1374945 Mounting microfilm MEDICAL IDENTIFICATION SYSTEMS Inc 23 Jan 1973 3471/73 Heading A4X A health information card comprises a rectangular paper body 16 having printing on one surface and folded along a central line 18 into two overlapping card sections 12, 14 with the printing on the outer surfaces thereof, each card section having in aligned relation with the other operative portions 34 adjacent said fold line 18 bounding a removed area 32 serving as a display window, a microfilm 20 having data 22 in a central location and border areas 24 thereabout occupying an operative display position in said window, the border areas 24 being engaged by said operative portions 34 to minimize slippage, and a transparent plastics cover 36 larger than body 16 having connected front and back panels 38, 42 disposed about the body 16 and having overlapping marginal areas (44) heat sealed together. As shown, the removed area 32 is rectangular and has one side originating at the fold line 18.
