Description
- La présente invention a pour objet un procédé de réalisation d'une prothèse dentaire, qu'il s'agisse d'une prothèse partielle fixe ou mobile ou d'une prothèse complète.
- La réalisation de telles prothèses nécessite, habituellement, les opérations suivantes :
- - coulée des empreintes ;
- - recherche de l'occlusion après fabrication d'une maquette ;
- - montage sur cire pour vérification esthétique ;
- - fabrication par la technique de la cire perdue des parties métalliques ou en résine ;
- - vérification et polissage ;
- - pose en bouche.
- Cette méthode, qui est très ancienne, présente de nombreux inconvénients et notamment :
- - l'importance du travail en bouche ;
- - des délais de réalisation importants ;
- - une précision du travail liée à l'expérience du prothésiste ;
- - une discordance entre les appréciations du dentiste et celles du prothésiste ;
- - un coût élevé en main d'oeuvre ;
- - une dépendance vis-à-vis des matériaux de fonderie ;
- - la nécessité de plusieurs interventions en bouche qui sont longues et pénibles pour le patient.
- La présente invention vise à remédier à ces inconvénients.
- A cet effet, le procédé qu'elle concerne mettant en oeuvre une saisie de la forme de la zone d'implantation, par empreinte, micro-palpage ou empreinte optique, consiste à fabriquer la pièce prothétique elle-même, une ébauche de celle-ci, ou un moule permettant l'obtention de la pièce prothétique, sur un centre de travail assisté d'un calculateur tenant compte, outre de la saisie de forme, du matériel et du matériau à usiner.
- Selon un mode de mise en oeuvre, dans le cas de la réalisation d'un ensemble prothétique de plusieurs éléments, tel qu'un appareil mobile ou un bridge, ce procédé consiste à déterminer la forme de la partie de la prothèse située du côté de la gencive en tenant compte dans le logiciel de trois paramètres, à savoir : les surfaces des deux extrémités de la prothèse et le point bas du pontique pour un bridge ou le profil du support pour un appareil mobile. Il convient également de tenir compte d'une notion très importante constituée par l'axe d'insertion des dents.
- La figure 1 du dessin schématique annexé représente un bridge (2) pour lequel il convient de tenir compte des surfaces, respectivement (3) et (4), des deux extrémités de la prothèse, et du point bas du pontique portant la référence (5).
- Selon une autre caractéristique de ce procédé, il convient de réaliser le positionnement des dents que comporte la prothèse eh faisant, tout d'abord, apparaître la totalité de l'arcade (6), comme représenté à la figure 2, puis en déterminant des plans proximaux (7, 8), chaque plan proximal (7) étant défini comme étant perpendiculaire à la droite (9) réunissant deux points de contact de la prothèse avec des dents (10) demeurant en bouche, tandis que chaque plan proximal (8) est défini comme étant perpendiculaire à la droite réunissant un point de contact à un autre plan proximal.
- Après détermination des plan proximaux, le procédé consiste à placer des dents théoriques (12) sur ceux-ci en leur faisant, éventuellement, subir une déformation en vue du respect de l'enveloppe, comme montré à la figure 3.
- Pour réaliser l'usinage d'une prothèse comportant plusieurs éléments, il est intéressant, comme montré aux figures 4 et 5 du dessin annexé, de sectionner l'arcade à réaliser en un ensemble de tronçons (13) formant des droites ou des arcs de grand rayon. Chaque tronçon est usiné séparément avec un système de fixation intermédiaire (14, 15). Le repérage se fait au niveau de l'élément central, dans la mesure où la courbure est régulière.
- Le découpage d'une arcade en un ensemble de tronçons est effectué en tenant compte de l'angle d'attaque de l'outil d'usinage pour que celui-ci puisse travailler dans les meilleures conditions possibles, de préférence selon trois axes, au maximum selon quatre axes pour des raisons de simplicité.
- Selon un mode de mise en oeuvre, dans le cas de la réalisation d'une prothèse mobile, ce procédé consiste à réaliser les deux parties d'un moule destiné à former la plaque support des dents, par usinage automatique de ces deux parties à partir de la saisie des formes de la zone d'implantation de la prothèse et à partir d'un logiciel tenant compte de l'axe d'insertion des dents, du calcul biomécanique pour la stabilité de l'appareil, des zones de pression pour le respect des zones buccales fragiles, et des crochets de fixation de la prothèse.
- Selon une première possibilité, ce procédé consiste à réaliser la seule plaque support, et à fixer sur celle-ci des dents ou des séries de dents pré-usinées de façon manuelle ou automatique en tenant compte des contraintes indiquées précédemment.
- La figure 6 du dessin annexé représente une plaque (16) comportant une ouverture centrale (17), et des découpes périphériques ménageant des passages pour les dents (18) demeurant en bouche.
- Cette plaque (16) est obtenue par moulage à l'intérieur d'un moule comprenant une partie femelle (19) et une partie mâle (20). Les parties (19) et (20) sont usinées automatiquement à l'aide d'une machine-outil à commande numérique, à partir des données du logiciel qui intègre la saisie des formes effectuées en bouche.
- La figure 7 représente une vue en coupe de ce moule au niveau de la partie femelle, permettant de visualiser le profil de la plaque (16) à obtenir. Après fermeture du moule, comme montré à la figure 8, il est procédé à la coulée ou à l'injection d'un matériau tel qu'une résine ou du métal destiné à former la plaque (16).
- Il ressort clairement des dessins, et notamment de la figure 8 et de la figure 9, qui est une vue à échelle agrandie d'un détail de figure 8, que le logiciel tient compte, lors de l'usinage des deux parties (19) et (20) du moule, de l'épaisseur de la plaque (16) à obtenir.
- Comme montré à la figure 9, il est possible de prévoir dans la partie (20) du moule, des évidements respectivement (22) assurant la formation de tenons pour la fixation des dents et (23) assurant la formation des crochets.
- Selon une autre possibilité illustrée à la figure 10, ce procédé consiste à usiner les deux parties d'un moule dont celle (23), réalisée en une matière rigide, reproduit la forme de la zone de la gencive sur laquelle l'appareil doit être fixé, et dont celle (24) est réalisée en une matière semi-souple et présente autant de cavités (25) que la prothèse doit comporter de dents.
- Dans chaque cavité, est insérée une dent pré-usinée dont le profil tient compte de l'enveloppe de la prothèse, de l'occlusion et de l'axe d'insertion. Après fermeture du moule, la partie souple est enserrée dans un capot rigide (26), empêchant sa déformation lors de l'injection de matière dans la cavité (27) destinée à la formation de la plaque. Lors de cette injection, la matière pénètre dans l'espace (27), ainsi que dans les zones d'ancrage (28) ménagées dans chaque dent pré-usinée.
- Selon un autre mode de mise en oeuvre, ce procédé consiste à réaliser les deux parties du moule par usinage automatique, puis à emboutir la plaque support entre les parties mâle et femelle pour lui donner sa forme générale, avant de réaliser sa finition par un découpage obtenu par fraisage, bombardement électronique, laser, ou électro-érosion.
- Avantageusement, et comme montré aux figures 11 à 13 du dessin schématique annexé, il est tout d'abord procédé à l'usinage de la zone d'empreinte (29) correspondant à la gencive, occupant l'ensemble du palais ou de la zone linguale, puis à l'usinage de la contre-pièce d'emboutissage (30) calculée en fonction des caractéristiques de la plaque (épaisseur, fluage ...).
- Après insertion dans le moule, il est procédé à l'emboutissage de la plaque (32), qui possède la forme représentée à la figure 12, puis à son découpage selon le tracé souhaité pour tenir compte des zones d'appui dans la bouche, ainsi que du passage des dents. Il est ensuite procédé au montage des crochets de fixation sur les dents existantes, puis à la fixation des dents proprement dites par soudure, collage, vissage ou toute autre méthode.
- Il est à noter que les différentes parties du moule ou la plaque elle-même dans les derniers modes de mise en oeuvre décrits, sont usinées par des techniques connues telles que fraisage, tournage, électro-érosion, électro-chimie, ultra-sons, ou bombardement laser ou électronique.
- Avantageusement, dans tous les cas, les dents ou séries de dents fixées sur les plaques sont en stock et sont numérotées en fonction de leur forme et de leur teinte, l'ordinateur désignant en fonction de la saisie des formes, de l'enveloppe, de l'occlusion et de l'axe d'insertion, les dents à travailler pour limiter au maximum l'usinage.
- Dans la mesure où l'usinage est réalisé à l'aide d'une fraise (33) représentée à la figure 14, dans le cas de l'usinage d'une couronne, le diamètre (D) de l'extrémité de la fraise est au moins égal au double de la partie la plus en contre-dépouille devant être usinée. Ceci permet de disposer d'un outil d'usinage qui travaille seulement selon trois axes. Dans un premier temps, l'élément à usiner étant solidaire d'un support (34), il est procédé à l'usinage de la face externe de la prothèse (35), par déplacement axial de la fraise (33) et par déplacement autour de la prothèse, comme montré à la figure 15. Après retournement de la pièce (35), il est procédé à l'usinage de l'intérieur de celle-ci, comme montré à la figure 16. L'usinage des sillons est effectué par une fraise spéciale dite fraise fissure.
- La séparation de la pièce prothétique et de son support est réalisée après finition de la prothèse, et avant polissage de celle-ci.
- Ce procédé permet également la réalisation de couronnes à incrustation esthétique.
- Il peut s'agir, par exemple, d'une couronne céramo-métallique, comme montré à la figure 17, ou d'une couronne métallique comportant une incrustation esthétique en résine, comme montré à la figure 18.
- Les contraintes prises en compte par le logiciel ne sont pas les mêmes dans les deux cas puisque, dans le cas d'une couronne comportant une partie métallique (36) et une partie céramique (37), la partie céramique peut s'étendre jusqu'au contact de la gencive (38), tandis que dans le cas d'une couronne présentant une partie métallique (39) et une inscrustation (40) en résine, cette dernière ne doit pas venir au contact de la gencive (38), nécessitant un léger retour (42) du métal à ce niveau.
- Les incrustations peuvent être fixées sur la couronne par différents systèmes connus, usinés automatiquement en même temps que les pièces par la machine outil à commande numérique. C'est ainsi que la facette usinée (43), représentée à la figure 19, comporte un système de fixation par queue d'aronde (44) sur la couronne. La facette (45) représentée à la figure 20 est fixée par vissage sur la partie métallique (46), à l'aide d'une vis (47) engagée depuis l'extérieur, avec dépôt d'un joint esthétique (48) recouvrant la tête de la vis. Dans la forme d'exécution représentée à la figure 21, une facette (49) est fixée sur la partie métallique (50) à l'aide d'une vis (52) engagée depuis la partie métallique vers la facette, qui présente à cet effet un perçage taraudé (53).
- . Selon un premier mode de réalisation d'une couronne comportant une incrustation esthétique, la couronne métallique (54) et la facette (55) en résine ou en céramique, représentées à la figure 22, sont usinées de manière définitive indépendamment l'une de l'autre, en tenant compte, lors de l'usinage de la couronne, de l'épaisseur de la facette, du système de fixation de celle-ci et du type d'inclusion. Après fixation de la facette (55) sur la couronne (54), la prothèse est entièrement terminée.
- Selon un second mode de mise en oeuvre illustré aux figures 23 et 24, ce procédé consiste tout d'abord à usiner la partie métallique (56) de la couronne en tenant compte, lors de l'usinage de celle-ci, de l'épaisseur de la facette (57), du système de fixation de celle-ci et du type d'inclusion, à fixer la facette (57) sur la partie métallique (56), puis à réaliser l'usinage de la facette.
- Dans le cas de la réalisation d'une couronne céramo-métallique, c'est-à-dire comprenant une partie intérieure métallique et une coiffe en céramique, un premier mode de mise en oeuvre de ce procédé consiste, disposant en stock de couronnes métalliques (58) et de facettes en céramique (59) pré-usinées, de telle sorte que la face externe de la partie métallique possède le même profil que la face interne de la facette, comme montré à la figure 25, à choisir un couple couronne métallique facette, puis à usiner la face interne de la partie métallique à la forme du moignon, et la face externe de la facette en fonction de l'enveloppe et de l'occlusion, ces différents usinages, étant effectués selon les indications données par la saisie des formes et les éléments du logiciel.
- Un second mode de mise en oeuvre pour l'obtention d'une couronne céramo-métallique consiste, en partant d'une saisie des formes effectuée en bouche, à réaliser, dans un bloc métallique (60), l'usinage des faces interne et externe de la couronne métallique (62), à faire une nouvelle saisie de la face externe de la couronne métallique, à réaliser en fonction de celle-ci et à partir d'un bloc de céramique (63) l'usinage de la face interne de la facette, puis en fonction de l'enveloppe et de l'occlusion, l'usinage de la face externe de celle-ci, avant de procéder à l'assemblage de la couronne métallique (62) et de la facette (64).
- Selon une possibilité, dans le cas d'une couronne comprenant un support métallique et une facette en résine, ce procédé consiste, partant d'une saisie des formes effectuée en bouche, à réaliser, dans un bloc métallique (65), l'usinage de la face interne de la couronne métallique à la forme du moignon, puis un encochage (66) de sa face externe. En fonction de l'enveloppe et de l'occlusion de la prothèse considérée, il convient ensuite d'usiner dans un bloc de matière (67), le contour extérieur (68) de la couronne. L'assemblage de la partie métallique et du bloc de matière (67) usiné assure la formation d'un moule désigné par la référence générale (69) à la figure 27, dans lequel est injecté de la résine formant la facette de la prothèse.
- Une autre solution consiste, comme illustré à la figure 28, partant d'un bloc métallique (70), à usiner automatiquement les faces interne et externe de celui-ci, afin d'obtenir la partie métallique (72) de la couronne, puis à réaliser, manuellement, le montage sur celle-ci d'une facette pré-usinée en résine ou en céramique, et de réaliser le façonnage manuel de celle-ci.
- Le procédé selon l'invention est également applicable aux prothèses destinées aux dents non délabrées. Dans ce cas, et comme montré à la figure 29, le procédé consiste à faire une saisie des formes de la dent (73) avant travail en bouche, puis à effectuer la taille d'un moignon (74) qui, dans d'autres applications, pourrait être une cavité. Il est alors procédé à une nouvelle saisie des formes du moignon ou de la cavité avec possibilité de correction sur le moniteur à l'aide du logiciel pour tenir compte notamment des éléments de fixation de la prothèse, tels que l'épaisseur de ciment. Le logiciel assure la superposition des deux empreintes pour obtenir le profil de toutes les faces de la prothèse (75) qui est usinée automatiquement avant d'être fixée sur le moignon (74) ou dans la cavité qui lui est destinée, selon les cas.
- Dans la mesure où ce procédé est appliqué à la réalisation d'une prothèse sur une dent saine ou très peu cariée, comme montré à la figure 30, le procédé consiste à faire une saisie des formes de la dent (76), à modifier le profil de la dent conformément au tracé (77) pour rétablir le profil réel, à tailler une cavité (78), dans la dent, dont on effectue une saisie des formes en tenant compte du tracé (77) et de la forme de la cavité (78), la machine-outil à commande numérique réalisant l'usinage de la prothèse (79) qui s'adapte parfaitement dans la cavité (78) qui lui est destinée.
- Dans le cas d'une dent très cariée, la saisie des données se fait soit à partir de la dent symétrique avec inversion des données, soit à partir d'une dent théorique correspondante.
- Afin de faciliter la fixation des différentes prothèses obtenues par le procédé selon l'invention, il est intéressant d'utiliser, dans certains cas, des pivots pré-calibrés possédant, à leur partie externe, un système de fixation standard, tel que par vis, et d'usiner à l'aide de la machine-outil à commande numérique la contrepartie adaptable sur la prothèse.
- La figure 31 du dessin schématique annexé représente un mode de fixation d'une prothèse sur une dent.
- Cette solution consiste à usiner dans la prothèse (80) un trou taraudé (82) de grand diamètre, lors de la réalisation automatique de celle-ci.
- Dans ce trou taraudé est mis en place une vis (83) ne dépassant pas de la prothèse du côté de la zone d'appui de celle-ci sur son support.
- La prothèse est alors positionnée sur son support, après quoi il est procédé, par l'intermédiaire d'un micro-foret (84) guidé dans le passage axial de la vis (83), à la réalisation dans le support d'un trou taraudé (85) de diamètre inférieur à celui (82).
- La prothèse est alors retirée de son support, en vue du démontage de la vis (83). Elle est ensuite remise en place sur son support et fixée sur celui-ci par l'intermédiaire d'une vis (86) possédant deux diamètres successifs correspondant, respectivement, à ceux des trous taraudés (82) et (85).
- Pour la mise en place de cette vis, il est possible d'utiliser un tournevis monté sur le micro-tour de la turbine, tournant très doucement.
- Ce procédé est également susceptible de passage par des stades intermédiaires, c'est-à-dire n'assurant pas immédiatement l'obtention de la prothèse définitive, mais permettant le passage par un stade intermédiaire permettant de vérifier l'exactitude de la saisie des formes, de la réalisation de l'enveloppe et de l'occlusion de la prothèse.
- Selon une première possibilité, le procédé consiste à usiner une maquette de l'enveloppe à l'aide de la machine-outil à commande numérique, à vérifier en bouche l'exactitude du tracé de l'enveloppe, notamment en ce qui concerne l'occlusion statique et dynamique, éventuellement à retoucher cette maquette, puis à saisir sur celle-ci la forme de l'enveloppe en vue de l'usinage de la prothèse elle-même.
- Selon un autre mode de mise en oeuvre, ce procédé consiste à réaliser tout d'abord une ébauche de la prothèse à l'aide de la machine-outil à commande numérique, à positionner cette ébauche en bouche en vue de déterminer l'enveloppe exacte de la prothèse et en particulier l'occlusion statique et dynamique, puis d'après les retouches en bouche, à terminer la prothèse sur la machine-outil.
- Selon une autre caractéristique, ce procédé consiste à stocker, sous forme numérique en mémoire, les données d'usinage de chaque prothèse usinée et placée en bouche, en vue d'éventuelles comparaisons avec des données provenant de prothèses existantes. Ceci est particulièrement intéressant pour la recherche fondamentale et pour l'identification de cadavres en médecine légale, domaine dans lequel il est bien connu que la dentition constitue un moyen très précis et très sûr d'identification d'un individu.
- Comme il ressort de ce qui précède, l'invention apporte une grande amélioration à la technique existante, en fournissant un procédé de réalisation d'une prothèse dentaire dont la mise en oeuvre est extrêmement rapide, puisque toute prothèse, même complexe, peut être mise en bouche le jour même où le dentiste réalise la saisie des formes. En outre, ce procédé assure une excellente qualité de la réalisation de la prothèse indépendamment de la dextérité du dentiste, et de l'expérience de celui-ci. Enfin, ce procédé constitue, par rapport à ceux traditionnels, un gain de main d'oeuvre important, tant pour le dentiste lui- même que pour le prothésiste dont les interventions sont supprimées dans la plupart des cas.
- Comme il va de soi, l'invention ne se limite pas aux seuls modes de mise en oeuvre de ce procédé, ni à ses seules applications, décrits ci-dessus ; elle en embrasse, au contraire, toutes les variantes.
