Abstract
Description
体内管腔の開存性を維持するために、ステントや、グラフト、ステント−グラフト、大静脈フィルタといった他の植込み可能な医療器具(以下、総称してステントと記載する)を用いることはよく知られている。
通常、ステントは、配置するための非拡張状態(閉じた状態)の径と、血管又は管路に配置された後の拡張された(開いた状態)の径とに変化する。ステントには、自己拡張型ステントや、(例えばバルーンの膨張により)ステント内部から径方向外側へ向かう力により機械的に拡張されるタイプのステントや、自己拡張型ステントと機械的に拡張可能なステントの双方と共通する特徴を一つ以上有する、ハイブリッドステントとして知られるステントがある。
(発明の概要)
本発明は、ステント留置の正確性を向上させ、ステントジャンプの発生及び程度を減少させるための実施例に関する。
(発明の詳細な説明)
本発明は多くの異なる態様にて実施可能であるが、本明細書においては特定の好ましい実施例について詳細に説明する。この説明は本発明の原理を例示するものであり、本発明を例示された特定の実施例に限定するものではない。
上述したように、本発明は、様々な態様にて実施することができる。例えば、図1に示される実施例においては、本発明は、全体が符号10にて示されるステント保持バンド又はカラーの態様で実施される。ステント保持バンド又はカラー10は、1個以上の突起14を備えた外面12を有する。図1及び図2に示されるように、突起14は、互いに類似する寸法及び配向を有していてもよく、互いに異なる寸法及び配向を有していてもよい。また、突起14は、縦列又は横列をなすように外面12上に配置されていてもよく、あるいはその他所望のパターンをなすように外面12上に配置されていてもよい。
上述したように、突起14は任意の形状又は構成とすることができる。例えば、図1及び図2に示される実施例においては、突起はバンド表面12の高くなった部分又は隆起部であり、例えばバンド及び突起を図示される形状に成形する等、様々な賦形装置により形成することができる。実施例によっては、突起14は、内側シャフト16をバンド10の材料と同質の材料に変更することにより形成することができる。あるいは、突起14を別個の要素とし、溶接、スタンピング、打ち抜き、接着、射出成形、溶融、又はその他の方法でバンド10の表面12上に配置及び/又は係合することができる。しかしながら、図4に示されるように、突起14は、バンド10に1個以上の開口40を切り込むことにより形成してもよい。管状体10から切り出された材料又はフラップ42は、少なくとも1本の係合線又は少なくとも1箇所の係合点44に沿って管状体10と係合した、管状体10と一体的な状態を保つ。管状体材料から形成されたフラップ42は、少なくともその一部が管状体表面12から外側に向かって延びるように配向され、突起14として機能する。複数のフラップ42が設けられる場合には、フラップ42は所望される任意の形状とすることができ、互いに同一の又は異なる構成を有していてもよい。
Claims (14)
- 内側シャフトを有するカテーテルであって、前記内側シャフトの少なくとも一部がステント装着領域を形成することと、
ステント壁により形成され、非拡張状態と拡張状態とに変化するステントであって、前記非拡張状態にあってはステントが前記ステント装着領域の少なくとも一部の周囲に配置されることと、
前記内側シャフトの少なくとも一部の周囲に配置され、ステント装着領域の少なくとも一部を形成する少なくとも1個のバンドであって、前記バンドは、外面と、同外面から径方向外側へ延びる複数の突起とを有する本体領域を備え、前記ステントが非拡張状態にあるときには、ステントの少なくとも一部が前記バンドの本体領域の少なくとも一部の周囲に配置されてこれと係合することと、各突起がステント壁により形成される空間に進入することと
を備えたステント搬送システム。 - 前記バンドの少なくとも一部が放射線不透過性を有する請求項1に記載のステント搬送システム。
- 前記ステントは自己拡張型ステントである請求項1に記載のステント搬送システム。
- 後退可能なシースをさらに備え、前記ステントが前記後退可能なシースにより非拡張状態に保持されることと、前記シースがステントから後退させられると、ステントが拡張状態に自己拡張することと、前記後退可能なシースが完全にステントから後退させられるまでは、ステントは前記複数の突起のうち少なくとも1個と係合し続けることとからなる、請求項1に記載のステント搬送システム。
- 前記複数の突起がそれぞれ前記バンドの外面上の隆起部である請求項1に記載のステント搬送システム。
- 前記複数の突起がそれぞれ前記バンドの外面から延びるフラップである請求項1に記載のステント搬送システム。
- 各フラップが、前記バンドの本体領域に開口を形成することにより形成されるとともに、フラップが形成される開口近傍のバンドの外面の少なくとも1箇所と係合する、請求項6に記載のステント搬送システム。
- 前記複数の突起のうち少なくとも1個がほぼフィン形状を有する請求項1に記載のステント搬送システム。
- 前記少なくとも1個のほぼフィン形状を有する突起が、前記バンドの外面の少なくとも一部を径方向外側へ引っ張ることにより形成される、請求項8に記載のステント搬送システム。
- 前記バンドの少なくとも一部が押出成形により形成される請求項1に記載のステント搬送システム。
- 前記バンドの少なくとも一部が射出成形により形成される請求項1に記載のステント搬送システム。
- 前記少なくとも1個のバンドが前記内側シャフトと一体的に構成される請求項1に記載のステント搬送システム。
- 内側シャフト及び後退可能なシースを有するカテーテルであって、前記内側シャフトは、これと係合される少なくとも1個のバンドを有し、この少なくとも1個のバンドは、外面と、同外面から径方向外側に延びる複数の突起とを有する本体領域を備えることと、
非拡張状態から拡張状態へ拡張可能なステントであって、非拡張状態にあっては、前記ステントの少なくとも一部が内側シャフトの一部の周囲に配置され、前記少なくとも1個のバンドの本体領域の少なくとも一部と係合されることと、非拡張状態においては、前記複数の突起のうち少なくとも1個が、ステントにより形成される空間の少なくとも一つの内部に延びることにより、ステントの少なくとも一部と係合することと、非拡張状態においては、後退可能なシースがステント上に位置し、突起のうち少なくとも1個が、ステントが内側シャフトに対して長手方向に移動することを妨げることと
を備えたステント搬送システム。 - カテーテルシャフトと、後退可能なシースと、非拡張状態から拡張状態へ拡張可能なステントと、前記ステントの少なくとも一部の下方に位置するカテーテルシャフトの一領域の周囲に配置される少なくとも1個のバンドであって、同バンドは径方向外側へ延びる複数の突起を有することとを備える搬送カテーテルから、搬送されるステントが長手方向へ移動することを防止する方法であって、
前記後退可能なシースがステントから完全に引き戻された状態となるまで、前記径方向に延びる突起のうち少なくとも1個と非拡張状態のステントが係合することを含む方法。
