Description
- Die vorliegende Erfindung bezieht sich auf ein Wirbelsäulenimplantat, um benachbarte Rückenwirbel einer Wirbelsäule miteinander zu vereinigen.
- Ein bekanntes Wirbelsäulenimplantat besitzt eine rechteckige Form und ein sich verjüngendes Ende. Das Wirbelsäulenimplantat weist Noppen auf, um benachbarte Rückenwirbel festzuhalten. Die Noppen haben geneigte Flächen, welche ein Vorwärtsgleiten des Wirbelsäulenimplantats in Kanäle erlaubt, die in benachbarte Rückenwirbel geschnitten sind. Dieses bekannte Wirbelsäulenimplantat ist im U.S. Patent No. 4,834,757 beschrieben, welches als Basis für den Oberbegriff des Anspruchs 1 dient. Beim Schneiden von Kanälen in die Rückenwirbel zur Aufnahme des Wirbelsäulenimplantats besteht die Gefahr, Nervenwurzeln zu verletzen.
- Ein Wirbelsäulenimplantat, offenbart in einem älteren EP-A-0 720 455 Dokument unter Art. 54(3) EPÜ, umfasst einen starren, rechteckigen und keilförmig ausgebildeten Block, mit oberen, unteren, seitlichen und endseitigen Wänden. Die oberen und unteren Wände weisen keine Öffnungen auf, laufen vom hinteren zum vorderen Ende des Kanals zusammen und erlauben denjenigen Rückenwirbeln, die den Block im Kanal aufnehmen, zusammengestossen zu werden, um die normale, scheibenförmige Kontur und die Ausrichtung in die Sagittalebene wiederherzustellen. Um eine Irritation der Seiten der Kanäle zu vermeiden, sind die Seitenwände des Blocks glatt. Die Seitenwände haben einen Schlitz, der eine Kammer bildet, in der das wachsende, den Block umfassende Knochenmaterial aufgenommen werden kann. Eine der endseitigen Wände des Blocks weist eine Bohrung auf, an der ein Werkzeug angesetzt werden kann, um das Einsetzen des Blocks in die Kanäle zu erleichtern. Die oberen und unteren Wände des Blocks weisen Zähne auf, um in die Böden der Kanäle zu beissen.
- Die vorliegende Erfindung gemäss Anspruch 1 sieht ein neues und verbessertes Wirbelsäulenimplantat vor, um benachbarte Rückenwirbel einer Wirbelsäule miteinander zu vereinigen. Das Wirbelsäulenimplantat wird zwischen benachbarte Rückenwirbel eingefügt, wobei die ersten und zweiten, im wesentlichen parallelen Seitenflächen den benachbarten Rückenwirbeln zugewandt sind. Das Wirbelsäulenimplantat wird in eine Position verdreht, in der die parallelen Seitenflächen sich von einem der benachbarten Rückenwirbel zum anderen der benachbarten Rückenwirbel erstrecken und die oberen und unteren Oberflächenmittel in den benachbarten Rückenwirbel eingreifen. Es werden keine Kanäle in den benachbarten Rückenwirbel geschnitten. Dadurch wird für die Operation weniger Zeit benötigt, und die Risiken für den Patienten werden verringert.
- Die vorgängigen und andere Merkmale der vorliegenden Erfindung werden für den Fachmann offensichtlich, nachdem die folgende Beschreibung der vorliegenden Erfindung unter Bezugnahme auf die beiliegenden Zeichnungen gelesen wurde. In diesen Zeichnungen zeigen:

- Ein erstes Ausführungsbeispiel eines Wirbelsäulenimplantats gemäss der vorliegenden Erfindung und ein Verfahren zum Einsetzen des Wirbelsäulenimplantats zwischen benachbarte Rückenwirbel sind in den Fig. 1-12 gezeigt.
- Eines oder mehrere im wesentlichen starre Wirbelsäulenimplantate 10 (von denen eines in der Fig. 1 gezeigt ist) werden nebeneinander zwischen benachbarte Rückenwirbel 12 und 14 einer Wirbelsäule 16 eingesetzt, um die benachbarten Rückenwirbel miteinander zu vereinigen. Die Wirbelsäulenimplantate 10 werden vorzugsweise im Spritzgiessverfahren aus einem mit geschnittenen Kohlenfasern verstärktem Polymer hergestellt. Trotzdem können die Wirbelsäulenimplantate 10 auch aus anderen passenden, implantierbaren Materialien wie rostfreier Stahl oder Titan gefertigt werden. Ausserdem sollte die Zugfestigkeit der Wirbelsäulenimplantate 10 bevorzugterweise zumindest 6,9 N/mm² (10000 psi) betragen, so dass die Wirbelsäulenimplantate eine relative Bewegung zwischen den benachbarten Rückenwirbeln 12 und 14 verhindern und die Belastbarkeit der Wirbelsäule auf Druck unterstützen.
- Jedes der Wirbelsäulenimplantate 10 (Fig. 2) weist parallele Seitenflächen 20 und 22 auf. Eine obere Fläche 24 und eine untere Fläche 26 zum Eingreifen in die benachbarten Rückenwirbel 12 und 14 erstrecken sich zwischen den Seitenflächen 20 und 22. Die oberen und unteren Flächen 24 und 26, die an einen ersten Endteil 30 des Wirbelsäulenimplantats 10 angrenzen, liegen um eine erste Distanz auseinander. Die oberen und unteren Flächen 24 und 26, die an einen zweiten Endteil 32 des Wirbelsäulenimplantats 10 angrenzen, liegen um eine zweite Distanz auseinander. Die zweite Distanz ist vorzugsweise grösser als die erste Distanz, um dem Wirbelsäulenimplantat eine keilförmige Form zu verleihen, damit es in Teilen der Wirbelsäule mit einer lordotischen Krümmung eingesetzt werden kann. Obwohl das Wirbelsäulenimplantat 10 in einer keilförmigen Form dargestellt ist, um in Teilen der Wirbelsäule mit einer lordotischen Krümmung eingesetzt werden zu können, kann das Wirbelsäulenimplantat 10 auch keilförmig ausgebildet sein, um in Teilen der Wirbelsäule mit einer kypothetischen Krümmung eingesetzt zu werden, oder schliesslich kann es auch nicht keilförmig ausgebildet sein.
- Die oberen und unteren Flächen 24 und 26 weisen eine Mehrzahl von dreieckig geformten Zähnen 36 auf, welche sich von der Seitenfläche 20 zu der Seitenfläche 22 erstrecken, um in die Rückenwirbel 12 und 14 einzugreifen. Jeder Zahn 36 (Fig.3) weist eine zum Endteil 30 gerichtete Fläche 40 auf. Eine Fläche 42 des Zahns 36 ist zum Endteil 32 des Wirbelsäulenimplantats 10 hin gerichtet. Die Flächen 40 und 42 des Zahns 36 schneiden einander, um eine Kante 44 zu bilden. Die Flächen 40 und 42 von benachbarten Zähnen 36 schneiden sich, um Kanten 46 zu bilden. Die Kanten 46 sind parallel zueinander und liegen in einer Ebene 48.
- Die Fläche 40 des Zahns 36 erstreckt sich unter einen spitzen Winkel X zu der Fläche 48. Die Fläche 42 des Zahns 36 erstreckt sich unter einem spitzen Winkel Y zu der Ebene 48. Vorzugsweise sind die Winkel X und Y gleich gross und weisen einen Wert von 45º auf, so dass die Flächen 40 und 42 senkrecht zueinander verlaufen. Dadurch haben die Zähne 36 keine bevorzugte Wirkungsrichtung. Die Zähne 36 verhindern einerseits, dass sich das Wirbelsäulenimplantat 10 in Richtung des vorderen Endes der Wirbelsäule 16 verschiebt, wie auch, dass sich das Wirbelsäulenimplantat 10 in Richtung des hintere Endes der Wirbelsäule 16 verschiebt. Trotzdem können die Winkel X und Y, wie von einem Chirurgen gewünscht, unterschiedliche Werte einnehmen.
- Eine Mehrzahl von Öffnungen 56 und 58 erstreckt sich von der Seitenfläche 20 zu der Seitenfläche 22, um das Fliessen von Körperflüssigkeiten und das Knochenwachstum von einer Seite des Implantats 10 auf die andere Seite des Implantats zu ermöglichen. Die Öffnungen 58 sind nahe dem Endteil 30 des Implantats angeordnet. Die Öffnungen 56 und 58 erstrecken sich senkrecht zu den Seitenflächen 20 und 22. Vorzugsweise sind fünf Öffnungen 56 und zwei Öffnungen 58 vorgesehen, welche sich zwischen den Seiten 20 und 22 erstrecken. Die Durchmesser der Öffnungen 56 sind grösser als die Durchmesser der Öffnungen 58. Die Grössen, Formen und Positionen der Öffnungen 56 und 58 können gemäss den Wünschen eines Chirurgen verändert werden. Obwohl die Öffnungen 56 und 58 senkrecht verlaufend zu den Seitenflächen 20 und 22 gezeigt sind, können die Öffnungen 56 und 58 auch unter einem spitzen Winkel zu den Seitenflächen 20 und 22 verlaufen. Ausserdem ist zu berücksichtigen, dass das Wirbelsäulenimplantat 10 in den Seitenflächen 20 und 22 Ausnehmungen aufweisen kann, um Autotransplantat- oder Allotransplantat-Knochen, Knochenproteine, Knochensubstitute oder dergleichen anstelle der Öffnungen 56 und 58 oder zusammen mit diesen aufzunehmen.
- Die Seitenfläche 20 weist eine Ausnehmung 60 und die Seitenfläche 22 eine Ausnehmung 62 auf. Die Öffnungen 58 sind in den Ausnehmungen 60 und 62 angeordnet. Jede der Ausnehmungen 60 und 62 weist eine plane Bodenfläche 64 auf. Die Ausnehmungen 60 und 62 weisen zudem parallele Seitenflächen 66 und 68 auf, welche senkrecht zu der Bodenfläche 64 verlaufen und sich vom Endteil 30 des Wirbelsäulenimplantats 10 zu dem Endteil 32 hin erstrecken. Eine bogenförmige Seitenfläche 70 erstreckt sich zwischen den parallelen Seitenflächen 66 und 68. Die Ausnehmungen 60 und 62 können irgendwo auf den Flächen 20 und 22 angeordnet werden, je nachdem ob das Implantat 10 von hinten, von vorne oder seitlich von vorne eingesetzt wird. Die Ausnehmungen 60 und 62 in den Seitenflächen 20 und 22 dienen dem Ansetzen eines Instruments 80 (Fig. 4), welches das Wirbelsäulenimplantat 10 festhält, um das Einsetzen des Wirbelsäulenimplantats 10 zwischen die benachbarten Rückenwirbel 12 und 14 und das Drehen des Wirbelsäulenimplantats 10, nachdem es zwischen den benachbarten Rückenwirbeln eingesetzt ist, zu erleichtern.
- Jedes Instrument, das das Implantat sicher festhält und ein Drehen des Implantats in die Position erlaubt, kann verwendet werden. Ein solches Instrument 80 (Fig. 4) umfasst einen mittleren Abschnitt 82 (Fig. 5), einen Handgriff 84 (Fig. 6) und einen Klemmteil 86 (Fig. 7 und 8).
- Der mittlere Abschnitt 82 (Fig. 5) ist im wesentlichen zylindrisch ausgebildet und umfasst eine sich entlang der Achse des mittleren Abschnitts erstreckenden Öffnung 92. Die Öffnung 92 umfasst einen ersten, zylindrischen Bereich 94, der sich von einem Endteil 96 des mittleren Abschnitts 82 zu einem zentralen Bereich 98 erstreckt. Ein zweiter, zylindrischer Bereich 100 der Öffnung 92 erstreckt sich vom ersten zylindrischen Abschnitt 94 zu einem konisch zulaufenden Abschnitt 102 der Öffnung 92. Der zweite, zylindrische Bereich 100 der Öffnung 92 weist einen Durchmesser auf, der grösser ist als der Durchmesser des ersten zylindrischen Bereichs 94. Der konisch zulaufende Bereich 102 der Öffnung 92 verläuft konisch von einem an den Bereich 100 angrenzenden, kleinen Durchmesser zu einem an den Bereich 104 angrenzenden, grösseren Durchmesser des mittleren Abschnitts 82.
- Der mittlere Abschnitt 82 weist einen Bereich 108 mit einem grösseren Durchmesser auf, der sich vom Endteil 96 zum zentralen Bereich 98 erstreckt. Der Bereich 108 verjüngt sich von einem grossen Aussendurchmesser zu einem sich vom Bereich 108 zum Endbereich 104 erstreckenden Bereich 110 mit kleinem Aussendurchmesser. Die Oberfläche des Bereichs 108 ist vorzugsweise gerändelt, um einem Chirurgen das Erfassen des mittleren Abschnitts 82 zu erleichtern. Der Endteil 96 besitzt einen Durchmesser, der kleiner ist als derjenige der Abschnitte 108 und 110.
- Der Handgriff 84 (Fig. 6) weist einen gerändelten Bereich 116 mit grossem Durchmesser auf. Eine Spindel mit kleinem Durchmesser 118 steht vom gerändelten Bereich 116 ab. Die Spindel 118 weist ein mit einem Gewinde versehenes Ende auf, der in den Klemmteil 86 eingeschraubt werden kann.
- Der Klemmteil 86 (Fig. 7 und 8) weist einen Schaft 126 auf, in den eine mit einem Innengewinde versehene Öffnung 128 eingelassen ist, um das mit einem Gewinde versehene Ende 120 des Handgriffs 84 aufzunehmen. Ein Klemmhälftenpaar 130 ist voneinander beabstandet und steht vom Schaft 126 ab. Die Klemmhälften 130 haben nach aussen konisch verlaufende Flächen 132. Die Flächen 132 verlaufen konisch vom Schaft 126 zu einer Nut 134.
- Die Klemmhälften 130 weisen Verlängerungen 136 auf, welche in den Ausnehmungen 60 und 62 der Seitenflächen 20 und 22 des Wirbelsäulenimplantats 10 aufgenommen werden. Die Verlängerungen 136 weisen ebene, innere Oberflächen 138 (Fig. 7) auf, um in die planen Bodenflächen 64 der Ausnehmungen 60 und 62 einzugreifen. Parallele Seitenflächen 140 und 142 (Fig. 8) der Verlängerungen 136 greifen in Seitenflächen 66 und 68 der Ausnehmungen 60 und 62 ein, wenn das Instrument 80 gebraucht wird, um das Wirbelsäulenimplantat 10 festzuhalten. Eine gebogene Seitenfläche 144, zum Erfassen der Seitenfläche 70, erstreckt sich zwischen den Seitenflächen 140 und 142.
- Der Schaft 118 des Handgriffs 84 erstreckt sich in die Öffnung 92 im mittleren Abschnitt 82 und ist mit der Welle 126 des Klemmteus 86 (Fig. 4) verschraubt. Da der Handgriff 84 in die Öffnung 128 des Klemmteus 86 geschraubt ist, wird der Klemmteil in die Öffnung 92 im mittleren Abschnitt 82 gezogen. Die konischen Flächen 132 der Klemmhälften 130 greifen in den konischen Bereich 102 der Öffnung 92 ein. Da der Klemmteil 86 in die Öffnung 92 gezogen wird, werden die Klemmhälften 130 durch den konischen Bereich 102 der Öffnung 92 gegeneinander gedrückt, um das Wirbelsäulenimplantat 10 zwischen den Verlängerungen 136 einzuklemmen.
- Das Verfahren zum Einsetzen des Wirbelsäulenimplantats 10 zwischen die benachbarten Rückenwirbel 12 und 14, um die benachbarten Rückenwirbel miteinander zu vereinigen, wird nun beschrieben. Der grösste Teil der zwischen den Rückenwirbeln 12 und 14 vorhandenen Bandscheibe wird entfernt. Die zueinander gerichteten Flächen der Rückenwirbel 12 und 14 werden mit einem Bandscheibenschaber und einer Hohlmeisselzange gereinigt. Vorzugsweise wird ein Kreisring der Bandscheibe zwischen den Rückenwirbeln 12 und 14 belassen.
- Das Instrument 80 wird benutzt, um ein Wirbelsäulenimplantat 10 festzuhalten. Das Wirbelsäulenimplantat 10 wird von hinten, von vorne oder seitlich von vorne zwischen die Rückenwirbel 12 und 14 eingesetzt, wie dies von einem Chirurgen gewünscht wird, wobei die parallelen Seitenflächen 20 und 22 gegen benachbarte Rückenwirbel 12 und 14 gerichtet sind. Das gezeigte Wirbelsäulenimplantat 10 wurde in Fig. 9 von hinten eingesetzt. Das Wirbelsäulenimplantat 10 wird so eingesetzt, dass der Endteil 32 nahe der vorderen Seite der Wirbelsäule 16 und der Endteil 30 nahe der hinteren Seite der Wirbelsäule ist. Das Wirbelsäulenimplantat 10 wird um 90º in die in Fig. 10 gezeigte Position verdreht, so dass die an der oberen und unteren Fläche 24 und 26 angeordneten Zähne 36 in die Rückenwirbel 12 und 14 eingreifen und sich die Seitenflächen 20 und 22 vom Rückenwirbel 12 zum Rückenwirbel 14 erstrecken. Die keilförmige Gestaltung des Wirbelsäulenimplantats 10 vermindert die Notwendigkeit, den hinteren Teil der Wirbelsäule um eine grosse Distanz auseinanderzuziehen und danach den hinteren Teil wieder zusammenzudrücken, um die benötigte Lordose zu erhalten. Nur der hintere Teil muss auf die Höhe der gewünschten Zwischenscheibe auseinandergezogen werden.
- Ein zweites Wirbelsäulenimplantat 10 wird Seite an Seite zum ersten Wirbelsäulenimplantat 10 zwischen die Rückenwirbel 12 und 14 eingesetzt. Das zweite Wirbelsäulenimplantat 10 wird in ähnlicher Weise wie das erste Implantat eingesetzt. Das Instrument 80 wird benutzt, um das Wirbelsäulenimplantat 10 festzuhalten. Das Wirbelsäulenimplantat 10 wird eingesetzt, indem die parallelen Seitenflächen 20 und 22 gegen die Rückenwirbel 12 und 14 (Fig. 11) gerichtet sind. Danach wird das zweite Wirbelsäulenimplantat 10 um 90º in die in Fig. 12 gezeigte Position verdreht, so dass die Zähne 36 der oberen und unteren Flächen 24 und 26 in die Rückenwirbel 12 und 14 eingreifen.
- Der zwischen dem Wirbelsäulenimplantat 10 und den angrenzenden Rückenwirbeln 12 und 14 verbleibende Raum wird mit Autotransplantat- oder Allotransplantat-Knochen, Knochenproteinen, Knochensubstituten oder dergleichen aufgefüllt. Eine Vorrichtung, um die Rückenwirbel 12 und 14 in einer gewünschten, räumlichen Beziehung zueinander zu halten, wie dies im U.S. Patent Nr. 4,696,290 offenbart ist, wird an der Wirbelsäule 16 angebracht, bis die Rückenwirbel 12 und 14 komplett miteinander vereint sind. Die Vorrichtung, um die Rückenwirbel 12 und 14 in der gewünschten, räumlichen Beziehung zueinander zu halten, verhindert, dass sich das Wirbelsäulenimplantat 10 aus der Position verschiebt und das Knochen-Transplantat aus den Zwischenräumen zwischen den Wirbelsäulenimplantaten und den Rückenwirbeln 12 und 14 fällt.
- Bei dem in den Figuren 1-12 gezeigten Ausführungsbeispiel der Erfindung besitzt das Wirbelsäulenimplantat 10 relativ kleine Öffnungen 56 und 58. Bei dem in den Figuren 13 und 14 gezeigten Ausführungsbeispiel der Erfindung weist das Wirbelsäulenimplantat eine relativ grosse, sich zwischen den Seitenflächen erstreckende Öffnung auf. Da das in den Figuren 13 und 14 gezeigte Ausführungsbeispiel der Erfindung sehr ähnlich zu dem in den Figuren 1-12 gezeigten Ausführungsbeispiel der Erfindung ist, werden ähnliche Bezugszeichen verwendet, um ähnliche Komponenten zu bezeichnen: Der Suffix "a" wird den Bezugszeichen der Fig. 13 und 14 beigefügt, um Verwechslungen zu vermeiden.
- Eines oder mehrere im wesentlichen steife Wirbelsäulenimplantate 10a (von denen eines in der Fig. 13 gezeigt ist) werden Seite an Seite zwischen benachbarte Rückenwirbel eingefügt, um die benachbarten Rückenwirbel fest miteinander zu vereinigen. Die Wirbelsäulenimplantate 10a werden vorzugsweise im Spritzgiessverfahren aus einem mit geschnittenen Kohlenfasern verstärktem Polymer hergestellt. Trotzdem können die Wirbelsäulenimplantate 10a auch aus anderen passenden, implantierbaren Materialien wie rostfreier Stahl oder Titan gefertigt werden. Auch sollte die Zugfestigkeit des Materials, das zur Herstellung der Wirbelsäulenimplantate 10a verwendet wird, bevorzugterweise zumindest 6,9 N/mm² (10000 psi) betragen, so dass die Wirbelsäulenimplantate eine relative Bewegung zwischen den benachbarten Rückenwirbeln 12 und 14 verhindern und die Belastbarkeit der Wirbelsäule auf Druck unterstützen.
- Jedes der Wirbelsäulenimplantate 10a (Fig. 13) weist parallele Seitenflächen 20a und 22a auf. Um in die benachbarten Rückenwirbel einzugreifen, erstreckt sich zwischen den Seitenflächen 20a und 22a eine obere Fläche 24a und eine untere Fläche 26a. Die oberen und unteren, an einen ersten Endteil 30a des Wirbelsäulenimplantats 10a angrenzenden Flächen 24a und 26a sind um eine erste Distanz voneinander beabstandet. Die oberen und unteren, an einen zweiten Endteil 32a des Wirbelsäulenimplantats 10a angrenzenden Flächen 24a und 26a sind um eine zweite Distanz voneinander beabstandet. Die zweite Distanz ist vorzugsweise grösser als die erste Distanz, um dem Wirbelsäulenimplantat eine keilförmige Form zu verleihen, damit es in Teilen der Wirbelsäule mit einer lordotischen Krümmung eingesetzt werden kann. Das Wirbelsäulenimplantat 10a kann in Abhängigkeit von demjenigen Teil der Wirbelsäule, in welchen das Implantat eingesetzt werden muss, eine unterschiedliche Keilform oder gar keine Keilform aufweisen.
- Die oberen und unteren Flächen 24a und 26a weisen eine Mehrzahl von dreieckig geformten Zähnen 36a auf, welche sich von der Seitenfläche 20a zu der Seitenfläche 22a erstrecken, um in die Rückenwirbel einzugreifen. Jeder Zahn 36a (Fig.14) weist eine zum Endteil 30a gerichtete Fläche 40a auf. Eine Fläche 42a jedes Zahns 36a ist zum Endteil 32a des Wirbelsäulenimplantats 10a gerichtet. Die Flächen 40a und 42a des Zahns 36a schneiden einander, um eine Kante 44a zu bilden. Die Flächen 40a und 42a von benachbarten Zähnen 36a schneiden sich, um Kanten 46a zu bilden. Die Kanten 46a sind parallel zueinander und liegen in einer Ebene 48a.
- Die Fläche 40a von jedem Zahn 36a erstreckt sich unter einem spitzen Winkel C zu der Ebene 48a. Die Fläche 42a jedes Zahns 36a erstreckt sich unter einem spitzen Winkel D zu der Ebene 48a. Vorzugsweise sind die Winkel C und D gleich gross und weisen einen Wert von 45º auf, so dass die Flächen 40a und 42a senkrecht zueinander verlaufen. Deshalb haben die Zähne 36a keine bevorzugte Wirkungsrichtung. Dennoch können die Werte der Winkel C und D auch unterschiedlich sein.
- Eine relativ grosse Öffnung 156 erstreckt sich von der Seitenfläche 20a zu der Seitenfläche 22a. Die Öffnung 156 wird mit Autotransplantat- oder Allotransplantat-Knochen, Knochenproteinen, Knochensubstituten oder dergleichen gefüllt und ermöglicht das Fliessen von Körperflüssigkeiten und ein Knochenwachstum von einer Seite des Implantats 10a auf die andere Seite des Implantats. Die Öffnung 156 erstreckt sich senkrecht zu den Seitenflächen 20a und 22a und kann unter einem Winkel zu den Seitenflächen verlaufen. Die Grösse, Gestaltung und Position der Öffnung 156 kann nach den Wünschen eines Chirurgen verändert werden.
- Die Seitenfläche 20a weist eine Ausnehmung 60a und die Seitenfläche 22a eine Ausnehmung 62a auf. Jede der Ausnehmungen 60a und 62a weist eine plane Bodenfläche 64a, parallele Seitenflächen 66a und 68a, die senkrecht zu der Bodenfläche 64a verlaufen, und eine bogenförmige Seitenfläche 70a, welche sich zwischen den parallelen Seitenflächen 66a und 68a erstreckt, auf. Die Ausnehmungen 60a und 62a in den Seitenflächen 20a und 22a dienen dem Ansetzen des Instruments 80 (Fig. 4), welches das Wirbelsäulenimplantat 10a festhält, um das Einsetzen des Wirbelsäulenimplantats zwischen die benachbarten Rückenwirbel 12 und 14 und das Drehen des Wirbelsäulenimplantats, nachdem es zwischen den benachbarten Rückenwirbeln eingesetzt ist, zu erleichtem.
- Jedes Instrument, das das Implantat 10a sicher festhält und ein Drehen des Implantats in die Position erlaubt, kann verwendet werden. Das Instrument 80 ist ein solches Instrument, das verwendet werden kann.
- Das Verfahren zum Einsetzen des Wirbelsäulenimplantats 10a zwischen benachbarte Rückenwirbel, um die benachbarten Rückenwirbel miteinander zu vereinigen, ist ähnlich dem Verfahren zum Einsetzen des Wirbelsäulenimplantats 10 zwischen benachbarte Rückenwirbel wie in den Fig. 4-12 beschrieben und wird daher nicht im Detail erklärt. Der grösste Teil der zwischen den Rückenwirbeln vorhandenen Bandscheibe wird entfernt. Die zueinander gerichteten Flächen der Rückenwirbel werden mit einem Bandscheibenschaber und einer Hohlmeisselzange gereinigt. Vorzugsweise wird ein Kreisring der Bandscheibe zwischen den Rückenwirbeln belassen.
- Die Öffnung 156 wird mit einem Knochen-Transplantat gefüllt. Das Instrument 80 wird benutzt, um ein Wirbelsäulenimplantat 10a, wie in den Ausführungsbeispielen der Fig. 1-12 umschrieben, festzuhalten. Das Wirbelsäulenimplantat 10a wird von hinten, von vorne oder seitlich von vorne zwischen den Rückenwirbeln eingesetzt, wobei die parallelen Seitenflächen 20a und 22a zu den benachbarten Rückenwirbeln gerichtet sind. Das Wirbelsäulenimplantat 10a wird so eingesetzt, dass der Endteil 32a nahe der vorderen Seite der Wirbelsäule und der Endteil 30a nahe der hinteren Seite der Wirbelsäule ist. Das Wirbelsäulenimplantat 10a wird um 90º verdreht, so dass die an der oberen und unteren Fläche 24a und 26a angeordneten Zähne 36a in die Rückenwirbel eingreifen und sich die Seitenflächen 20a und 22a zwischen den benachbarten Rückenwirbeln erstrecken.
- Ein zweites Wirbelsäulenimplantat 10a wird Seite an Seite mit dem ersten Wirbelsäulenimplantat wischen die Rückenwirbel eingesetzt. Das zweite Wirbelsäulenimplantat 10a wird in ähnlicher Weise wie das erste Implantat eingesetzt. Die Öffnung 156 wird mit einem Knochen-Transplantat gefüllt. Das Instrument 80 wird benutzt, um das Wirbelsäulenimplantat 10a festzuhalten. Das Wirbelsäulenimplantat 10a wird eingesetzt, indem die parallelen Seitenflächen 20a und 22a zu den Rückenwirbeln gerichtet sind. Danach wird das zweite Wirbelsäulenimplantat 10 um 90º verdreht, so dass die Zähne 36a der oberen und unteren Flächen 24a und 26a in die Rückenwirbel eingreifen.
- Der zwischen dem Wirbelsäulenimplantat 10a und den benachbarten Rückenwirbeln verbleibende Raum wird mit Autotransplantat- oder Allotransplantat-Knochen, Knochenproteinen, Knochensubstituten oder dergleichen aufgefüllt. Eine passende Vorrichtung, um die Rückenwirbel in einer gewünschten, räumlichen Beziehung zueinander zu halten, wird an der Wirbelsäule angebracht, bis die Rückenwirbel miteinander vereint sind.
- In den in den Fig. 1-14 gezeigten Ausführungsbeispielen der Erfindung weisen die Wirbelsäulenimplantate Öffnungen auf, die sich nur zwischen den Seitenflächen erstrecken. In dem in den Fig. 15 und 16 gezeigten Ausführungsbeispiel der Erfindung weist das Wirbelsäulenimplantat eine sich zwischen Seitenflächen erstreckende Öffnung und eine sich zwischen der oberen und unteren, in die Rückenwirbel eingreifenden Fläche erstreckende Öffnung auf. Da das in den Fig. 15 und 16 gezeigte Ausführungsbeispiel der Erfindung im grossen und ganzen ähnlich zu den in den Fig. 1-14 gezeigten Ausführungsbeispielen ist, werden ähnliche Bezugszeichen verwendet, um ähnliche Komponenten zu bezeichnen: Der Suffix "b" wird den Bezugszeichen der Fig. 15 und 16 beigefügt, um Verwechslungen zu vermeiden.
- Eines oder mehrere im wesentlichen steife Wirbelsäulenimplantate 10b (von denen eines in der Fig. 13 gezeigt ist), werden Seite an Seite zwischen benachbarte Rückenwirbel eingefügt, um die benachbarten Rückenwirbel fest miteinander zu vereinigen. Die Wirbelsäulenimplantate 10b werden vorzugsweise im Spritzgiessverfahren aus einem mit geschnittenen Kohlenfasern verstärktem Polymer hergestellt. Trotzdem können die Wirbelsäulenimplantate 10b auch aus anderen passenden, implantierbaren Materialien wie rostfreier Stahl oder Titan gefertigt werden. Auch sollte die Zugfestigkeit des Materials, das zur Herstellung der Wirbelsäulenimplantate 10a verwendet wird, bevorzugterweise zumindest 6,9 N/mm² (10000 psi) betragen, so dass die Wirbelsäulenimplantate eine relative Bewegung zwischen den benachbarten Rückenwirbeln verhindern und die Belastbarkeit der Wirbelsäule auf Druck unterstützen.
- Jedes der Wirbelsäulenimplantate 10b (Fig. 15) weist parallele Seitenflächen 20b und 22b auf. Um in die benachbarten Rückenwirbel einzugreifen, erstreckt sich zwischen den Seitenflächen 20b und 22b eine obere Fläche 24b und eine untere Fläche 26b. Die oberen und unteren, an einen ersten Endteil 30b des Wirbelsäulenimplantats 10b angrenzenden Flächen 24b und 26b sind um eine erste Distanz voneinander beabstandet. Die oberen und unteren, an einen zweiten Endteil 32b des Wirbelsäulenimplantats 10b angrenzenden Flächen 24b und 26b, sind um eine zweite Distanz voneinander beabstandet. Die zweite Distanz ist vorzugsweise grösser als die erste Distanz, um dem Wirbelsäulenimplantat eine keilförmige Form zu verleihen, damit es in Teilen der Wirbelsäule mit einer lordotischen Krümmung eingesetzt werden kann. Das Wirbelsäulenimplantat 10b kann in Abhängigkeit des Teils der Wirbelsäule, in welchen das Implantat eingesetzt werden muss, eine unterschiedliche Keilforn oder gar keine Keilform aufweisen.
- Die oberen und unteren Flächen 24b und 26b weisen eine Mehrzahl von dreieckig geformten Zähnen 36b auf, welche sich von der Seitenfläche 20b zu der Seitenfläche 22b erstrecken, um in die Rückenwirbel einzugreifen, Jeder Zahn 36b (Fig.16) weist eine zum Endteil 30b gerichtete Fläche 40b auf. Eine Fläche 42b jedes Zahns 36a ist zum Endteil 32b des Wirbelsäulenimplantats 10b gerichtet. Die Flächen 40b und 42b des Zahns 36a schneiden einander, um eine Kante 44b zu bilden. Die Flächen 40b und 42b von benachbarten Zähnen 36b schneiden sich, um Kanten 46b zu bilden. Die Kanten 42b sind parallel zueinander und liegen in einer Ebene 48b.
- Die Fläche 40b von jedem Zahn 36b erstreckt sich unter einem spitzen Winkel F zu der Fläche 48b. Die Fläche 42b jedes Zahns 36b erstreckt sich unter einem spitzen Winkel G zu der Ebene 48b. Vorzugsweise sind die Winkel F und G gleich gross und weisen einen Wert von 45º auf, so dass die Flächen 40b und 42b senkrecht zueinander verlaufen. Deshalb haben die Zähne 36b keine bevorzugte Wirkungsrichtung. Dennoch können die Werte der Winkel F und G auch unterschiedlich sein.
- Eine relativ grosse, kreisförmige Öffnung 156b erstreckt sich von der Seitenfläche 20b zu der Seitenfläche 22b. Eine relativ grosse, rechteckige Öffnung 160 erstreckt sich von der oberen Fläche 24b zu der unteren Fläche 26b und kreuzt die Öffnung 156b. Die Öffnungen 156b und 160 werden mit Autotransplantatoder Allotransplantat-Knochen, Knochenproteinen, Knochensubstituten oder dergleichen gefüllt und ermöglichen das Fliessen von Körperflüssigkeiten und ein Knochenwachstum durch das Implantat 10b hindurch. Die Grössen, Gestaltungen und Positionen der Öffnungen 156b und 160 können den Wünschen des Chirurgen entsprechend verändert werden.
- Die Seitenfläche 20b weist eine Ausnehmung 60b und die Seitenfläche 22b eine Ausnehmung 62b auf. Jede der Ausnehmungen 60b und 62b weist eine plane Bodenfläche 64b, parallele Seitenflächen 66b und 68b und eine bogenförmige Seitenfläche 70b, welche sich zwischen den parallelen Seitenflächen 66b und 68b erstreckt, auf. Die Ausnehmungen 60b und 62b in den Seitenflächen 20b und 22a dienen dem Ansetzen des Instruments 80 (Fig. 4), welches das Wirbelsäulenimplantat 10b festhält, um das Einsetzen des Wirbelsäulenimplantats zwischen benachbarte Rückenwirbel und das Drehen des Wirbelsäulenimplantats, nachdem es zwischen den benachbarten Rückenwirbeln eingesetzt ist, zu erleichtern.
- Jedes Instrument, das das Implantat 10b sicher festhält und ein Drehen des Implantats in die Position erlaubt, kann verwendet werden. Das Instrument 80 ist ein solches Instrument, das verwendet werden kann.
- Das Verfahren zum Einsetzen des Wirbelsäulenimplantats 10b zwischen benachbarte Rückenwirbel, um die benachbarten Rückenwirbel miteinander zu vereinigen, ist ähnlich dem Verfahren zum Einsetzen des Wirbelsäulenimplantats 10 zwischen benachbarte Rückenwirbel wie in den Fig. 4-12 beschrieben und wird daher nicht im Detail erläutert. Der grösste Teil der zwischen den Rückenwirbeln vorhandenen Bandscheibe wird entfernt. Die zueinander gerichteten Flächen der Rückenwirbel werden mit einem Bandscheibenschaber und einer Hohlmeisselzange gereinigt. Vorzugsweise wird ein Kreisring der Bandscheibe zwischen den Rückenwirbeln belassen.
- Die Öffnungen 156b und 160 werden mit einem Knochen-Transplantat gefüllt. Das Instrument 80 wird benutzt um ein Wirbelsäulenimplantat 10b festzuhalten, wie in den Ausführungsbeispielen der Fig. 1-12 umschrieben. Das Wirbelsäulenimplantat 10b wird von hinten, von vorne oder seitlich von vorne zwischen die Rükkenwirbel eingesetzt, wobei die parallelen Seitenflächen 20b und 22b zu den benachbarten Rückenwirbeln gerichtet sind. Das Wirbelsäulenimplantat 10b wird so eingesetzt, dass der Endteil 32b nahe der vorderen Seite der Wirbelsäule und der Endteil 30b nahe der hinteren Seite der Wirbelsäule ist. Das Wirbelsäulenimplantat 10b wird um 90º verdreht, so dass die an der oberen und unteren Fläche 24ba und 26b angeordneten Zähne 36b in die Rückenwirbel eingreifen und sich die Seitenflächen 20b und 22b zwischen den benachbarten Rückenwirbeln erstrecken.
- Ein zweites Wirbelsäulenimplantat 10b wird Seite an Seite mit dem ersten Wirbelsäulenimplantat wischen die Rückenwirbel eingesetzt. Das zweite Wirbelsäulenimplantat 10b wird in ähnlicher Weise wie das erste Implantat eingesetzt. Die Öffnungen 156b und 160 werden mit einem Knochen-Transplantat gefüllt. Das Instrument 80 wird benutzt, um das Wirbelsäulenimplantat 10b festzuhalten. Das Wirbelsäulenimplantat 10b wird eingesetzt, indem die parallelen Seitenflächen 20b und 22b zu den Rückenwirbeln gerichtet sind. Danach wird das zweite Wirbelsäulenimplantat 10b um 90º verdreht, so dass die Zähne 36b der oberen und unteren Flächen 24b und 26b in die Rückenwirbel eingreifen.
- Der zwischen dem Wirbelsäulenimplantat 10b und den benachbarten Rückenwirbeln verbleibende Raum wird mit Autotransplantat- oder Allotransplantat-Knochen, Knochenproteinen, Knochensubstituten oder dergleichen aufgefüllt. Eine passende Vorrichtung, um die Rückenwirbel in einer gewünschten, räumlichen Beziehung zueinander zu halten, wird an der Wirbelsäule angebracht, bis die Rückenwirbel fest miteinander verbunden sind.
- Aufgrund der vorgängigen Beschreibung der Erfindung können mit der Materie vertraute Fachleute Verbesserungen, Anderungen und Modifikationen vornehmen, welche unter den Schutzumfang der nachfolgenden Ansprüche fallen.
Claims (13)
1. Wirbelsäulenimplantat (10, 10a, 10b), um benachbarte
Rückenwirbel (12, 14) einer Wirbelsäule (16) miteinander zu
vereinigen, umfassend:
- erste und zweite im wesentlichen parallel zueinander
verlaufende Seitenflächen (20, 20a, 20b; 22, 22a, 22b);
- obere und untere, zwischen den genannten ersten und zweiten
Seitenflächen (20, 20a, 20b; 22, 22a, 22b) verlaufende
Oberflächenmittel (24, 24a, 24b; 26, 26a, 26b), um in die
benachbarten Rückenwirbel (12, 14) einzugreifen;
- erste und zweite Endteile (30, 30a, 30b; 32, 32a, 32b); und
- Mittel (60, 60a, 60b; 62, 62a, 62b) zum Ansetzen eines
Werkzeugs, um das genannte Wirbelsäulenimplantat (10, 10a, 10b)
zu drehen,
dadurch gekennzeichnet, dass
- die genannten Mittel (60, 60a, 60b; 62, 62a, 62b) zum
Ansetzen eines Werkzeugs, um das genannte Wirbelsäulenimplantat
(10, 10a, 10b) zu drehen, Ausnehmungen (60, 60a, 60b, 62,
62a, 62b) aufweisen, die an den genannten, ersten Endteil
(30, 30a, 30b) des genannten Wirbelsäulenimplantats (10, 10a,
10b) angrenzen und in die genannten ersten und zweiten
Seitenflächen (20, 20a, 20b; 22, 22a, 22b) zum Ansetzen des
Werkzeugs (80) zum Ergreifen und Drehen des genannte
Wirbelsäulenimplantats (10, 10a, 10b) eingelassen sind,
- wobei das genannte Wirbelsäulenimplantat (10, 10a, 10b) um
eine sich durch den genannten ersten und zweiten Endteil (30,
30a, 30b; 32, 32a, 32b) erstreckende Achse drehbar ist, aus
einer Position zwischen den benachbarten Rückenwirbeln (12,
14), in welcher die genannten ersten und zweiten
Seitenflächen (20, 20a, 20b; 22, 22a, 22b) gegen die benachbarten
Rückenwirbel (12, 14) gerichtet sind, in eine Position, in
welcher die genannten oberen und unteren Oberflächenmittel
(24, 24a, 24b; 26, 26a, 26b) in die benachbarten Rückenwirbel
(12, 14) eingreifen und die genannten ersten und zweiten
Seitenflächen (20, 20a, 20b; 22, 22a, 22b) sich von einem der
benachbarten Rückenwirbel (12) zum anderen der benachbarten
Rückenwirbel (14) erstrecken.
2. Wirbelsäulenimplantat (10, 10a, 10b) gemäss Anspruch 1, in
welchem die genannten oberen und unteren Oberflächenmittel < 24,
24a, 24b; 26, 26a, 26b) sich von dem genannten ersten Endteil
(30, 30a, 30b) zu dem genannten zweiten Endteil (32, 32a, 32b)
erstrecken, wobei die genannten oberen und unteren, an den
genannten ersten Endteil (30, 30a, 30b) angrenzenden
Oberflächenmittel (24, 24a, 24b; 26, 26a, 26b) um eine erste Distanz
beabstandet sind und die genannten oberen und unteren, an den
genannten zweiten Endteil (30, 30a, 30b) angrenzenden
Oberflächenmittel (24, 24a, 24b; 26, 26a, 26b) um eine zweite Distanz
beabstandet sind, welche grösser ist als die erste Distanz.
3. Wirbelsäulenimplantat (10, 10a, 10b) gemäss Anspruch 1,
welches ferner zumindest eine sich durch das genannte
Wirbelsäulenimplantat (10, 10a, 10b) erstreckende Öffnung (56, 58, 156,
156b, 160) aufweist.
4. Wirbelsäulenimplantat (10b) gemäss Anspruch 3, in welchem
sich die genannte Öffnung (156) von dem genannten oberen
Oberflächenmittel (22b) zu dem genannten unteren Oberflächenmittel
(24b) erstreckt.
5. Wirbelsäulenimplantat (10, 10a, 10b) gemäss Anspruch 3, in
welchem sich die genannte Öffnung (56, 58, 156, 156b) von der
genannten ersten Seitenfläche (20, 20a, 20b) zu der genannten
zweiten Seitenfläche (22, 22a, 22b) erstreckt.
6. Wirbelsäulenimplantat (10) gemäss Anspruch 5, welches ferner
eine Mehrzahl von Öffnungen (56, 58) aufweist, welche sich von
der genannten ersten Seitenfläche (20, 20a, 20b) zu der
genannten zweiten Seitenfläche (22, 22a, 22b) erstrecken.
7. Wirbelsäulenimplantat (10) gemäss Anspruch 6, in welchem
sich die genannten Öffnungen (56, 58) im wesentlichen senkrecht
zu den genannten ersten und zweiten Seitenflächen (20, 22)
erstrecken.
8. Wirbelsäulenimplantat (10b) gemäss Anspruch 5, welches
ferner eine sich von dem genannten oberen Oberflächenmittel (22b)
zu dem genannten unteren Oberflächenmittel (24b) erstreckende
Öffnung (160) aufweist.
9. Wirbelsäulenimplantat (10b) gemäss Anspruch 8, bei welchem
die sich von dem genannten oberen Oberflächenmittel (22b) zu

die benachbarten Rückenwirbel (12, 14) einzugreifen.
11. Wirbelsäulenimplantat (10, 10a, 10b) gemäss Anspruch 10, in
welchem die genannten Zähne (36, 36a, 36b) an den genannten
oberen und unteren Oberflächenmitteln (24, 24a, 24b; 26, 26a,
26b) dreieckig ausgebildet sind.
12. Wirbelsäulenimplantat (10, 10a, 10b) gemäss Anspruch 11,
bei welchem jeder der genannten Zähne (36, 36a, 36b) eine gegen
den genannten ersten Endteil (30, 30a, 30b) gerichtete, erste
Oberfläche (40, 40a, 40b) und eine zweite, sich von der
genannten ersten Oberfläche (40, 40a, 40b) des genannten Zahns < 36,
36a, 36b) erstreckende Oberfläche (42, 42a, 42b) aufweist,
welche gegen den genannten zweiten Endteil (30, 30a, 30b)
gerichtet ist, wobei die genannten ersten und zweiten Oberflächen
(40, 40a, 40b; 42, 42a, 42b) des genannten Zahns (36, 36a, 36b)
einander schneiden, um eine Kante (44, 44a, 44b) zu bilden,
wobei sich die genannte erste Oberfläche (40, 40a, 40b) unter
einem spitzen Winkel (X, C, F) zu einer Ebene (48, 48a, 48b)
erstreckt,
die eine Kante (46, 46a, 46b) der genannten ersten
Fläche (40, 40a, 4db) umfasst, welche gegenüber der genannten,
durch die Schnittlinie der genannten ersten und zweiten Flächen
(40, 40a, 40b; 42, 42a, 42b) gebildeten Kante (46, 46a, 46b)
liegt, und eine Kante (46, 46a, 46b) der genannten zweiten
Seitenfläche (42, 42a, 42b) umfasst, die gegenüber der genannten,
durch die Schnittlinie der genannten ersten und zweiten Flächen
(40, 40a, 40b; 42, 42a, 42b) gebildeten Kante (46, 46a, 46b)
liegt, und wobei die genannte zweite Fläche (42, 42a, 42b)
gegenüber der Ebene (48, 48a, 48b) unter einem spitzen Winkel
(Y,D,G) verläuft, der gleich ist wie der spitze Winkel (X, C,
F), unter dem sich die erste Fläche (40, 40a, 40b) zu der Ebene

13. Wirbelsäulenimplantat (10, 10a, 10b) gemäss Anspruch 12, in
welchem die genannte erste Fläche (40, 40a, 40b) senkrecht zu
der genannten zweiten Fläche (42, 42a, 42b) verläuft.
