Description

The present invention relates to a wound dressing. In particular, the present invention belongs to the technical field of wound dressings suitable for covering and treating deep wounds such as pressure ulcers (bed slippage) that occur in bedridden elderly people and the like.

Conventionally, treating pressure ulcers (bed sores) has been an important issue in the care of bedridden elderly people and the like. A pressure ulcer is a condition in which a specific part of the body is pressed or rubbed for a long time, blood flow is deteriorated, oxygen and nutrients are not supplied, and skin and muscles gradually collapse.
As a method for treating this pressure ulcer, wound dressing is generally used together with nutrition improvement, massage and the like.

Conventionally, various dressing materials such as polyurethane foam, alginate coating materials, and the like are known as wound dressings, and as one of them, dressings using chitin / chitosan are attracting attention. Chitin / chitosan is a substance obtained by purifying and chemically treating chitin, which is the main component of crab shells and shrimp shells, and has excellent biocompatibility and antibacterial properties. It is possible to treat the pressure ulcer by purifying the affected area by suppressing the pressure. Moreover, since it has moisturizing properties, it is said that it has the effect of maintaining the vicinity of the affected area in a moist state and promoting the formation of defective granulation.

As an example of a wound dressing material using the above chitin / chitosan, (Patent Document 1) includes a normal viscose corporation rather than a mixed viscose in which chitin / chitosan viscose and cellulose viscose are mixed in an arbitrary ratio. A functional wound dressing made of cotton, which can be directly applied to a skin ulcer surface, or a woven fabric, a woven fabric, a knitted fabric or a non-woven fabric, produced by a wet spinning method of a synthetic fiber, which can be directly applied to a skin ulcer surface It is disclosed.
Further, (Patent Document 2) discloses a wound covering protective material comprising a nonwoven fabric composed of chitin fibers having a thickness of less than 1 denier and a strength of 2 g / d or more, and a fibrous binder. Yes.
Although these dressings actually have some therapeutic effect on various wounds, chitin / chitosan has a moisturizing performance of about 20% of its own weight, so it is a deep wound like a pressure ulcer. When the amount of exudate is relatively large, the exudate may not be sufficiently absorbed or discharged. For this reason, there has been a problem that exudate accumulates in the vicinity of the affected area, and the antibacterial action of chitin / chitosan decreases, and sometimes chitin / chitosan fibers are melted by the exudate. Further, the exudate has accumulated, so that the affected area becomes wet and it takes time to heal.

Further, (Patent Document 3) discloses a chitin or chitosan flocculent made of a flocculent aggregate obtained by opening chitin or chitosan fibers or fibrils. According to this, it is said that the exfoliation effect of exudate is enhanced by forming it in a cotton shape. However, since the whole is composed of chitin / chitosan cotton, the ability to absorb exudate is still limited, which is not sufficient.
Therefore, it has been desired to develop a wound dressing material that can effectively remove excess exudate and maintain high antibacterial properties over a long period of time while maintaining the affected area in a moderately moist state.

Furthermore, especially in the case of pressure ulcers, the wound may be deep and the meat may be lost, resulting in unevenness in the affected area. In such a case, the conventional wound dressing has a problem that it cannot sufficiently follow the unevenness of the surface and creates a gap, and even if chitin / chitosan is used, its antibacterial action cannot be sufficiently exhibited. Therefore, there has been a demand for a wound dressing that is well adhered to an uneven affected part such as pressure ulcer and has a high therapeutic effect.

Japanese Patent No. 3046099

Therefore, in view of the above-described conventional situation, the present invention is suitably used particularly for serious wounds such as pressure ulcers, can absorb and remove exudate well, exhibit high antibacterial properties, and moderately affect the affected area. An object is to provide a novel wound dressing that is kept moist and promotes granulation formation.
It is another object of the present invention to provide a wound dressing material that follows well on the surface of an affected area having irregularities as seen in severe pressure sores and the like and has excellent adhesion, resulting in a high therapeutic effect.

In order to solve the above-mentioned problems, a wound dressing according to the present invention comprises, as claimed in claim 1, a woven or non-woven fabric containing chitosan used on the skin side, and a sheet of superabsorbent polymer laminated on the woven or non-woven fabric. It is characterized by having.

According to the above configuration, the superabsorbent polymer sheet absorbs and eliminates a large amount of exudate from the affected area. At that time, the superabsorbent polymer sheet is separated from the woven fabric / nonwoven fabric containing chitosan and does not directly contact the affected area, so that the tissue does not harden due to excessive drying. The formation of granulation is promoted in a moderately moist state. And by removing the exudate, the antibacterial action of chitosan is effectively exhibited, and the affected part is cleaned.

The wound dressing according to claim 2 comprises a woven or non-woven fabric used on the skin side and containing chitosan, and a sheet of superabsorbent polymer laminated on the woven or non-woven fabric, wherein the sheet absorbs water. The woven fabric or non-woven fabric is deformed as it expands.

According to the said structure, in addition to the effect | action of Claim 1, according to the quantity of exudate, the sheet | seat of a super absorbent polymer swells, and the woven fabric and nonwoven fabric of a skin side deform | transform according to it. Since the amount of the exudate tends to be proportional to the depth of the wound (that is, unevenness), the sheet is greatly expanded at the deeply scratched portion, and the woven / nonwoven fabric is pressed against the affected area. As a result, the woven fabric / nonwoven fabric containing chitosan adheres along the uneven surface of the affected area, and the therapeutic effect is enhanced.

A third aspect is characterized in that an adhesive is laminated on the wound dressing according to the first or second aspect and can be fixed to the skin.

According to the said structure, the adhesive bandage type coating | covering material stuck and used for the location of a wound is provided. Especially for pressure ulcers, the affected area is very delicate and the skin may be damaged even if the dressing is slightly deviated. Therefore, a high therapeutic effect can be obtained by securely fixing the dressing to the affected area. .

In addition, a fourth aspect is characterized in that, in the wound dressing according to any one of the first to third aspects, the content of chitosan with respect to the entire woven fabric or non-woven fabric is 20 to 80 wt%.

According to the said structure, the content rate of chitosan in a woven fabric and a nonwoven fabric is specified from a viewpoint of keeping an affected part in a moist state and fully exhibiting an antimicrobial effect. In the present invention, chitosan-containing woven fabrics / nonwoven fabrics are prepared by kneading chitosan fine particles into general woven fabrics / nonwoven fabrics made of natural or synthetic fibers and adhering them, or chitosan is directly formed into fibers. The woven fabric / nonwoven fabric produced from the fiber can be used, but the above content range is the former case, that is, when chitosan fine particles are kneaded and adhered to a general woven fabric / nonwoven fabric. Applied.

Moreover, Claim 5 is the wound dressing according to any one of Claims 1 to 4, wherein the deacetylation degree of chitosan is 60% or more.

According to the above configuration, the degree of deacetylation of chitosan is optimized in order to obtain high antibacterial properties.

Furthermore, claim 6 is the wound dressing according to any one of claims 1 to 5, wherein the superabsorbent polymer is a polyacrylate-based, starch-based, cellulose-based, poval-based, or polyoxyethylene-based polymer. It is characterized by being.

According to the said structure, the specific kind of superabsorbent polymer is specified from a viewpoint which absorbs exudate favorably and does not have a bad influence with respect to a biological body.

As described above, the wound dressing of the present invention is a laminate of a woven fabric or nonwoven fabric containing chitosan and a sheet of superabsorbent polymer, and exudates by applying the side of the woven fabric / nonwoven fabric containing chitosan to the affected area. The liquid can be absorbed and removed effectively, and the antibacterial action of chitosan can be maximized. In addition, the moisturizing property of chitosan keeps the affected area moist and promotes granulation.
The wound dressing of the present invention can be applied to various wounds such as ulcers and contusions, but is particularly effective for deep wounds such as pressure ulcers.

In addition, the wound dressing of the present invention may be along the surface even for affected areas with irregularities because the woven fabric or nonwoven fabric deforms as the sheet of superabsorbent polymer absorbs exudate and expands. Adhering to each other, chitosan and bacteria are sufficiently in contact with each other, and a high therapeutic effect can be obtained.
Since the wound dressing according to the present invention contains chitosan, which is a natural substance, it does not discharge harmful substances even when incinerated or disposed of, or has biodegradability and contributes to environmental protection. It is.

Hereinafter, the present invention will be described in detail.
First, an embodiment (1) of the present invention is shown in FIG. As shown in FIG. 1, the wound dressing 1 of the present invention includes a woven or nonwoven fabric 10 containing chitosan used on the skin side, and a superabsorbent polymer sheet 11 laminated on the woven or nonwoven fabric 10. It is comprised roughly with.

As the woven or non-woven fabric 10 containing chitosan, for example, chitosan fine particles are kneaded into natural fibers such as cotton and general woven or non-woven fabrics made of (semi) synthetic fibers such as rayon and acrylic. It is possible to appropriately select and use an attached material or chitosan directly formed into a fiber and a woven fabric / nonwoven fabric produced from the fiber. Chitosan can be obtained by deacetylating chitin contained in shrimp, crab shells, etc. with alkali, etc., and its structure is a linear polysaccharide with β-1,4-linked D-glucosamine, which has high antibacterial properties , Has moisture retention. Therefore, there is an effect of killing bacteria of the wound, effectively cleaning the affected part, and keeping the wound part moist to promote granulation regeneration.

As described above, when the chitosan fine particles are kneaded or adhered to the woven / nonwoven fabric, the content of chitosan is preferably 20 to 80 wt% with respect to the entire woven or non-woven fabric 10. If it is less than 20 wt%, the necessary antibacterial properties cannot be obtained, and if it is attempted to exceed 80 wt%, the flexibility of the woven fabric or nonwoven fabric 10 tends to be lost, which is not suitable.

Further, when chitosan is directly formed into a fiber, it can be carried out by appropriate means. For example, chitosan fiber is first prepared by dissolving chitosan powder in a solvent such as acetic acid, passing through processes such as filtration, spinning in a coagulation bath of water, various alcohols, acetone, etc., winding up and drying. Can be obtained. When spinning, not only chitosan but also cotton, acrylic fiber or the like may be blended, and a woven fabric or nonwoven fabric may be produced from the obtained blended yarn.

As described above, chitosan can be obtained by N-deacetylation of chitin by hydrolysis with concentrated alkali, but it is preferable that the degree of deacetylation at that time is as high as possible because it is superior in antibacterial properties. . This is considered to be because the amino group of chitosan produced by deacetylation attracts the negative charge on the cell wall of the bacterium and inhibits the freedom of the bacterium to suppress the growth. Specifically, when the degree of acetylation is 60% or more, it acts very effectively on the amount of bacteria in general pressure ulcers. However, depending on the degree of wound and the like, chitosan having a lower degree of acetylation may be used, and is not limited to the above numerical values.

Next, as the superabsorbent polymer sheet 11, a woven fabric / nonwoven fabric made from a fibrous superabsorbent polymer, or the fibrous superabsorbent polymer and other synthetic fibers and natural fibers are mixed. Thus, a woven fabric / nonwoven fabric obtained or a surface absorbent material such as a water-absorbing pulp formed into a sheet can be appropriately selected and used in which a superabsorbent polymer powder is embedded. As the type of the superabsorbent polymer, a conventionally known substance can be appropriately used as long as it does not adversely affect the living body, and specifically, polyacrylate, starch, cellulose, and poval. And polyoxyethylene polymers are preferably used. In addition, in order to increase the absorption rate of exudate containing a large amount of sodium ions, etc., a highly water-absorbing polymer improved by copolymerizing a strongly dissociable sulfonate group or adding an ion sequestering agent is used. Also good.

The wound dressing 1 of the present invention comprises the above-described superabsorbent polymer sheet 11 to quickly absorb and remove excess exudate from the affected area, thereby increasing the antibacterial properties of chitosan without deteriorating it. Can be maintained at a level. In addition, the superabsorbent polymer sheet 11 is not in direct contact with the skin and has a moisturizing chitosan-containing woven or non-woven fabric 10 interposed therebetween, thereby preventing excessive drying of the affected area due to the absorption of moisture. In addition, it is possible to ensure an appropriate wet state necessary for granulation formation. Furthermore, since the highly water-absorbing polymer itself has elasticity (cushioning properties), it also has an effect of preventing compression of the affected area.

In addition, as shown in FIG. 2, the wound dressing 1 of the present invention can be configured such that the woven fabric or the nonwoven fabric 10 is deformed as the highly water-absorbing polymer sheet 11 absorbs water and expands.
That is, when the wound A is covered, the sheet 11 of the superabsorbent polymer swells more as it is closer to the center of the wound A where the amount of exudate is larger, and the woven fabric or the nonwoven fabric 10 is deformed accordingly. The cloth or non-woven fabric 10 fits well to the affected area depending on the state of the wound A, and the bacteria and chitosan can be brought into contact with each other to effectively exert the antibacterial action.

As shown in FIG. 2, in order to increase the swelling degree of the superabsorbent polymer sheet 11, it can be obtained by increasing the filling ratio of the superabsorbent polymer in the sheet or adjusting the degree of crosslinking of the superabsorbent polymer itself. it can. In addition, a sheet having a high degree of swelling can also be obtained by means such as containing an acrylate polymer in a hydrophilic polyurethane foam, but is not limited thereto.

When laminating the above-mentioned woven or nonwoven fabric 10 and the highly water-absorbing polymer sheet 11, it can be carried out by various means in consideration of the actual use form and the like. Specific examples include a method of sewing the woven or non-woven fabric 10 and the highly absorbent polymer sheet 11 and a method of bonding the peripheral edge of the sheet using an adhesive. Alternatively, separately produced woven or non-woven fabric 10 and superabsorbent polymer sheet 11 may be overlapped when actually used, and fixed to the affected area with a bandage or the like from the outside.

Next, an embodiment (2) of the present invention is shown in FIG. In this embodiment, an adhesive 2 is laminated on the wound dressing 1 shown in the above-described embodiment (1) to form a bandage-type dressing that can be fixed to the skin.
By sticking this covering material so that the woven or non-woven fabric 10 covers the wound site, the antibacterial and moisturizing properties of chitosan are sufficiently exhibited, and the wound can be effectively treated. Here, the shape and size of the pressure-sensitive adhesive body 2 can be appropriately set depending on the location of the affected part and the scale of the wound.

The pressure-sensitive adhesive body 2 can be generally constituted by laminating various supports 20 and pressure-sensitive adhesive layers 21. The support 20 and the pressure-sensitive adhesive layer 21 are preferably made of a material that is strong against moisture and has little irritation (rash, etc.). Specifically, as the support 20, a soft polyvinyl chloride film, a polyethylene film, a polyurethane film, a cotton cloth, a rayon, etc. Non-woven fabric, Japanese paper, Spantex (registered trademark) cloth, etc. are applicable. Acrylic adhesive, rubber adhesive, polyurethane adhesive, rubber / resin / zinc oxide, etc. However, the present invention is not limited to these. In addition, it is preferable that the adhesive body 2 has air permeability in order not to store the exudate and to prevent maceration (blur) of the affected skin and the surrounding healthy skin. This air permeability can be obtained by means such as making the support 20 and the pressure-sensitive adhesive layer 21 themselves breathable, or making small holes on the pressure-sensitive adhesive body 2 at intervals.

As described above, the wound dressing of the present invention can quickly remove exudate from the affected area, sufficiently exhibit the chitosan's original antibacterial action, and obtain an unprecedented high therapeutic effect. In addition, when disposed after use, no harmful substances are generated even if incinerated, and basically, since the raw materials are naturally derived, even if discarded, the environment is not adversely affected.

EXAMPLES Hereinafter, although an Example demonstrates this invention further in detail, it is not limited to these.
(Example 1)
First, chitin powder refined from crab shell was treated with a 30% aqueous solution of caustic soda to obtain chitosan powder having a deacetylation degree of 80%. Subsequently, this chitosan powder was dissolved in a 3% aqueous solution of acetic acid to give a 5% dope, extruded from a nozzle into hot water at 80 ° C., wound up at a constant speed, and the solvent was removed to remove about 8 denier. A fiber was produced. And this fiber was cut to about 1 cm, and the chitosan nonwoven fabric of thickness 0.5mm was produced with the card machine.
The above chitosan nonwoven fabric and a sheet (thickness 3 mm) coated with water-absorbing pulp of acrylate-based superabsorbent polymer particles were laminated and sewn to obtain the intended wound dressing.

About the obtained wound dressing, the test was implemented for 30 test subjects (average age 78 years old). All the subjects have stage 2 to 3 pressure ulcers in the sacrum, coccyx region, and the like.
A wound dressing was applied to the pressure ulcer site and fixed with sterile gauze and bandage from the outside. The covering material was changed every 3 days, and a total of 40 days was observed. As a result, after 25 days at the latest, all the subjects were confirmed to have increased granulation, and no exudate accumulation was observed. Moreover, the wound surface was cleaned and the healing effect was remarkable.

(Example 2)
A dope similar to that in Example 1 was prepared and dropped into hot water at 80 ° C. to obtain chitosan fine particles having a particle size of 5 μm. The chitosan fine particles were mixed with cotton at 30 wt% to prepare a chitosan-containing cotton woven fabric.
This chitosan-containing woven fabric and a superabsorbent polymer sheet were laminated, and the peripheral edge was adhered to obtain the intended wound dressing. As the superabsorbent polymer sheet, a sponge-like sheet having a thickness of 5 mm in which a small amount of an acrylate polymer was contained in a hydrophilic polyurethane foam was used.

When the wound dressing obtained was tested in the same manner as in Example 1, it was swollen and deformed by 400% or more in the thickness direction along with the absorption of exudate, and was well adhered along the affected area. After 18 days, granulation growth was confirmed in all the subjects, and the healing effect was remarkable.
