Heading A5R A surgical instrument comprises a gun and a cartridge portion (50) mounted thereon, the cartridge portion comprising a rail assembly including a pair of parallel anvil 52 with a knife guide 54 extending between and parallel thereto, a suture carrier slidable on the rails assembly including a plurality of wire sutures 148 ejectable by a pusher which drives the sutures against the anvils, and a knife 174, the gun including means to cause a tube held against the anvil to be sutured and severed by the knife when a trigger 14 (see Fig. 1) on the gun is operated. The gun includes a barrel 20 containing a rod 22 with a collar 24 attached to a rod 18 and biased by a spring 38, a sleeve 26 mounted on the rod being biased by spring 42. The suture carrier comprises two barrels 102, rotatable by a rachet plate 134, which carry sutures in slots on their peripheries. One end of a cam follower 128 is slidably located for transverse movement in plate 118 and also attached to the rachet plate 134, so that longitudinal movement of plate 118 results in transverse movement of rachet plate 134. In operation, the gun trigger is moved to cause rod 18 to move forwards and hence cause the cartridge portion to slide on the rail assembly to the position of Fig. 5. Further movement of the trigger causes rod 22 to move relative to barrel 26 against spring 42 and case the knife to move forwards while sutures 148 are driven against the anvils and wrapped around the tube by push rods 164. Collar 24 also engages and moves the cam plate 118 forward which in turn causes rachet plate 134 to move transversely. The tube is then sutured and severed. When the trigger is released the springs 42 and 38 drive the cartridge portion back along the rails to the position shown in Fig. 4. As the rachet plate 134 returns it engages teeth on barrels 102, rotating them to cause another set of sutures located thereon, to align with push rods 164 for the next operation. The cartridge portion is detachably lockable to the gun by means of a guard 82. The sutures (Figs. 6, 10, 11, not shown) have pointed forward ends (150) a rear end (154) defined by a right angle and a hump (152) adjacent the rear end. They are wrapped round the tube in a loop and compressed by the hump (152) being collapsed (Fig. 11).
