Claims (1)
- 【特許請求の範囲】 1. 病的な脈管を治療するためのステントであって、少なくとも2つの細長い フィラメント(2,2’)の形で埋め込み装置(70)によって脈管(21,6 0)内に導入可能であり、埋め込み中埋め込み場所において初めてその所定の形 状になる形式のものにおいて、フィラメント(2,2’)の少なくとも2つがそ の所定の埋め込み状態で、ステント(1)の長手方向の延びの少なくとも一部に わたって、互いに逆向きのらせん(3,3’)の形で存在することを特徴とする 、病的脈管を治療するためのステント。 2. フィラメント(2,2’)が例えばニチノール(Nitinol)のような超弾 性的な性質及び又は熱記憶性質を有する材料から成っていることを特徴とする、 請求項1記載のステント。 3. 互いに逆向きのらせん(3,3’)のフィラメント(2,2’)がただ1 つの線材から成っており、この線材は両方のらせん(3,3’)の結合箇所にお いて折り曲げ部又はループ(4)を、互いに逆向きのらせん(3,3’)の形成 のために有していることを特徴とする、請求項1又は2記載のステント。 4. 互いに逆向きのらせん(3,3’)のフィラメント(2,2’)が一体的 に、管形の工作物から、例えばレーザによって、切り出されていることを特徴と する、請求項1又は2記載のステント。 5. 互いに逆向きのらせん(3,3’)のフィラメント(2,2’)が2つの フィラメント線材から成っており、これらのフィラメント線材は、少なくとも1 つの結合箇所(4,5)で互いに固く結合されていることを特徴とする、請求項 1又は2記載のステント。 6. 互いに逆向きのらせん(3,3’)を形成する両方のフィラメント(2, 2’)が結合箇所(4,5)において、互いに接着、ろう接又は溶接されている ことを特徴とする、請求項5記載のステント。 7. 互いに逆向きのらせん(3,3’)を形成する両方のフィラメント(2, 2’)が結合箇所(4,5)において、両方のフィラメント(2,2’)をつか むスリーブ(5,27)によって互いに結合されていることを特徴とする、請求 項5記載のステント。 8. 結合箇所(4,5)が、ステント(1)が所定の埋め込み状態にある場合 に、ステントの外周の範囲に配置されていることを特徴とする、請求項1から7 までのいずれか1項記載のステント。 9. 互いに逆向きのらせん(3,3’)が少なくとも部分的に、それらの交差 点(24)において互いに結合されていることを特徴とする、請求項1から8ま でのいずれか1項記載のステント。 10.互いに逆向きのらせん(3,3’)の、交差点(24)に置ける結合が、 フィラメント(2,2’) の少なくとも1つに配置されている糸材(25)のループによって行われ、その 際フィラメント(2,2’)は一定の限度内で互いに相対的に可動であることを 特徴とする、請求項9記載のステント。 11.所定のらせん形状を有しているほぼ同じ長さの少なくとも3つのフィラメ ント(2,2’,12)が設けられており、その際第3の、若しくは第3以下複 数のフィラメント(12)は、その所定の形状において、少なくとも部分的に、 互いに逆向きのらせん(3,3’)を形成する両方のフィラメント(2,2’) とは異なったピッチを有していることを特徴とする、請求項1から10までのい ずれか1項記載のステント。 12.フィラメント(2,2’)がステント(1)の全長の少なくとも一部にわ たって、同じ回転方向を有していることを特徴とする、請求項1から11までの いずれか1項記載のステント。 13.少なくとも1つのフィラメント(2,2’)が、その所定の埋め込み状態 で、ステント(1,10)の外周に沿って延びる弧形の区分(29,29’)を 有していることを特徴とする、請求項1から12までのいずれか1項記載のステ ント。 14.それぞれ2つのフィラメント(2,2’)が、ステント(1,10)の長 手方向の延びの少なくとも一部にわたって、それぞれ同じような、互いに逆向き の弧形の区分(29,29’)を有していることを特徴とする、請求項13記載 のステント。 15.弧形の区分はそれらが最大に接近している範囲において、例えば、フィラ メント(2,2’)を結合しているスリーブ及び又は溶接、ろう接又は接着によ って結合され、あるいはフィラメント(2,2’)相互の一定限度内の可動性が 保証されているように、例えばフィラメント(2,2’)の少なくとも1つに配 置された糸材(25)のループによって結合されていることを特徴とする、請求 項14記載のステント。 16.フィラメント(2,2’)の2つが、それらの所定の形状において、それ ぞれ1つの交番する構造を有しており、この交番する構造においては、少なくと も1つの完全ならせんループ(30,30’)が少なくとも1つの弧形の区分( 29,29’)に続いており、この場合、フィラメント(2,2’)は互いにず らされて配置されていて、一方のフィラメント(2)のそれぞれ1つのらせんル ープ(30,30’)が他方のフィラメント(2’)の弧形の区分(29,29 ’)と交差していることを特徴とする、請求項13記載のステント。 17.ステント(1,10)が少なくとも部分的にだ円形又はD形の内腔を有し ていることを特徴とする、請求項1から16までのいずれか1項記載のステント 。 18.フィラメント(2,2’)が、それらの所定の形状において、ステント( 1)の長手方向の延びの少なくとも一部にわたって、少なくとも2つの副ステン ト(61,61’)から成る複ステントを形成していることを特徴とする、請求 項1から17までのいずれか1項記載のステント。 19.複ステントの副ステント(61,61’)がそれらのフィラメント(2, 2’)のらせんループ(30,30’)において互いに結合されていることを特 徴とする、請求項18記載のステント。 20.ステント(1)が、その所定の埋め込み状態において、少なくともその遠 位端部において、ステント(1)の内腔がほぼ8の形状を有しているように、成 形されていることを特徴とする、請求項18記載のステント。 21.殊に弾性的に変形可能な薄膜包囲体(57)が設けられていることを特徴 とする、請求項1から20までのいずれか1項記載のステント。 22.薄膜包囲体(57)が繊維織物から成っていることを特徴とする、請求項 21記載のステント。 23.繊維の薄膜包囲体(57)の織物組織内に、殊に金属から成る補強糸(5 9)が編み込まれていることを特徴とする、請求項22記載のステント。 24.フィラメント(2,2’)の少なくとも1つが、その所定の構成において 、少なくとも部分的に、こ のフィラメントの外周を越えて突出している織物組織体(45)及び又は半径方 向でフィラメントから離れて延びている繊維(48,49)を備えていることを 特徴とする、請求項1から23までのいずれか1項記載のステント。 25.フィラメント(2,2’)によって形成されている個々の隣接するらせん (46,47)及び又は弧区分(29,29’)から出ている織物組織体(45 )及び又は繊維(48,49)がその自由端部において少なくとも部分的に互い に接触していることを特徴とする、請求項24記載のステント。 26.少なくとも1つのフィラメント(2,2’)が、突出している織物組織体 (45)及び又は半径方向に突出している繊維(48,49)を含めて、包囲及 び又は巻き付けられていることを特徴とする、請求項24又は25記載のステン ト。 27.複数の部分フィラメント(55,55’)が、突出している織物組織体( 45)及び又は半径方向に突出している繊維(48,49)を含めて、ただ1つ のフィラメント(2,2’)に撚られていることを特徴とする、請求項26記載 のステント。 28.フィラメント(2,2’)から半径方向に離れて延びている織物組織体( 45)及び又は繊維(48,49)が部分的に、かつ又は半径方向の優先方向に 応じて、異なった長さであることを特徴とする、請求 項24から27までのいずれか1項記載のステント。 29.織物組織体(45)及び又は繊維(48,49)が、フィラメント(2, 2’)によって形成されている個々の隣接するらせんループ(46,47)及び 又は弧区分(29,29’)の間で、波線形の移行線(51)を形成して、少な くとも部分的に互いに接触していることを特徴とする、請求項28記載のステン ト。 30.織物組織体(45)及び又は繊維(48,49)が、フィラメント(2, 2’)によって形成されている個々の隣接するらせんループ(46,47)及び 又は弧区分(29,29’)の間で、少なくとも部分的に重なり合っていること を特徴とする、請求項24から29までのいずれか1項記載のステント。 31.織物組織体(45)が少なくとも部分的に繊維組織体から成っていること を特徴とする、請求項24から30までのいずれか1項記載のステント。 32.織物組織体(45)が少なくとも部分的に金属織物から成っていることを 特徴とする、請求項24から30までのいずれか1項記載のステント。 33.織物組織体(45)が少なくとも部分的にフリンジ状に切り込まれている ことを特徴とする、請求項31又は32記載のステント。 34.織物組織体(45)が結合手段、殊にマジックテープ結合部(64)を、 互いに重なり合っている織 物組織体の結合のために、備えていることを特徴とする、請求項31から33ま でのいずれか1項記載のステント。 35.織物組織体(45)の横断面が、フィラメント(2,2’)からの間隔が 増大するにつれて先細になっていることを特徴とする、請求項31から34まで のいずれか1項記載のステント。 36.シールステント(67,68,69)が織物組織体(45)及び又は繊維 (48,49)を有しており、この織物組織体及び又は繊維は殊に半径方向でス テント(1,10)の内腔内に突入し、かつ又は半径方向でステント(1,10 )の外周を越えて突出していることを特徴とする、請求項1から35までのいず れか1項記載のステント。 37.フィラメント(2,2’)の少なくとも1つがマークを備えており、この マークはステント(1)を診断手段、例えばレントゲン診断装置によって外部か ら観察することを可能にすることを特徴とする、請求項1から36までのいずれ か1項記載のステント。 38.フィラメント(2,2’)が、ステント(1)の長手方向の延びに沿って 所定の間隔で、それぞれ互いに逆向きに巻かれた、ステントの外径を決定するル ープ(66)を形成していることを特徴とする、請求項1から37までのいずれ か1項記載のステント。 39.請求項1から38までのいずれか1項記載の特 徴を具備したステント(1,10)を脈管(21)内に埋め込むための埋め込み 装置において、2つのプッシャ(71,72)がカテーテルによって脈管(21 )内に導入可能であり、その際外側のプッシャ(71)は軸方向の孔を有してお り、この孔によって、内側のプッシャ(72)が案内可能であり、外側のプッシ ャ(71)はその遠位端部(74)に、ステント(1,10)の近位端部をカテ ーテル(22)の内部で支えるための支持面を有しており、内側のプッシャ(7 2)には、ステント(1,10)の遠位端部を解離可能に結合するための手段( 73)が配置されていることを特徴とする、埋め込み装置。 40.内側のプッシャ(72)は、ステント(1,10)の遠位端部と結合する ために、ねじ山(73)を有していることを特徴とする、請求項39記載の埋め 込み装置。 41.ステント(1,10)が、内側のプッシャ(72)のねじ山(73)に適 合した結合手段を有していることを特徴とする、請求項40記載の埋め込み装置 。 42.外側のプッシャ(71)の支持面の範囲に、ステント(1,10)の近位 端部と解離可能に結合する手段が設けられていることを特徴とする、請求項39 から41までのいずれか1項記載の埋め込み装置。 43.請求項1から38までのいずれか1項記載の特 徴を具備したステント(1,10)を脈管(21)内に埋め込むための埋め込み 装置において、カテーテル(22)によって脈管(21)内に導入可能なプッシ ャ(72)が設けられており、このプッシャは、その遠位端部に、埋め込み過程 中ステント(1,10)を支持する支持手段(78,86)を有していることを 特徴とする、埋め込み装置。 44.遠位端部の支持手段として、ステント(1,10)のフィラメント(2, 2’)の交差点あるいはフィラメント(2,2’)を結合するスリーブ(27) を案内するためのフォーク(78)が設けられていることを特徴とする、請求項 43記載の埋め込み装置。 45.遠位端部の支持手段として、はさみ装置が設けられており、このはさみ装 置は埋め込み過程中フィラメント(2,2’)の、ステント遠位端部においてフ ィラメントを互いに結合しているループ(4)あるいは折り曲げ部及び又はフィ ラメントを結合しているスリーブ(27)を、固く、しかし解離可能に、つかん でいることを特徴とする、請求項43記載の埋め込み装置。 46.遠位端部の支持手段として、フックエレメント(83)が設けられており 、このフックエレメントは、ステント(1,10)のフィラメント(2,2’) をその近位端部において互いに結合している端部スリーブ(81)に相応して形 成されているノッチ(82 )内に形状結合で、かつカテーテル(22)の内部で軸方向に不動に、結合可能 であることを特徴とする、請求項43記載の埋め込み装置。
