Description
- Instrument zur transkutanen und subkutanen Erwärmung und Iontophorese und Verfahren zu seiner Anwendung Die vermittels Massage oder Akupunktur bestimmter Hautstellen auf die mit ihnen in Verbindung stehenden Organen des Menschen oder Wirbeltieres ausgeübte Wirkung kann wesentlich gesteigert oder leichter in dem gewünschten Sinne geleitet werden, wenn die bestimmten Hantstellen trans- oder subkutan erwärmt werden oder vermittels Thermo- oder Elektro-Iontophorese Spuren bestimmter Metalle oder Medikamente zugeführt erhalten. Als besonders wirltungsvoll hat es sich erwiesen, wenn die bestimmten Hautstellen gleichzeitig trans- und subkutan beeinflußt werden, indem die transkutane Erwärmung oder Iontophorese durch eine subkutane unterstützt wird, oder umgekehrt.
- Mit der Erfindung ist ein Instrument geschaffen worden, das an den bestimmten Hautstellen gleichzeitige trans- und subkutane Erwärmungen bzw.
- Iontophorese ermöglicht. Es ist in der Zeichnung Fig. I bis 7 in verschiedenen Ausführungsformen schematisch und beispielhaft dargestellt. Es zeigt Fig. I das Instrument nach der Erfindung zusammengesetzt in Seitenansicht, Fig. 2 den unteren Teil des Stabes im Schnitt, Fig. 3 eine Thermonadel im Schnitt, Fig. 4 eine Thermonadel zur lontophorese von Medikamenten, Fig. 5 eine Elektronadel im Schnitt, Fig. 6 eine Elektronadel zur Iontophorese von Medikamenten, Fig. 7 ein Schaltschema.
- Das Instrument nach der Erfindung (Fig. I) besteht aus zwei Teilen, dem Stab I und der Nadel 2, die durch eine Öffnung 2I im Stab hindurchgeführt ist. Der Stab trägt an seinem unteren Ende den Kegel 3. Der obere Teil dient als Handgriff 4, aus dessen oberem Ende ein zweiadriges Kabel 5 herausführt, das mit den beiden Polen einer elektrischen Stromquelle 6 verbunden ist.
- An dem Stab befindet sich eine Klemmvorrichtung 7, die es ermöglicht, die Nadel in jeder gewünschten Stellung mit dem Stab fest zu verbinden.
- Das obere Ende der Nadel 2, die in Fig. I als Elektronadel dargestellt ist, ist durch ein einadriges Kabel 8 mit dem positiven Pol der Stromquelle 6 verbunden, während der andere Pol an eine Kontaktplatteg oder wahlweise an eine Kontaktnadel 10 angeschlossen ist.
- In Fig. 2 ist das untere Ende des Stabes I beispielsweise in einer Ausführungsform dargestellt.
- Der Kegel 3 ist mit dem Stab I so verbunden, z. B. vermittels Verschraubung, Bajonettverschluß u. daß er auswechselbar ist. Beide tragen eine gemeinsame Bohrung 21 zur Aufnahme der Nadel 2.
- Zur Erwärmung des Kegels 3 sind Heizspiralen 22 vorgesehen, die in einer ringförmigen und oben und seitlich mit Wärmeisolationsmitteln 23 ausgekleidet,en Nut des Stabes so gelagert sind, daß die von der Stromquelle 6 erzeugte Wärme sich nur auf den Kegel 3 überträgt.
- Der Stab I wird zweckmäßigerweise aus einem schlechten Wärmeleiter, z. B. Kunstharz, hergestellt, dagegen der Kegel 3 atis gut wärmeleitendem Metall, z. B. Gold, Silber, Kupfer, Eisen (Fe) u. ä., das zugleich in der Lage ist, die erwünschten Spurenelemente infolge der schwarten Strahlung bei der Erwärmung abzugeben.
- Anstatt massive Kegel aus dem zu emanierenden Metall zu verwenden, ist es vorgesehen, den Kegel 3 aus einem besonders gut wärmeleitenden Material, z. B. Kupfer, herzustell,en, und nur die mit der Haut in Berührung kommende Fläche mit dem zu emanierenden Metall, z. B. Gold, Silber, Eisen u. ä., zu plattieren oder auf andere Weise zu bedecken.
- Die Hautberührungsfiächen des Kegels 3 sind mit Vertiefungen 26 zur Aufnahme von Medilkamenten versehen.
- Um den Kegel 3 auch zur Elektro-Iontophorese verwenden zu können, ist vorgesehen, daß er mit einem in -dem Stab eingebetteten Kontaktstück 24, in. der -Zeichnung beispielsweise- als Schraubfassung für den Kegel dargestellt, verbunden ist, das wahlweise an den positiven oder negativen Pol der Stromquelle 6 anschließbar ist.
- Die Zuführung des elektrischen Stromes zur Heizspirale-22 und dem Kegel 3 erfolgt über den im Handgriff 4 gelagerten Umschalter II und den Einschalter I2. Der Umschalter ermöglicht in bekannter Weise, den elektrischen Strom wahlweise dem Kegel 3, dem Kegel und der HeiizspiraLe 22 oder der Heizspirale allein zuzuführen. Die entsprechenden Stellungen des Umschalters sind in der Zeichnung (Fig. I) mit E = Elektro-Iontophoirese, ET = Elektro- und Thermo-Iontophorese und T = Th'ermo-Iontophorese bezeichnet. Das Schaltschema Fig. 7 zeigt die näheren Einzelheiten, wobei 70 eine in den drei Schaltstellungen gezeichnete Kontaktieder ist.
- Zur Regulierung der Temperatur des Kegels 3 ist vorgesehen, entweder mit bekannten Geräten (Quecksilber-,Widerstands-, Thermoelementthermometer u. ä.) die Temperatur des Kegels laufend zu messen und die gewünschte Temperatur durch Ein- und Ausschalten des Heizstromes durch Betätigung des Druckschalters 12 oder vermittels bekannter Temperaturregler, z. B. Bimetallschalter, auf eine eingestellte Temperatur einregulieren zu lassen.
- Um eine subkutane Erwärmung hervorzurufen, ist die in Fig. 3 dargestellte Thermonadel vorgeueben. Die eigentliche Nadel 30 hat die übliche Form einer Injektionshohlnadel. Durch ihr Inneres ist ein elektrischer Leitungsdraht 31 hindurchgeführt, der von einem Isolationsmaterial 32 so umgeben ist, daß ein unmittelbarer Stromübertritt vom Leitungsdraht zur Nadel nicht möglich ist.
- An das untere Ende des Leitungsdrahtes ist ein Heizdraht 33 angeschlossen, dessen anderes Ende bei dem Punkt 34 mit der Nadelspitze in leitender Verbindung steht. Um heim Einstechen in die Haut zu verhindern, daß Hautteile oder Körperflüssigkeit in den vorderen Teil der Nadel eindringen, ist der Heizdraht 33 in eine wärme- und f'euchtigkei-tsbeständige Masse 35, z. B. Schamotte oder Ton, eingebettet.
- Die Zuführung des Heizstromes erfolgt, wie in Fig. 7 bei der Thermonadel a dargestellt, indem die Nadel 30 und der Leitungsdraht 31 an die beiden Pole der Stromquelle angeschlossen werden.
- Soll die bei der subkutanen Erwärmung auftretende schwarze Strahlung zur Thermo-Iontophorese ausgenutzt werden, so ist vorgesehen, die Nadel 30 aus dem zu emanierenden Metall herzustellen oder ihre Spitze damit zu überzichen.
- Zur subkutanen Thermo-Iontophorese von Medikamenten ist vorgesehen, wie in Fig. 4 dargestellt, an der Spitze einen Hohlraum 40 zur Aufnahme des zu emanierenden Medikaments anzubringen.
- Um die Wirkung der subkutanen Thermo-Iontophorese durch eine gleichzeitig von derselben Thermonadel ausgehende Etektro-Iontophorese zu steigern, wird erfindungsgemäß nur der Leitungsdraht 31 mit dem positiven Pol der Stromquelle verbunden (Fig. 7, -Thermonadel b). Der Strom fließt dann durch den Leitungsdraht 3I, bringt den Heizdraht 33 zur Erwärmung, geht über die Nadelspitze in den Körper des Patienten und über die Kontaktplatteg oder die Kontaktuadel 10 oder wahlweise über den Kegel 3 zur Stromquelle zurück.
- Für die subkutane Elektro-Iontophorese sind Elektron adeln wie in Fig. 5 beispielhaft angegeben vorgesehen. Ein elektrischer Leitungsdraht 50, der von- einem Mantel 51 aus Isolationsmaterial umgeben ist, trägt in seinem unteren Ende eine mit ihm fest verbundene Spitze 52 aus dem zu emanierenden Metall. Es genügt jedoch, wenn die Spitze lediglich mit dem betreffenden Metall überzonen worden ist.
- Um eine subkutane Elelçtro-Iontophorese von Medikamenten vornehmen zu können, ist die in Fig. 6 beispielhaft dargestellte Elektronadel an ihrer Spitze mit Vertiefungen 6I, z. B. Ringnuten oder Bohrungen, zur Aufnahme der zu emanierenden Medikamente vorgesehen.
- Mit dem Instrument läßt sich eine rein lakale lontophorese zwischen Kegel und einer Elektro-oder Thermonadel oder umgekehrt durchführen.
- Hierfür wird die Elektro- oder Thermonadel (Fig. 7, b oder c) nur mit dem Leitungsdraht 31 oder 50 in den positiven oder negativen Pol der Stromquelle angeschlossen und der Kegel 3 mit dem entgegengesetzten Pol verbunden. Um einen direkten Stromübertritt von der Thermonadel 30 auf den sie umgebenden Kegel 3 zu verhindern, ist entweder die Thermonadel bis auf die frei zu lassende Spitze mit einer Isolationsschicht überzogen oder die Bohrung des Kegels mit einem Niantel 25 aus Isolationsmaterial ausgekleidet.
- Die Anwendung des Instruments geschieht folgendermaßen: Die Nadel 2 (Thermo- oder Elektronadel) wird so weit in den Stab 1 eingeführt, daß ihre Spitze der gewünschten Einstichtiefe entsprechend aus diesem hervorragt, und in dieser Stellung vermittels der Klemmvorrichtung 7 festgesetzt. An einem geeigneten Punkt der Hautoberfläche wird die Kontaktplatte 9 in bekannter Weise leitend angelegt oder die Kontaktnadel IO in die Haut gestochen. Bei unmittelbarer Iontophorese- zwischen Kegel und Nadel spitze ist das Anlegen der Kontaktmittel g und IO nicht erforderlich. Dann wird in der zu beeinflussenden Hautstelle die Nadel 2 so tief in die Haut gestochen, bis der Kegel 3 an der Haut zur Anlage kommt.
- Durch sinngemäßes Einschalten der Stromquelle 6 wird die trans- oder subkutane Erwärmung oder lontophorese eingeleitet.
- Das Instrument nach der Erfindung ermöglicht zwischen transkutaner und subkutaner Erwärmung, Thermo- und Elektro-Iontophorese etwa fütnfundzwanzig verschiedene Kombinationen, die allen vorkommenden Forderungen der Praxis genügen.
- PATEXTANSPRÜCHE I. Instrument zur transkutanen und subkutanen Erwärmung und lontophorese, dadurch gekennzeichnet, daß es aus einem Kegel (3) für die transkutane Erwärmung, Thermo- und Elektro-Iontophorese und einer Nadel (2) für die subkutane Erwärmung, Thermo- und Elektro-Iontophorese besteht.
Claims (1)
- 2. Instrument nach Anspruch I, dadurch gekennzeichnet, daß der Kegel (3) und der Stab (I) mit einer Offnung (2I) zur Aufnahme der Nadel (2) versehen sind.3. Instrument nach den Ansprüchen I und 2, dadurch gekennzeichnet, daß der Kegel (3) vermittels einer elektrischen Heizspirale (22) heizbar ist.4. Instrument nach den Ansprüchen I bis 3, dadurch gekennzeichnet, daß der Kegel (3) an den positiven oder negativen Pol einer elektrischen Stromquelle (6) anschließbar ist.5. Instrument nach den Ansprüchen I bis X, dadurch gekennzeichnet, daß der Kegel (3) an den mit dem Körper des Patienten in Berührung kommenden Stellen mit Vertiefungen (26) zur Aufnahme von Medikamenten versehen ist.6. Instrument nach den Ansprüchen I bis 5, dadurch gekennzeichnet, daß die Ciffnung (2I) des Kegels (3) mit einem Mantel (25) aus Isolationsmaterial ausgekleidet ist.7. Instrument nach den Ansprüchen I bis 6, dadurch gekennzeichnet, daß der Stab (1) mit einem elektrischen Schalter (I2) zum Ein- und Ausschalten und einem Schalter (11) zum wahlweisen Umschalten auf die Heizspirale (22), den Kegel (3) oder auf beide gemeinsam ausgerüstet ist.S. Instrument nach den Ansprüchen I und 2 dadurch gekennzeichnet, daß die Länge des aus dem Kegel (3j hervorragenden Teils der Nadel (2) einstellbar ist und daß die Nadel (2) vermittels einer Klemmvorrichtung (7) mit dem Stab (I) fest verbunden ist.9. Thermonadel für das Instrument nach den Ansprüchen I, 2 und 8, dadurch gekennzeichnet. daß eine hohle Nadel (30) im Innern mit einem Heizdraht (33) versehen ist, dessen eines Ende an der Nadel angeschlossen ist, während zum anderen Ende ein isolierter Leitungsdraht (31) durch die Nadel hindurchführt.IO. Thermonadel nach Anspruch 9, dadurch gekennzeichnet, daß der Heizdraht (33) in eine wärme- und feuchtigheitsbeständige Masse (35) eingebettet ist.II. Thermonadel nach den Ansprüchen 9 und IO, dadurch gekennzeichnet, daß die Spitze der Nadel (30) Hohlräume (40) zur Aufnahme von Medikamenten besitzt.12. Thermonadel nach den Ansprüchen g bis II, dadurch gekennzeichnet, daß der Schaft der Nadel (30) äußerlich mit Ausnahme der für die lontophorese erforderlichen Fläche mit einem elektrischen Isolationsmittel überzogen ist.13. Elektronadel für das Instrument nach den Ansprüchen I, 2 und 8, dadurch gekennzeichnet, daß sie aus einem mit elektrischen Isolationsmaterial (5 I) umgebenen Leitungsdraht (50)1 der auf seinem einen Ende eine Spitze trägt, besteht.14. Elektronadel nach Anspruch I3, dadurch gekennzeichnet, daß sie an ihrer Spitze (52) mit Vertiefungen (61) zur Aufnahme von Medikamenten versehen ist.15. Kegel, Thermo- und Elektronadeln für das Instrument nach den Ansprüchen I bis I4, dadurch gehennzeichnet, daß die mit dem Körper des Patienten in Berührung kommenden wirksamen Teile aus dem zu emanierenden Metall hergestellt oder mit ihm plattiert oder überzogen sind.I6. Verfahren zur Verwendung des Instrumentes nach den Ansprüchen I bis I5, dadurch gekennzeichnet, daß wahlweise vermittels sinn- gemäßer Schaltung (Fig. 7) der Kegel (3) zum Erwärmen, zur Thermo-Iontophorese oder zur Elektro-Iontophorese (als Anode oder Kathode) oder zu allen dreien gleichzeitig und die Thermonadel (Fig. 3) zum Erwärmen, zur Thermo-Iontophorese oder zur Elektro-Iontophorese (als Anode oder Kathode) oder zu allen dreien gleichzeitig oder die Elektronadel (Fig. 5) zur Elektro-Iontophorese (als Anode oder Kathode) verwandt werden.
