Description
- 1. Technisches Gebiet der Erfindung
- Die vorliegende Erfindung bezieht sich allgemein auf eine Vorrichtung zum Fördern einer intervertebralen (zwischen Rückenwirbeln stattfindenden) Fusion und insbesondere auf eine Vorrichtung zum Einführen in einen Zwischenraum zwischen benachbarten Wirbeln, um eine intervertebrale Fusion zu fördern, während eine im wesentlichen natürliche Lordose der menschlichen Wirbelsäule aufrecht erhalten wird.
- 2. Beschreibung des Stands der Technik
- Bandscheiben, die aufgrund verschiedener Faktoren wie beispielsweise Trauma oder Alterung degeneriert worden sind, müssen typischerweise teilweise oder vollständig entfernt werden. Die Entfernung einer Bandscheibe kann die Wirbelsäule destabilisieren, so dass es notwendig wird, die Bandscheibe zu ersetzen, um die Höhe des Wirbels aufrecht zu erhalten und die Stabilität wiederherzustellen. Rückenimplantate werden häufig verwendet zum Verhindern eines Zusammenfallens der Wirbelsäule und zum Fördern der Fusion. Die am 24. Oktober 1996 eingereichte U.S. Patentanmeldung Seriennr. 08/740,123 bezieht sich auf Verfahren und Vorrichtungen zum Fördern einer Wirbelfusion.
- Nachdem eine Bandscheibe entfernt worden ist, wird typischerweise eine Implantatvorrichtung zwischen benachbarte Wirbel eingesetzt, um einen normalen Scheibenabstand aufrecht zu erhalten und die Wirbelsäulenstabilität wiederherzustellen, und dadurch eine zwischen den Wirbeln stattfindende Fusion zu fördern. Eine zwischen benachbarten Wirbeln angeordnete herkömmliche Implantatvorrichtung ist in den
1 und2 dargestellt. Die Implantatvorrichtung umfasst ein Paar Eingreifelemente20 , die typischerweise ein Gewinde10 zum Eingreifen mit den Wirbeln aufweisen. Bevor die Eingreifelemente eingesetzt werden, wird typischerweise ein Wirbelbohrer in die chirurgische Wunde eingeführt, um in die kortikale Endplatte hineinzubohren und Fasergewebe und Kernmaterial zu entfernen. Ein Wirbel-Gewindebohrer kann dann verwendet werden, um Gewindegänge in die Enden der benachbarten Wirbel zu schneiden. Die Eingreifelemente neigen dazu, relativ unflexibel und im wesentlichen nicht biegsam zu sein. Die Eingreifelemente sind typischerweise mit Knochentransplantat bepackt, um eine Wirbelfusion zu fördern. - Herkömmliche Implantatvorrichtungen neigen dazu, die "Lordose", oder die natürliche Krümmung der unteren Lendenwirbelsäule, nicht zu unterstützen. Wie in
1 gezeigt umfasst die Implantatvorrichtung parallele eingreifende Seiten12 und13 zum Berühren der Wirbel15 . Typischerweise ist es erforderlich, dass die eingreifenden Seiten parallel sind, um zu verhindern, dass der Fusionskäfig aus dem Zwischenwirbelraum herausrutscht. Der parallele Aufbau des Fusionskäfigs neigt dazu, die Lordose der Wirbelsäule zu verändern. Solch ein Verlust der Lordose kann zu einem erhöhten Risiko führen für andere Bandscheiben, die benachbart zu dem Fusionsniveau angeordnet sind und aufgrund der geänderten Übertragungskraft in der Wirbelsäule degenerieren können. -
2 stellt eine Vorderansicht der eingreifenden Elemente20 der Implantatvorrichtung dar. Die eingreifenden Elemente sind im wesentlichen zylindrisch und der Kontaktbereich zwischen einem Eingreifelement und einem Wirbel ist durch einen gekrümmten Bereich22 bestimmt. Die zylindri sche Geometrie der Eingreifelemente neigt dazu, einen relativ kleinen Kontaktbereich zwischen dem Fusionskäfig und dem Wirbel zur Verfügung zu stellen. Das Gewicht der Wirbelsäule erzeugt Druck auf die Wirbel, der unmittelbar an den gekrümmten Bereichen konzentriert ist. Ein Absinken oder eine Verformung der kortikalen Schicht der Wirbel neigt davon herzurühren. - Das an Michelson erteilte U.S. Patent Nr. 5,522,899 bezieht sich auf ein Wirbelsäulenimplantat zum Einsetzen in den Bandscheibenraum, um die Wirbelsäule zu stabilisieren und in einer Wirbel-zu-Wirbelknochenfusion teilzunehmen. Das an Kuslich et al. erteilte U.S. Patent Nr. 5,498,308 bezieht sich auf ein Implantat zur Verwendung bei einer Wirbelsäulenstabilisierung, wobei das Implantat einen zylindrischen Körper mit einem äußeren Gewinde und radial angebrachten Öffnungen, die angeordnet sind, um Knochen in einen inneren Bereich des Körpers abzubröckeln, wenn das Implantat eingebaut wird.
- Die oben genannten bekannten Verfahren und Systeme richten sich unter anderem ungenügend auf die Notwendigkeit, die natürliche Lordose der Wirbelsäule zu unterstützen. Es ist daher wünschenswert, dass ein verbessertes Rückenimplantat zum Fördern einer intervetebralen Körperfusion entwickelt wird.
- WO-A-97/00054 offenbart ein Rückenimplantat, das ein Paar eingreifender Platten umfasst. Eine Beabstandungsvorrichtung ist zwischen den eingreifenden Platten einführbar, um zu ermöglichen, dass der Abstand zwischen den Platten verändert werden kann. Die Beabstandungsvorrichtung umfasst ein Einstellmittel mit zwei Beabstandungsgliedern, die in einer sich in Längsrichtung erstreckenden und durch ausgefräste Furchen definierten Hohlräumen angeordnet sind.
- Zusammenfassung der Erfindung
- Entsprechend der vorliegenden Erfindung wird ein Wirbelsäulenimplantat bereitgestellt, das die vorgenannten Nachteile herkömmlicher Implantatvorrichtungen zum größten Teil beseitigt oder verringert.

Es sollte jedoch verstanden werden, dass die Zeichnungen und die zugehörige ausführliche Beschreibung nicht beabsichtigen, die Erfindung auf die speziell offenbarte Form zu beschränken, sondern im Gegenteil beabsichtigt, alle Veränderungen, Äquivalente und Alternativen, die in den durch die beigefügten Patentansprüche definierten Schutzbereich der vorliegenden Erfindung fallen, abzudecken.
- Ausführliche Beschreibung der bevorzugten Ausführungsformen
- Eine bevorzugte Ausführungsform einer Zwischenkörperfusionsimplantatvorrichtung
10 zum Fördern der Bildung einer Wirbelsäulenfusion ist in den3 bis5 dargestellt. Eine Draufsicht der Fusionsvorrichtung ist in3 dargestellt. Die Fusionsvorrichtung10 umfasst vorzugsweise ein Paar Seiten oder eingreifende Platten12 und14 zum Eingreifen in Wirbelkörper16 und18 . Die eingreifenden Platten können gebogene Kanten aufweisen, so dass die äußere Fläche15 der Platten der Form des Querschnitts der Wirbelkörper entspricht, wie in3 gezeigt. Die Fusionsvorrichtung hat eine Höhe20 , die durch den vertikalen Abstand zwischen den äußeren Flächen15 der eingreifenden Platten12 und14 festgelegt ist. Die Höhe20 der Fusionsvorrichtung ist vorzugsweise einstellbar und kann entlang der Fusionsvorrichtung zwischen dem vorderen Ende22 und dem hinteren Ende24 variieren, um die natürliche Lordose der Wirbelsäule aufrecht zu erhalten. Die Höhe20 kann auch entlang der Vorrichtung10 von der ersten Seite 26 zu der zweiten Seite28 variieren, um eine laterale Abweichung in der Wirbelsäule, die bei Skoliose auftreten kann, zu korrigieren. Die Fusionsvorrichtung10 umfasst weiterhin vorzugsweise eine Ausrichtvorrichtung zum Einstellen der Höhe20 , so dass die natürliche Lordose der Wirbelsäule im wesentlichen beibehalten wird, nachdem die Fusionsvorrichtung implantiert worden ist. Die Ausrichtvorrichtung kann verwendet werden, um die Höhe zwischen den eingreifenden Platten unmittelbar am vorderen Ende einzustellen und unabhängig die Höhe zwischen den eingreifenden Platten unmittelbar am hinteren Ende einzustellen. - Eine Wirbelsäulenfusion wird typischerweise eingesetzt, um Schmerzen, die durch die Bewegung des degenerierten Scheibenmaterials verursacht werden, zu beseitigen. Bei erfolgreicher Fusion wird die Fusionsvorrichtung
10 permanent innerhalb des Bandscheibenraums fixiert. Die Fusionsvorrichtung ist vorzugsweise mit Knochentransplantat40 bepackt, um Knochenwachstum durch und um die Fusionsvorrichtung herum zu fördern. Derartiges Knochentransplantat kann vor, nach oder während der Implantation der Fusionsvorrichtung zwischen die eingreifenden Platten12 und14 gepackt werden. Knochenersatzmaterial, das dem Fachmann wohlbekannt ist, kann anstelle des Knochentransplantats verwendet werden. Eine Knochenertragsausrüstung, die von Spine-Tech, Inc. mit Sitz in Minneapolis, MN kommerziell erhältlich ist, kann verwendet werden, um Knochentransplantat zwischen die eingreifenden Platten zu injizieren. Die Broschüre mit dem Titel "Bone Harvester: Minimaly Invasive Bone Harvesting Kit" (erhältlich von Spine-Tech, Inc.) beschreibt die Verwendung der Knochenertragsausrüstung ausführlich. - In einer Ausführungsform der Erfindung umfassen die Flächen
15 der eingreifenden Platten12 und14 eine Vielzahl von Öffnungen34 , die darin angeordnet sind, um die Entwicklung und das Wachsen von Knochen durch die eingreifenden Platten12 und14 und zwischen der Fusionsvorrichtung10 und den benachbarten Wirbeln16 und18 zu ermöglichen. In einer Ausführungsform weisen die Öffnungen34 ein zusammengesetztes Flächenmaß auf, das größer als etwa 50% des Flächenmaßes der Fläche15 (einschließlich des Flächenmaßes der Öffnungen34 ) ist, vorzugsweise zwischen etwa 60% und etwa 80% des Flächenmaßes der Fläche15 , und noch weiter bevorzugt etwa 70% oder mehr des Flächenmaßes der Fläche15 . - Die Fusionsvorrichtung kann eine Zurückhalteplatte
36 unmittelbar am hinteren Ende24 umfassen, um ein Futter bereitzustellen, gegen das Knochentransplantat gepackt werden kann und um das Knochentransplantat zwischen den eingreifenden Platten zu halten. Die Zurückhalteplatte36 kann im wesentlichen eben sein und kann Öffnungen enthalten, die Knochenwachstum dort hindurch ermöglichen. Eine entfernbare Endkappe25 kann unmittelbar am vorderen Ende22 angeordnet sein, um Knochentransplantat innerhalb der Fusionsvorrichtung zu enthalten und um das Wegwandern des Knochentransplantats aus den eingreifenden Platten zu verhindern. Die Endkappe25 kann eine oder mehrere Öffnungen enthalten, um Knochenwachstum zwischen einem Wirbelkörper und zwischen den eingreifenden Platten enthaltenes Knochentransplantat zu ermöglichen. Die Endkappe25 ist vorzugsweise hergestellt aus einem Kunststoffmaterial, wie beispielsweise Polyäthylen, das dazu neigt, nicht reizend und nicht abreibend in Bezug auf die umliegenden Gewebe zu sein. - Ein Querschnitt der Fusionsvorrichtung durch die Ebene I der
3 ist in4a und4b dargestellt.4a zeigt die relative Position der eingreifenden Platten12 und14 , bevor die Höhe20 mittels einer Ausrichtvorrichtung ausgerichtet worden ist, um eine im wesentlichen natürliche Lordose zu erzielen.4b zeigt die relative Position der Platten nachdem die Höhe20 eingestellt und Knochentransplantat40 zwischen die eingreifenden Platten gepackt worden ist.4b zeigt, dass die Höhe20 in der Nähe des vorderen Endes22 größer als im Vergleich zu dem hinteren Ende24 ist, um die natürliche Lordose der Wirbelsäule zu unterstützen. Die Flächen15 der eingreifenden Platten12 und14 sind vorzugsweise planar, um eine relativ große Kontaktfläche zwischen den eingreifenden Platten und den benachbarten Wirbeln bereitzustellen. Ruf diese Art kann ein Absinken der Wirbel verhindert werden, weil die durch die Fusionsvorrichtung auf die Wirbel ausgeübte Kraft nicht auf eine relativ kleine Fläche der Wirbel konzentriert ist, so wie das in einigen herkömmlichen Implantaten der Fall ist. Alternativ können die eingreifenden Platten nicht planar sein. Die eingreifenden Platten umfassen auch vorzugsweise eine Vielzahl von Spitzen oder Vorsprüngen38 , die aus der Fläche15 hervorstehen zum Verstärken eines Angriffs zwischen den Wirbeln und der eingreifenden Platte. Die Vorsprünge können in den Wirbel ragen, um zu verhindern, dass sich die Fusionsvorrichtung aus dem Bandscheibenraum herausbewegt. Die eingreifenden Platten sind vorzugsweise aus Titan oder einer Titanlegierung aufgebaut, obwohl verständlich sein sollte, dass andere Materialien (beispielsweise Keramik, Metalle, Karbonzusammensetzungen) verwendet werden können. - Eine Vorderansicht der Fusionsimplantatvorrichtung ist in
5 dargestellt. In einer Ausführungsform der Erfindung umfasst die Ausrichtvorrichtung eine erste Stütze30 und eine zweite Stütze32 , die sich jeweils zwischen den eingreifenden Platten12 und13 entlang der Länge der Fusionsvorrichtung von dem vorderen Ende22 zum hinteren Ende24 erstrecken. Wie hierin beschrieben wird der Ausdruck "Stütze" in der Bedeutung eines beliebigen Stützglieds verwendet, das zwischen den eingreifenden Platten angeordnet ist, um die eingreifenden Platten zu trennen. Die Stütze30 erstreckt sich vorzugsweise entlang der nächstliegenden ersten Seite26 der Fusionsvorrichtung. Die Stütze32 ist vorzugsweise im wesentlichen parallel zur Stütze30 und kann sich entlang der nächstliegenden zweiten Seite28 der Fusionsvorrichtung erstrecken. Die Stützen30 und32 dienen dazu, einen vorbestimmten Abstand zwischen den eingreifenden Platten zu erzeugen. Der vorbestimmte Abstand ist vorzugsweise derart, dass die Höhe20 näherungsweise gleich der Höhe des Bandscheibenmaterials ist, das vorher den Bandscheibenraum zwischen den Wirbelkörpern besetzte. - Eine perspektivische Ansicht einer Ausführungsform der Stütze ist in
6a dargestellt. Die Stütze kann eine "I-Strahl"-Form aufweisen und umfasst vorzugsweise ein Paar Enden50 . Die Enden50 können eine laterale Breite51 aufweisen, die größer ist als die der Seiten53 . Die Enden weisen vorzugsweise einen "Schwalbenschwanz"-förmigen Querschnitt wie in6a gezeigt auf. Die eingreifenden Platten umfassen vorzugsweise längliche Schlitze60 (ge zeigt in den7 und8 ), die in der Größe angepasst sind um die Enden50 der ersten und zweiten Stützen aufzunehmen. Der Schlitz60 weist vorzugsweise eine komplementäre Schwalbenschwanzform auf, wie in8 dargestellt, die der Form des Endes50 entspricht. Die Stützen können mit den eingreifenden Platten verbunden werden, indem gleitende Enden50 in den Schlitzen60 in einer Richtung vom vorderen Ende22 zum hinteren Ende24 oder umgekehrt hineingleiten. - In einer Ausführungsform sind die Schlitze derart verjüngend, dass ihre Breite sich in einer Richtung vom vorderen Ende zum hinteren Ende wie in
7 gezeigt, verengt. Die Enden50 können derart verjüngend sein (wie in10 gezeigt), dass die laterale Breite51 sich entlang der Länge der Stütze verschmälert. Die Verjüngung der lateralen Breite der Stütze passt vorzugsweise mit der des Schlitzes60 zusammen. Die Breite des Schlitzes unmittelbar am vorderen Ende ist vorzugsweise in der Größe derart gewählt, dass es möglich ist, das Stützenende in den Schlitz hineinzugleiten. Die Breite des Schlitzes unmittelbar am hinteren Ende ist vorzugsweise kleiner als die laterale Breite51 des engsten Teils des Endes50 . Die Verjüngung der Schlitze ermöglicht vorzugsweise ein "verschließendes, verjüngendes Eingreifen" der Stützenenden in den Schlitzen. Der Begriff eines "verschließenden, verjüngenden Eingriffs" wird verwendet zum Bezeichnen einer festsetzbaren Eingriffpassung, die zwischen dem Ende50 und dem Schlitz60 gebildet wird, wobei die Stütze einem Verschieben widersteht, wenn von den benachbarten eine Kraft auf die Fusionsvorrichtung Wirbeln ausgeübt wird. In einer alternativen Ausführungsform können die Schlitze derart verjüngend sein, dass sich die Breite der Schlitze in einer Richtung vom hinteren Ende zum vorderen Ende verjüngt. - Die erste und zweite Stütze weist jeweils eine vorbestimmte Höhe auf, die die Höhe der Fusionsvorrichtung festlegt. Die eingreifenden Platten
12 und14 sind vorzugsweise angepasst zum Aufnehmen von Stützen verschiedener Höhen, um zu ermöglichen, dass die Höhe20 variiert werden kann zum Anpassen an die Bedürfnisse des Patienten. Eine Seitenansicht einer sich verjüngenden Stütze ist in6b dargestellt. Die sich verjüngende Stütze weist vorzugsweise eine Höhe auf, die entlang ihrer Länge variiert. Auf diese Weise wird die sich verjüngende Stütze zwischen den eingreifenden Platten12 und14 positionierbar, um zu bewirken, dass die Höhe20 in einer Richtung vom vorderen Ende22 zum hinteren Ende24 abnimmt, wobei die natürliche Lordose der menschlichen Wirbelsäule durch die Fusionsvorrichtung unterstützt wird. Der Grad der Verjüngung der Stütze entspricht einer gewünschten Lordose und kann in Abhängigkeit von der Größe des Patienten variieren. - In einer Ausführungsform weisen die erste und zweite Stütze verschiedene Höhen auf, um zu bewirken, dass die Höhe
20 zwischen dem ersten Ende14 und dem zweiten Ende16 variiert. Auf diese Weise kann die Fusionsvorrichtung verwendet werden, um eine laterale Abweichung in der Wirbelsäule, so wie sie bei einer Skoliose auftreten kann, zu korrigieren. Eine Vorderansicht einer Fusionsvorrichtung, die Stützen mit verschiedenen Höhen umfaßt, ist in9 dargestellt. Jede der Stützen umfaßt vorzugsweise einen Gelenkzapfen70 , um zu ermöglichen, dass sich ein oberes Glied72 der Stütze sich in Bezug auf ein unteres Glied74 der Stütze dreht. Auf diese Weise können die Stützen wie in9 gezeigt, derart gedreht werden, dass die Enden der Stützen passend mit den Schlitzen der eintreffenden Platten ausgerichtet sind, wenn ein Höhenunterschied zwischen der ersten und zweiten Stütze besteht. - Zum Einbau der Fusionsvorrichtung wird vorzugsweise eine Disektomie mit einem vorderen Zugang ausgeführt. Alle Knorpel und alles weiche Gewebe werden vorzugsweise von der Wirbelendplatte entfernt, so wie das normalerweise beim Einsetzen eines Oberschenkel-Stützimplantats ausgeführt werden würde. Ein derartiges Vorgehen gehört zum Wissen eines auf dem Gebiet erfahrenen Praktizierenden. Die eingreifenden Platten können in den Bandscheibenraum zwischen den benachbarten Wirbeln eingesetzt werden. Eine Ablenkkraft kann auf die eingreifenden Platten angewendet werden unter Benutzung eines Plättchenaufweiters (englisch: laminae spreader) oder einer ähnlichen Vorrichtung, um die Wirbel in eine ausgewählte Höhe und lordotische Ausrichtung zu bringen. Die Verwendung eines Plättchenaufweiters ist dem Fachmann wohl bekannt. Die richtigen Höhen für die erste und zweite Stütze kann zuvor mittels Röntgentechniken, bei denen die hinteren und vorderen Bereiche des Zwischenwirbelbandscheibenraums untersucht werden, bestimmt werden.
- In ihrer Größe angemessen gewählte und sich verjüngende Stützen werden vorzugsweise in Schlitzen
60 hineingeschoben und punktiert, bis ein verschließender, verjüngender Eingriff zwischen den Stützenden und dem Schlitz erzielt wird. Wenn Stützen verschiedener Höhen verwendet werden, um eine laterale Abweichung der Wirbelsäule zu korrigieren, kann jede Stütze vor dem Einsetzen um den Gelenkzapfen70 gedreht werden, so dass die Enden50 richtig ausgerichtet sind zum Platzieren in die Schlitze60 . Knochentransplantatmaterial wird vorzugsweise durch das vordere Ende eingeführt und zwischen die eingreifenden Platten gepackt. Die Rückhalteplatte36 verhindert vorzugsweise, dass Knochentransplantatmaterial während des Packens durch die Fusionsvorrichtung hindurchläuft. Die Endkappe25 kann dann auf das vordere Ende aufgesetzt werden. - Eine Ausführungsform der lastverteilenden Fusionsvorrichtung ist in den
10 bis12 dargestellt. Eine Draufsicht auf eine Stütze30 , die ein lastverteilendes Glied enthält, ist in10 dargestellt. Die10 und12 stellen Querschnittsansichten der Stütze entlang der Ebene V der10 dar. Das lastverteilende Glied110 ist vorzugsweise axial durch die Stütze hindurch angeordnet. Das lastverteilende Glied kann in einer sich durch die Stütze erstreckenden Bohrung angeordnet sein. Die Bohrung weist vorzugsweise eine Breite (d. h. Durchmesser) auf, die größer ist als die des lastverteilenden Gliedes, um dem lastverteilenden Glied genügend Raum zum Durchbiegen zu geben. Die Bohrung ist vorzugsweise innerhalb des unteren Glieds74 angeordnet. Der Teil118 des oberen Gliedes kann den Knochen und das lastverteilende Glied im wesentlichen umringen, wobei es eine Befestigung des oberen und unteren Gliedes ermöglicht. In einer Ausführungsform ist das lastverteilende Element ein Zapfen, der einen im wesentlichen runden Querschnitt aufweist. Der Zapfen passt vorzugsweise lose derart in die Bohrung, dass seine Drehfreiheit beibehalten bleibt. Der Zapfen kann der Gelenkzapfen70 sein, um den sich das obere Glied72 in Bezug auf das untere Glied74 dreht. Das lastverteilende Glied umfasst vorzugsweise einen Einschnitt114 , der eine im wesentlichen planare Oberfläche bildet, um die herum das lastverteilende Glied gebogen werden kann. - Ein Verbinder
112 erstreckt sich vorzugsweise durch eine Öffnung116 im Ende50 der Stütze. Der Verbinder befestigt vorzugsweise das lastverteilende Glied mit dem oberen Glied72 und kann das lastverteilende Glied am Auflagepunkt126 , der vorzugsweise auf der innerhalb der Einkerbung114 gebildeten planaren Oberfläche angeordnet ist, berühren. Der Verbinder122 ist vorzugsweise eine Stell schraube, und die Öffnung116 enthält vorzugsweise ein Gewinde zum Eingreifen für die Stellschraube.11 stellt eine Stütze in einer "nicht belasteten" Position dar, bei der ein vorbestimmter Abstand122 zwischen dem oberen Glied72 und dem Teil120 des unteren Glieds74 besteht. Der vorbestimmte Abstand122 kann eingestellt werden, indem die Position des Verbinders112 innerhalb der Öffnung116 verändert wird. Beispielsweise kann die Schraube durch die Öffnung116 hindurch zum Vergrößern des Abstands122 gedreht werden. In der nicht belasteten Position bleibt das lastverteilende Glied im wesentlichen nicht durchgebogen. - Bei Anwendung einer Kompressionskraft auf das Ende
50 des oberen Glieds72 wird Kraft vorzugsweise von dem Verbinder112 an dem Auflagepunkt126 auf das lastverteilende Glied ausgeübt. Die Druckkraft reicht vorzugsweise aus, um eine Durchbiegung des lastverteilenden Gliedes und eine Bewegung des oberen Glieds72 auf den Teil120 des unteren Glieds, so dass der vorbestimmte Abstand122 vergrößert wird, zu verursachen. Das Durchbiegen des lastverteilenden Glieds kann den Teil118 des oberen Glieds in einen in der axialen Bohrung gebildeten Hohlraum115 bringen. Das lastverteilende Glied ist vorzugsweise in einer Dreipunktbiegeanordnung wie in19 gezeigt gebogen. -
12 stellt die Stütze in der "belasteten" Position dar, wobei das lastverteilende Glied durchgebogen ist. Der vorbestimmte Abstand22 ist vorzugsweise justierbar, und kann justiert sein, um die maximale Belastung, die durch das lastverteilende Glied ausgeübt werden kann, einzustellen. Wenn das lastverteilende Glied sich um einen vertikalen Abstand verbogen hat, der gleich mit dem vorbestimmten Abstand22 ist, dann berührt das obere Glied72 den Teil120 , und verhindert dabei weitere Belastung auf das lastverteilende Glied. Auf diese Weise kann der maximale Grad der Zugbelastung auf das lastverteilende Glied beschränkt werden, um die Möglichkeit zu verringern, dass das Glied einen Ausfall durch Materialermüdung erleidet. - Das lastverteilende Glied kann aus einem beliebigen einer Vielzahl von Metallen oder Legierungen aufgebaut sein. In einer bevorzugten Ausführungsform ist das lastverteilende Glied aus Titan oder einer Titanlegierung aufgebaut. Die Materialeigenschaften und die Querschnittsfläche des lastverteilenden Glieds sind vorzugsweise kontrolliert, um zu ermöglichen, dass ein vorbestimmter Grad an Stress in der Fusionsvorrichtung auftritt. Der horizontale Abstand
124 oder Hebelarm zwischen dem Auflagepunkt126 und dem Tragepunkt128 auf dem unteren Glied ist vorzugsweise derart ausgewählt, dass die Fusionsvorrichtung ein "effektives" Elastizitätsmodul in der Nähe des Elastizitätsmoduls von Knochen aufweist, um Knochenentwicklung zu fördern. Der Begriff "effektives" Elastizitätsmodul der Fusionsvorrichtung wird benutzt, um das Verhältnis der Druck- zur Zugbelastung in der Fusionsvorrichtung in einer Richtung entlang der Höhe20 zu bezeichnen, wobei die Vorrichtung sich beim Aufnehmen einer Druckkraft aus der nicht belasteten Position in die belastete Position bewegt. Wie hierin beschrieben wird der Ausdruck "in der Nähe des Elastizitätsmoduls von Knochen" benutzt, zum Bezeichnen eines Young-Moduls zwischen etwa 3 GPa und etwa 25 GPa. In einer Ausführungsform ist der effektive Modul der Fusionsvorrichtung zwischen etwa 16 GPa und etwa 25 GPa. Die Veröffentlichung mit dem Titel "Variation of Young's Modulus and Hardness in Human Lumbar Vertebrae Measured by Nanoindentation" von Marcel Roy und Jae-Young Rho (Lehrstuhl für Biomedizinisches Ingenieurwesen, University of Memphis, Memphis, TN) und Ting Y. Tsui und George M. Pharr (Lehrstuhl für Materialwissenschaften, Rice University, Hou ston, TX) bezieht sich auf die mechanischen Eigenschaften von Knochen. - Die auf die Wirbelsäule ausgeübten Belastungen werden vorzugsweise durch die Fusionsvorrichtung und die umgebenden Knochenimplantate verteilt. Wenn sich die Wirbelsäulenfusion entwickelt, nimmt der Anteil der durch das umgebende Knochenmaterial gebildeten Belastung vorzugsweise zu und die benötigte Belastung auf die Fusionsvorrichtung nimmt vorzugsweise ab. Nach Vervollständigen der Fusion verbleibt die Fusionsvorrichtung vorzugsweise in der nicht belasteten Position während der normalen täglichen Aktivitäten des Patienten.
- Die Fusionsvorrichtung
10 stellt vorzugsweise eine relativ große Kontaktfläche zwischen den eingreifenden Platten und den Wirbelkörpern, die den durch die Fusionsvorrichtung eingenommenen Bandscheibenraum definieren, zur Verfügung.13 stellt eine Draufsicht einer Ausführungsform einer erfindungsgemäßen Fusionsvorrichtung dar.14 stellt eine Querschnittsansicht der Fusionsvorrichtung entlang der Ebene VI der13 dar. In den15 bis17 wird ein herkömmlicher Fusionskäfig dargestellt, so wie der in dem an Ray et al. erteilten U.S. Patent Nr. 4,961,740 beschrieben ist. Die Vorrichtungen in den13 bis17 sind: in ihrer Größe angepasst zur Verwendung in dem L3 bis L4 Bandscheibenraum einer mittelgroßen Frau mittleren Alters. Die Abmessungen der Fusionsvorrichtungen sind in Millimetern angegeben. - Die "effektive Kontaktfläche" zwischen einer eingreifenden Platte und einem Wirbelkörper kann berechnet werden, indem man die Fensterverbandfläche a (d. h. das zusammengesetzte Flächenmaß der Öffnungen
34 , die für das Knochenwachstum vorgesehen sind) von der totalen Kontaktfläche A (das Flä chenmaß der Fläche15 einschließlich des Flächenmaßes der Öffnungen34 ) abzieht. Die totale Kontaktfläche und die Fensterverbandfläche der Fusionsvorrichtung in den13 und14 beträgt 581 mm2 bzw. 96 mm2. Daher ist die effektive Kontaktfläche zwischen der eingreifenden Platte und dem Wirbel 485 mm2. - Für den in den
15 bis17 dargestellten Fusionskäfig wird angenommen, dass Gewindebohrungen auf der äußeren Oberfläche des Fusionskäfigs in den Wirbel bis zu insgesamt drei Millimetern pro Seite, wie vom Hersteller empfohlen, eindringen. Es sollte festgehalten werden, dass dieses Eindringen oft schwierig zu erzielen ist. Zusätzlich ist die kortikale Schicht eines Wirbelkörpers häufig nur 1 bis 2 Millimeter dick. Jedes der zylindrischen Elemente des Fusionskäfigs weist eine totale Kontaktfläche von 283,5 mm2 und eine Fensterverbandfläche von 198,5 mm2 auf. Daher beträgt die zusammengesetzte effektive Kontaktfläche der beiden zylindrischen Elemente 170 mm2. Wenn die Gewindegänge des Fusionskäfigs eine kleinere Strecke als 3 mm pro Seite in den Wirbel eindringen, wird die Kontaktfläche kleiner als die oben berechnete. - Die von täglichen Aktivitäten herrührenden, maximalen axialen Kompressionskräfte in den Lendenwirbeln wurden in einer Veröffentlichung mit dem Titel "The BAKTM Interbody Fusion: An Innovative Solution" von Bagby et al. und verfügbar von Spine-Tech, Inc. in Minneapolis, Minnesota (siehe Seite 3, unterer Abschnitt) auf 3200 N abgeschätzt. Für eine Kompressionskraft von 3200 N wird die Belastung pro Fläche zu 18,8 N/mm2 für den in den
15 bis17 dargestellten Fusionskäfig berechnet, im Vergleich zu 6,6 N/mm2 für die in den13 und14 dargestellte Fusionsvorrichtung. Es wird angenommen, dass eine solche Verringerung der Belastung pro Fläche zu einer signifikan ten Verringerung des post-chirurgischen Einsinkens an der Grenzfläche der Fusionsvorrichtung und des Wirbelkörpers führt. Typischerweise wird die Abnahme der Bandscheibenhöhe nach einem Monat auf etwa 1 bis 3 mm abgeschätzt, wenn herkömmliche Vorrichtungen wie die in den15 bis17 dargestellten verwendet werden. - Zum Einbau der Fusionsvorrichtung wird vorzugsweise eine Disektomie mit einem Zugang von vorne ausgeführt. Alle Knorpel und weiches Gewebe werden vorzugsweise von der Wirbelendplatte entfernt, so wie das normalerweise beim Einsetzen eines Oberschenkel-Stützimplantats ausgeführt werden würde. Ein derartiges Vorgehen gehört zum Wissen eines in dem Gebiet erfahrenen Praktizierenden. Die eingreifenden Platten können in den Bandscheibenraum zwischen benachbarten Wirbeln eingesetzt werden.
Claims (12)
- Rückenimplantat zur Förderung einer Fusion zwischen benachbarten Wirbeln einer menschlichen Wirbelsäule, umfassend: – ein vorderes Ende gegenüber einem hinteren Ende; – ein Paar eingreifende Platten (
12 ,14 ), angepaßt, um zwischen die Wirbel zu passen und einzugreifen, um einen Handscheibenraum zwischen den Wirbeln während der Verwendung aufrechtzuerhalten; – eine zwischen den eingreifenden Platten positionierbare Stütze (30 ), wobei die Stütze angepaßt ist, um die eingreifenden Platten während der Verwendung derartig zu trennen, daß eine Höhe (20 ) dazwischen existiert, wobei die Höhe derartig einstellbar ist, daß die Höhe unmittelbar am vorderen Ende sich von der Höhe unmittelbar am hinteren Ende unterscheidet, um im wesentlichen eine natürliche Krümmung der menschlichen Wirbelsäule während der Verwendung aufrechtzuerhalten; und – jede der eingreifenden Platten einen Schlitz (60 ) umfaßt, dadurch gekennzeichnet, daß die Stütze (30 ) ein oberes und ein unteres Glied (72 ,74 ) umfaßt, die in den Schlitz (60 ) aufgenommen werden und aneinander durch ein biegsames lastverteilendes Glied (110 ) befestigt sind, so daß die Stütze (30 ) biegsam ist, um die eingreifenden Platten (12 ,14 ) zu trennen und um Belastung auf Knochen in der Nachbarschaft des Implantats anwenden zu lassen. - Rückenimplantat nach Anspruch 1, wobei die Stütze während der Verwendung bedienbar ist, um die Höhe zwischen den eingreifenden Platten (
12 ,14 ) unmittelbar am vorderen Ende einzustellen und simultan die Höhe zwischen den eingreifenden Platten unmittelbar am hinteren Ende einzustellen. - Rückenimplantat nach Anspruch 1, wobei die eingreifenden Platten (
12 ,14 ) im wesentlichen planar sind, um so ein Absinken der Wirbel zu verhindern. - Rückenimplantat nach Anspruch 1, wobei die eingreifenden Platten eine Vielzahl von Öffnungen (
34 ) umfassen, um zu erlauben, daß Knochenwachstum durch die eingreifenden Platten erfolgt. - Rückenimplantat nach Anspruch 1, wobei die eingreifenden Platten eine Vielzahl von Öffnungen (
34 ) umfassen und weiterhin Knochentransplantat (40 ) umfassen, das angepaßt ist, um zwischen die eingreifenden Platten gepackt zu werden. - Rückenimplantat nach Anspruch 1, wobei die Stütze ein Gelenk (
70 ) umfaßt, um dem oberen Glied (74 ) der Stütze zu erlauben, sich in Bezug auf das untere Glied (74 ) der Stütze zu drehen. - Rückenimplantat nach Anspruch 1, weiterhin umfassend Vorsprünge (
38 ), die sich von mindestens einer der eingreifenden Platten zu Erhöhung eines Eingriffs zwischen dem Wirbel und der eingreifenden Platte erstrecken. - Rückenimplantat nach Anspruch 1, weiterhin umfassend eine Zurückhalteplatte (
36 ) unmittelbar am hinteren Ende, zum Halten der Knochentransplantat (40 ) zwischen den eingreifenden Platten. - Rückenimplantat nach Anspruch 1, weiterhin umfassend eine entfernbare Endkappe (
25 ) unmittelbar am vorderen Ende zum Halten von Knochentransplantat zwischen den eingreifenden Platten. - Rückenimplantat nach Anspruch 1, wobei der Schlitz (
60 ) einen im wesentlichen schwalbenschwanzförmigen Querschnitt zur Aufnahme eines Endes der Stütze besitzt. - Rückenimplantat nach Anspruch 1, wobei jede der eingreifenden Platten (
12 ,14 ) einen Schlitz (60 ) zur Aufnahme eines Endes der Stütze umfaßt, wobei der Schlitz eine Weite besitzt, die sich in Richtung vom vorderen Ende zum hinteren Ende verengt, wobei die Weite des Schlitzes unmittelbar am hinteren Ende geringer ist als die weite des Endes der Stütze, so daß ein verschließender verjüngender Eingriff zwischen dem Schlitz und dem Ende der Stütze gebildet werden kann. - Rückenimplantat nach Anspruch 1, wobei die Stütze (
30 ) eine Höhe (20 ) umfaßt, die sich entlang ihrer Länge derartig ändert, daß die Höhe zwischen den eingreifenden Platten zwischen dem vorderen Ende und dem hinteren Ende unterschiedlich ist.
