Description
【0001】
【0030】

TECHNICAL FIELD OF THE INVENTION
The present invention relates to medical devices, and more particularly to trocar systems and methods.
[0002]
BACKGROUND OF THE INVENTION
Trocar systems have been developed for many years in endoscope applications in the medical field. These trocar systems conventionally include a cannula into which a trocar, obturator or other endoscopic instrument is inserted. The trocar may or may not be shielded. Examples of shielding trocars include U.S. Pat. No. 4,654,030 entitled "Trocar" by Maul et al. And U.S. Pat. No. 4,931,042 entitled "Improved Latched Trocar Assembly" by Holmes et al. No. 4,902,280 entitled "Trocar" by Lander and U.S. Pat. No. 5,030,206 entitled "Trocar" by Lander. However, there is a need for a sealed trocar that has excellent cost performance, is small in handling and shipping, and has high performance.
[0003]
It is also known to use one or more valves in or proximal to the cannula base of the trocar system. Examples of such trocar systems having one or more valves in a cannula include US Pat. No. 5,226,891 entitled "Sealing Protector" by Bushats et al. U.S. Patent No. 5,308,336, entitled "Trocar with Floating Partition Seal" by Hart et al., "Protective Assembly for Access Devices" by Hart et al. U.S. Pat. No. 5,782,812, Hart et al., U.S. Pat. No. 5,443,452, entitled "Protective Assembly for Access Devices," and Richart et al., "Lever-Driven Partition Enclosure." No. 5,209,737, entitled "U.S. Pat.
[0004]
However, these devices have mostly complicated mechanical valves and are difficult and expensive to manufacture. Also, after use in surgical procedures such as endoscopes, valved cannulas are often discarded, especially if they cannot be sterilized. This often results in the high cost being passed on to the insurance company to which the patient is subscribed.
[0005]
Other than the cannula valves of these trocar systems, they do not use complex mechanical valves, but are still expensive and can be more difficult to manufacture. As costs rise in the healthcare industry, insurance, consumer and government agencies make efforts to reduce the price of medical devices. Also, these other valves are often inflexible and often damaged during use by a physician or medical professional during a surgical procedure. Therefore, alternative cannula and valve geometries were needed for the trocar system. There is a need for a more flexible trocar system that is less expensive, improves the handling of medical users, such as physicians, and a trocar system that is effective for a variety of endoscopic surgical procedures.
[0006]
[Means for Solving the Problems]
With the above in mind, the present invention provides a trocar system that is relatively inexpensive to manufacture system components such as cannulas, trocars and valves, thereby reducing the costs associated with the trocar system. Things. The present invention advantageously provides a trocar system that improves the handling of a physician or medical personnel during a surgical procedure. The present invention also provides a more flexible trocar system that is effective during various endoscopic surgical procedures. Further, the present invention advantageously provides a cannula and trocar that are ergonomic and easy to use. The present invention also advantageously provides a method for improving the use and handling of a trocar system during a surgical procedure.
[0007]
In particular, there is provided a trocar system having a cannula having an elongated cannula body including an intermediate portion and a distal portion having a first diameter and a base connected to the intermediate portion and having a second diameter. . Desirably, the second diameter is greater than the first diameter. The trocar system also includes a cannula body end housing removably connected to a base of the cannula body and housing at least one valve therein, each having an opening in a central portion, and adjacent to each other. First and second valves (trocar valves) disposed within the end housing, and an elongated trocar body extending through the cannula body end housing, the first and second valves and the cannula body. It is possible. Further, it is preferable that the trocar has a handle connected to the trocar body so that the trocar can be picked up and handled by a user.
[0008]
For example, the first valve is formed of an elastomeric material that deforms around a trocar body disposed therein so as to shield an outer circumferential surface of the trocar body adjacent to an inner circumferential surface of the opening of the first valve. Is preferred. The first valve has a substantially flat base surface and a substantially flat peripheral surface. The elastomeric material preferably includes a fibrous material infused with silicone material to increase the strength of the valve and to increase the sliding and shielding of the trocar body.
[0009]
Due to the structure of the cannula body end housing (hereinafter referred to as the end housing) of the cannula, during a surgical procedure or other minimally invasive procedure, the user can easily and suitably fit the end housing to damage the sample from the cannula. Can be removed without the need. Because the first and second valves are located within the end housing, the retrieved material is free from concerns about damage and other contamination from the valves. If sample removal is not required, the first and second valves allow the user to conveniently maintain system pressure and to slide and move a trocar or other instrument through the cannula.
[0010]
A pinching means extends outward from the base of the cannula body so that the user can pinch the cannula with the fingers of the hand. The system also includes a trocar having an elongated trocar body extending through the cannula. The trocar also preferably has a handle on the trocar body so that the user can grasp and handle with the fingers of the hand.
[0011]
For example, the picking means may be provided as a pair of picks. Each of the pair of tabs has a finger support web. Each finger support web is mounted substantially 180 degrees apart from each other. With this position and arrangement, for example, the user can comfortably operate and handle the cannula. Since the cannula is desirably formed of a plastic disposable material, the support web can be easily molded or manufactured with the same positives as the formation of the cannula body. The knob is three-dimensional and has a substantially ergonomic shape.
[0012]
A trocar having an elongated trocar body is extended through the cannula. The elongate trocar body has a sharp distal portion, an intermediate portion having a first diameter, and a base portion having a second diameter. Preferably, the second diameter is greater than or equal to the first diameter. The trocar has a handle connected to the base of the trocar body so that the user can grasp and handle the trocar by hand, and is slidably provided at an intermediate portion of the trocar body, and is a shielding portion urged to an extended position. This protects the sharp distal portion of the trocar body until pressure is applied to the end of the shield, and the shield slides toward the base of the trocar body with that pressure. Move to the retracted position. Desirably, the shield has a third diameter that is at least less than or equal to the second diameter.
[0013]
The present invention further provides a method of using the trocar system. The method preferably comprises the step of the user pinching the cannula using the fingers of the hand so that at least one of the finger support webs extending outwardly from the cannula body is fingered and pinched with the finger of the user. Sliding the trocar through the wound cannula body.
[0014]
The method of using the trocar is preferably such that the trocar is inserted through the cannula body, the shield is slid from the distal end of the trocar body toward the base of the trocar body by pressure applied to the end of the shield, and And a method of stopping the shrinkage of the shield using a transition region between the middle and base of the trocar.
