Description
The present invention relates to an effect enhancer for regenerative treatment of central nerve damage and a regenerative treatment method used therefor, and more specifically, transplants olfactory nerve sheath cells or mucosal tissue containing the cells to a central nerve injury patient to obtain the central nerve. The present invention relates to an effect enhancer for regenerative treatment to be reconstructed, and a central nerve regenerative treatment method using the effect enhancer.
The central nervous system consists of the brain and spinal cord. Central nerve damage is a disease caused by damage to the central nerve. Specifically, for example, traumatic brain injury such as spinal cord injury, cerebral contusion, brain damage based on cerebrovascular disorder such as cerebral hemorrhage or cerebral infarction, brain Examples include brain damage after surgery.
Spinal cord injury is a pathological condition that causes paralysis of the peripheral motor, sensory, and autonomic nervous system below the injured part due to injury to the spinal cord parenchyma due to trauma such as a spinal dislocation fracture caused by a traffic accident or falling at a high place. is there. Currently, the number of spinal cord injuries is estimated to be approximately 100,000 in Japan and approximately 250,000 in the United States. The number of patients in Japan is 5,000 and more than 10,000 in the United States each year. With the advancement of medical treatment in recent years, it has become possible to survive even after being injured. However, the inconvenience of daily life and the mental burden are causing social problems, resulting in social problems. It is.
For many years, it has been thought that the central nervous system (brain and spinal cord) of an adult mammal does not regenerate once damaged. The central nervous system is difficult to regenerate, but it has long been known that the peripheral nervous system regenerates easily, and the difference is that in the central nervous system, oligodendrocytes, myelin, or extracellular matrix suppresses axonal outgrowth. It is explained that the factor exists, whereas such a factor does not exist in the peripheral nervous system. In other words, the peripheral nervous system axon environment is permissive as a whole for newly growing axons, whereas the central nervous system is non-permissive, ie, axonal neoplasia. Is in an environment where If this explanation is correct, it will be necessary to tolerately alter the rejecting environment to promote regeneration of the mammalian central nervous system. One of the attempts is transplantation of olfactory nerve sheath cells (see Non-Patent Document 1).
Olfactory neurons are specific cells that continue to regenerate throughout life in adult mammalian cells. Lu et al. Transplanted olfactory nerve sheath cells derived from the nasal olfactory mucosa of the rat into the spinal cord of a completely dissected rat, which markedly restores motor function and significantly reduces the descending inhibition of the spinal reflex. The knowledge that the axon has grown beyond the site has been obtained, and the possibility of application to regeneration treatment of spinal cord injury, which is the central nervous system of humans, has been shown (see Non-Patent Document 2).
Since the olfactory mucosa can be collected even in adults, the expectation has rapidly increased in that it can be autotransplanted, and clinical trials are being conducted. For example, in Portugal, Carlos Lima et al. Reported that autologous transplantation of the olfactory mucosa to spinal cord injury patients had a therapeutic effect (see Non-Patent Document 3). Although it is not autotransplantation, in China, led by Huang et al., Clinical trials of human fetal olfactory nerve sheath cell transplantation for patients with spinal cord injury have been conducted since 2002, and some patients reported improvements. (See Non-Patent Document 4).
As described above, the method of treating spinal cord injury, which is one of the central nerve injuries, by transplanting olfactory sheath cells and olfactory mucosa to the damaged site has achieved certain results, but the effect is still In order to obtain limited and further functional recovery, it is necessary to improve the method of treatment.
Another reason why axons in damaged spinal cord do not regenerate is that chondroitin sulfate proteoglycans (CSPG) that accumulate in glial scars at the site of injury and Nogo protein on myelin membrane inhibit axonal elongation It is believed to have activity. Therefore, if the action of these inhibitors is suppressed, spinal cord axonal regeneration may be promoted.
Rho, which is a signaling molecule that mediates most of the signal from the low molecular weight GTP-binding protein Rho, is one of the promising targets that can suppress the action of inhibitors such as CSPG and Nogo protein on the myelin membrane. Substances that inhibit kinases have been considered (see Non-Patent Document 5).
With this background, various attempts have been made to apply Rho kinase inhibitors to spinal cord axonal regeneration. For example, Dergham et al. Described Y27632 ((R)-(+)-trans-N- (4-pyridyl) -4- (1-aminoethyl) cyclohexane, a Rho kinase inhibitor, upon injury of the dorsal region of the mouse thoracic spinal cord. When carboxamide) was applied to the injured site, it was shown that the axon regeneration of the corticospinal tract was promoted, and that the inhibitor also promoted the recovery of motor function using the BBB score as an index (see Non-Patent Document 6). ).
Various substances have been known as Rho kinase inhibitors, and examples of such compounds include ROKα (ROCK-II), p160ROCK (ROKβ, ROCK-I) and other serine / threonine kinases. Examples thereof include compounds that inhibit a protein having activity. Specific examples thereof include hexahydro-1- (5-isoquinolinesulfonyl) -1H-1,4-diazepine (see Patent Documents 1 and 2), (R) -trans-N- (pyridin-4-yl) -4. Rho such as-(1-aminoethyl) cyclohexanecarboxamide, (R)-(+)-N- (1H-pyrrolo [2,3-b] pyridin-4-yl) -4- (1-aminoethyl) benzamide Rho kinase inhibitors such as kinase inhibitors (see Patent Document 2 or 3), 1- (5-isoquinolinesulfonyl) homopiperazine, 1- (5-isoquinolinesulfonyl) -2-methylpiperazine (Patent Document 4 or non-patent documents) 7), Rho kinase inhibitors such as (1-benzylpyrrolidin-3-yl)-(1H-indazol-5-yl) amine (see Patent Document 5) Rho kinase inhibitors such as (1-benzylpiperidin-4-yl)-(1H-indazol-5-yl) amine (see Patent Document 6), N- [2- (4-fluorophenyl) -6,7- Rho kinase inhibitors such as dimethoxy-4-quinazolinyl] -N- (1H-indazol-5-yl) amine (see Patent Document 7), N-4- (1H-indazol-5-yl) -6,7- Rho kinase inhibitors such as dimethoxy-N-2-pyridin-4-yl-quinazoline-2,4-diamine (see Patent Document 8), and 4-methyl-5- (2-methyl- [1,4] diazepane Rho kinase inhibitors (see Patent Document 9) such as -1-sulfonyl) isoquinoline are known.
An object of the present invention is to perform regenerative treatment for reconstructing the central nerve by transplanting olfactory sheath cells or mucosal tissue containing the cells to the damaged site of central nerve injury (for example, spinal cord injury). It provides a drug that enhances the effect. Another object of the present invention is to provide a method for enhancing the effect of the regenerative treatment using the drug.
As a result of various studies to solve the above-mentioned problems, the present inventors have transplanted olfactory sheath cells or mucosal tissues containing the cells to the damaged site for central nerve damage (for example, spinal cord injury). When performing regenerative treatment to reconstruct nerves, it was found that the regenerative treatment effect can be remarkably enhanced by administering a Rho kinase inhibitor to the patient, and the present invention was completed through further studies. .

Regenerative treatment is performed by administering a Rho kinase inhibitor to a patient during regenerative treatment in which a central nerve is reconstructed by transplanting an olfactory nerve sheath cell or a mucosal tissue containing the cell to a patient with central nerve injury (for example, spinal cord injury). The effect of can be remarkably enhanced.
The effect-enhancing agent of the present invention is an effect-enhancing agent in regenerative treatment for reconstructing the spinal cord by transplanting olfactory sheath cells or mucosal tissue containing the cells into a patient with central nerve injury, and comprising a Rho kinase inhibitor as an active ingredient It is characterized by doing.
The “central nerve injury” as used in the present invention refers to a disease caused by damage to the central nerve composed of the brain and spinal cord. For example, spinal cord injury, traumatic brain injury, cerebrovascular disorder (eg cerebral hemorrhage, cerebral infarction, etc.) ) Based brain injury, brain injury after brain surgery, and the like.
As used herein, “olfactory nerve sheath cell” refers to a cell that surrounds the axon of an olfactory nerve cell. The position of olfactory nerve sheath cells in humans is shown in FIG.
Specific examples of the “mucosal tissue containing the cells” in the present invention include “olfactory mucosa”.
Referred to in the present invention "olfactory mucosa" is in part surrounded by the upper nasal turbinate top of left and right nasal隔上portion of the nasal cavity as a main olfactory, for example in humans those having an area of about the right and left each about 2 cm 2 is there. The olfactory mucosa is composed of the olfactory epithelium and the olfactory mucosa lamina, and olfactory nerve sheath cells are mainly present in the olfactory mucosa lamina.
In carrying out the regenerative treatment according to the present invention, first, “olfactory nerve sheath cells or mucosal tissues containing the cells” are prepared.
Preparation of mucosal tissue containing olfactory nerve sheath cells is performed by removing mucosal tissue containing "olfactory mucosa" present in the nose of mammals, preferably removing the olfactory mucosa, more preferably the olfactory mucosa lamina propria Is done. The site shown in FIG. 2 is preferable as the excision site in humans, but if the olfactory mucosa is the main component, respiratory mucosa (not including olfactory nerve sheath cells) may be mixed. The excised olfactory mucosa can be made into a form that can be easily transplanted to a spinal cord injury site by chopping as necessary.
In addition, olfactory nerve sheath cells derived from the olfactory mucosa can be cultured and isolated for use, and the culture and isolation method therefor can be carried out by a method known per se. For example, the mucosal tissue containing the extracted olfactory nerve sheath cells is cultured in Dulbecco's modified Eagle medium (DMEM) supplemented with 10% fetal calf serum for 3 to 4 days, and then cultured in DMEM serum-free medium for 2 to 3 weeks. The olfactory nerve sheath cells may be isolated by using trypsin-EDTA after the latter medium is added twice a week and cultured. Further, the isolated olfactory nerve sheath cells can be suspended in an appropriate medium (for example, DMEM, serum-added DMEM, etc.) and used.
Olfactory nerve sheath cells and the source of mucosal tissue containing the cells include adult humans (autologous or allogeneic transplantation), human fetuses (allografts), human cadaver (allografts), and other mammals such as pigs (xenotransplantation) In consideration of donor problems, problems of immune rejection that can occur after transplantation, ethical problems, and the like, human adults who undergo transplantation (autologous transplantation) are preferable.
Transplantation of olfactory nerve sheath cells and mucosal tissues containing the cells is performed at the site of central nerve injury. The transplantation can be performed by various methods. When the central nerve injury is a spinal cord injury, for example, it can be performed as follows.
Under general anesthesia, the patient is made a midline incision and the muscles are removed under the periosteum at an appropriate level. Carefully handle muscles, bones, and epidural space during dural opening. A laminectomy is performed over the entire lesion. The entire vertebral arch is excised to avoid spinal deformity due to surgical procedures and to allow structural regeneration after olfactory nerve sheath cell transplantation. Inject the cultured suspension of olfactory sheath cells into the site adjacent to the cranial and caudal side of the spinal cord lesion, or fill the void in the excision site with mucosal tissue containing olfactory sheath cells Transplant by.
When the central nerve damage is brain damage, for example, it is performed as follows.
The patient is subjected to craniotomy under general anesthesia, and the affected part is opened to transplant olfactory mucosal sheath cells or mucosal tissue containing the cells (for example, olfactory mucosa). In cerebral infarction or the like, olfactory mucosal sheath cells may be injected stereotactically around the affected area. In the acute phase of cerebral infarction, olfactory mucosal sheath cells may be administered transvascularly to the affected area. Furthermore, in the chronic stage of brain injury, transplantation may be performed after removing already formed glial scar tissue, or olfactory mucosal sheath cells may be injected around the affected area.
Further, the transplantation time may be performed immediately after the central nerve injury or may be performed one to several weeks later depending on the condition of the patient. Furthermore, chronic phase transplantation is also applicable to cases where months to years have passed since injury, and in some cases, it may be performed on patients who are unable to recover due to rehabilitation or the like.
In the present invention, the following Rho kinase inhibitor is used to enhance the effect of the transplantation treatment.
(Amano et al.,Exp.Cell.Res.,261,44−51(2000))。 The “Rho kinase inhibitor” referred to in the present invention is a substance that inhibits the activity of Rho kinase. Rho kinase is a kind of low molecular weight GTP-binding protein (low molecular weight G protein) included in the category of GTPase which is a degrading enzyme of GTP (guanosine triphosphate).
