Abstract
The invention relates to a treatment and rehydration composition which is used to limit the side effects which occur during gastro-enteritis, such as feelings of malaise and fatigue, in adults and, in particular, among the elderly. The inventive composition comprises an intestinal antiseptic, such as nifuroxazide, or an agent used to alter the intestinal transit, such as loperamide or even clays, glucose and sucrose. Said composition is characterised in that the ratio of glucose to sucrose is such that the osmolality is between 260 and 290 mosm and efficiency and acceptability are combined.
Description
COMPOSITION DE TRAITEMENT ET DE REHYDRATATION POUR LIMITER LES EFFETS SECONDAIRES DES GASTRO- ENTERITES CHEZ TREATMENT AND REHYDRATION COMPOSITION FOR LIMITING SIDE EFFECTS OF GASTROENTERITIS IN
L'ADULTEADULTS
The present invention relates to a treatment and rehydration composition for limiting the side effects of gastroenteritis in adults.
These acute diarrheas are defined by pathophysiological concepts, namely a malabsorption of water which enters a large quantity every day into the intestine.
In addition, to this large liquid volume, the secretions of digestive origin, stimulated by the ingestion of food.
This volume of water is normally reabsorbed in normal regime by the small intestine and the colon, leaving only a small part in the stool.
Diarrhea therefore results from excess water in the stool, linked to poor reabsorption and / or abnormal secretion.
We know that dehydration is a major complication of diarrhea and we know that in infants, we can reduce the risk of death linked to this dehydration by forcibly administering rehydration products intravenously in hospitals or by oral ingestion. specific solutes with very precise compositions and osmolalities.
Acute gastroenteritis in adults is accompanied by great fatigue, feelings of discomfort and hypotension.
According to the inventor of the composition which will be described, these general signs are in reality signs directly linked to dehydration. This interpretation has never been described in this way to date. The solution of the prior art consisted in treating gastroenteritis with antiseptics, prebiotics, modifiers of intestinal transit or soils, especially clays.
We thus know as antiseptic Nifuroxazide and as intestinal transit retarder Loperamide. It is advisable to associate with this treatment the intake of glucose which we know it seems to act to improve the sodium absorption system because it is intimately linked to the movements of water in the body.
Glucose, but preferably sucrose allows, in a defined proportion and associated with electrolytes, the reversal of the flows of water and electrolytes, rehydrating the subject.
This is how the advice given consists notably in ingesting sugary drinks.
In the normally constituted adult, the corpulence is considered sufficient and the risks of death are less rare. The high loss of water recorded during acute diarrhea can be compensated. We also know that the volume of extra-cellular fluids in relation to fluid loss in adults is higher than that of infants or the elderly.
Nevertheless, in the middle-aged adult, the symptoms which the present invention relates to the dehydration associated with diarrhea, cause a feeling of discomfort and remain disabling.
As this risk of mortality is eliminated, the compositions administered in the usual manner consist solely of compositions of one or more of the drugs known from the prior art by giving dietary advice consisting in taking sweet or savory drinks.
First of all, it is known that such drinks do not have the necessary osmolality and are not very effective or even increase the amount of liquid in transit. In addition, the patient does not always follow the dietary instructions.
The composition according to the present invention consists in systematically associating, for adults, in the same galenic form, one or more known products for the treatment of diarrhea such as an antiseptic, Nifuroxazide or a modifier of intestinal transit, Loperamide or a prebiotic or a probiotic or even clays and, necessarily, a rehydrating product.
According to the present invention, the composition thus prepared is effective as will be described below since it is necessarily absorbed simultaneously. But there remains another problem, that of the certainty that said effective composition is well taken. However, the acceptability of such a composition is not acquired and if the drink is not taken, the patient does not receive any treatment. The criterion of taste is essential for the patient to ingest the product offered. Now we know that solutions based on glucose or bicarbonate are completely unacceptable for adults, more than for an infant who is in a state of severe dehydration by reflex of survival, even absorbs a drink whose taste would be unacceptable for an adult .
Glucose has been replaced in an interesting way, at least in part by sucrose. As for bicarbonate, it is substituted for bicarbonate citrate. However, no composition is known of which the acceptability has been worked to improve the setting as in the case of the present invention. Too much osmolαlité is linked to a bad taste but it is necessary to maintain a sufficient osmolality of the whole of the composition to make it as effective as possible.
This is the object of the present invention which describes such a composition and shows by test results, the surprising effectiveness that it can have on side effects.
A/ COMPOSITIONSThe composition according to the invention is now described with reference to specific examples to show its surprising capacities for the side effects of acute gastroenteritis in adults. A / COMPOSITIONS
1/ Antiseptique intestinal :1 / Intestinal antiseptic:
It is known that the intestinal antiseptic has an action on the etiological agent and a curative effect.
The composition proposes to combine Nifuroxazide and a rehydration solution.
The dosage is determined by the frequency of taking the rehydration solution and the amount of antiseptic is adjusted accordingly.
Une composition particulièrement convenable comprend :A particularly suitable composition includes:
- 909,2 mg de Nifuroxazide, - 16 g de glucose,- 909.2 mg of Nifuroxazide, - 16 g of glucose,
- 20 g de saccharose,- 20 g of sucrose,
- 23,1 milliéquivalents de citrate,- 23.1 milliequivalents of citrate,
- 26,5 milliéquivalents de chlorure,- 26.5 milliequivalents of chloride,
- 26 milliéquivalents de potassium, et - 67,8 milliéquivalents de sodium.- 26 milliequivalents of potassium, and - 67.8 milliequivalents of sodium.
L'ensemble présentant une osmolalité de 280 mosml/gThe whole having an osmolality of 280 mosml / g
- 909,2 mg de Nifuroxazide,Also to obtain this composition, the following are mixed in powder form: - 909.2 mg of Nifuroxazide,

The administration of this composition is carried out orally.
A dose of 200 mg of this powder makes it possible to obtain a composition of 220 ml by adding water. This corresponds to the dilution of these 200 mg in a glass of water. An osmolality of between 260 and 290 mmm should be observed by the proportions of glucose and sucrose.
Surprisingly, these ranges must be very narrow to maintain the desired efficiency and the best acceptability. This osmolality selection considerably increases the results for patients.
It is possible to add a flavor to optimize acceptability.
2/ Modificateur du transit intestinal :2 / Intestinal transit modifier:
On sait donc que le modificateur de transit intestinal a un effet curatif .We therefore know that the intestinal transit modifier has a curative effect.

- 9,08 mg de Loperamide,- 9.08 mg of Loperamide,
- 16 g de glucose,- 16 g of glucose,
- 20 g de saccharose- 20 g of sucrose
The administration of this composition is carried out orally.
A dose of 2 mg of this powder makes it possible to obtain a composition of 220 ml by adding water. This corresponds to the dilution of these 2 mg in a glass of water. An osmolality of between 260 and 290 mmm should be observed by the proportions of glucose and sucrose.
Surprisingly, these ranges must be very narrow to maintain the desired efficiency and the best acceptability.
B/ ESSAIS CLINIQUES :It is possible to add a flavor to optimize acceptability. B / CLINICAL TRIALS:
Trials are carried out on adult patients with acute diarrhea with commercial products in capsules versus sachets of powder combining the rehydrating agent and the same active ingredient.
All clinical trials are conducted in intra-individual cross-over, each patient necessarily receiving each product, in random order or in parallel groups.
EFFICACITEThese tests relate to the efficacy, tolerance and acceptability of the products. EFFICIENCY
1st trial: 31 adult patients, cross-analysis, randomized trial with sachets of powder with composition 1 / based on Nifuroxazide versus Nifuroxazide alone, in capsules.
Le test porte sur la sensation de fatigue et la sensation de malaise avec des valeurs exprimées selon l'EVA (Echelle Visuelle Analogique) de 0 à 100.The test relates to the feeling of fatigue and the feeling of discomfort with values expressed according to the EVA (Analog Visual Scale) from 0 to 100.
There is an improvement in the feeling of fatigue because the difference is greater in the case of the powder having the composition according to the invention than in the case of commercial capsules.
2eme essai : 36 patients adultes, analyse croisée, essai randomisé avec des sachets de poudre ayant la composition 2/ à base de Loperamide versus2 nd test: 36 adult patients, cross analysis, randomized trial with sachets of powder having the composition 2 / based Loperamide versus
Loperamide seul, en gélules.Loperamide alone, in capsules.
The test relates to the feeling of discomfort with values expressed according to the EVA (Analog Visual Scale) from 0 to 100.
There is an improvement in the feelings of discomfort because the difference is greater in the case of the powder having the composition according to the invention than in the case of capsules. The value obtained of 7.72 on the EVA scale is also low, which is important in addition to the concept of reduction in absolute value, the patient feels the discomfort more weakly.
•oe e essai :• this test:
This trial is conducted with 82 adult patients affected by acute diarrhea, for 48 hours, randomized, in 4 parallel groups, with the two active ingredients.
It is again noted that the results confirm the trend of the previous tests. The compositions according to the present invention make it possible to improve the general condition of the patient by eliminating the feelings of discomfort linked to dehydration.
It can be noted that the trials, even if significant, should be extended by clinical trials on larger populations and preferably on more severe cases of diarrhea.
In the same way that antiseptics and / or modifiers of the intestinal transit could be used, one can resort to prebiotics or to earths like clays, Smectite, Attapulgite and identical results are expected.
4imB essai4 imB test
This is not a full-fledged trial, but the conclusions result from statistical analyzes that focus on improving the consistency of stool, these analyzes being carried out in parallel with the treatments administered to patients.
There is a larger difference in the case of treatment with the composition of the invention, in the form of powder. We arrive at a value on the EVA scale which is interesting, less than 20. The test focused on the same 82 adult patients affected by acute diarrhea, for 48 hours, randomized, in parallel groups, with the two active ingredients. .
ACCEPTABILITE essaiACCEPTABILITY test
In order to be certain that the patient will ingest the powder diluted in a glass of water, ie more than 200 ml of liquid, the composition must still have a certain acceptability.
The tests carried out on 82 adult patients affected by acute diarrhea, for 48 hours, randomized, in parallel groups, with the two active ingredients, show, in the table below, less good results for the diluted powder than for the capsules. . These capsules, by principle, leave no taste and are swallowed, which does not really cause swallowing or taste problems.
The table below shows values for the powder which are nevertheless in the lower third of the range from 1 to 100 of the EVA scale, which is completely acceptable and which does not pose a general problem of acceptability. . We are far from the results that could be obtained with bicarbonate and / or glucose powders.
Having a powder requiring a volume of rehydration liquid, the osmolality of which is adapted and the acceptability of which is good, makes it possible to more quickly treat the sensations of discomfort associated with acute gastroenteritis. This combination of active product and a rehydration solution whose osmolality is specifically adapted, is particularly effective because rehydration is no longer a simple recommendation but an obligation. It is no longer a simple recommendation of more or less followed rehydration and which, if they are followed, are by ingesting liquids of unsuitable osmolality but indeed of a full-fledged composition whose synergy of effects is particularly concrete.
Given the proportions between the mass of intercellular water and the volume of stool, the elderly is more sensitive to this rehydration effect than a healthy middle-aged adult. The functional benefit observed in middle-aged adults is more marked in the elderly and generally translates into benefits in terms of morbidity (reduction in hospitalizations, hypotension, etc.). The benefit will be all the more important as the diarrhea is severe and the subject is weakened by undernutrition or other associated pathologies.
