Description

SOLID DOSAGE FORM OF A SMALL MOLECULE ANTIVIRAL AND USES THEREOF
CROSS-REFERENCE TO RELATED APPLICATIONS
[0001] This application claims the benefit of U.S. Provisional Patent Application Serial No. 63/515,965 filed July 27, 2023, the entire contents of which are incorporated by reference herein.
FIELD
[0002] The present disclosure relates to solid dosage formulations of a small molecule antiviral. The solid dosage formulations of the invention are typically tablets, such as compressed tablets. The present disclosure also relates to uses and methods of using the solid dosage forms for the treatment or prevention of viral infection in a patient in need thereof.
BACKGROUND
[0003] Viral infections, such as infections caused by Eastern Equine Encephalitis Vims (EEEV), Western Equine Encephalitis Vims (WEEV), and Venezuelan Equine Encephalitis Vims (VEEV), Chikungunya fever virus (CHIK), Ebola virus, influenza virus, respiratory syncytial vims (RSV), Zika vims, and coronaviruses, such as Severe Acute Respiratory' Syndrome Coronavirus (SARS-CoV or SARS-CoV-1), Middle East Respiratory Syndrome Coronavims (MERS-CoV), and, most recently, SARS-CoV-2 (or 2019-nCoV), continue to cause illnesses, which can be mild to severe to life-threatening and fatal, across the globe.
[0004] EEEV, WEEV, VEEV, and CHIK virus are vector-bome viruses (family Togaviridae, genus Alphavirus') that can be transmitted to humans through mosquito bites. The equine encephalitis vimses are Centers for Disease Control (CDC) Category B pathogens, and the CHIK virus is Category C.
[0005] Coronaviruses cause a large percentage of respiratory illnesses in humans, which can be severe or life-threatening. SARS-CoV-1, which emerged in 2002, has caused at least 8439 human illnesses globally and at least 812 deaths WHO Cumulative Number of Reported Probable Cases ofSARS, From 1 Nov 2002 To 4 July 2003, downloaded from www.who.int/csr/sars/country/2003_07_04/en/. on August 12, 2020). Similarly, MERS-CoV emerged in 2012 and has caused at least 2519 human illnesses globally and at least 866 deaths WHO Middle East respiratory syndrome, MERS situation update, January 2020, downloaded from www.emro.who.int/health-topics/mers-cov/mers-outbreaks.html, on August 12, 2020).
More recently, SARS-CoV-2 emerged in 2019, and it has caused at least 629 million confirmed human illnesses globally and at least 6.5 million deaths {World Health Organization COVID-19 Weekly Epidemiological Update. Issue No. 117. published 9 November 2022, downloaded from www.who.int/publications/m/item/weekly-epidemiological-update-on-covid-19 — 9-november- 2022, on November 10, 2022). SARS-CoV-2 causes the disease referred to as COVID-19, which can include severe respiratory disease and systemic disease manifestations in humans. SARS- CoV-2 infection is also associated with mental and neurological symptoms that can include delirium or encephalopathy, agitation, stroke, meningo-encephalitis, impaired sense of smell or taste, anxiety, depression, and sleep problems, and these neurological symptoms can occur in the absence of respiratory symptoms (see Clinical management of COVID- 19 (Interim guidance, 27 May 2020), downloaded from www.who.int/publications/i/item/clinical-management-of-covid- 19, on September 15, 2020). Additional studies are needed to further characterize the SARS- CoV-2 virus and to identify ways to prevent and treat the COVID-19 disease, as well as diseases caused by other human coronaviruses.
[0006] Many patients with COVID-19 recover with no or minimal medical intervention.
However, clinical progression to severe disease severely impacts both patients and healthcare systems, increasing an individual’s risks of requiring mechanical ventilation and of death and potentially overburdening hospital capacity7 and available healthcare resources during COVID-19 surges. Reducing the number of patients requiring hospitalization for COVID- 19 is therefore critical. Vaccination remains by far the most important medical intervention available to reduce the risk of hospitalization or death from COVID-19. However, early treatment soon after symptom onset has also been shown as effective. Monoclonal antibodies bamlanivimab/etesevimab, casirivimab/imdevimab, and sotrovimab were originally the only treatments authorized for at-risk outpatients with COVID- 19. Because monoclonal antibodies require administration via infusion or injection in a medical setting, a direct-acting, oral agent that can be self-administered at home after diagnosis may be more practical for non-hospitalized patients and would be an important new tool in treating COVID-19 caused by SARS-CoV-2. [0007] Antiviral agents continue to be developed to treat viral infections. For example, the antiviral nucleoside fyD-M4)-hydroxy cytidine (NHC, l-((2 ,3 .4<S’,57?)-3.4-dihydroxy-5- (hydroxymethyl) tetrahydrofuran-2-yl)-4-(hydroxyamino)pyrimidin-2(777)-one), has antipestivirus and antihepacivirus activities. ANTIM1CROB AGENTS CHEMOTHER, 2003, 47(l):244-54. While NEIC has been shown to have limited oral bioavailability in nonhuman primates (see Toots et al., SCI. TRANSL. MED. 11, eaax5866 (2019)), prodrugs ofNHC, including {(2R, 3.S'.4/ .5/?)-3.4-dihy droxy-5-[4-(hydroxyimino)-2-oxo-3,4-dihydropyrimidin- 1 (277) -yl] oxolan-2-yl } methyl 2-methy Ipropanoate) : Compound A, which is also known as uridine 4-oxime 5'-(2-methylpropanoate) and molnupiravir, are orally bioavailable and are understood to be potentially useful for the treatment of viral infections caused by at least Eastern Equine Encephalitis Virus, Western Equine Encephalitis Virus, and Venezuelan Equine Encephalitis Virus, Chikungunya fever virus, Ebola virus, influenza virus, respiratory syncytial virus, Zika virus, and coronaviruses, such as Severe Acute Respiratory7 Syndrome Coronavirus, Middle East Respiratory Syndrome Coronavirus, and, most recently, SARS-CoV-2.
[0008] P-D-JV(4)-hydroxycytidine, its prodrugs, its derivatives, and methods for making the same are illustrated in PCT International Patent Application No. PCT/US2015/066144, which published as PCT International Patent Application Publication No. W02016/106050, and United States Patent Application No. 15/537,087. which published as United States Patent Application Publication No. US2019/0022116, and United States Patent Application No. 16/921,359, which published as United States Patent Application No. US 2021/0060050 and issued as United States Patent No. 11, 628, 181, each of which are incorporated herein by reference in their entirety.
Compound A. and pharmaceutically acceptable salts, tautomers, and prodrugs thereof, as well as additional derivatives of P-D-/V(4)-hydroxy cytidine, and methods for making the same are illustrated in PCT International Patent Application No. PCT/US2018/064503, which published as PCT International Patent Application Publication No. WO2019/113462, and United States Patent Application No. 16/755,779. now United States Patent No. 11,331,331. and United States Patent Application No. 17/465,344. which published as US 2022/0016153 Al, and in United States Provisional Patent Application No. 63/127,484, and PCT International Patent Application No. PCT/US2021/064021, which published as PCT International Patent Application Publication No. WO2022/133205, each of which are incorporated herein by reference in their entirety7. [0009] Compound A is marketed for the treatment of viral infections caused by SARS-CoV-2 and other pathogens, as well as for the treatment of COVID-19 caused by SARS-CoV-2. However, the 800 mg dose, administered twice daily for five days, takes the form of four (4) capsules. Each of these capsules is a size 0 capsule that is 21.7 mm x 7.6 mm and has a 200 mg unit dosage strength, and each represents a unit dosage form. The size and number of unit dosage forms required at each dose administration creates a significant pill burden, both in terms of the size of each capsule and in terms of the number of capsules administered at each dosage interval. The pill burden can affect whether a patient takes the medication as prescribed (either by number of single dosage forms or by adherence to a schedule for dosing). With a high pill burden also come other risks, such as increased risks of medication errors and increased costs.
[0010] There is a need for oral dosage formulations of antiviral nucleosides, such as Compound A, that reduce pill burden by reducing the number of unit dosage forms required at each dose administration, by reducing the size of each unit dosage form, or both. Reducing the pill burden will ultimately improve compliance, and, therefore, efficacy, and will decrease risk of medication error and decrease costs. However, the design of such formulations remains largely unpredictable.

In certain embodiments, “about” can mean a variation of ± 0.100 %, 0.500 %, 1.00 %, 2.00 %, 3.00 %, 4.00 %, 5.00 %, or 10.0 %. [0035] Unless otherwise defined, all technical and scientific terms used herein have the same meaning as commonly understood by one of ordinary skill in the art to which this disclosure relates. In case of conflict, the present specification, including definitions, will control.
[0036] Throughout this specification and claims, the word “comprise,” or variations such as “comprises” or “comprising” are used in an inclusive sense, i.e., to specify7 the presence of the stated features but not to preclude the presence or addition of further features that may materially enhance the operation or utility of any of the embodiments of the invention, unless the context requires otherwise due to express language or necessary implication. Unless otherwise required by context, singular terms shall include pluralities and plural terms shall include the singular. Any example(s) following the term “e.g.” or “for example” is not meant to be exhaustive or limiting.
[0037] It is understood that wherever embodiments are described herein with the language “comprising,” otherwise analogous embodiments described in terms of “consisting of’ and/or “consisting essentially of’ are also provided. [0038] The phrase "consists essentially of.” or variations such as “consist essentially of’ or “consisting essentially of,” as used throughout the specification and claims, indicate the inclusion of any recited elements or group of elements, and the optional inclusion of other elements, of similar or different nature than the recited elements, that do not materially change the basic or novel properties of the specified dosage regimen, method, or composition.
[0039] The terms “pharmaceutically effective amount” and “effective amount” mean an amount whereby sufficient therapeutic composition or formulation is introduced to a patient to cause a therapeutic or prophylactic effect to treat a diseased or condition. One skilled in the art will recognize that this amount may vary according to a patient’s characteristics such as age, weight, etc.
[0040] “Pharmaceutically acceptable” refers to excipients (vehicles, additives) and compositions that can reasonably be administered to a subject to provide an effective dose of the active ingredient employed and that are “generally regarded as safe”, e.g., that are physiologically tolerable and do not typically produce an allergic or similar untoward reaction, such as gastric upset and the like, when administered to a human. In another embodiment, this term refers to molecular entities and compositions approved by a regulatory agency of the federal or a state government or listed in the United States Pharmacopeia or another generally recognized pharmacopeia for use in animals, and more particularly in humans.
[0041] The term “pharmaceutically acceptable carrier” refers to any inactive substance that is suitable for use in a formulation for the delivery7 of a therapeutic agent. A carrier may be an antiadherent, binder, coating, disintegrant. filler or diluent, lubricant, preservative (such as antioxidant, antibacterial, or antifungal agent), sweetener, absorption delaying agent, wetting agent, emulsifying agent, buffer, and the like. Examples of suitable pharmaceutically acceptable carriers include water, ethanol, polyols (such as glycerol, propylene glycol, polyethylene glycol, and the like), dextrose, vegetable oils (such as olive oil), saline, buffer, buffered saline, and isotonic agents such as sugars, polyalcohols, sorbitol, and sodium chloride.
[0042] The term “pharmaceutical formulation” refers to preparations that are in such form as to permit the active ingredients to be effective. The term “formulation” and “pharmaceutical formulation” are used interchangeably throughout.
[0043] “Direct compression” is defined as a process involving pre-milling each ingredient of a formulation (e.g., the active drug substance and excipients) into a powder, mixing the powdered ingredients and any lubricants, and compressing the mixed powders into tablets. When the formulation is to be processed by direct compression, the active ingredient, herein an antiviral compound, may be provided for pre-milling either directly, “as is”. from synthesis or after purification by a suitable method, such as recrystallization.
[0044] “Wet granulation” is defined as a process involving granulating a powder with liquid (aqueous, non-aqueous, hot melt, etc.) to achieve the desired properties for subsequent downstream processes. The liquid (e.g., water, binder solution) is added to a powder blend while the powder blend is being continuously mixing, leading to granule nucleation and growth. When the formulation is to be processed by wet granulation, the antiviral compound may be provided directly, “as is”, from synthesis or after purification by a suitable method, such as recrystallization.
[0045] “Roller compaction” is defined as an agglomeration process where a powder is compressed into a dense compact (or ribbon) using two counter-rotating rollers. The pressing forces from the rollers cause the particles to deform plastically to achieve the dense compact. The dense compact is subsequently broken up and milled into granules. The advantages of roller compaction include improved flow, better content uniformity, and/or less sticking. Roller compaction is also preferred for moisture sensitive and/or heat sensitive formulations.
[0046] Excipients that are mixed with other ingredients (including the antiviral compound) prior to granulation, and are thus incorporated within the tablets, comprise the intragranular component of the formulation. Excipients that are mixed with the dry granules before the complete mixture is compacted comprise the extragranular component of the formulation. Excipients that may be suitable for inclusion in the oral dosage forms set forth herein include compression aids, diluents, disintegrants, lubricants, salts, glidants, surfactants, solubilizers, wetting agents, binders, and/or fillers.
[0047] The term “compression aid” refers to any substance used in the preparation of pharmaceutical dosage forms, such as tablets, that is a pharmacologically inactive substance that may be compacted easily even when mixed with an active drug substance, and that improves flowability and compressibility during manufacture of tablets. Examples of suitable pharmaceutically acceptable compression aids for use in the oral dosage forms described herein include directly compressible starches, dibasic calcium phosphate, spray-dried lactose, anhydrous lactose, spray cry stallized maltose, cry stalline sorbitol, mannitol, microfine cellulose, and microcrystalline cellulose.

[0062] In aspects of embodiments, the antiviral nucleoside may be used either directly from synthesis or after purification and may be directly formulated as a solid dosage form by blending or granulating with excipients and compressed into tablets. The granulation process may be a wet granulation process, such as high-shear wet granulation or fluidized-bed granulation, or it may be a dry granulation process, such as roller-compaction.
[0063] The pharmaceutical formulations may be roller compacted or wet granulated to densify, improve flow attributes, and/or reduce the risk of segregation of components during subsequent handling (e.g., compression into tablets). Granulation steps can also be used to minimize the impact of raw material property variability (e.g., excipient particle size) on subsequent processing (e.g., tablet compression) and ultimate product performance. Lubrication is typically performed prior to roller compaction and tablet compression to reduce the tendency of material to adhere to compression surfaces (e.g., tablet tooling). In particular embodiments, the lubricant system is a combination of sodium stearyl fumarate and magnesium stearate. These methods can be carried out by those skilled in the art. See, e.g.. Ansel, INTRODUCTION TO PHARMACEUTICAL DOSAGE FORMS, Seventh Edition, 1999.
[0064] Tablets may contain the active ingredient in admixture with non-toxic pharmaceutically acceptable excipients that are suitable for the manufacture of tablets. The tablets may be uncoated or they may be film coated to modify their appearance or may be coated so as to modulate the onset and/or rate of release in the gastrointestinal tract, so as to optimize or maximize the biological exposure of the patient to the antiviral compound. In embodiments, the tablets may be coated with a functional coat to delay disintegration and absorption in the gastrointestinal tract and thereby provide a sustained action over a longer period.

[0158] Many patients with COVID-19 recover with no or minimal medical intervention.
However, clinical progression to severe disease severely impacts both patients and healthcare systems, increasing an individual’s risks of requiring mechanical ventilation and of death and potentially overburdening hospital capacity and available healthcare resources during COVID- 19 surges. Reducing the number of patients requiring hospitalization for COVID-19 is therefore critical. Vaccination remains by far the most important medical intervention available to reduce the risk of hospitalization or death from COVID-19. However, early treatment soon after symptom onset has also been shown as effective. A direct-acting, oral agent such as the herein described oral dosage forms that can be administered at home after diagnosis may be more practical for non-hospitalized patients and would be an important new tool in treating viral infection and viral diseases, including COVID- 19 caused by SARS-CoV-2.
[0159] Additional embodiments of the disclosure include the pharmaceutical compositions, combinations, uses, and methods set forth in above, wherein it is to be understood that each embodiment may be combined with one or more other embodiments, to the extent that such a combination is consistent with the description of the embodiments. It is further to be understood that the embodiments provided above are understood to include all embodiments, including such embodiments as result from combinations of embodiments.
EXAMPLES
[0160] The following examples are meant to be illustrative and should not be construed as further limiting. The contents of the figures and all references, patents, and published patent applications cited throughout this application are expressly incorporated herein by reference.
Comparative Example: Compound A Capsule Formulation
[0161] The capsule formulation for Compound A uses a wet granulated mixture filled into size 0 hypromellose capsules at a unit dose strength of 200mg (fill weight of 285.7mg). The composition of the capsule formulation is shown in Table 2.
Table 2
Example 1: Compound A Tablet Formulation: High-Shear Wet Granulation, Roller Compaction, and Direct Compaction Methods [0162] Tablets were prepared using different tableting methods, to determine whether the tableting process used would affect the size and drug load achievable as well as to assess different formulations for processability. The unit dose strength for each tablet was 400. Omg. [0163] A high-shear wet-granulation (HSWG) process was used to prepare tablets of Formulation A, by dispensing Compound A, hydroxypropyl cellulose, and sodium lauryl sulfate into a high shear wet granulator bowl. The ingredients were granulated with water. The wet granules were de-agglomerated, fluid-bed dried and milled. The granules were blended with extragranular ingredients and lubricated with magnesium stearate.
[0164] A high-shear wet-granulation (HSWG) process was used to prepare tablets of Formulation D, by dispensing Compound A, microcrystalline cellulose, hydroxypropyl cellulose, and croscarmellose sodium into a high shear wet granulator bowl. The ingredients were granulated with water. The wet granules were de-agglomerated, fluid-bed dried and milled. The granules were blended with extragranular ingredients and lubricated with magnesium stearate.
[0165] A roller compaction (RC) process was used to prepare tablets by dispensing Compound A, microcrystalline cellulose, lactose monohydrate, croscarmellose sodium, tri-basic calcium phosphate and sodium lauryl sulfate into a blender vessel preserving the following proportion. The ingredients were blended. The blend was granulated by roller compaction. The granules were added to a blender vessel with magnesium stearate added, and the ingredients were blended. Subsequently, the blend was compressed into tablets. This process was used to prepare Formulation B.
[0166] A direct compression (DC) process was used to prepare tablets by dispensing Compound A and tri-basic calcium phosphate into a blender vessel preserving the following proportion. The ingredients were pre-blended to improve flow. Then, the Compound A and tri- basic calcium phosphate blend, microcrystalline cellulose, lactose monohydrate, croscarmellose sodium, sodium lauryl sulfate, and magnesium stearate were dispensed into a blender, and the ingredients were blended. Subsequently, the blend was compressed into tablets. This process was used to prepare Formulation C.
Example 2: Comparison of Tablet Formulations Having Similar Drug Load
[0167] Tablet formulations A, B, C, and D were prepared according to the process and proportions indicated in Table 3. Table 3
[0168] To reduce pill burden and to achieve the requisite tablet mechanical strength for push through blisters and film coating, multiple formulations and processes were developed across roller compaction, direct compression, and high shear wet granulation (HSWG) platforms. Figure 1 shows the tablet-ability profile of the granules and powder blend of Compound A prepared using different techniques, specifically Formulation A, Formulation B, and Formulation C. A common formulation was chosen to be processed by RC and DC, while a different formulation composition was selected for HSWG (Table 3). Granules (HSWG and RC) and DC blend were compressed at different stresses and the tensile strength of each tablet was determined. The tablet strength was impacted by the manufacturing process. At a similar drug load of 50% (400.0 mg) and tablet core weight of 800.0 mg, the HSWG formulation (Formulation A) yielded tablets the best mechanical strength, while the tablets prepared by RC (Formulation B) were the weakest. The granules manufactured by roller compaction showed loss in tablet-ability, while the DC tablets (Formulation C) had improved tablet-ability since the formulation was not granulated. Tablets produced by HSWG (Formulation A) had the best tensile strength (Figure 1).
[0169] The HSWG tablets of Formulation D demonstrated improved strength relative to the RC tablets of Formulation B as indicated by better friability (Figure 2), which increases robustness for downstream operations such as film coating and packaging. The superior strength of the HSWG tablets of Formulation D relative to the RC tablets of Formulation B may be attributed to the flexibility of the HSWG process, which allows the use of significant levels of extragranular compression aid/filler (see Formulation A and Formulation D in Table 3) to boost tablet-ability while maintaining superior processability (Figure 3).
[0170] Significantly, each of the tablet formulations contained 400mg of Compound A in a single tablet, which is equivalent amount of Compound A in two capsules of the approved and marketed formulation, as shown in the Compartative Example.
Example 3: Impact of Sodium Lauryl Sulfate on In-Vitro Tablet Dissolution for HSWG Formulations of Compound A
[0171] Tablet formulations E, F, G, and H were prepared using a HSWG process by dispensing the following ingredients into a high shear wet granulator bowl preserving the following proportions as indicated in Table 4. The ingredients were granulated with water. The wet granules were de-agglomerated, fluid bed dried and milled. The granules were blended with extragranular ingredients as indicated in Table 4 and lubricated with magnesium stearate. Sodium lauryl sulfate (SLS) was added intragranularly to Formulations E, F, and G at 1.00% (6.000mg), 0.75% (4.500mg) and 0.25% (1.500mg) levels, respectively, while SLS was added extragranularly to Formulation H at a 0.50% (3.000 mg) level.
Table 4

[0172] A 100 RPM USP I (20 mesh) dissolution method was used to evaluate these samples. The dissolution media consisted of 500mL of 0.1N HC1. Following dissolution, samples were analyzed using a gradient reversed phase HPLC method. When SLS was added intragranularly (Formulations E. F, and G), the dissolution rate of tablets increased with increasing levels of SLS. Figure 4 shows the dissolution of these formulations. Although Compound A (>10 mg/mL in water) has good aqueous solubility at 37°C, it exhibits poor wettability' in aqueous media. Intragranular addition of SLS improves wettability and enhanced the contact between tablet surface and dissolution media, which in turn resulted in rapid tablet disintegration and dissolution. In contrast, dissolution rate was slowest with extragranular addition of SLS (Formulation H indicative that SLS needs to be in close proximity (intimately mixed) with Compound A to have a notable impact on dissolution rate. Example 4: Impact of Extragranular Components on In-Vitro Tablet Dissolution for HSWG Formulations of Compound A
[0173] For HSWG formulations of Compound A, and in the absence of a surfactant, excipients may be used to modulate the release of Compound A from a formulation. The type of excipient used, quantitative levels of excipient, and placement of excipients - either intragranular or extragranular - all can affect the release of Compound A. The effects of excipients on Compound A release were evaluated using a 50 RPM USP II dissolution method using 0. IN HC1 as the dissolution media with a 500 mL vessel fill. Following dissolution, samples were analyzed using a gradient reversed phase HPLC method. An increase in the extragranular levels of a water soluble compression aid and disintegrant, lactose monohydrate and croscarmellose sodium, respectively, results in increase in dissolution rate of HSWG tablets. Formulation D releases Compound A faster compared to Formulation I, the slow variant (Table 5 and Figure 5). Croscarmellose sodium facilitates the rapid release of Compound A from the tablet, due to the inherent wicking and swelling ability when exposed to water. Because it is water soluble and hydrophilic, lactose monohydrate facilitates gel formation and shortens the penetration time of the dissolution media into the tablet matrix.

Example 5: Absorption of Compound A, Systemic Pharmacokinetics, and Bioequivalence [0174] To characterize the human absorption of Compound A and systemic pharmacokinetics (PK) of NHC and to assess the bioequivalence of the capsule formulation (Comparative Example) and the tablet formulation (Formulation D). a physiologically based biopharmaceutics model (PBBM) was developed using a dissolution method as per FDA guidance document. The model was qualified with multiple clinically observed PK results (See Painter WP, Holman W, Bush JA, Almazedi F, Malik H, Eraut NCJE, Morin MJ, Szewczyk LJ, Painter GR, Human safety/, tolerability, and pharmacokinetics of molnupiravir, a novel broad-spectrum oral antiviral agent with activity against SARS-CoV-2, 65(5) ANTIMICROB. AGENTS CHEMOTHER. 1-14 (May 2021); available as 65:e02428-20 from doi.org/10.1128/AAC.02428-20). Briefly, physiologically based biopharmaceutics models are newer tools that can be used throughout drug product development and post approval and have found broader use in the FDA with a draft guidance (See U.S. FOOD AND DRUG ADMINISTRATION. DRAFT GUIDANCE FOR INDUSTRY: THE USE OF PHYSIOLOGICALLY BASED PHARMACOKINETIC ANALYSES— BIOPHARMACEUTICS APPLICATIONS FOR ORAL DRUG PRODUCT DEVELOPMENT. MANUFACTURING CHANGES, AND CONTROLS 2020; Available from: www.fda.gov/regulatory-information/searchfda-guidance-documents/use-physiologically-based- pharmacokinetic-analyses-biopharmaceutics-applications-oral-drug-product). PBBM focuses on the generation of mechanistic understanding of how7 drug product quality attributes interact with physiology to influence the in vivo drug performance and pharmacokinetics. PBBM allows establishing a dissolution bioequivalence safe space; dissolution profiles that are within the safe space lead to bioequivalent pharmacokinetics in Cmax and AUC. (See Wu F, Shah H, Li M, Duan P, Zhao P, Suarez S, Raines K, Zhao Y, Wang M, Lin HP, Duan J, Yu L, Seo P, Biopharmaceutics Applications of Physiologically Based Pharmacokinetic Absorption Modeling and Simulation in Regulatory Submissions to the U.S. Food and Drug Administration for New Drugs. 23(31) The AAPS J. (Feb. 2021); available as doi: 10.1208/sl2248-021-00564-2. PMID: 33619657).
[0175] The Compound A dissolution curves w ere obtained for the capsule formulation and tablet Formulation D using a USP 2 method. The dissolution profiles are a key input data to the PBBM and are used together with the in vitro dissolution conditions (volume, solubility in the dissolution medium, and dose). The Compound A dissolution data w ere then modeled to calculate z-factors on the basis of Takano et al. (Takano R, Sugano K, Higashida A, Hayashi Y, Machida M, Aso Y, Yamashita S, Oral absorption of poorly water-soluble drugs: computer simulation of fraction absorbed in humans from, a miniscale dissolution test, 23(6) PHARM RES. 1144-1 156 (June 2006); available as doi: 10. 1007/s 11095-006-0162-4. Epub 2006 May 25.
PMID: 16715363.). The z-factor calculation tool in GastroPlus was used, and the results of the z- factors from in vitro data were used for the simulation by setting the dissolution model as z-factor model in GastroPlus (See Wu D. Li M, Current State and Challenges of Physiologically Based Biopharmaceutics Modeling (PBBM) in Oral Drug Product Development, 40(2) PHARM RES. 321-336 (Feb. 2023, published online 8 Sep. 2022); available as doi: 10. 1007/sl 1095-022-03373- 0; see also Kollipara, S., Bhatiprolu, A.K , Boddu, R., Ahmed T., Chachad S., Best Practices for Integration of Dissolution Data into Physiologically Based Biopharmaceutics Models (PBBM): A Biopharmaceutics Modeling Scientist Perspective, 24(59) AAPS PHARMSCITECH (Feb. 2023); available as doi.org/10.1208/sl2249-023-02521-y.).
[0176] Overall, the bioequivalence safe space lower bound is Q = 80% dissolved in 37 min in 0. IN HC1 dissolution media in Figure 6. Based on the dissolution safe space (shaded area), the dissolution capsule formulation of Compound A is bioequivalent to the Formulation D tablet. The dissolution profile for Formulation I, the slow variant, is shown for illustration purposes to be outside of the bioequivalence safe space. Formulation I is expected to be bioequivalent to, or nearly bioequivalent to, Formulation D and the capsule formulation of the Comparative Example.
