Abstract
Description
CYTOLOGICAL PREPARATIONS
The present invention relates to a device for analyzing cytological preparations making it possible to detect cellular abnormalities on a sample taken from at least one individual.
The object of the invention also relates to a method making it possible to obtain analyzes with a more reliable result.
Another object of the invention is to provide a device and a method for analyzing cytological preparations, the implementation of which allows a high number of analyzes per unit of time without necessarily using the aid of an automatic machine.
In its preferred application, the invention relates to a device and an analysis method for detecting precancerous, early cellular abnormalities, or even advanced cancers, on cell samples taken from the cervix.
In a known manner, cytological analyzes consist in taking, from the human body or in a cavity thereof, a sample containing cells, such a taking being for example carried out at the level of the cervix, then in performing one or more elution / centrifugation stages with the final aim of obtaining a preparation containing only the cells to be analyzed.
This cytological preparation must then be deposited on a microscope slide to form an imprint obtained by evaporation or aspiration of the eluents, said imprint being then treated with colored reagents known to those skilled in the art to detect by subsequent examination under a microscope d 'possible cellular anomalies.
The deposit with a spatula of a cytological preparation on a slide has the well-known drawback of damaging or masking cells which may be abnormal or even malignant cells, which will then no longer be detectable by subsequent microscopic preparations and observations. Furthermore, only part of the cell population sampled is found on the slide, the rest (about 80%) being discarded with the spatula.
In an analysis laboratory, in addition to its unreliable aspect (20 to 25% of false negatives), this process does not have the advantage of being very fast.
To overcome these drawbacks, various devices or methods have been proposed for carrying out the deposition and sedimentation of a cytological preparation on a slide.
Un tel dispositif correspond par exemple à celui décrit dans le document DE-U-91 04 438.These devices essentially consist in using a tube, or chamber, plated on a microscope slide to form on this one single imprint of a cytological preparation. Such a device corresponds for example to that described in document DE-U-91 04 438.
In order to be able to carry out a greater number of analyzes per unit of time, various manufacturers have proposed automata bringing together, in one or more trays, several chamber / slide assemblies so as to successively carry out the sedimentation operations and cell staining by stages in which elutions and aspirations of the supernatant succeed one another. Such a device of this type is for example described in document EP 0 590 506.
Such devices have the advantage of providing a shorter treatment time per blade, but have the disadvantage of requiring an expensive investment in equipment. In addition, the removal of the supernatant by aspiration, an operation which must be repeated about ten times, can remove cells from subsequent observation under a microscope and lead to a false negative diagnosis.
The object of the present invention is therefore to propose a method for the analysis of cytological preparations which can be implemented without necessarily requiring the use of an automated device, by virtue of a relatively simple and inexpensive device, which also makes it possible to obtain diagnoses with a greater reliability.
- un ensemble de chambres cylindriques en un matériau plastique, pourvues chacune d'une ouverture supérieure et d'une ouverture inférieure munie d'une collerette;
- un ensemble de lames de microscope sur lesquelles seront formées des empreintes de préparations cytologiques;
To this end, the subject of the invention is an analysis device for depositing on microscope slides cytological preparations of at least one sample of cells, comprising
- A set of cylindrical chambers made of a plastic material, each provided with an upper opening and a lower opening provided with a collar;
- a set of microscope slides on which fingerprints of cytological preparations will be formed;
a rigid upper frame, of generally parallelepipedal shape, provided with as many openings as there are chambers, intended to hold said chambers and to position their lower openings in the same plane, and
- a rigid lower frame, of generally parallelipiped shape, provided on its surface with as many housings as microscope slides, said analysis device being characterized in that the upper frame is configured to position the chambers in pairs above each blade disposed in a housing of the lower frame, and in that the upper frame and the lower frame comprise assembly / disassembly means making it possible to position and seal the junction between the lower opening of a chamber and the surface of a blade.
Thus, the device makes it possible to have two fingerprints on a single slide, which can lead to a more reliable diagnosis as will be explained below. To obtain these two imprints on the same blade, it is advantageous to combine the two chambers which must form the imprint on a single blade in the same module, thereby reducing the cost of this support discarded after use. To facilitate the assembly operations of the device, it is also advantageous to provide protuberances, such as fins ensuring the wedging of the chambers in the upper frame to allow easier handling to assemble said upper frame on the lower frame.
- la figure 1 est une vue éclatée en perspective de l'ensemble du dispositif;
Although the pressure exerted by the assembly system may be sufficient to establish the necessary seal between the lower opening of a chamber and the blade, it is also advantageous to establish at this level an additional seal. The most usual way is to provide in the periphery of said lower part a groove intended to receive an O-ring, which may require additional manipulation to put it in place. According to the present invention, this drawback can be eliminated by forming, during the manufacture of the chambers or the pairs of chambers, either a lip inclined towards the interior of the chamber, or a compressible bead. The device according to the invention, its characteristics and advantages will appear more clearly on reading the detailed description below given by way of illustrative and nonlimiting example with reference to the appended figures in which
- Figure 1 is an exploded perspective view of the entire device;
FIG. 2 is a perspective view of a blade and a pair of chambers, and
- Figures 3 and 4 are sectional representations along lines III-III and IV-IV of Figure 2 at the blade / chamber junction.
FIG. 1 represents an exploded perspective view of the entire device, the middle part cut away showing that the number of impressions that can be made with said device is only limited by the length of said device. Preferably, the length of said device must make it possible to set up 12 microscope slides of standard format, and therefore to take 24 impressions. This device essentially comprises two frames 10, 20 for assembling and ensuring the seal between the chambers 1a, 1b and the associated blades 2 in order to make two impressions 3a, 3b on each blade.
The upper U-shaped frame has two parallel series of openings 11a, 11b on its junction face, and openings 17 on its sides. The openings 11a, 11b are intended to receive the outer tubular part of the chambers 1a, 1b. The openings 17, which can have any shape, are intended on the one hand to lighten the device as a whole, on the other hand to facilitate the circulation of ventilated air for the formation of imprints. This upper frame further comprises at each of its ends an assembly device 15, formed by a bore 15a which will be used for positioning and a hook 15b of a locking system called "toggle". This locking device is supported by a reinforcement 18 provided between the two sides of the U-shaped structure. The frame 10 can be produced either by machining or molding a metal, or by assembling different parts.
The lower frame 20 also in the shape of a U, but inverted with respect to the upper frame 10, has on its surface oriented towards the frame 10 housings 22 intended to receive the blades 2. In order to facilitate the handling of the blades 2, each housing 22 has on the edge of the frame a recess 23. It will further be observed that, according to the embodiment shown, the width of the lower frame 20 is substantially the same as the length of the microscope slides, but greater than the width of the upper frame 10 .
This configuration in fact makes it possible to leave visible a portion 4 of the end of each blade 2 in order to affix on said portion means of identification of the cytological preparations subjected to analysis, for example in the form of a bar code, as is more visible in Figure 2. It also allows to set up a bar 30 made of a ferromagnetic material and intended to cooperate with magnets disposed at each free end of the frame 20 to hold the blades 2 on said frame during disassembly. The ends of the frame 20 also include the complementary locking elements 25 intended to cooperate with those carried by the frame 10, namely a pin 25a and a loop 25b of the latch device. The lower frame 10 can be produced as indicated above for the upper frame 10 and includes, in its sides, openings referenced 27, having a function equivalent to that of the openings 17 of the frame 10.
Referring now more particularly to Figures 2, 3 and 4, there is shown the disposable products of the device according to the invention. In Figure 2 we see in particular a perspective view from below, two chambers 1a, 1b joined by the same base 5, the dotted representation showing the flanges 5a, 5b at the bottom of said chambers if these were separated. Figures 3 and 4 are enlarged sections of embodiments of the additional sealing means. In Figure 3 there is shown a bead 8 formed at the same time as the chamber la, but with a material having greater elasticity. In Figure 3 this sealing means consists of a lip 7 inclined towards the inside of the chamber and coming integrally with it. FIGS. 3 and 4 also show an embodiment of a means for wedging the chambers in the upper frame 10. This wedging means consists of fins 6 extending radially between a portion of the outer wall of a chamber and a portion of the collar 5a, 5b.
After having described the structure of the device, we indicate below the analysis method that said device allows to implement, with reference essentially to Figure 1.
First, in a known manner, a cytological preparation is carried out in a liquid medium, for example distilled water, no longer containing undesirable biological material, from a sample taken from the human body or in a cavity thereof. . Microscope slides 2 are available in the housings 22 of the lower frame 20, each slide having previously been provided with a locating means, such as a bar code. The cylindrical chambers, or pairs of chambers 1a, 1b are blocked in the corresponding openings 11a, 11b of the upper frame 10, thus allowing easy manipulation of the upper frame / chamber assembly for its positioning on the lower frame 20 by means of the pins. 25a. The sealing of the assembly is then ensured by locking the latching device 15b, 25b. An aligned portion of the same cytological preparation is then deposited in each pair of chambers disposed above the same slide. After having been left to stand for approximately 15 minutes in order to obtain cell sedimentation, the supernatant is removed and drying is carried out in a ventilated oven at 50-600C. These operations being completed, the blades 2 can be removed from the device by unlocking the frames 10, 20, the blades being during this operation preferably maintained by the bar 30 with magnetic fixing. Each slide then has two fingerprints corresponding to a single sample corresponding to an average number of 115,000 cells, that is to say a number much greater than that obtained with the methods allowing only one fingerprint to be produced per slide. After having performed the colorings, which will not be described here as being well known to those skilled in the art, observation under a microscope is thereby facilitated. The reliability of the diagnosis resulting from this observation is further increased if each of the two chambers is filled with an aligned portion corresponding to a different dilution, for example 1/3.
It will be appreciated that the device which has just been described is susceptible, on the part of a person skilled in the art, to numerous variations, both in terms of the frames and of the disposable material after use and in particular of the chambers or pairs of chambers, without leaving of the scope of the present invention.
