部分は、重鎖および軽鎖を含み、ここで、該重鎖の重鎖可変ドメインのアミノ酸配列および該軽鎖の軽鎖可変ドメインのアミノ酸配列は、以下:
(a)配列番号3のアミノ酸配列および配列番号9のアミノ酸配列;
(b)配列番号15のアミノ酸配列および配列番号21のアミノ酸配列;
(c)配列番号27のアミノ酸配列および配列番号33のアミノ酸配列;
(d)配列番号1の核酸配列によりコードされるアミノ酸配列および配列番号7の核酸配列によりコードされるアミノ酸配列;
(e)配列番号13の核酸配列によりコードされるアミノ酸配列および配列番号19の核酸配列によりコードされるアミノ酸配列;
(f)配列番号25の核酸配列によりコードされるアミノ酸配列および配列番号31の核酸配列によりコードされるアミノ酸配列;ならびに
(g)抗体10D1の可変ドメインのアミノ酸配列
からなる群から選択される、方法。 - 請求項9に記載の方法であって、ここで、前記抗体またはその抗原結合部分は、以下:
(a)配列番号4、配列番号5、配列番号6、配列番号10、配列番号11および配列番号12に示されているアミノ酸配列を含む抗体;
(b)配列番号16、配列番号17、配列番号18、配列番号22、配列番号23および配列番号24に示されているアミノ酸配列を含む抗体;ならびに
(c)配列番号28、配列番号29、配列番号30、配列番号34、配列番号35および配列番号36に示されているアミノ酸配列を含む抗体
からなる群から選択される抗体である、方法。 - 請求項9に記載の方法であって、ここで、前記抗体またはその抗原結合部分は、配列番号27に示されているアミノ酸配列を有する重鎖可変領域および配列番号33に示されているアミノ酸配列を有する軽鎖可変領域を含む、方法。
- 請求項9に記載の方法であって、ここで、前記抗体は、以下:
(a)配列番号2および配列番号8に示されているアミノ酸配列を含む抗体;
(b)配列番号14および配列番号20に示されているアミノ酸配列を含む抗体;ならびに
(c)配列番号26および配列番号32に示されているアミノ酸配列を含む抗体
からなる群から選択される、方法。 - 請求項1〜12または13に記載の方法であって、ここで、前記抗体は、前記CpG ODN PF3512676の投与の1〜7日前に投与される、方法。
- 請求項1〜13または14に記載の方法であって、ここで、前記CpG ODN PF3512676は、前記抗体の約1〜100日後に投与される、方法。
- 請求項1〜14または15に記載の方法であって、ここで、前記CpG ODN PF3512676は、皮下に投与される、方法。
- 請求項1〜15または16に記載の方法であって、ここで、前記CpG ODN PF3512676は、1日当たり1mg〜50mgの量で投与される、方法。
- 癌の処置のための薬学的組成物であって、該組成物は、治療的に有効な量の抗CTLA−4抗体またはその抗原結合部分、および治療的に有効な量のCpG ODN PF3512676、および薬学的に受容可能なキャリアを含む、組成物。
