Abstract
L'invention concerne un instrument pour l'insémination artificielle, la transplantation d'embryons in-utéro ou le prélèvement de liquide folliculaire par voie transpéritonéale ches les animaux, notamment sur les ovins, caprins, bovins ou équins, cet instrument est caractérisé en ce qu'il est constitué d'une sonde rigide (5) comprenant deux canaux tangentiels (5₁, 5₂) définis par deux conduits de diamètres différents, logés l'un dans l'autre, l'un des conduits (5₂) étant relié à une pompe d'aspiration l'autre (5₁) servant de guide à un cathéter punctiforme, la base de ce dernier étant encochée radialement pour permettre de visionner, sous endoscopie, le déplacement de l'aiguille (12).
Description
- L'invention concerne un procédé et un instrument pour l'insémination artificielle, la transplantation d'embryons in-utéro ou la ponction de liquides folliculaires, par voie transpéritonéale, chez les animaux, notamment les ovins, caprins, bovins, équins ou autres.
- S'agissant des ovins et des caprins, on sait les difficultés que l'on rencontre lors de l'insémination artificielle ou du transfert d'embryons par les voies naturelles. Le corps de l'utérus étant difficilement franchissable chez les caprins et, infranchissable chez les ovins, on était donc contraint, jusqu'alors, de procéder à une intervention chirurgicale, sous anesthésie générale, avec les inconvénients que cela engendre du point de vue médical et économique. Une telle opération chirurgicale suppose, en effet, l'existence d'un matériel sophistiqué qui n'est évidemment pas adpaté à la souplesse requise pour les techniques de terrain. A cet investissement vient s'ajouter les frais d'anesthésie ainsi que les soins post-opératoires, ce qui grève sensiblement le coût de l'intervention.
- Pour remédier à ces inconvénients, on fait appel, selon l'invention, à un procédé et un instrument de conception simple permettant l'insémination artificielle ou le transfert d'embryons, sous endoscopie, sans anesthésie générale, ce qui écarte les risques post-opératoires, tout en permettant d'augmenter la fiabilité et l'efficacité de l'intervention.
- Le procédé selon l'invention est caractérisé en ce que l'on pratique dans la paroi abdominale de l'animal, de fines incisions rapprochées, l'une étant destinée à recevoir un endoscope permettant de visionner le champ opératoire, l'autre étant destinée au passage d'un trocart pourvu d'un manchon guide, le retrait de ce trocart, après perforation, permettant le passage, dans le manchon guide resté en place, d'un instrument d'insémination, de transplantation ou de prélèvement de liquides folliculaires, présentant l'originalité d'être relié à un moyen d'aspiration et de comporter par ailleurs, une seringue d'injection pour permettre d'une part, la fixation, par effet de ventouse, de la paroi d'une des deux cornes utérine et, d'autre part, de traverser l'endomètre pour amener l'aiguille dans la lumière de la corne à traiter, afin d'y déposer la semence ou les embryons ou encore de pratiquer les prélèvements souhaités, après quoi on traite, toujours sous endoscopie, la deuxième corne, après retrait de l'aiguille et nouvelle orientation de l'instrument.
- L'instrument pour la mise en oeuvre de ce procédé est constitué d'une sonde rigide formée de deux conduits coaxiaux séparés, l'un relié à une pompe d'aspiration, l'autre servant de guide à un cathéter punctiforme dont la base est encochée radialement pour que l'opérateur puisse visionner le déplacement de l'aiguille et notamment contrôler son introduction dans la lumière de la corne.
- Selon une caractéristique de l'invention le cathéter est constitué d'une seringue comprenant d'une part une gaine, fermée à une de ses extrémités par un embout prolongé par une aiguille, ledit embout comportant une cavité interne tronconique, d'autre part un piston coulissant avec étanchéité dans cette gaine.
- Selon un mode de réalisation préférentiel, le cathéter renferme une paillette fine de conditionnement de la semence ou des embryons, rendu solidaire de l'embout par emmanchement à force dans sa cavité tronconique, l'injection étant alors réalisée par le piston coulissant placé en appui, par son extrémité frontale, sur le bouchon obturant usuellement ce type de paillettes.
- D'autres caractéristiques et avantages ressortiront de la description ci-après et des dessins annexés dans lesquels :
- - la Fig. 1 est une vue schématique illustrant, en coupe, l'instrument d'insémination, de transfert d'embryons ou de ponction de liquides folliculaires, l'endoscope ayant été volontairement représenté bien que dans la pratique il serait masqué par ledit instrument,
- - la Fig. 2 est une vue en coupe selon la ligne AA de la figure 1,
- - la Fig.3 est une vue partielle en coupe de l'extrémité postérieure de l'instrument,
- - la Fig.4 est une vue en coupe longitudinale de l'extrémité introductive de l'instrument utilisant une paillette fine de conditionnement.
- Le but essential de l'invention est de mettre à la disposition des praticiens une technologie et une instrumentation souples, de conception simple, qui soient néanmoins d'une grande fiabilité et d'un coût le plus réduit possible.
- Pour ce faire, et sans récuser la chirurgie sous anesthésie générale, on fait appel ici à de la petite chirurgie non stressante pour l'animal et qui, en raison de son innocuité, peut être facilement renouvelée sans grand risque potentiel.
- Conformément au procédé, on pratique sur l'abdomen de l'animal, (sensiblement au voisinage de ce que l'on dénomme la "ligne blanche" pour les ovins et caprins et sur le flanc pour les bovins et équins) deux incisions 1 et 2, fines et rapprochées, et on introduit dans l'une d'elles, un endoscope 3 permettant de visualiser le champ opératoire. On perfore ensuite la paroi abdominale et le péritoine à l'aide d'un trocart, constitué de façon classique d'un mandrin (non représenté) et de son manchon guide 4 qui servira, après retrait du mandrin, de fourreau pour l'introduction et le guidage de l'instrument 5 spécifique de l'invention.
- Comme illustré en figure 1, cet instrument s'apparente à une sonde rigide comprenant deux canaux tangentiels 5₁, 5₂ (figure 2), ceux-ci étant définis par deux tubes 6 et 7, de diamètres différents, logés l'un dans l'autre et réunis à leurs deux extrémités par brasure, le conduit 5₁ étant séparé du conduit 5₂ et comportant une encoche radiale 5₃ de visualisation de l'aiguille.
- Le canal extérieur 5₂ est relié par raccord 8 à un moyen d'aspiration, tandis que le canal intérieur 5₁ sert de guide à un cathéter punctiforme 9 formé essentiellement (figure 4) d'une gaine 10, dont la base est obturée par un embout 11 prolongé par une aiguille 12, cette gaine introduite dans le canal 5₁ recevant un piston 13, étalonné à l'aide de repères 13₁, dont la tête coulisse avec étanchéité dans le conduit intérieur de la gaine.
- Dans la version la plus simple, en particulier lorsque l'on procède à l'injection de semences fraiches ou à la transplantation immédiate d'embryons, le cathéter est constitué, comme décrit précédemment, d'une simple gaine à aiguille et d'un piston d'injection. Lorsque l'on fait appel à de la semence ou des embryons qui ont été congelés, on a alors recours à des paillettes fines 14, que l'on fixe à l'intérieur du cathéter, et dont le bouchon d'obturation 15 sert de piston. On sait en effet que ces paillettes fines servent à la conservation, en milieu cryogénique, des semences ou embryons et qu'elles sont formées d'un tube capillaire soudé à une de ses extrémités et fermé à l'autre par un bouchon contenant une poudre polymérisable au contact de la substance. Lorsque l'on utilise ici de telles paillettes on doit sectionner l'extrémité antérieure et l'introduire à force dans la cavité conique 16 de l'embout du cathéter afin de l'immobiliser. Il suffit ensuite d'utiliser le piston 13, après avoir retiré sa tête amovible, afin que celui-ci puisse coulisser dans la paillette fine et venir en appui frontal sur le bouchon 15 qui joue alors de rôle de tête de piston.
- Le fonctionnement de cet instrument est simple puisque lorsque la sonde 5 est amenée en position, sous endoscopie, en regard d'une des deux cornes utérines, on ouvre le robinet 17, prévu sur le raccord 8, et l'on crée une dépression dans le canal 5₂ de manière à fixer par effet de ventouse, la paroi de la corne qui, comme on le sait est fuyante et a, par conséquent, tendance à se dérober. Lorsque la corne est ainsi immobilisée par succion, on descend le cathéter et l'on contrôle l'introduction de l'aiguille 12 au travers de l'endomètre 18 jusqu'à ce que son extrémité vienne se situer dans la lumière 19 de la corne considérée. A ce stade le cathéter, qui comporte à son extrémité arrière une collerette 21 vient en butée sur la face postérieure 22 de l'instrument de manière à ce que l'aiguille ne traverse pas la paroi opposée de la corne. Dans cette position on procède à la descente du piston 13 qui expulse dans ladite cavité 19 la semence ou les embryons contenus dans la gaine 10 ou dans la paillette 14.
- En matière d'insémination d'ovins et caprins, le cathéter renferme deux doses de quinze millions de spermatozoïdes chacune pour traiter les deux cornes 20. Lorsqu'une dose a été injectée, il suffit de procéder au retrait partiel du cathéter et en particulier de l'aiguille 12, puis d'incliner, sous endoscopie, l'instrument, pour amener son extrémité frontale en regard de la deuxième corne, puis de renouveler les opérations.
- Etant donné que l'on doit, préalablement à l'intervention, repousser les viscères de la paroi abdominale, afin d'avoir un champ optique maximum et une bonne visualisation de l'appareil génital, il est souhaitable de prévoir à l'extrémité postérieure de l'instrument (figure 3) un presse-étoupe 23 à joint torique 24 permettant d'obturer efficacement le canal extérieur 5₂ afin d'éviter une remontée de l'air à travers ce conduit.
- Le procédé et l'instrument selon l'invention présentent de nombreux avantages, car à la finesse de la chirurgie utilisée et son innocuité, viennent s'ajouter ceux liés à la souplesse et au caractère économique de l'intervention. En effet, l'instrument de conception simple permet de traiter les animaux directement sur le terrain, puisqu'on n'a plus recours aux installations propres à la chirurgie lourde et à l'anesthésie générale. En outre, les incisions étant fines, l'animal est peu stressé et les soins post-opératoires pratiquement inexistants. Enfin, et il s'agit là d'un avantage particulièrement important dans le cadre de l'insemination artificielle des ovins et caprins, on peut mettre en place à l'aide de cette instrumentation, un faible nombre de spermatozoïdes (trente millions pour les deux cornes) là où il fallait, avec les techniques traditionnelles 400 millions de spermatozoïdes. Par conséquent, le rendement est élevé, l'injection de semences, directement dans l'utérus, améliorant à la fois le taux de fertilité et le pouvoir de diffusion des mâles. On doit souligner en outre, que cet instrument est particulièrement bien adapté au transfert embryonnaire et aux prélèvements de liquides folliculaires chez les ovins, les caprins, les bovins ou les équins, seules les dimensions de l'appareil changeant en fonction de la taille de l'animal. En outre, cet instrument est d'une manipulation aisée car l'opérateur peut d'une seule main tenir l'endoscope et la sonde qui comporte à cet effet, un anneau de préhension 25.
Claims (6)
1°. Instrument pour l'insémination artificielle, la transplantation d'embryons in-utéro ou le prélèvement de liquide folliculaire par voie transpéritonéale chez les animaux, notamment sur les ovins, caprins, bovins ou équins, caractérisé en ce qu'il est constitué d'une sonde rigide (5) comprenant deux canaux tangentiels (5₁,5₂) définis par deux conduits de diamètres différents, logés l'un dans l'autre, l'un des conduits (5₂) étant relié à une pompe d'aspiration l'autre (5₁) servant de guide à un cathéter punctiforme (10-13), la base de ce dernier étant encochée radialement pour permettre de visionner, sous endoscopie, le déplacement de l'aiguille (12).
2°. Instrument selon la revendication 1, caractérisé en ce que le cathéter est constitué d'une part, d'une gaine semi-rigide (10) fermée à une de ses extrémités par un embout (11) prolongé par une aiguille (12), ledit embout comportant une cavité interne tronconique (16), d'autre part un piston (13) coulissant avec étanchéité dans cette gaine.
3°. Instrument selon la revendication 2, caractérisé en ce que le cathéter renferme une paillette fine (14) de conditionnement de la semence, des embryons ou des liquides folliculaires, rendue solidaire de l'embout (11) par emmanchement à force dans sa cavité tronconique (16), l'injection ou la ponction étant réalisée par le piston coulissant (13) en appui frontal sur le bouchon (15), obturant usuellement ces paillettes.
4°. Instrument selon l'une quelconque des revendications de 1 à 3, caractérisé en ce que l'extrémité supérieure de la sonde est obturée hermétiquement par un presse-étoupe (23), traversé par la gaine (10) de la seringue.
5°. Instrument selon l'une quelconque des revendications de 1 à 4, caractérisé en ce que le piston est étalonné.
6°. Procédé pour la mise en place et la mise en oeuvre de l'instrument selon les revendications de 1 à 5, caractérisé en ce que l'on pratique deux fines incisions dans la paroi abdominale de l'animal, on introduit dans l'une un endoscope pour visionner le champ opératoire et dans l'autre un trocart pourvu de son manchon guide, puis on procède au retrait du trocart et on engage dans le manchon resté en place l'instrument d'insémination et, de transplantation ou de prélèvement, relié à un moyen d'aspiration et comportant une seringue, on fixe ensuite par effet de ventouse la paroi d'une des cornes utérines puis on traverse l'endomètre pour amener l'aiguille dans la lumière de cette corne afin de déposer la semence ou les embryons où diffère le prélèvement souhaité, après quoi on traite, toujours sous endoscopie, la deuxième corne dans les mêmes conditions, après retrait de l'aiguille et nouvelle orientation de l'instrument.
