Abstract

393,010. Suturing; tying and knotting. RENVALL, E. J., 60, Norr Malarstrand, Stockholm. Jan. 14, 1932, No. 1161. [Class 81 (ii).] A hµmostat of the pivotal forcep type for the surgical tying of veins is provided at its front end with two guiding channels or the like for the tying thread which terminates at a point behind said front end. The pivotal legs 2, 3 of the hµmostat 1 are provided with longitudinal channels 6, 7, Fig. 3, near the rear openings of which are located the edges 8 of knives 9, 10 pivoted to the legs at 11, 12. A lazytongs 15.. 18 is pivoted at one end to the legs and carries a pulley 19. The thread 21 is made into a double loop, Fig. 5, both sections 22, 23 of which are pushed over the nose of the instrument, the ends being inserted from the front through channels 6, 7 and tied together round the pulley 19. The vein 20 is gripped in the jaws and continued pressure on the handles tensions the thread 21 causing the loop to slide on to the vein and tighten round the same. Pressure on the arched portions of the knives 9,10 serves to sever the thread ends. In a modification, Fig. 6, an arm 25 is pivoted to the arms 17, 18 and is slotted at 26 to receive a guide pin 27. A flat spring 28 serves for securing the thread ends. The arms 15, 16 are slotted to receive pins 31, 32 on the legs and also screws attached to links 37, 38 pivoted to the knives. Flat springs 39, 40 are adapted to retain the loop in position. According to Fig. 8, the two channels are arranged in the same leg and merge together behind the nose. An arm 43, having a spring 44 for securing the thread, is slidable along the leg 2 and formed with a rack 45 co-operating with. a pinion 47. A pinion 48 on either side of the latter co-operates with a toothed segment 49 on the leg 3. The knife 10 is actuated by a link 42. Co-operating detent and abutment 50, 51 limit the opening movement.
