Abstract
Description
The present invention relates to an intracorporeal system for heating the human body or homeothermic animals during surgical treatments. By intracorporeal means in the present invention that the heating elements are placed inside the body on which the treatment takes place. In the treatment of peritoneal cancer in humans, one of the methods of treatment includes the combination of intra-abdominal chemotherapy and hyperthermia, immediately following a surgical cytoreduction. Indeed, it has been found that the effect of anti-cancer agents, usually of the antimitotic type, was potentiated at a temperature of 42 ° C to 43 ° C. Thus, the treatment consists of filling the abdomen of the patient on whom an operation has been performed with a liquid containing one or more anti-cancer agents and maintained at a temperature of the order of 42 ° C. to 43 ° C. One of the current difficulties is that, to maintain this hyperthermia, bulky, complex and long extracorporeal circulation is available only to systems that are relatively implementable and extremely expensive. These extracorporeal circulation systems include a heat source, a reservoir, pumps, heat exchangers and drain circuits for circulating a hot liquid to the abdomen and then recovering it. The difficulty of implementation and the cost of the apparatus limit today the diffusion of these techniques of treatment. In addition, another difficulty of these systems is that to maintain a hyperthermia of 42 ° C to 43 ° C in the patient's abdomen, the liquid must be fed through the inlet drains to a higher temperature, due to continuous heat loss, this temperature ranging from 44 ° C to 48 ° C. However, there is a risk of burns of RAT001-FR-4 TEXTE REMOVAL JS / CD - 2 - human tissue from 44 ° C. Two variants of implementation of this treatment exist, closed belly or open belly. With a closed belly, the risk of burning prompts to preferentially position the inlet drains under the right diaphragm and the left diaphragm respectively, at a distance from the intestine, which is the viscera most sensitive to hyperthermia. The liquid flowing between the inlet drains and the outlet drain placed in the pelvis tends to take lateral preferential paths, the most direct paths between the ends of said drains. As a result, the central regions of the abdomen may be poorly exposed to hyperthermia and the effectiveness of treatment may be reduced. In the technique of open-mouth operation, it is necessary to constantly stir the liquid manually to ensure satisfactory diffusion of hyperthermia and to prevent burns. For example, it will be possible to refer to the French patent FR 2,819,726 in the name of the Applicant which describes a device applicable in this type of treatment and which allows manual mixing of the hyperthermic treatment liquid while effectively protecting the manipulator from exposure to the agents. anti-cancer. With a closed belly and open belly, problems of circulation of the heated liquid in the drainage circuits can be encountered, in particular by suction of the viscera on the outlet drain which sucks them or by obstruction of the circuits by small clusters of tissues mobilized by the treatment procedure. These liquid circulation problems require specific manipulations that lead to temperature drops. Moreover, in the context of long surgical operations, particularly on the trunk and in which an opening on the pleural cavities, RAT001-FR4 TEXTE DEPOSE JS / CD-3-pericardial or abdominopelvic is practiced, there is a phenomenon heat loss leading to hypothermia in patients. Even mild hypothermia has been shown to increase the risk of infection, myocardial infarction, postoperative hemorrhage, blood transfusion and increase the duration of the necessary postoperative mechanical ventilation. delay the patient's awakening. In order to fight against this hypothermia, various heating systems all having an extracorporeal heat source associated with a pump, an engine and ducts have been devised. Thus, there are known mattresses, heating blankets with forced air, clothes with water circulation or heat transfer heat plates.
These heating systems are placed outside the body of the patient and allow him to spread some heat to fight against heat loss due to surgical opening. The object of the present invention is to obviate the drawbacks of these known systems and to propose an intracorporeal heating system of simple design, inexpensive and making it possible to avoid risks of burns in the context of hyperthermic treatments, for example. In this respect, the subject of the present invention is a system for heating the human or animal body during surgical treatments, remarkable in that it comprises at least one heating element placed inside the body, in a natural cavity, said heating element comprising a solid or viscous and electrically resistant heating material, providing heat by ohmic heating. It is understood that the system according to the invention avoids the need for extracorporeal circulation systems of liquids, particularly bulky RAT001-FR-4 TEXT JS / CD REMOVED and expensive. In addition, with an electric resistance heating, we can see all the advantages of fineness of regulation and responsiveness of such a system. In the context of a closed abdominal type intra-abdominal chemo-hyperthermia, the system according to the invention ensures a homogeneous distribution of hyperthermia to the entire abdominopelvic cavity, without risk of burning. In the case of the open-bellied variant, the system will dispense with constant mixing of the contents of the abdomen. According to one characteristic of the invention, the heating material is an electrically resistant material; the heat emission is then provided by ohmic heating. Advantageously, a metallic material, such as copper or aluminum, will be used. But of course, any other synthetic polymer or material capable of releasing heat when it is electrically powered can be implemented without departing from the scope of the present invention.
Other advantages and features will become more apparent from the following description of several variants given as non-limiting examples of the invention. According to a first preferred embodiment, the heating element is in the form of a flexible cable. Those skilled in the art will advantageously choose the diameter and the length of said cable and the flexibility thereof so that a surgeon can route this cable in the boxes of the abdomen at which it wishes to bring heat , and can even form loops in the largest boxes, to increase the calorie intake. An alternative embodiment of this cable is a multi-segment cable with several heating zones provided RAT001-FR-3 TEXT REMOVES JS / CD 4 -5- each independently of temperature control means. According to another embodiment, the heating element may be in the form of heating plates, for example of oval and curved shape. These plates may be made entirely of a resistive material or have a winding or mesh with a resistive wire around a support member, the latter being preferably made of a material capable of accumulating and / or distribute the heat produced. According to another embodiment of the invention, the heating element consists of an integrated heating resistance boiler. Advantageously, the resistor (s) will be immersed in a liquid or a gel capable of accumulating and / or diffusing the heat produced, such as water for example. According to another characteristic of the invention, the heating element comprises a coating consisting of a biocompatible and thermally conductive insulator allowing direct contact with the organs and tissues. Such a material will advantageously be silicone. According to yet another characteristic of the invention, the heating element may be positioned in a protective cage or spongy material avoiding direct contact of the heating element with the organs so that said element may possibly exceed a temperature of 43. --44 degrees without risk of burns to the surrounding tissues.
Finally, according to an important characteristic, the system comprises means for regulating the temperature emitted by the heating element. These means are advantageously constituted by rheostat and thermostat couples. RAT001-EN-4 INSTALLATION TEXT JS / CD - 6 - It goes without saying that the system according to the invention will find preferential application in the context of intra-abdominal hyperthermia chemotherapy treatments in which the objective is to maintain the fluid. containing the antimitotic agent (s) at an intra-abdominal temperature of 42 ° C to 43 ° C. The system according to the invention may be used in the context of laparoscopic chemo-hyperthermia, the cable being introduced into the abdomen by means of a trocar. This system can also be used in the context of thoracotomy or thoracoscopy thoracoscopic thoracoscopic chemohyperthermia with open or closed thorax. The system according to the invention will also be implemented in the context of surgical or veterinary operations on the trunk, with a heating temperature kept below 40 ° C-41 ° C, in order to maintain the central temperature to a value close to normal. It can be placed in the abdominopelvic cavity, pleural cavity or pericardial cavity depending on the type of procedure. In the context of such use, the system according to the invention will obviously be placed out of the field of surgical intervention in one or more lodges located at a distance from the latter. Liquid may be poured into these boxes, in order to open them and obtain a diffuse distribution of heat. Again, in the form of a cable, the system can be used in the context of endoscopic surgery.
