PROTHESE DISCALE APTE A ETRE INTERPOSEE ENTRE DEUX VERTEBRES CONSECUTIVES PROSTHETIC DISCALE TO BE INTERPOSED BETWEEN TWO CONSECUTIVE VERTEBERS
La présente invention concerne les prothèses discales aptes à être 5 interposées entre deux vertèbres consécutives en remplacement du disque intervertébral situé originellement entre ces deux vertèbres. II est déjà connu des prothèses discales aptes à être implantées entre deux vertèbres consécutives pour remplacer un disque intervertébral endommagé, mais elles ne sont en fait pas polyvalentes, c'est-à-dire ne 10 permettent pas de réaliser le remplacement du disque intervertébral tout en autorisant une certaine liberté de rotation des deux vertèbres entre lesquelles elles sont implantées. Aussi, la présente invention a-t-elle pour but de réaliser une prothèse discale apte à être interposée entre deux vertèbres consécutives en 15 remplacement du disque intervertébral situé originellement entre ces deux vertèbres, qui réponde de façon très satisfaisante aux buts définis ci-dessus et qui, tout en étant d'une structure relativement simple, puisse être mise en place aussi bien par voie antérieure que par voie postérieure. Plus précisément, la présente invention a pour objet une prothèse 20 discale apte à être interposée entre deux première et seconde vertèbres consécutives en remplacement du disque intervertébral situé à l'origine entre ces deux vertèbres, caractérisée par le fait qu'elle comporte : • un premier plateau comportant une première surface de contact avec la première vertèbre, 25 • un second plateau comportant une seconde surface de contact avec la seconde vertèbre, ce second plateau étant en outre délimité par au moins deux couples de faces latérales, ces faces latérales étant sensiblement perpendiculaires à la seconde surface de contact, ce second plateau étant entièrement compris dans l'espace délimité par les plans passant 30 respectivement par ces faces latérales, • un corps intermédiaire apte à être interposé au contact glissant entre les deux dits premier et second plateaux, • des moyens pour commander le déplacement du dit corps intermédiaire entre deux première et seconde positions, le corps intermédiaire et les premier et second plateaux étant agencés les uns par rapport aux autres de façon que, dans la première position du corps intermédiaire, les deux plateaux soient à une distance donnée l'un de l'autre et que, dans la seconde position du corps intermédiaire, les deux plateaux soient à une distance l'un de l'autre supérieure à ladite distance donnée, et • des moyens aptes à lier ledit corps intermédiaire avec le premier plateau et la première vertèbre quand ce dit corps intermédiaire est dans sa seconde position. En référence aux figures annexées, la présente invention concerne une prothèse discale apte à être interposée entre deux première et seconde vertèbres consécutives schématiquement illustrées en V1, V2, pour remplacer le disque intervertébral situé à l'origine entre ces deux vertèbres et qui s'est révélé avoir été endommagé. La prothèse discale comporte un premier plateau 10 comportant une première surface de contact 11 avec la première vertèbre VI et un second plateau 20 comportant une seconde surface de contact 22 avec la seconde vertèbre V2, ce second plateau étant en outre délimité par au moins deux couples de faces latérales 24, 26 sensiblement perpendiculaires à la seconde surface de contact 22, bien que pouvant ne pas être rigoureusement planes mais considérées comme planes. De plus, ce second plateau est entièrement compris dans l'espace délimité par les plans passant respectivement par ces faces latérales 24, 26. La prothèse discale comporte en outre un corps intermédiaire 30 apte à être interposé au contact glissant entre les deux premier et second plateaux 10, 20, et des moyens 40 pour commander le déplacement de ce corps intermédiaire 30 entre deux première et seconde positions P1, P2. De plus, le corps intermédiaire 30 et les premier et second plateaux 10, 20 sont agencés les uns par rapport aux autres de façon que, dans la première position P1 du corps intermédiaire, illustrée en traits-doubles points sur les figures 1 et 2, les deux plateaux 10, 20 soient, figure 1, à une distance donnée l'un de l'autre dnd et que, dans la seconde position P2 du corps intermédiaire, illustrée en traits continus sur les figures 1 et 2, les deux plateaux soient à une distance Dd l'un de l'autre supérieure à la distance donnée dnd, cette différence entre les valeurs Dd et dnd constituant ce que les techniciens désignent par "distraction" entre deux vertèbres consécutives. La prothèse discale comporte aussi des moyens 50 aptes à lier le corps intermédiaire 30 avec le premier plateau 10 et la première vertèbre VI quand ce corps est dans seconde position P2, ou au moins lorsqu'il arrive dans sa seconde position P2 en venant de sa première position P1. Selon une autre caractéristique de l'invention, le corps intermédiaire 30 comporte, figure 1, une première partie de surface latérale 31 et deux deuxième et troisième parties de surface latérale consécutives 32, 33, la première partie de surface latérale d'une part et les deuxième et troisième parties de surface latérale d'autre part étant aptes à coopérer par glissement, lorsque le corps passe de sa première position P1 à sa seconde position P2 et réciproquement, avec respectivement une première surface de frottement 12 réalisée sur le premier plateau 10 et une seconde surface de frottement 28 réalisée sur le second plateau 20. II est précisé que les premier et second plateaux 10, 20 et le corps intermédiaire 30 sont agencés de façon que ces première et seconde surfaces de frottement 12, 28 soient sensiblement situées en regard l'une de l'autre, comme cela apparaît sur les figures 1 et 2. Selon une réalisation particulièrement avantageuse, le corps intermédiaire 30 est apte à passer de sa première position P1 à sa seconde position P2 et réciproquement, au moins par rotation autour d'un axe de rotation 34 sensiblement parallèle à deux 24 des faces latérales du second plateau 20. Il est souligné que, selon la configuration de la première partie de surface latérale 31 et des deux deuxième et troisième parties de surface latérale successives 32, 33, cet axe de rotation 34 peut se déplacer parallèlement à lui-même quand le corps intermédiaire 30 passe de sa première position à sa seconde et réciproquement. A des fins d'industrialisation, surtout dans le cas d'une application comme celle qui sera explicitée ci-après notamment en regard de la figure 2, la seconde surface de frottement 28 réalisée sur le second plateau 20 est une surface de révolution autour d'au moins un point 29, un seul point si elle est sphérique, un ensemble de points situés sur une droite si elle est cylindrique de révolution. Selon le mode de réalisation illustré sur les figures 3 et 4, ce point 29 est situé dans l'espace compris entre les deux plans 70, 71 passant respectivement par les deux faces latérales opposées 24 du second plateau 20 qui sont parallèles à l'axe de rotation 34 défini ci-dessus. Cette réalisation trouve une application dans le cas où il est prévu d'implanter une seule prothèse discale entre les deux vertèbres, généralement par voie antérieure, par exemple pour une nucléo-plastie. Cependant, pour une autre application qui sera explicitée notamment en référence à la figure 2, cette surface de révolution autour d'au moins un point 29 est réalisée sur le second plateau 20 de façon que ce point 29 soit situé, comme illustré sur les figures 1 et 2, en dehors de l'espace compris entre les deux plans 70, 71 définis ci-dessus. Selon un mode de réalisation particulièrement avantageux, les moyens 50 définis ci-avant, aptes à lier le corps intermédiaire 30 avec le premier plateau 10 et la première vertèbre V1 quand le corps est dans seconde position P2, sont constitués, comme illustré sur les figures, par au moins une lame 51 solidaire du corps 30 de façon que, lorsque ce dernier passe de sa première position P1 à sa seconde position P2, la lame 51 se déplace dans une rainure partielle 52, figure 1 et 2, réalisée dans le premier plateau 10. De plus, cette lame 51 présente des dimensions et est positionnée sur le corps 30 de façon que, lorsque le corps 30 est dans sa seconde position P2, la lame 51 émerge de la première surface de contact 11 du premier plateau 10 pour s'ancrer dans le corps osseux de la première vertèbre V1, comme illustré en trait continu sur les figures 1 et 2, par exemple en pénétrant par découpe la partie osseuse de la vertèbre VI. Cette lame sera ensuite emprisonnée dans l'os par ostéosynthèse pour lier le plateau 10 et le corps 30 à la vertèbre V1.
