Abstract
Die Erfindung betrifft Kombinationen umfassend mindestens einen Wirkstoff aus der Pantothensäure und Pantothensäurederivate umfassenden Gruppe, insbesondere Dexpanthenol, und mindestens einen antiallergischen Wirkstoff, zur Behandlung von allergischen Symptomen der Haut oder Schleimhaut, diese enthaltende Arzneimittel und dessen Herstellung.
Description
Arzneimittel mit einer Wirkstoffkombination zur Behandlung von allergischen Symptomen
Die Erfindung betrifft Kombinationen umfassend mindestens einen Wirkstoff aus der Pantothensäure und Pantothensäurederivate umfassenden Gruppe, insbesondere Dexpanthenol, und mindestens einen antiallergischen Wirkstoff, zur Behandlung von allergischen Symptomen der Haut oder Schleimhaut, diese enthaltende Arzneimittel und dessen Herstellung.
Die für eine antiallergische Therapie zur Verfügung stehenden Pharmaka wirken im allgemeinen symptomatisch, da eine kausale Therapie nur durch völlige Antigenkarenz bewirkt werden könnte. Bei der symptomatischen Therapie von allergischen Erkrankungen kommen hauptsächlich Wirkstoffe aus der Wirkstoffklasse der Antihistaminika und aus der Klasse der Corticosteroide zum Einsatz.
Bei der Entstehung allergischer Reaktionen und der dabei auftretenden Symptome kommt der Ausschüttung von Mediatoren (z. B. Histamin) durch Mastzellen und basophile Granulozyten eine zentrale Rolle zu. Die freigesetzten Mediatoren bewirken eine erhöhte Durchlässigkeit von Gefäßen, spastische Kontraktionen der glatten Muskulatur, und - durch Aktivierung weiterer Zelltypen des Immunsystems - lokale Entzündungsfeaktionen. Antihistaminika verhindern das Auftreten oder die Auswirkungen von Mediatoren allergischer Reaktionen (Prostaglandine, insbesondere PGD2, und Histamin) und haben bei atopischen Patienten einen antagonistischen Effekt auf die Migration von eosinophilen Granulozyten. Des weiteren kann der allergischen Reaktion durch Verabreichung von entzündungshemmenden und immunsuppressiv wirkenden Corticosteroiden (Glucocorticoiden) entgegengewirkt werden. Hierauf beruht die Verwendung dieser Wirkstoffklassen bei der symptomatischen Therapie allergischer Erkrankungen.
Die von allergischen Reaktionen hervorgerufenen Symptome betreffen vor allem die Haut, die Schleimhäute, insbesondere die Schleimhäute der Atemwege (Nasenhöhle, Nasennebenhöhlen, Mundhöhle, Rachen, Kehlkopf, Trachea, Bronchien, Bronchiolen), die Muskulatur der Bronchien, den Magen-Darm-Trakt, die Bindehaut des Auges sowie das Gefäßsystem. Häufige Symptome bzw. klinische Erscheinungsbilder von allergischen Erkrankungen sind z. B. folgende: Hautjucken, Hautrötungen, Ekzeme, Exantheme, Urtikaria, Purpura; Ödembildung in der Schleimhaut, Schwellung der Schleimhaut, Sekretabsonderung aus Schleimhäuten, allergische Rhinitis, allergisches Asthma bronchiale, allergische Alveolitis, sowie allergische Bindehautentzündung.
Antihistaminika und Glucocorticoide sind sowohl als orale (d. h. systemisch wirkende) als auch als topisch wirksame Arzneimittel für die Behandlung allergischer Symptome zugelassen. Allerdings
ist die therapeutische Wirksamkeit der bekannten Antihistaminika und Glucocorticoide oft unzureichend, und eine Dosissteigerung ist aufgrund des damit verbundenen Nebenwirkungsrisikos nicht vertretbar. Häufig kommt es bei der symptomatischen antiallergischen Therapie unter Verwendung von Wirkstoffen aus den Klassen der Antimstaminika und Glucocorticoide zu unerwünschten Nebenwirkungen, wie z. B. Hauttrockenheit, Hautatrophie, Anfälligkeit der Haut oder Schleimhaut für Pilzinfektionen sowie für bakterielle und virale Infektionen.
Bei der symptomatischen Therapie von allergischen Erkrankungen werden des weiteren Wirkstoffe aus der Klasse der Mastzelldegranulationshemmer und aus der Klasse der Leukotrien-Rezeptor- Antagonisten eingesetzt. Ferner ist aus WO 01/19330 Al bekannt, daß durch Pantothensäure und deren Derivate (z. B. Dexpanthenol), aber auch durch die Aminosäure Glycin, eine Hemmung der Freisetzung von Mediatoren aus Mastzellen bewirkt und dadurch die allergische Reaktion abgemildert werden kann.
Üblicherweise wird Dexpanthenol als Bestandteil in topischen Arzneimitteln verwendet. Beispielsweise wird in EP 1159958 Al eine Nasensalbe beschrieben, die Dexpanthenol als pflegenden Bestandteil enthält. DE 10005936 Al offenbart eine Creme zur äußeren Anwendung im Nasenbereich, welche eine Kombination von Allantoin und Dexpanthenol enthält. DE 19541919 Al beschreibt eine pharmazeutische Zubereitung zur Behandlung akuter Rhinitiden, die ein zur topischen Anwendung geeignetes Sympathomimetikum mit 2-Imidazolinstruktur (z. B. Oxymetazolin) in Kombination mit einem Pantothenolderivat, z. B. Dexpanthenol, enthält. Durch die Kombination mit Dexpanthenol sollen die durch das Sympathomimetikum hervorgerufenen Nebenwirkungen abgemildert werden.
Die der vorliegenden Erfindung zugrunde liegende Aufgabe bestand darin, Arzneimittel für die therapeutische Behandlung allergischer Symptome der Haut oder Schleimhaut bereitzustellen, welche eine verbesserte Wirksamkeit und/oder ein geringeres Nebenwirkungspotential aufweisen.
Gemäß vorliegender Erfindung wird diese Aufgabe durch eine Kombination umfassend mindestens einen Wirkstoff aus der Pantothensäure und Pantothensäurederivate umfassenden Gruppe, vorzugsweise Dexpanthenol, und mindestens einen anti-allergischen Wirkstoff aus der
AntiWstaminika, Corticosteroide, synthetische Mastzelldegranulationshemmer und Leukotrien-
Rezeptor-Antagonisten umfassenden Gruppe gelöst. Überraschenderweise hat sich gezeigt, daß
Wirkstoffe aus der Pantothensäure und Pantothensäurederivate umfassenden Gruppe, insbesondere Dexpanthenol, die Wirksamkeit des mit der Wirkstoffkombination gemeinsam verabreichten
Antiallergikums in synergistischer Weise verstärken. Dies ermöglicht es, die Dosis des
Antiallergikums zu reduzieren, im Vergleich zu herkömmlichen Arzneimitteln zur
symptomatischen Therapie der Allergie, welche Dexpanthenol nicht enthalten. Ferner wurde überraschenderweise eine - im Vergleich zu den genannten herkömmlichen Arzneimitteln - eine Reduktion der Nebenwirkungen festgestellt. Die erfindungsgemäßen Kombinationen eignen sich in vorteilhafter Weise zur Behandlung allergischer Symptome oder Erkrankungen der Haut, der Schleimhäute, vor allem der Atemwege und insbesondere der Nase, oder auch der Augenbindehaut. Eine Kombination von Hydrocortison mit Dexpanthenol ist aufgrund ihrer anti-inflammatorischen Eigenschaften besonders vorteilhaft für die Behandlung allergischer Symptome oder Erkrankungen.
Die erfindungsgemäßen Kombinationen sind weiterhin vorteilhaft, da sie ein verringertes Nebenwirkungspotential aufweisen. Das Auftreten von Nebenwirkungen wie z. B. Hauttrockenheit kann aufgrund der Verwendung der erfindungsgemäßen Wirkstoffkombination erheblich reduziert werden. Dies ist insbesondere deshalb von Bedeutung, da einige anti-allergische Wirkstoffe bei topischer Anwendung selbst allergische Reaktionen hervorrufen können, und eine Dosisreduzierung aus diesem Grunde erstrebenswert ist. Zusätzliche Vorteile ergeben sich bei den erfindungsgemäßen Kombinationen dadurch, daß Dexpanthenol aufgrund seiner antiinflammatorischen Eigenschaften Entzündungssymptome, wie z. B. Rötung, Schwellung und Wärmeentwicklung sowie trockene oder brennende Schleimhäute, oder Krustenbildung, zu reduzieren vermag.
Die Gruppe der Pantothensäurederivate umfaßt insbesondere Dexpanthenol, DL-Panthenol, Salze der Pantothensäure (z. B. Na-Pantothenat, Ca-Pantothenat), Ester der Pantothensäure (z. B. Ethyl-, Methylester), Panthenol-Ether (z. B. Ethyl- oder Methylether), Panthenol-Thioether sowie Panthenyltriacetat. Dexpanthenol (= D-(+)-Pantothenylalkohol) ist am meisten bevorzugt.
Der relative Anteil des Dexpanthenols (bzw. des Pantothensäurederivates und/oder der Pantothensäure) läßt sich in weiten Bereichen variieren, abhängig von den jeweils angestrebten Wirkungen. Vorzugsweise beträgt der Dexpanthenol-Anteil 0,1 bis 95 Gew.-%, insbesondere 0,5 bis 50 Gew.-%, besonders bevorzugt 0,5 bis 30 Gew.-%, jeweils bezogen auf die gesamte Wirkstoffmenge. Die erwähnte synergistische Wirkung wird insbesondere dann beobachtet, wenn das Gewichtsverhältnis von Dexpanthenol (bzw. des Pantothensäurederivates) zu dem genannten mindestens einen anti-allergischen Wirkstoff im Bereich von 1: 100 bis 100:1, bevorzugt im Bereich von 50:1 bis 1:50, besonders bevorzugt im Bereich von 1:10 bis 10:1 liegt.
Gemäß einer bevorzugten Ausführungsform der Erfindung ist vorgesehen, daß die Wirkstoffkombination zusätzlich Glycin enthält. Durch die zusätzliche Verabreichung von Glycin läßt sich eine weitere Steigerung der therapeutischen Wirksamkeit erzielen. Vorzugsweise ist
Glycin in einem Mengenanteil von 0,1 bis 25 Gew.-%, insbesondere 0,5 bis 10 Gew.-%, jeweils bezogen auf den gesamten Wirkstoffanteil, in der Wirkstoffkombination enthalten.
Als antiallergische Wirkstoffe, die erfindungsgemäß in Kombination mit Dexpanthenol (bzw. mit Pantothensäure und/oder Pantothensäurederivaten) verwendet werden, kommen insbesondere Wirkstoffe aus der Wirkstoffklasse der Antihistaminika und aus der Wirkstoffklasse der Corticosteroide in Betracht. Bei den erfϊndungsgemäß verwendeten Corticosteroiden handelt es sich in der Regel um Glucocorticoide.
Durch die kombinierte Verabreichung mit Dexpanthenol bzw. mit Pantothensäure und/oder Pantothensäurederivaten läßt sich überraschenderweise eine synergistische Steigerung der therapeutischen Wirkung der Antihistaminika wie auch der Glucocorticoid- Wirkstoffe erzielen.
Gemäß einer weiteren bevorzugten Ausführungsform ist deshalb vorgesehen, daß die erfindungsgemäße Kombination mindestens einen Wirkstoff aus der Pantothensäure und Pantothensäurederivate umfassenden Gruppe, vorzugsweise Dexpanthenol, und mindestens einen Wirkstoff aus der Wirkstoffklasse der Antihistaminika oder/und mindestens einem Wirkstoff aus der Wirkstoffklasse der Corticosteroide umfaßt.
Antihistaminika (Histamin-HI -Rezeptorantagonisten, Histamin-H2-Rezeptorantagonisten), die für eine therapeutische Behandlung allergischer Symptome der Haut oder der Schleimhäute geeignet sind und deshalb für die Verwendung in der erfindungsgemäßen Wirkstoffkombination in Betracht kommen, sind dem Fachmann grundsätzlich bekannt. Antihistaminika sind Substanzen, die die Wirkung des freigesetzten Histamins am Hl -Rezeptor (oder am H2 -Rezeptor) antagonisieren, wodurch die Wirkung von Histamin an Hl -Rezeptoren vor allem an den peripheren Gefäßen aufgehoben wird. Als Antihistaminika kommen im Zusammenhang mit der vorliegenden Erfindung insbesondere folgende Wirkstoffe in Betracht: Ketotifen, Thonzylamin, Mepyramin, Thenalidin, Tripelennamin, Chlorpyramin, Promethazin, Tolpropamin, Dimetinden, Clemastin, Bamipin, Loratadin, Isothipendyl, Diphenhydramin, Diphenhydraminmethylbromid, Chlorphenoxamin, Pheniramin, Diphenylpyralin, Dioxopromethazin, Dimenhydrinat, Thiethylperazin und Meclozin, Azelastin, Levocabastin, Astemizol, Mebhydrolin, Terfenadin, Mequitazin, Cetirizin, Emedastin, Mizolastin, Olopatadin, Epinastin und Antazolin. Besonders bevorzugte Antihistaminika sind: Bamipin, Clemastin, Chlorphenoxamin, Azelastin, Terfenadin, Loratadin.
Der relative Anteil des Antihistaminikums (bzw. der Antihistaminika) wird in Abhängigkeit von den pharmakologischen Eigenschaften und der Art der jeweils verwendeten Darreichungsform auf dem Fachmann bekannte Weise festgelegt.
Als Corticosteroide kommen grundsätzlich alle Glucocorticoid-Wirkstoffe in Betracht, deren therapeutische Wirksamkeit bei der Behandlung allergischer Erkrankungen oder Symptome bekannt ist. Insbesondere kommen im Zusammenhang mit der vorliegenden Erfindung folgende Wirkstoffe in Betracht: Triamcinolon, Dexamethason, Hydrocortison, Hydrocortisonacetat, Hydrocortisonbutyrat, Hydrocortisonbuteprat, Prednisolon, Betamethason, Methylprednisolon, Clobetason, Flumetason, Fluocortin, Fluperolon, Fluorometholon, Flupredniden, Desonid, Triamcinolon, Alclometason, Dexamethason, Clocortolon, Betamethason, Fluclorolon, Desoximetason, Fluocinolonacetonid, Fluocortolon, Diflucortolon, Fludroxycortid, Fluocinonid, Budesonid, Diflorason, Amcinonid, Halometason, Mometason, Methylprednisolonaceponat, Beclometason, Hydrocortisonaceponat, Fluticason, Prednicarbat, Difluprednat, Ulobetasol, Clobetasol, Halcinonid, Medryson, Desonid, Formocortal, Rimexolon, Mazipredon, Flunisolid und Tixocortol. Besonders bevorzugte Corticosteroide sind: Hydrocortison, Beclometason-dipropionat, Dexamethason.
Die Erfindung umfaßt ferner Ausführungsformen, die vorsehen, daß die Wirkstoffkombination mindestens einen synthetischen antiallergischen Wirkstoff enthält, der nicht aus der AntiWstaminika und Corticosteroide umfassenden Gruppe ausgewählt ist. Hierbei kommen insbesondere Wirkstoffe aus der Gruppe, die synthetische Mastzelldegranulationshemmer und Leukotrien-Rezeptor- Antagonisten umfaßt, in Betracht.
Mastzelldegranulationshemmer, auch als Mastzellstabilisatoren bezeichnet, sind Substanzen, welche die Freisetzung von Histamin aus Mastzellen hemmen. Beispielhafte und bevorzugte Vertreter dieser Wirkstoffklasse sind Cromoglicinsäure, Spagluminsäure, Nedocromil und Lodoxamid.
Leukotrien-Rezeptor-Antagonisten sind Substanzen, welche die Leukotrien-Synthese oder/und Leukotrien-Wirkungen hemmen. Leukotriene sind an der Entstehung und Chronifizierung allergischer entzündlicher Symptome beteiligt. Beispielhafte und bevorzugte Vertreter dieser Wirkstoffklasse sind Montelukast, Pranlukast, Ibudilast und Zafirlukast.
Die in den vorangehenden Absätzen beispielhaft angeführten Wirkstoff-Substanzen stellen keine vollständige und abschließende Aufzählung von Wirkstoffen aus den Klassen der Antihistaminika, Glucocorticoide, synthetischen Mastzelldegranulationshemmer und Leukotrien-Rezeptor-
Antagonisten dar. Die Nennung dieser Substanzen dient lediglich der Veranschaulichung dieser Wirkstoffklassen. Selbstverständlich können auch andere Wirkstoffe aus den genannten Wirkstoffklassen im Sinne der Erfindung verwendet werden. Darüber hinaus können die genannten Wirkstoffe gemäß vorliegender Erfindung auch in Form ihrer pharmazeutisch akzeptablen Salze verwendet werden.
Pharmazeutisch akzeptable Salze der erfindungsgemäßen Verbindungen können Säureadditionssalze der Verbindungen mit Mineralsäuren, Carbonsäuren oder Sulfonsäuren sein. Besonders bevorzugt sind z.B. Salze mit Chlorwasserstoffsäure, Bromwasserstoffsäure, Schwefelsäure, Phosphorsäure, Methansulfonsäure, Ethansulfonsäure, Toluolsulfonsäure, Benzolsulfonsäure, Naphthalindisulfonsäure, Essigsäure, Propionsäure, Milchsäure, Weinsäure, Zitronensäure, Fumarsäure, Maleinsäure, Oxalsäure, Succinsäure, Buttersäure, Decansäure, Asparaginsäure oder Benzoesäure.
Als Salze können aber auch Salze mit üblichen Basen genannt werden, wie beispielsweise Alkalimetallsalze (z.B. Natrium- oder Kaliumsalze), Erdalkalisalze (z.B. Calcium- oder Magnesiumsalze) oder Ammoniumsalze, abgeleitet von Ammoniak oder organischen Aminen wie beispielsweise Diethylamin, Triethylamin, Ethyldiisopropylamin, Prokain, Dibenzylamin, N- Methylmorpholin, Dihydroabiethylamin, 1-Ephenamin oder Methyl-piperidin.
Bevorzugt umfaßt die erfindungsgemäße Wirkstoffkombination a) Dexpanthenol und mindestens ein AntiWstaminikum, oder b) Dexpanthenol, Glycin und mindestens ein Antihistaminikum, oder c) Dexpanthenol und mindestens ein Corticosteroid, oder d) Dexpanthenol, Glycin und mindestens ein Corticosteroid, oder e) Dexpanthenol, mindestens ein Antihistaminikum und mindestens ein Corticosteroid, oder f) Dexpanthenol, Glycin, mindestens ein Antihistaminikum und mindestens ein Corticosteroid, oder g) Dexpanthenol und mindestens einen aus der synthetische Mastzelldegranulationshemmer und Leukotrien-Rezeptor- Antagonisten umfassenden Gruppe ausgewählten Wirkstoff, oder h) Dexpanthenol, Glycin und mindestens einen aus der synthetische Mastzelldegranulationshemmer und Leukotrien-Rezeptor-Antagonisten umfassenden Gruppe ausgewählten Wirkstoff.
Die vorstehend beschriebenen Wirkstoffkombinationen können optional zusätzlich mindestens einen weiteren Wirkstoff enthalten, der nicht aus den vorstehend erwähnten Gruppen der antiallergischen Wirkstoffe ausgewählt ist. Der genannte weitere Wirkstoff kann vorzugsweise aus
der Gruppe ausgewählt werden, die pflanzliche Wirkstoffe, Pflanzenextrakte (z. B. Nachtkerzenöl, Borretschöl, Johanniskrautextrakt, Aloe-vera-Extrakt, Hamamelis-Extrakt, Ringelblumenextrakt, Kamillenextrakt), pflanzliche Wirkstoffpräparate, Vitamine (z. B. Vitamin E und dessen Salze, Tretinoin), Dermatika (z. B. Harnstoff, Heparin, Hyaluronsäure, Allantoin, Milchsäure und deren Salze, Pyroglutaminsäure, Salicylsäure, Gerbstoffe, Steinkohlenteerlösung, Bisabolol, Glycyrrhizinsäure und ihre Salze, Bituminosulfonate wie z. B. Ammoniumbituminosulfonat oder Na-Bituminosulfonat), Antibiotika (z. B. Sulfonamide, Erythromycin), Antiseptika (z. B. Chlorhexidin), Lokalanästhetika (z. B. Lidocain, Polidocanol), Dekongestiva (z. B. Sympathomimetika wie Xylometazolin), sowie Hautpflegesubstanzen (z. B. Jojoba-Öl, Aloe Vera, Lanolin, pflanzliche Öle und Fette) und Lichtschutzsubstanzen (UV-A-Filter, UV-B-Filter, Breitbandfilter) umfaßt. Besonders vorteilhaft ist der Zusatz von Substanzen oder Stoffgemischen mit anti-inflammatorischer Wirkung, wie z. B. Steinkohlenteerlösung, Na-Bituminosulfonat, Ammoniumbituminosulfonat, Tretinoin, Nachtkerzenöl, Borretschöl, Johanniskrautextrakt, Aloe- vera-Extrakt, Hamamelis-Extrakt, Ringelblumenextrakt, Kamillenextrakt, Vitamin E und seine Salze, Bisabolol, Allantoin, Glycyrrhinsäure und -salze sowie Polidocanol.
Der Anteil des/der genannten weiteren Wirkstoffe(s) kann frei gewählt werden, vorzugsweise beträgt dieser Anteil 0,05 bis 50 Gew.-%, insbesondere 0,1 bis 20 Gew.-%, jeweils bezogen auf das Arzneimittel.
Unter „Kombination" in Sinne der Erfindung werden nicht nur Darreichungsformen verstanden, die alle Komponenten in einer gemeinsamen Darreichungsform enthalten (sog. Fixkombinationen), und Kombinationspackungen, die die Komponenten voneinander getrennt in einer einzigen Verpackung enthalten („Kit"), sondern auch gleichzeitig oder zeitlich versetzt applizierbare Komponenten, sofern sie zur Behandlung derselben Krankheit/desselben Symptoms eingesetzt werden.
Die Wirkstoffe der erfindungsgemäßen Medikamente und pharmazeutischen Formulierungen sind besonders geeignet, in einer fixen Kombination formuliert zu werden. Es ist bekannt, daß die Anwendungszuverlässigkeit (Compliance) bei Patienten entscheidend von den Faktoren Anzahl der Darreichungsformen pro Anwendungszeitpunkt, sowie gegebenenfalls von Größe und Gewicht der (festen peroralen) Arzneiform abhängig ist. Daher sollte sowohl die Anzahl der verschiedenen getrennt einzunehmenden Arzneimittel so gering wie möglich sein (Vorteil einer fixen Kombination), als auch gegebenenfalls die Größe und das Gewicht einer festen peroralen Darreichungsform so klein wie möglich gehalten werden, ohne die therapeutische Wirkstärke zu beeinträchtigen. Dadurch wird die Anwendung für den Patienten so angenehm wie möglich
gestaltet. Durch Realisieren fixer Kombinationen wird eine höchstmögliche Patienten-Compliance geboten und die Sicherheit sowie Zuverlässigkeit der Therapie entscheidend verbessert.
Bevorzugt enthalten die erfϊndungsgemäßen Arzneimittel die erfindungsgemäße Wirkstoffkombination in Form einer "fixen Kombination"; dies bedeutet, daß sämtliche Wirkstoffe, welche die jeweilige Wirkstoffkombination bilden, gemeinsam in einer Darreichungsform vorhanden sind und gemeinsam verabreicht werden.
Zur therapeutischen Anwendung werden die erfindungsgemäßen Wirkstoffkombinationen in Form von Arzneimittel im allgemeinen auf diejenigen Bereiche der Haut oder Schleimhaut, welche von allergischen Symptomen betroffen sind, lokal (d. h. topisch) appliziert.
Die erfindungsgemäßen Arzneimittel werden vorzugsweise als feste, halbfeste, flüssige oder gasförmige Arzneiformen zur topischen Anwendung auf der Haut oder Schleimhaut formuliert, insbesondere als Pulver, Puder, Pflaster, Staubaerosol, Granulat, Pastille, Lutschtablette, Kaugummi, Gel, Salbe, Lotion, Paste, Shampoo, Emulsion, Suspension, Lösung, Gurgelmittel, Saft, Sirup, Öl, Aerosol oder Inhalat.
Für die Behandlung allergischer Symptome an oder im Auge kommen insbesondere folgende Darreichungsformen in Betracht: Augensalben, Augensprays, Augentabletten, Augentropfen, Augenwässer, Augenbäder, Lidsalben.
Für die Behandlung allergischer Symptome der Nase oder Nasennebenhöhlen kommen insbesondere folgende Darreichungsformen in Betracht: Nasentropfen, Nasensprays, Nasensalben, Cremes, Lutschtabletten, Suspensionen oder Lösungen, Tinkturen, Lotionen, Emulsionen, Nasenpulver, Puder, Gele, Nasendusche, Nasenspülungen, Aerosole, Inhalate. Die Erfindung umfaßt ferner auch Darreichungsformen zur äußerlichen Behandlung der Nase (z. B. Salben, Gele, Cremes).
Für die Behandlung allergischer Symptome des Rachenraums oder der unteren Atemwege (Luftröhre, Bronchien, Bronchiolen, Lungenbläschen) kommen insbesondere folgende Darreichungsformen in Betracht: Aerosol, Inhalat (Suspensionen, Lösungen), Rachenspray, Mundspray, Pulver zum Inhalieren, Tropfen, Tabletten, Lutschtabletten, Pastillen, Kapseln, Sirup, Tinkturen, Gurgellösungen.
Für die Behandlung allergischer Symptome der Haut oder Schleimhäute kommen insbesondere folgende Darreichungsformen in Betracht: Gele, Salben, Lösungen, Puder, Cremes, Sprays, Emulsionen, Suspensionen, Tinkturen, Lotionen.
Die vorstehend genannten festen, halbfesten, flüssigen oder gasförmigen Arzneiformen sind dem Fachmann grundsätzlich bekannt und in der Literatur beschrieben; dasselbe gilt im Hinblick auf die Herstellung solcher Arzneiformen und die dafür geeigneten, pharmazeutisch akzeptablen Hilfsstoffe. Die Herstellung der erfindungsgemäßen Arzneimittel kann unter Berücksichtigung der erfindungsgemäß zu verwendenden Wirkstoffkombinationen mit den für die jeweilige Darreichungsform bekannten Verfahren unter Verwendung von pharmazeutischen Hilfsstoffen, wie sie dem Fachmann geläufig sind, erfolgen.
Als Hilfsstoffe kommen beispielsweise in Betracht: partikuläre Trägerstoffe (z. B. Talk, Zinkoxid, Stärke, Stärkederivate, Kieselgur); gelbildende Substanzen (z. B. Gelatine, Tragant, Cellulosederivate, Alginate, Polyacrylsäure); Befeuchtungsmittel (z. B. Harnstoff, Glycerin, Propylenglykol), haftklebende Polymere (z. B. Polyacrylate, sowie Klebharze); Salbengrundlagen (z. B. Vaselin, Fette, Cellulosederivate, Polyacrylsäure, Polyethylenglykole); Emulgatoren (z. B. Wollwachs, Sorbitanester, Monoglyceride); Konservierungsmittel (z. B. Benzalkoniumchlorid), Antioxidantien (z. B. Butylhydroxyanisol), Verdickungsmittel (z. B. Hydroxypropylmethylcellulose), pH-Wert-Korrigenzien; Bindemittel (z. B. Polyvinylpyrrolidon, Stärke, Hydroxypropylmethylcellulose, Polyethylenglykole), Füllstoffe (z. B. mikrokristalline Cellulose, Sorbitol), Farbstoffe, Aromastoffe, Süßstoffe (z. B. Sorbitol, Aspartam); Lösemittel (z. B. Wasser, Ethanol, Ethanol-Wasser-Gemische); Lösungsvermittler (z. B. Glycerol, Propylenglykol); Haut-Penetrationsverbesserer (z. B. Propylenglykol); Weichmacher (z. B. Sorbitol, Glycerin, Phthalsäureester); Netzmittel (z. B. Natriumlaurylsulfat, Polysorbate); synthetische und natürliche Öle (z. B. mittelkettige Triglyceride); Treibmittel für inhalative Darreichungsformen (z. B. Norfluran, Cryofluran, Dichlorfluormethan, Trichlorfluormethan, Propan, Butan, Isobutan, Stickstoff).
Flüssige Darreichungsformen werden ebenfalls nach Standardmethoden mit pharmazeutisch annehmbaren Hilfsstoffen hergestellt und enthalten die Wirkstoffe entweder gelöst oder suspendiert. Typische Applikationsvolumina dieser Arzneizubereitungen liegen zwischen 1 und 10 ml. Beispiele für Hilfsstoffe in diesen flüssigen Formulierungen sind: Lösungsmittel (z.B. Wasser, Alkohol, natürliche und synthetische Öle wie mittelkettige Triglyceride), Lösungsvermittler (z.B. Glycerol, Glykolderivate), Netzmittel (z.B. Polysorbat, Natriumlaurylsulfat), sowie weitere Hilfsstoffe, die zur Herstellung von Arzneiformulierungen der gewünschten Eigenschaften benötigt
werden, z.B. viskositätserhöhende Mittel, pH-Wert-Korrigenzden, Süßstoffe und Aromen, Antioxidantien, Stabilisatoren und/oder Konservierungsmittel.
Ferner können gemäß weiteren Ausführungsformen der Erfindung Arzneiformen verwendet werden, die eine verzögerte oder gesteuerte Wirkstofffreisetzung ermöglichen. Arzneiformen mit kontrollierter oder protrahierter Wirkstofffreisetzung (z. B. Depotpräparate und Retardpräparate, sowie Therapeutische Systeme) sind dem Fachmann allgemein bekannt. Durch Modifikation der
Zusammensetzung der Arzneimittel mittels bekannter Maßnahmen, beispielsweise mittels
Umhüllungen, Einbettungen oder Mikroverkapselung, kann die Wirkstofffreisetzung aus dem Arzneimittel gesteuert werden. Die verzögerte oder kontrollierte Freisetzung kann wahlweise nur einen, mehrere oder sämtliche Wirkstoffkomponenten der fixen Wirkstoffkombination betreffen.
Der gesamte Wirkstoffgehalt liegt bei den erfindungsgemäßen Arzneimitteln vorzugsweise im Bereich von 0,05 bis 70 Gew.-%, insbesondere im Bereich von 0,1 bis 50 Gew.-%, besonders bevorzugt im Bereich von 0,5 bis 15 Gew.-%, jeweils bezogen auf das Gesamtgewicht eines Arzneimittels. Die Wirkstoffe können in den Arzneimitteln insbesondere in partikulärer, dispergierter, gelöster, suspendierter oder emulgierter Form vorliegen.
Vorzugsweise handelt es sich bei den erfindungsgemäßen Arzneimitteln um Arzneimittel zur intranasalen, intraokulären oder dermalen Anwendung, oder um Arzneimittel zur inhalativen
Verabreichung zur Behandlung der Atemwege (insbesondere der Bronchien), oder um Arzneimittel zur lokalen Anwendung im Mund- und/oder Rachenraum. Dadurch eignen sich die erfindungsgemäßen Arzneimittel in besonderer Weise zur Behandlung von allergischen
Symptomen der Nasenschleimhaut, der Augenbindehaut, der Haut, der Mundschleimhaut, der Schleimhaut des Rachenraums, oder der unteren Atemwege (z. B. Trachea, Bronchien,
Bronchiolen).
Gemäß einer bevorzugten Ausführungsform umfaßt die vorliegende Erfindung dermale Darreichungsformen, die Dexpanthenol in Kombination mit einem Hl -Antihistaminikum enthalten. Bevorzugte Darreichungsformen sind Salben, Cremes und Gele. Bevorzugte Hl -Antihistaminika sind Bamipin, Clemastinfumarat und Chlorphenoxamin-HCl. Der Dexpanthenol-Gehalt liegt vorzugsweise im Bereich von 0,5 bis 20 Gew.-%, insbesondere von 5 bis 15 Gew.-%. Der Anteil des Antihistaminikums liegt vorzugsweise im Bereich von 0,01 bis 10 Gew.-%, insbesondere von 0,01 bis 5 Gew.-%. Diese dermalen Darreichungsformen können insbesondere zur Behandlung von allergischen Hauterkrankungen oder allergischen Symptomen der Haut verwendet werden.
Gemäß einer weiteren bevorzugten Ausführungsfoπn umfaßt die Erfindung Darreichungsformen zur intranasalen oder nasalen Anwendung, welche Dexpanthenol in Kombination mit einem Hl- Antihistaminikum enthalten. Bevorzugte Darreichungsformen sind Sprays, insbesondere Dosiersprays, oder Lösungen. Bevorzugte Hl -Antihistaminika sind Azelastin-HCl sowie Cromoglicinsäure. Der Dexpanthenol-Gehalt liegt vorzugsweise im Bereich von 0,5 bis 20 Gew.- %, insbesondere von 5 bis 15 Gew.-%. Der Anteil des AntiMstaminikums liegt vorzugsweise im Bereich von 0,01 bis 10 Gew.-%, insbesondere von 0,01 bis 5 Gew.-%. Diese Darreichungsformen können insbesondere zur Behandlung von allergischen Erkrankungen oder allergischen Symptomen der Nase verwendet werden. Vorzugsweise erfolgt die Verabreichung mittels eines Dosiersprays, wobei mit jedem Dosierstoß ein Volumen von 0,05 bis 0,2 ml verabreicht wird.
Gemäß einer weiteren bevorzugten Ausführungsform umfaßt die Erfindung dermale Darreichungsformen, die Dexpanthenol in Kombination mit einem Corticosteroid enthalten. Bevorzugte Darreichungsformen sind Salben, Cremes, Gele, Lösungen und Suspensionen. Bevorzugte Corticosteroide sind Hydrocortison, Beclometason- 17,21-dipropionat und Dexamethason. Der Dexpanthenol-Gehalt liegt vorzugsweise im Bereich von 0,5 bis 20 Gew.-%, insbesondere von 5 bis 15 Gew.-%. Der Anteil des Corticosteroids liegt vorzugsweise im Bereich von 0,01 bis 10 Gew.-%, insbesondere von 0,01 bis 5 Gew.-%. Diese dermalen Darreichungsformen können insbesondere zur Behandlung von allergischen Hauterkrankungen oder allergischen Symptomen der Haut verwendet werden.
Gemäß einer weiteren bevorzugten Ausführungsform umfaßt die Erfindung Darreichungsformen zur nasalen Anwendung, die Dexpanthenol in Kombination mit einem Corticosteroid enthalten. Bevorzugte Darreichungsformen sind Salben, Sprays, Aerosole, Lösungen und Suspensionen. Bevorzugte Corticosteroide sind Hydrocortison, Beclometason- 17,21 -dipropionat und Dexamethason. Der Dexpanthenol-Gehalt liegt vorzugsweise im Bereich von 0,5 bis 20 Gew.-%, insbesondere von 5 bis 15 Gew.-%. Der Anteil des Corticosteroids liegt vorzugsweise im Bereich von 0,01 bis 10 Gew.-%, insbesondere von 0,01 bis 5 Gew.-%. Diese Darreichungsformen können insbesondere zur Behandlung von allergischen Hauterkrankungen oder allergischen Symptomen der Nase verwendet werden.
Gegebenenfalls kann es erforderlich sein, von den genannten Mengen abzuweichen, und zwar in Abhängigkeit von Körpergewicht bzw. der Art des Applikationsweges, vom individuellen Verhalten gegenüber den Medikamenten, der Art von deren Formulierung und dem Zeitpunkt bzw. Intervall, zu welchem die Verabreichung erfolgt. So kann es in einigen Fällen ausreichend sein, mit weniger als der vorgenannten Mindestmenge auszukommen, während in anderen Fällen die
genannte obere Grenze überschritten werden muß. Im Falle der Applikation größerer Mengen kann es empfehlenswert sein, diese in mehreren Einzelgaben über den Tag zu verteilen.
Die Erfindung erstreckt sich ferner auf die Verwendung einer Wirkstoffkombination wie vorstehend definiert zur Behandlung von allergischen Symptomen der Haut, Schleimhaut (insbesondere Schleimhäute der Atemwege) oder Augenbindehaut, sowie auf die Verwendung einer solchen Wirkstoffkombination zur Herstellung eines Medikaments zu Behandlung der erwähnten allergischen Symptome. Der Begriff "Behandlung" schließt die vorbeugende (prophylaktische) Behandlung mit ein.
Bei den oben genannten allergischen Symptomen handelt es sich insbesondere um solche, die bei folgenden allergischen Erkrankungen auftreten: atopisches Ekzem, Psoriasis, Kontaktekzem, Hautallergien, Kontaktdermatitis, photoallergische Dermatitis, allergische Urtikaria, allergische Rhinitis (z. B. saisonale allergische Rhinitis, perenniale allergische Rhinitis), allergische Rhinokonjunktivitis, allergische Konjunktivitis, allergische Sinusitis, allergisches Larynxödem, allergische Alveolitis, allergische Bronchitis, allergisches Ödem, allergische Rosacea, Insektenstiche, Arzneimittelallergie, Arzneimittelexanthem.
Zu den genannten allergischen Symptomen gehören insbesondere folgende: Juckreiz; Hautrötung; Erytheme; Ekzeme; Schuppung der Haut; Krustenbildung; Nässen der Haut; Hauttrockenheit;
Hautentzündungen; Quaddeln, Papeln, Pusteln, Schwellungen; Schleimhautatrophie;
Entzündungszustände einer Schleimhaut, insbesondere der Nasenschleimhaut; Brennen im Nasen- und Rachenraum, Hustenreiz, Husten; Schluckschmerzen, Kratzen, Trockenheitsgefühl im Hals;
Hyperämie, Ödeme, Hypertrophie oder Schwellung einer Schleimhaut, insbesondere der Nasenschleimhaut; gesteigerte Sekretabsonderung (serös, mukös) einer Schleimhaut, insbesondere der Nasenschleimhaut; Niesreiz, Niesattacken; Behinderung der Nasenatmung; Rötung und
Schwellung der Bindehaut des Auges, Wucherungen der Bindehaut; Lidödem, Lidekzem.
Die Erfindung umfaßt ferner ein Verfahren zur therapeutischen Behandlung einer Person, die ein oder mehrere allergische Symptome an der Haut, Schleimhaut (insbesondere Schleimhäute der
Nase und der Atemwege) oder Bindehaut aufweist, und einer therapeutischen Behandlung bedarf.
Ein solches erfindungsgemäßes therapeutisches Verfahren umfaßt das Verabreichen einer fixen
Wirkstoffkombination, wie oben definiert, in einer therapeutisch wirksamen Dosis. Falls erforderlich, wird die Verabreichung der Wirkstoffkombination in geeigneten Zeitabständen wiederholt, um die angestrebte therapeutische Wirkung herbeizuführen.
Die Verabreichung der Wirkstoffkombination erfolgt vorzugsweise mittels einer festen, halbfesten, flüssigen oder gasförmigen Arzneiform, wie oben erwähnt. Die Wirkstoffkombination, oder ein Arzneimittel, das eine solche Kombination enthält, wird im allgemeinen lokal appliziert, d. h. auf diejenigen Bereiche der Haut oder Schleimhaut, die von allergischen Symptomen betroffen sind. Vorzugsweise wird das Arzneimittel bzw. die Wirkstoffkombination auf die Haut, auf eine Schleimhaut, oral, intranasal, intratracheal, intrabronchial oder intraokulär appliziert.
Die jeweils geeignete therapeutische Dosis hängt von der Art der in der jeweiligen Wirkstoffkombination enthaltenen Wirkstoffe und der Art der zu behandelnden Symptome ab, und kann auf dem Fachmann bekannte Weise ermittelt werden. Die erfindungsgemäßen Arzneimittel können die genannten Wirkstoffe in den üblichen Dosierungen enthalten. Aufgrund der erwähnten synergistischen Wirkung kann die in den erfindungsgemäßen Arzneimitteln enthaltene Dosierung eines jeden Wirkstoffs der Wirkstoffkombination niedriger gewählt werden als die Dosierung, welche für ein Monopräparat des jeweiligen Wirkstoffes üblich ist.
Die erfindungsgemäßen Wirkstoffkombinationen und Arzneimittel können nicht nur im Bereich der Humanmedizin, sondern auch im Bereich der Veterinärmedizin zur Behandlung von allergischen Symptomen der Haut oder Schleimhaut verwendet werden.
Beispiele
Die Erfindung wird an Hand der in den nachfolgenden Beispielen angegebenen Rezepturen näher erläutert:
A) Dermale Darreichungsformen, enthaltend Dexpanthenol in Kombination mit einem Hl- Antihistaminikum.
Beispiel 1 : Gel oder Creme 2 Gew.-% Bamipin
5 Gew.-% Dexpanthenol
Hilfsstoffe: gereinigtes Wasser, Makrogol (= Polyethylenglykol), Propylenglykol.
Das Gel bzw. die Creme wird zur Behandlung von allergischen Hauterkrankungen, insbesondere von juckenden Hauterkrankungen (z. B. Urtikaria) verwendet. Zur Anwendung wird das Gel bzw. die Creme ein- oder mehrmals täglich auf die betroffenen Hautpartien aufgetragen und in die Haut einmassiert.
Beispiel 2: Gel oder Creme 0,04 Gew.-% Clemastinfumarat 5 Gew.-% Dexpanthenol Hilfsstoffe: Carbomer, Propylenglykol, Polyolfettsäureester.
Das Gel bzw. die Creme wird zur Behandlung von allergischen Hauterkrankungen, insbesondere von juckenden Hauterkrankungen verwendet. Die Anwendung erfolgt wie in Beispiel 1.
Beispiel 3: Creme 1,5 Gew.-% Chlorphenoxamin-HCl
5 Gew.-% Dexpanthenol
Hilfsstoffe: gereinigtes Wasser, weißes Vaselin, Propylenglykol, mittelkettige Triglyceride,
Macrogol-Fettsäureester.
Die Creme wird zur Behandlung von allergischen Hauterkrankungen, insbesondere von juckenden Hauterkrankungen verwendet. Die Anwendung erfolgt wie in Beispiel 1.
B) Darreichungsformen zur intranasalen oder nasalen Anwendung, enthaltend Dexpanthenol in Kombination mit einem Hl -Antihistaminikum.
Beispiel 4: Nasenspray (Dosier-Spray) oder Lösung
0,1 Gew.-% Azelastin-HCl
5 Gew.-% Dexpanthenol
Hilfsstoffe: gereinigtes Wasser, Benzalkoniumchlorid, Dinatrium-EDTA.
Das Sprühstoß-Volumen beträgt 0,14 ml, entsprechend einer Einzeldosis von 0,14 mg Azelastin- HCl pro Sprühstoß.
Das Nasenspray wird zur Behandlung von allergischen Rhinitiden verwendet. Zur Anwendung wird 1- bis 3mal täglich jeweils ein Sprühstoß in jedes Nasenloch eingesprüht.
Beispiel 5: Nasenspray (Dosier-Spray) oder Lösung 3,1 Gew.-% Cromoglicinsäure, Dinatriumsalz 5 Gew.-% Dexpanthenol Hilfsstoffe: gereinigtes Wasser, Benzalkoniumchlorid, Dinatrium-EDTA.
Das Sprühstoß-Volumen beträgt 0,09 ml, entsprechend einer Einzeldosis von 2,8 mg Cromoglicinsäure pro Sprühstoß. Falls das gewählte Sprühstoß- Volumen 0,14 ml beträgt (wie in
Beispiel 4), ist die Wirkstoffkonzentration gegebenenfalls entsprechend anzupassen (2 Gew.-%), um die angegebene Einzeldosis von 2,8 mg Cromoglicinsäure pro Sprühstoß zu erhalten.
Das Nasenspray wird zur Behandlung von allergischem Schnupfen verwendet. Zur Anwendung wird 1- bis 5mal täglich jeweils ein Sprühstoß in jedes Nasenloch eingesprüht.
C) Darreichungsformen enthaltend Dexpanthenol in Kombination mit einem Corticosteroid.
Beispiel 6: Creme oder Salbe zur dermalen Anwendung 0,5 Gew.-% Hydrocortison
5 Gew.-% Dexpanthenol
Hilfsstoffe:
Creme: Weißes Vaselin, Isopropylmyristat, Sorbitanmonolaurat, Propylenglykol, Dinatrium-
EDTA, gereinigtes Wasser. Salbe: Weißes Vaselin, gereinigtes Wasser, Propylenglykol, flüssiges Paraffin, Wollwachs.
Die Creme bzw. Salbe wird zur Behandlung von allergischen Hauterkrankungen (z. B. Ekzeme, insbesondere Kontaktekzeme, atopisches Ekzem; Hautentzündungen, Urtikaria, Hautdermatosen) verwendet. Zur Anwendung wird die Creme bzw. die Salbe ein- oder mehrmals täglich auf die betroffenen Hautpartien aufgetragen und leicht eingerieben.
Beispiel 7: Lösung zur dermalen Anwendung 1 Gew.-% Hydrocortison 10 Gew.-% Dexpanthenol Hilfsstoffe: gereinigtes Wasser, Isopropylalkohol, Konservierungsmittel (Methyl-4- hydroxybenzoat, Propyl-4-hydroxybenzoat) . Verwendung und Anwendung: wie in Beispiel 6.
Beispiel 8: Suspension zur dermalen oder nasalen Anwendung 0,05 Gew.-% Beclometason- 17,21 -dipropionat 5 Gew.-% Dexpanthenol
Hilfsstoffe: gereinigtes Wasser, Benzalkoniumchlorid, wasserfreie Glucose, Phenylethylalkohol, Polysorbat 80.
Die vorstehend beschriebene Suspension kann auch als Pumpspray formuliert werden, beispielsweise zur Verwendung als Nasenspray.
Die Suspension bzw. das Spray wird zur Vorbeugung oder Behandlung von allergischen Rhinitiden verwendet. Zur Anwendung wird 2- bis 5mal täglich jeweils ein Sprühstoß (0,09 ml) in jedes Nasenloch eingesprüht.
Beispiel 9: Dosier-Aerosol zur dermalen oder nasalen Anwendung
0,1 Gew.-% Beclometason- 17,21-dipropionat
5 Gew.-% Dexpanthenol
Hilfsstoffe: Ethanol, Norfluran (1,1,1,2-Tetrafluorethan)
Sprühstoß- Volumen: 0,05 ml.
Intranasale Anwendung: wie in Beispiel 8 (oben).
Dermale Anwendung: Das Dosier-Aerosol wird zur Behandlung des allergischen Ekzems oder der allergischen Dermatitis verwendet. Zur Anwendung wird das Aerosol 2- bis mehrmals täglich auf die betroffenen Hautpartien gesprüht.
Beispiel 10: Nasensalbe
0,2 Gew.-% Dexamethason
3 bis 5 Gew.-% Dexpanthenol
Hilfsstoffe: weißes Vaselin, Paraffin, Wollwachs, gereinigtes Wasser.
Die Nasensalbe wird zur Behandlung von allergischen Rhinitiden verwendet. Zur Anwendung wird eine etwa erbsengroße Menge in jede Nasenöffnung eingeführt und durch Massieren von außen verteilt.
Beispiel 11 : Nasenspray (Dosier-Spray) oder Lösung 3,5 Gew.-% Dexpanthenol
Hilfsstoffe: steriles isotonisiertes Meerwasser, Kaliurndihydrogenphosphat,
Kaliummonohydrogenphosphat
Das Nasenspray wird zur Behandlung von allergischem Schnupfen und/oder von allergischen Rhinitiden verwendet und ist mit anderen Präparaten, die mindestens einen anti-allergischen Wirkstoff aus der Gruppe Antihistaminika, Corticosteroide, synthetische Mastzellendegranulationshemmer und/oder Leukotrien-Rezeptor-Anatagonisten enthalten, zeitgleich oder zeitversetzt zu applizieren. Zur Anwendung wird 1- bis 5mal täglich jeweils ein Sprühstoß in jedes Nasenloch eüigesprüht.
Erste klinische Erkenntnisse — offene Beobachtung:
Die Wirkung eines Dexpanthenol Nasensprays aus Beispiel 11, enthaltend 3.5 % Dexpanthenol, wurde an fünf Patienten getestet, die an diagnostiziertem Heuschnupfen (Gräser Pollinosis) litten.
Alle Patienten waren schon premedikamentiert mit Hilfe entweder eines topischen Antiallergikums in Form von Spray (Azelastin) oder eines oral eingenommenen Antiallergikums (Cetirizin) in den empfohlenen Dosen (Azelastin: max. 2 nasale Applikationen pro Tag, Cetirizin: 10 mg einmal täglich). In einem schweren Fall wurden beide Antiallergika verwendet.
Die beobachteten Symptome wie laufende oder verstopfte Nase, Niesen, tränende Augen, rote und entzündete Haut in und um die Nase wurden als mittel bis schwer (in Abhängigkeit vom Zeitabstand zur Medikamenteneinnahme) klassifiziert.
Die Patienten klagten ebenfalls über Nasentrockenheit und Nasenbluten. Diese Symptome wurden der Verwendung der Antiallergika zugeschrieben, deren Nebenwirkungen allgemein bekannt sind.
Die Patienten wurden aufgefordert, das Dexpanthenol Nasenspray aus Beispiel 11 in Situationen, in dienen sie sich hohen Dosen eines Allergens ausgesetzt sahen (z.B. beim Durchqueren einer Wiese mit blühenden Gräsern) zusätzlich zu ihrer eingenommenen Medikation an Antiallergika zu verwenden. Über je zwei bis drei Anwendungen wurde berichtet.
Befunde:
Zwei Patienten, die allein Azelastin zur Behandlung ihrer Symptome applizierten, berichteten, dass die Verwendung des Dexpanthenol enthaltenden Nasensprays aus Beispiel 11 zu einer Besserung bei den Symptomen der Nasentrockenheit und der Entzündung der Haut in und um die Nase und zu einem Rückgang der Symptome der laufenden und verstopften Nase führte. Sie sahen sich in der Lage, die Applikation von Azelastin von zwei Anwendungen täglich auf eine Anwendung zu reduzieren. Drei Patienten, die Cetirizin einnahmen, berichteten von einer Besserung bei den Symptomen der Nasentrockenheit oder rissiger Haut, von weniger Entzündungsreaktion der Haut in und um die Nase und reduzierter allergischer Rhinitis. Eine Verlängerung des Zeitintervalls zwischen den Tabletteneinnahmen an Cetirizin wurde möglich.
Fazit:
Diese ersten Befunde bestätigen die synergistische Wirkung von Dexpanthenol mit Antiallergika bei Anwendung in Kombination.
Darüber hinaus bewirkt Dexpanthenol eine Befeuchtung der Nasenhaut sowie -Schleimhaut und reduziert so Nebenwirkungen wie Nasentrockenheit oder Nasenbluten, die oft während der Anwendung von Antiallergika oder Antihistaminika auftreten, und verbessert damit das
Nebenwirkungsprofϊl der entsprechenden Wirkstoffe.
Claims
1. Kombination umfassend mindestens einen Wirkstoff aus der Pantothensäure und Pantothensäurederivate umfassenden Gruppe und mindestens einen anti-allergischen Wirkstoff aus der Antihistaminika, Corticosteroide, synthetische Mastzelldegranulationshemmer und Leukotrien- Rezeptor- Antagonisten umfassenden Gruppe.
2. Kombination nach Anspruch 1, dadurch gekennzeichnet, daß der Wirkstoff aus der Pantothensäure und Pantothensäurederivate umfassenden Gruppe Dexpanthenol ist.
3. Kombination nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß die Kombination zusätzlich Glycin enthält.
4. Kombination nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, daß sie mindestens einen Wirkstoff aus der Pantothensäure und Pantothensäurederivate umfassenden
Gruppe, und mindestens einem Wirkstoff aus der Wirkstoffklasse der Antihistaminika oder/und mindestens einem Wirkstoff aus der Wirkstoffklasse der Corticosteroide umfasst.
5. Kombination nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, daß sie zusätzlich einen oder mehrere Bestandteile umfasst, die aus der pflanzliche Wirkstoffe,
Pflanzenextrakte, pflanzliche Wirkstofrpräparate, Vitamine, Dermatika, Antibiotika, Antiseptika, Lokalanästhetika, Dekongestiva, Hautpflegesubstanzen und Lichtschutzsubstanzen umfassenden Gruppe ausgewählt sind.
6. Kombination nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, daß die Kombination mindestens einen Wirkstoff aus der Wirkstoffklasse der Antihistaminika enthält, vorzugsweise ausgewählt aus der Ketotifen, Thonzylamin, Mepyramin, Thenalidin, Tripelennamin, Chlorpyramin, Promethazin, Tolpropamin, Dimetinden, Clemastin, Bamipin, Isothipendyl, Diphenhydramin, Diphenhydraminmethylbromid, Chloφhenoxamin, Pheniramin, Diphenylpyralin, Dioxopromethazin, Dimenhydrinat, Thiethylperazin und Meclozin, Azelastin, Levocabastin, Astemizol, Mebhydrolin, Terfenadin, Mequitazin, Cetirizin, Emedastin, Mizolastin, Olopatadin, Epinastin und Antazolin umfassenden Gruppe der Antihistaminika.
7. Kombination nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, daß die Kombination mindestens einen Wirkstoff aus der Wirkstoffklasse der Corticosteroide enthält, vorzugsweise ausgewählt aus der Triamcinolon, Dexamethason, Hydrocortison,
Hydrocortisonacetat, Hydrocortisonbutyrat, Hydrocortisonbuteprat, Prednisolon, Betamethason, Methylprednisolon, Clobetason, Flumetason, Fluocortin, Fluperolon, Fluorometholon, Flupredniden, Desonid, Triamcinolon, Alclometason, Dexamethason, Clocortolon, Betamethason, Fluclorolon, Desoximetason, Fluocinolonacetonid, Fluocortolon, Diflucortolon, Fludroxycortid, Fluocinonid, Budesonid, Diflorason, Amcinonid, Halometason, Mometason, Methylprednisolonaceponat, Beclometason, Hydrocortisonaceponat, Fluticason, Prednicarbat, Difluprednat, Ulobetasol, Clobetasol, Halcinonid, Medryson, Desonid, Formocortal, Rimexolon, Mazipredon, Flunisolid und Tixocortol umfassenden Gruppe der Corticosteroide.
8. Kombination nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, daß die Kombination mindestens einen synthetischen anti-allergischen Wirkstoff enthält, der nicht aus der
Antihistaminika und Corticosteroide umfassenden Gruppe ausgewählt ist.
9. Kombination nach Anspruch 8, dadurch gekennzeichnet, daß der genannte antiallergische Wirkstoff aus der synthetische Mastzelldegranulationshemmer und Leukotrien-Rezeptor-Antago- nisten umfassenden Gruppe ausgewählt ist, vorzugsweise aus der Cromoglicinsäure, Spagluminsäure, Lodoxamid und Nedocromil umfassenden Gruppe.
10. Kombination nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, daß die Kombination a) Dexpanthenol und mindestens ein Antihistaminikum, oder b) Dexpanthenol, Glycin und mindestens ein Antihista-minikum, oder c) Dexpanthenol und mindestens ein Corticosteroid, oder d) Dexpanthenol, Glycin und mindestens ein Antihista-minikum, oder e) Dexpanthenol, mindestens ein Antihistaminikum und mindestens ein Corticosteroid, oder f) Dexpanthenol, Glycin, mindestens ein Antihistaminikum und mindestens ein Corticosteroid, oder g) Dexpanthenol und mindestens einen aus der synthetische Mastzelldegranulationshemmer und Leukotrien-Rezeptor- Antagonisten umfassenden Gruppe ausgewählten Wirkstoff, oder h) Dexpanthenol, Glycin und mindestens einen aus der synthetische Mastzelldegranulationshemmer und Leukotrien-Rezeptor-Antagonisten umfassenden Gruppe ausgewählten Wirkstoff umfasst.
11. Kombination nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, daß die Kombination eine fixe Kombination ist.
12. Arzneimittel enthaltend eine Kombination nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, daß es als feste, halbfeste, flüssige oder gasförmige Arzneiform zur topischen Anwendung auf der Haut, Schleimhaut oder Bindehaut vorliegt, vorzugsweise als Pulver, Puder, Pflaster, Staubaerosol, Granulat, Pastille, Lutschtablette, Kaugummi, Gel, Salbe, Lotion, Paste, Shampoo, Emulsion, Suspension, Lösung, Gurgelmittel, Öl, Spray, Aerosol oder Inhalat.
13. Arzneimittel nach Anspruch 12, dadurch gekennzeichnet, daß es ein Arzneimittel zur intranasalen, intraokulären oder dermalen Anwendung ist.
14. Arzneimittel nach einem der Ansprüche 12 bis 13, dadurch gekennzeichnet, daß es ein Arzneimittel zur lokalen Anwendung im Mund- und/oder Rachenraum ist.
15. Arzneimittel oder Kombination nach einem der Ansprüche 1 bis 14 zur Behandlung von allergischen Symptomen der Haut, Schleimhaut oder Augenbindehaut, insbesondere zur Behandlung von Symptomen, die bei einer der nachfolgend genannten allergischen Krankheiten auftreten: atopisches Ekzem, Psoriasis, Kontaktekzem, Hautallergien, Kontaktdermatitis, photoallergische Dermatitis, allergische Urtikaria, allergische Rhinitis, allergische Rhinokonjunktivitis, allergische Konjunktivitis, allergische Sinusitis, allergisches Larynxödem, allergische Alveolitis, allergische Bronchitis, allergisches Ödem, allergische Rosacea, Insektenstiche, Arzneimittelallergie, Arzneimittelexanthem.
16. Verwendung einer Kombination nach einem der Ansprüche 1 bis 11 zur Herstellung eines Arzneimittels zur Behandlung von allergischen Symptomen der Haut, Schleimhaut oder Augenbindehaut, insbesondere zur Behandlung von Symptomen, die bei einer der folgenden Krankheiten auftreten: atopisches Ekzem, Psoriasis, Kontaktekzem, Hautallergien, Kontaktdermatitis, photoallergische Dermatitis, allergische Urtikaria, allergische Rhinitis, allergische Rhinokonjunktivitis, allergische Konjunktivitis, allergische Sinusitis, allergisches Larynxödem, allergische Alveolitis, allergische Bronchitis, allergisches Ödem, allergische Rosacea, Insektenstiche, Arzneimittelallergie, Arzneimittelexanthem.
17. Verwendung einer Kombination nach einem der Ansprüche 1 bis 11 zur Herstellung eines Arzneimittels zur Behandlung von allergischen Erkrankungen oder allergischen Symptomen der Nase.
18. Verwendung einer Kombination nach einem der Ansprüche 1 bis 11 zur Herstellung eines Arzneimittels zur Behandlung von allergischen Erkrankungen oder allergischen Symptomen der Atemwege, insbesondere des Rachens, der Trachea, der Bronchien und Bronchiolen.
19. Verfahren zur therapeutischen Behandlung einer Person, die ein oder mehrere allergische Symptome an der Haut, Schleimhaut oder Bindehaut aufweist und einer therapeutischen Behandlung bedarf, wobei das Verfahren das Verabreichen einer Wirkstoffkombination gemäß einem der vorangehenden Ansprüche 1 bis 11 in einer therapeutisch wirksamen Dosis umfaßt.
