Description
Implant communication system and method for communicating with an implantable medical device
The invention relates to an implant communication system. Furthermore, the invention relates to a computer implemented method for communicating with an implantable medical device.
US 2020/0357513 Al discloses techniques for remote monitoring of a patient and corresponding medical device(s). The remote monitoring comprises providing an interactive session configured to allow a user to navigate a plurality of sub sessions, determining a first set of data items in accordance with a first sub session, the first set of data items including the image data, determining a second set of data items in accordance with a second sub session of the interactive session, determining, based at least in part on the first set of data items and the second set of data items, an abnormality, and outputting a post-implant report of the interactive session.
WO 2007/070669 A2 discloses a patient management device for portably interfacing with a plurality of implantable medical devices and method thereof is presented. Permission to interrogate one or more implantable medical devices is authenticated. Patient device data is individually exchanged through interrogation of at least one authenticated implantable medical device through short range telemetry. External device data is exchanged via communication with at least one external device through long range telemetry. At least one of the patient device and external device data is maintained contemporaneously to execution of operations to perform one or more of relay, processing, and outputting of the patient device and external device data subsequent to the interrogation of the implantable medical device.
Conventional implant systems hence comprise a programmer capable of reading out data from and writing data to an implantable medical device. Such proprietary programming devices can also be used solely for reading out data from the implantable medical device.
If a patient wearing an implantable medical device is admitted to a hospital and/or medical practice due to an acute medical condition data is read out from the implantable medical device by means of said programmer by a trained expert and/or medical practitioner in order to evaluate medical parameters of the patient recorded by the implantable medical device as well as technical parameters of the implantable medical device in order to evaluate if the implantable medical device is operating normally or whether a technical issue is present.
Programmers however are not universally available at all medical facilities such as hospitals and/or medical practices. Furthermore, such programmers may be difficult to use for untrained medical staff, acceptance of such devices is generally limited. In addition, programming devices are equipped with many functions which, if used incorrectly, can adversely affect the health of the patient / implant carrier.
Furthermore, programmers give limited visual feedback to the user (e.g. a health care provider, a physician or another person of medical staff) and the user interacts with it only in a physical way by pressing buttons.
It is therefore an object of the present invention to provide an improved implant communication system and method that is usable not only by medical practitioners but also by untrained medical staff and is generally widely accepted and universally available.
The object is solved by an implant communication system having the features of claim 1.
Furthermore, the object is solved by a computer implemented method for communicating with an implantable medical device having the features of claim 13.
In addition, the object is solved by a computer program with program code to perform the method according to the present invention when the computer program is executed on a computer having the features of claim 14.
Moreover, the object is solved by a computer-readable data carrier containing program code of a computer program for performing the method according to the present invention when the computer program is executed on a computer having the features of claim 15.
Further developments and advantageous embodiments are defined in the dependent claims.
The present invention provides an implant communication system, comprising a front-end communication unit comprising a first communication interface configured to read out data from an implantable medical device, and a user communication device comprising an app communicatively connected to the front-end communication unit, wherein the front-end communication unit further comprises a second communication interface configured to transfer the data read out via the first communication interface to the user communication device.
At this point, it should be explicitly pointed out that the front-end communication unit and the user communication device are two separate devices.
Furthermore, the present invention provides a computer implemented method for communicating with an implantable medical device comprising the steps of reading out data from the implantable medical device by means of a front-end communication unit comprising a first communication interface, and transferring the data read out via the first communication interface to the user communication device via a second communication interface of the front-end communication unit, wherein the user communication device comprises an app communicatively connected to the front-end communication unit.
In addition, the present invention provides a computer program with program code to perform the method according to the present invention when the computer program is executed on a computer.
Moreover, the present invention provides a computer-readable data carrier containing program code of a computer program for performing the method according to the present invention when the computer program is executed on a computer.
An idea of the present invention is to provide medical staff with an easy to use and widely available tool for accessing a patient’s implantable medical device for the purposes of reading out data related to a condition of the patient, i.e. medical and/or technical parameters recorded by the implantable medical device.
By including an app (e.g. a companion app) on the user communication device (e.g. a tablet or smartphone), the use cases of the front-end communication unit can be extended, the frontend communication unit is better handleable by a wider range of users, it is less error prone and the user satisfaction and compliance with the workflow of the front-end communication unit can be increased.
Further, due to steady increasing means to interact with technology and increasing numbers of digital natives becoming medical personal, users might expect to use medical devices the same way they are used to use consumer devices - meaning also connected devices controlled by or enhanced by companion apps.
The implantable medical device may be formed by a purely therapeutic implant, a purely diagnostic implant and/or an implant with therapeutic and diagnostic functions.
An example of a purely therapeutic implant / implantable medical device is, e.g. a stimulator / electrode for deep brain stimulation. The therapy consists of the delivery of pulse trains without collecting diagnostic data from the stimulator.
An example of a purely diagnostic implant is, e.g. a cardiac rhythm monitor. The diagnostic function consists of continuous recording of the patient's ECG and automatic evaluation of abnormalities of the heart rhythm. If such are detected, an ECG recording is stored and typically automatically transmitted to a remote monitoring system.
An example of an implant with therapeutic and diagnostic functions is e.g. a cardiac pacemaker. The pacemaker is typically implanted subcutaneously in the upper right thoracic region and the electrode is placed in the patient's heart via a large vein. The therapeutic function consists of delivering stimulation pulses to trigger a cardiac action, provided there is no spontaneous cardiac action in the patient. The diagnostic function consists, for example, in the continuous recording of the patient's ECG and automatic evaluation of abnormalities of the heart rhythm. If such are detected, an ECG recording is stored and typically automatically transmitted to a remote monitoring system.
According to an aspect of the invention, the front-end communication unit is configured to transfer contact information data of a health service provider, in particular of a manufacturer of the implantable medical device, to the user communication device.
Thus, a specific a contact number can be obtained from the front-end communication unit. This can be implemented as a one-click solution to start a voice or video call with an employee of the health service provider without the additional need to type in the number manually.
According to a further aspect of the invention, the app of the user communication device is configured to, using the contact information data of the health service provider, initiate a voice or video call to the health service provider and/or perform screen sharing of the data of the implantable medical device received via the front-end communication unit. This advantageously enables an enhanced user friendliness and efficient use of the patient medication device.
According to a further aspect of the invention, the app of the user communication device is configured to output patient medical and/or device technical data received from the implantable medical device, in particular if a status change of the patient medical and/or device technical data occurs and/or to output workflow information for interrogating the implantable medical device by means of the front-end communication unit.
Once a regular point-of-care workflow is started, the app displays additional textual information whenever changes in the implant communication system (e.g. in/at the implantable medical device, a contained backend system and/or the user communication device) occur and displays different non-textual icons/symbols of the point-of-care frontend communication unit, especially regarding main steps such as “Place Wand”, “Interrogation”, “Remove Device”, “Uploading to health service provider” and “Upload done”. This additional information is e.g. more explanations, an expected duration of a process step, etc.
According to a further aspect of the invention, the device technical data is a battery state of charge, a software version installed in the front-end communication unit, successful/unsuccessful transmissions to a (remote) backend system (at least one backend server and/or another backend device), an average duration of the workflow on the front-end communication unit and/or a troubleshooting report of the implantable medical device. Said data is collected from the implantable medical device via the first interface regularly. All device information is then assembled into a troubleshooting data set/report and transmitted to a remote backend service (in/of the backend system).
According to a further aspect of the invention, the front-end communication unit is configured to connect to the backend system and to relay information to the app of the user communication device, said information comprising a from the backend system generated medical report and/or workflow statistics of the front-end communication unit. This allows to display a report if said report has already been created in the backend. It further allows to display the full report to the user. Moreover, information about workflow statistics can be provided.
According to a further aspect of the invention, the app of the user communication device is configured to store the received report as a file on the user communication device, wherein the app of the user communication device is configured to forward the received report to a predefined recipient via electronic messaging. The user communication device thus advantageously provides a functionality to send an e-mail with an attached file, a MMS, an
iMessage or a WhatsApp message, and/or to save messages/files to a cloud (e.g. Google Drive).
According to a further aspect of the invention, the app of the user communication device is configured to generate a QR-code, said QR-code comprising an IP-address for accessing the report, and wherein the app of the user communication device is configured to forward the received report (and/or to provide access to the report) to an electronic health record system of a medical service provider (e.g. a clinic or a pharmacy). The user communication device thus advantageously comprises the functionality to create a QR code with the address and also to store/send the QR code. The QR-Code may be sent/stored via email, MMS, WhatsApp, Google Drive, etc.
According to a further aspect of the invention, the app of the user communication device is configured to provide instructions on performing the workflow on the front-end communication unit. This advantageously allows to change important settings such as the display language and/or login information to the remote backend service. This provides an enhanced user friendliness and an easier operation of the front-end communication unit for the user.
According to a further aspect of the invention, the app of the user communication device is configured to search for and install software updates on the front-end communication unit. This allows the user to manually trigger maintenance options such as software updates. The user is thus in full control if and when an action is triggered.
According to a further aspect of the invention, the app of the user communication device is configured to change a configuration setting of the front-end communication unit, wherein the configuration setting comprises an accessibility to the remote backend service of the backend system. The user communication device such as a tablet/smartphone is further configured to display said settings to the user.
According to a further aspect of the invention, the app of the user communication device is configured to acquire voice data, in particular of medical staff, via a recording means and/or
a speech-to-text transcribing functionality of the user communication device, wherein the app of the user communication device is configured to transmit the acquired voice data (or a transcript of it) to the remote service of the backend system. This allows medical staff to give feedback on the situation the patient is in. This feedback is transmitted to the remote service and can then be analyzed/assessed.
The herein described features of the implant communication system are also disclosed for the computer implemented method for communicating with an implantable medical device and vice versa.
