Description
- Die vorliegende Erfindung betrifft ein Implantat für die Verbindung von Knochen und insbesondere ein Wirbelsäulenimplantat für die Verschmelzung von Wirbelknochen, das zwischen zwei zu verschmelzenden Wirbelknochen eingeführt wird.
- Durch die Degenerierung der Bandscheibe, insbesondere des Bandscheibenkerns (Nucleus pulposus) kommt es oft zu einem Höhenverlust in dem betroffenen Bandscheibenraum, der mit einer Lockerung des Bandscheibenfaserrings (annulus fibrosus) und der Ligamente einhergeht. Dadurch wird die Wirbelsäule an dieser Stelle instabil. Das Ergebnis ist eine horizontale Verschiebbarkeit der Wirbelkörper relativ zueinander (Spondylolisthesis), was zur Schädigung der Nervenwurzeln in diesem Bereich und/oder des Rückenmarks zusammen mit daraus resultierenden Schmerzen führt.
- Das Prinzip zur Behandlung dieser Symptome besteht in der operativen Exkavation des Bandscheibenkerns und dem Einlegen bzw. Einsetzen eines, in dem Bereich der Halswirbelsäule – oder zweier – in dem Bereich der Lendenwirbelsäule – ausreichend stabilen/r Körper/s, um die normale Höhe des Bandscheibenraumes wieder herzustellen. Gleichzeitig muss die horizontale Verschiebbarkeit verhindert werden. Dies erfolgt entweder durch das Implantat selbst oder durch zusätzliche Metallimplantate. Diese Implantate sind besonders in der Lendenwirbelsäule beträchtlichen Kräften unterworfen, die zu dem Bruch des Metallimplantats führen können. Daher wird versucht, den Zwischenwirbel-Einsatz so schnell und so fest wie möglich mit den benachbarten Wirbelkörpern zusammenwachsen zu lassen bzw. zu verschmelzen.
- Eine bekannte Möglichkeit, zwei Wirbel zu verschmelzen, besteht in dem Einsetzen eines geeignet geformten Zylinders oder Dübels in einen vorbereiteten Hohlraum, der die beiden Wirbel, die verschmolzen werden sollen, erreicht. Das dafür erforderliche Material wird vorher von dem Patienten entnommen, zum Beispiel von dem Beckenkamm. Aus dem so gewonnenen autogenen Knochenmaterial, das heißt, Knochenmaterial, das von dem selben Patienten stammt, wird ein Implantat hergestellt und dann in den Bandscheibenraum des Patienten zwischen den beiden zu verschmelzenden Wirbeln eingesetzt (Autotransplantat).
- Das Ziel der vorliegenden Erfindung besteht daher in der Erzeugung eines Implantats für die Verschmelzung von Knochen, das den korrekten Abstand zwischen den Wirbelkörpern und die Stabilität der Wirbelsäule wieder herstellt, das keine Immunreaktionen auslöst und das den Heilungsprozess beschleunigt.
- Dieses Ziel wird durch ein Wirbelsäulenimplantat mit den Merkmalen von Anspruch 1 erreicht.
- Ein besonderer Vorteil des Wirbelsäulenimplantats gemäß der Erfindung ist durch das verwendete Material gegeben, das, als Ergebnis seines biologischen Ursprungs, keinen Fremdkörper darstellt. Dadurch trägt das Implantat, das aus Knochenmaterial hergestellt ist, in seiner Gesamtheit, das heißt, das Unterstützungselement aus kortikalem Knochenmaterial wie auch der Grundkörper aus spongiösem Knochenmaterial, zu der Verschmelzung der Wirbelkörper bei, indem es sich während des Heilens in körpereigenes Gewebe umwandelt.
- Da der Grundkörper keine äußere Schale aus hartem Knochenmaterial aufweist sondern vielmehr aus spongiösem Knochen besteht, kommt das spongiöse Knochenmaterial des Grundkörpers in direkten Kontakt mit den Knochenoberflächen der Wirbelkörper, um eine knöcherne Verbindung mit letzterer zu bilden und diese zu verschmelzen bzw. miteinander zu verbinden.
- Die Erfinder haben erkannt, dass es besonders vorteilhaft ist, den Grundkörper aus spongiösem, das heißt, relativ porösem Knochenmaterial zu bilden, um den Heilungsprozess zu beschleunigen. Um jedoch in der Lage zu sein, die nicht unerheblichen Kräfte aufzunehmen, die zwischen den zwei Wirbelkörpern entstehen können, die miteinander verschmolzen werden sollen, ist gemäß der Erfindung zumindest ein Last tragendes Unterstützungselement in den Grundkörper aus spongiösem Knochenmaterial eingebettet, das sich von einer Seite zu einer gegenüberliegenden Seite des Grundkörpers erstreckt. Das Unterstützungselement dient somit als Last tragender Teil, wohingegen der Grundkörper im Wesentlichen die Funktion hat, Platz für das Untersützungselement zu bieten und den Prozess des Zusammenwachsens mit dem umgebenden Knochenmaterial zu ermöglichen.
- Das zumindest eine Unterstützungselement besteht aus kortikalem Knochen. Dadurch ist einerseits sichergestellt, dass von dem Wirbelsäulenimplantat hohe Kräfte aufgenommen werden können. Andererseits weist das Implantat keine Artefakte auf, was den Heilungsprozess weiter beschleunigt.
- In der Beschreibung, den Zeichnungen und den Nebenansprüchen sind weitere vorteilhafte Ausführungsformen des Wirbelsäulenimplantats gemäß der vorliegenden Erfindung dargelegt.
- Das Unterstützungselement kann vorzugsweise auf die Art eines Stiftes bzw. einer Säule gebildet sein, wobei es auch möglich ist, dass eine Vielzahl von Unterstützungselementen parallel in den Grundkörper eingebettet ist. Abhängig von der Spannungsverteilung können die Unterstützungselemente in diesem Fall auch unterschiedliche Querschnitte aufweisen.
- Gemäß einer weiteren Ausführungsform der Erfindung ist das Unterstützungselementflächig oder wandartig gebildet, wodurch noch höhere Spannungen an dem Implantat möglich sind. Dabei ist es vorteilhaft, wenn eine Vielzahl von flächigen Unterstützungselementen, deren Ebenen sich schneiden, in den Grundkörper eingebettet ist.
- Das Unterstützungselement oder die Unterstützungselemente kann/können an der äußeren Kontur des Grundkörpers enden oder über die äußere Kontur des Grundkörpers hinaus vorstehen, um das Einsetzen des Implantats zwischen zwei zu verschmelzenden Wirbelkörpern zu erleichtern.
- Gemäß einer weiteren Ausführungsform der Erfindung kann der Grundkörper zumindest einen Hohlraum für die Aufnahme von Knochenpulver oder Knochengranulat umfassen, der von seiner Außenseite aus zugänglich ist. Das Einführen von Materialien dieser Art in einen Hohlraum dieser Art verbessert den Heilungsprozess noch weiter.
- Um einen Einbau des Grundkörpers zwischen zwei zu verschmelzenden Wirbeln zu erleichtern, kann der Grundköper des Weiteren eine Oberfläche aufweisen, die gewellt, gezahnt oder mit Höckern versehen ist. Des Weiteren kann die äußere Form des Grundkörpers auch variieren. Letzterer kann blockförmig, ein rechteckiges Parallelepiped oder keilförmig sein, womit es möglich ist, dass Konturen mit abgerundeten Ecken oder gekrümmte Grundkörper von Vorteil sind.
- Das Implantat wird vorzugsweise in seiner Größe an den Raum zwischen den Wirbeln angepasst, der nach der Exkavation des Bandscheibenkörpers zwischen den benachbarten Wirbeln vorhanden ist. Die Anwendung des Wirbelsäulenimplantats gemäß der Erfindung erfolgt nach der Exkavation der Bandscheibe mit einem nachfolgenden Freilegen der darüber und darunter liegenden Wirbelkörper, ohne dass es notwendig ist, hierzu gesundes Knochenmaterial zu beschädigen. Das bringt den Vorteil mit sich, dass der vorhandene Raum zwischen den Wirbeln nicht vergrößert wird und die unterstützenden Strukturen intakt bleiben.
- Gemäß der Erfindung wird ein geeignetes allogenes oder xenogenes Knochenmaterial derart aufgearbeitet, dass es konserviert ist, gelagert werden kann und zweckentsprechend verwendet werden kann. Die Konservierung des Knochenmaterials kann zum Beispiel mit Hilfe von Gefriertrocknung erfolgen. Das spongiöse Knochenmaterial wird vorzugsweise durch Lösungsmittel-Wasserentzug von natürlichem Knochenmaterial mittels eines mit Wasser mischbaren organischen Lösungsmittels wie z. B. Methanol, Ethanol, Propanol, Isopropanol, Azeton, Methylethylketon oder Gemischen dieser Lösungsmittel hergestellt. Die Konservierung und Sterilisierung des Knochenmaterials gemäß diesem Verfahren ist auch Gegenstand des Patents
DE 29 06 650 . - Dieses Verfahren dient zur Herstellung von Transplantatkonserven und ermöglicht eine/n Wasserentzug und Freilegung direkt in die Feinstruktur der Fibrillen des Knochenmaterials, so dass das aufgearbeitete Knochenmaterial histologisch gesehen eine morphologische Struktur aufweist, die sehr ähnlich zu der des natürlichen Knochens ist, und somit die gewünschten Eigenschaften des Knochenmaterials erhalten bleiben. Dieses Verfahren des Lösungsmittel-Wasserentzugs besitzt auch den Vorteil, dass im Vergleich mit der Gefriertrocknung (eine) wesentlich niedrigere apparative Kosten und geringere Komplexität erforderlich ist.
- Des Weiteren kann spongiöses Knochenmaterial auch durch eine Lösungsmittel-Entwässserung von Knochenmaterial mit der nachfolgenden abschließender Sterilisierung, insbesondere durch Bestrahlung mit Gammastrahlen, hergestellt werden. Alternativ kann das spongiöse Knochenmaterial durch aseptische Aufarbeitung von Knochenmaterial ohne abschließende Sterilisierung hergestellt werden.
- Nachfolgend wird die vorliegende Erfindung rein beispielhafter unter Bezugnahme auf beispielhafte Ausführungsformen des Wirbelsäulenimplantats gemäß der Erfindung und unter Bezugnahme auf die beiliegenden Zeichnungen beschrieben. In diesen zeigt:

1 eine schematische perspektivische Darstellung einer ersten Ausführungsform eines Wirbelsäulenimplantats gemäß der vorliegenden Erfindung; -
2 eine schematische perspektivische Darstellung einer weiteren Ausführungsform eines Wirbelsäulenimplantats gemäß der vorliegenden Erfindung; und -
3 eine schematische perspektivische Darstellung einer dritten Ausführungsform eines Wirbelsäulenimplantats gemäß der vorliegenden Erfindung. - In den Figs. bezeichnen gleiche Bezugszeichen in jedem Fall die gleichen Komponenten der veranschaulichten Ausführungsformen. Die veranschaulichten beispielhaften Ausführungsformen sind sowohl für die Halswirbel- als auch für die Lendenwirbelverschmelzung von Wirbelknochen geeignet.
- Die beispielhafte Ausführungsform eines Wirbelsäulenimplantats gemäß der Erfindung, die in
1 veranschaulicht ist, besteht aus einem Grundkörper10 aus spongiösem Knochenmaterial, der derart entworfen ist, dass er im Wesentlichen die Form eines rechteckigen Parallelepipeds aufweist. In der veranschaulichten beispielhaften Ausführungsform ist eine Vielzahl von, in der veranschaulichten beispielhaften Ausführungsform fünf, Unterstützungselemente/n12 , die aus kortikalem Knochen bestehen, in den Grundkörper10 eingebettet. Die Unterstützungselemente sind als zylindrische Stifte geformt und sind parallel zueinander beabstandet in den Grundkörper10 eingebettet. Hierzu ist der Grundkörper10 aus spongiösem Knochenmaterial mit entsprechenden parallelen Bohrungen versehen, in die die Unterstützungselemente12 eingesetzt werden. Die Unterstützungselemente12 sind im Wesentlichen so lang wie der Grundkörper10 hoch ist, so dass jedes Unterstützungselement12 sich von einer Seite des Grundkörpers zu dessen gegenüberliegender Seite erstreckt. - In der in
1 veranschaulichten beispielhaften Ausführungsform sind die Endseiten der Last tragenden Unterstützungselemente12 im Wesentlichen mit der äußeren Kontur des Grundkörpers10 ausgerichtet. -
2 zeigt eine weitere Ausführungsform eines Wirbelsäulenimplantats, das aus einem Grundkörper20 aus spongiösem Knochenmaterial besteht, das derart entworfen ist, dass es die Form eines rechteckigen Parallelepipeds aufweist. Ein Last tragendes Unterstützungselement22 , das aus vier Wandabschnitten22a ,22b ,22c und22d besteht und deren Ebenen sich in der Mitte des Unterstützungselements22 schneiden, ist in den Grundkörper20 eingebettet. Das Unterstützungselement22 ist in einem Stück aus einem kortikalen Knochen gefräst und bildet in der Draufsicht die Form eines Diagonalkreuzes. Der Grundkörper20 aus spongiösem Knochenmaterial ist mit einer Ausfräsung versehen, die der äußeren Kontur des Unterstützungselements22 entspricht, so dass das Unterstützungselement22 aus kortikalem Knochen in diese Ausfräsung eingesetzt werden kann. - In dieser Ausführungsform sind die Endseiten des Unterstützungselements
22 wieder mit der äußeren Kontur des Grundkörpers20 ausgerichtet, d. h. die untere Seite des Unterstützungselements22 ist im Wesentlichen mit der unteren Seite des Grundkörpers20 ausgerichtet, und die obere Seite des Unterstützungselements22 ist im Wesentlichen mit dessen oberer Seite ausgerichtet. -
3 zeigt eine weitere Ausführungsform eines Wirbelsäulenimplantats, das aus einem Grundkörper30 aus spongiösem Knochenmaterial mit der Form eines rechteckigen Parallelepipeds besteht. Ähnlich wie in der Ausführungsform von1 ist ein stiftartiges Unterstützungselement32 , das aus kortikalem Knochen besteht, in der Mitte des Grundkörpers30 eingebettet. Wie erkennbar, sind die Endseiten des Unterstützungselements32 in der Art einer Kuppel abgerundet, so dass das Unterstützungselement32 mit seiner kuppelförmigen Endseite34 über die äußere Kontur des Grundkörpers30 hinaus vorsteht. Die gegenüberliegende Endseite des Unterstützungselements32 ist auf die gleiche Weise ausgebildet und steht in gleicher Weise über den Grundkörper30 hinaus vor. - Die Befestigung der Unterstützungselemente in dem Grundkörper kann durch Presspassung oder durch Aufkleben oder ein anderes geeignetes Befestigungsmittel erfolgen. Ebenso ist eine große Zahl von Abwandlungen in der Konstruktion der Unterstützungselemente möglich. Anstelle der veranschaulichten beispielhaften Ausführungsformen kann lediglich ein wandartiges Unterstützungselement eingesetzt sein. Stiftartige und wandartige Unterstützungselemente können jedoch auch miteinander kombiniert sein.
- Der Grundkörper aus spongiösem Knochenmaterial kann verschiedene andere Oberflächenkonstruktionen zusätzlich zu der veranschaulichten rechteckigen Parallelepiped-Form aufweisen. Zum Beispiel kann seine Oberfläche mit Höckern versehen sein; sie kann gewellt oder gezahnt geformt sein. Die äußere Form des Grundkörpers kann keilförmig, abgerundet oder auch in der Art einer Banane gekrümmt sein.
- Des Weiteren kann zumindest ein Hohlraum, der von der Außenseite des Grundkörpers aus zugänglich ist und der mit Knochenpulver oder Knochengranulat gefüllt sein kann, in dem Grundkörper vorgesehen sein.
- Im Prinzip wird die Größe des Grundkörpers an die vorbestimmte Stelle angepasst, an der das Implantat eingesetzt werden soll. Die Außenabmessungen eines Wirbelsäulenimplantats dieser Art können in Abhängigkeit davon, ob es in einer Halswirbelsäule oder einer Lendenwirbelsäule angewendet wird, zum Beispiel wie folgt sein: Länge 15 bis 23 mm, Breite 8 bis 13 mm, Höhe 5 bis 13 mm.
- Die Kanten des Grundkörpers können abgerundet sein, um das Einführen des Implantats zu erleichtern. Des Weiteren kann zumindest eine Aufnahmeöffnung für ein Einsatzwerkzeug vorgesehen sein.
- Das spongiöse Knochenmaterial des Grundkörpers kann menschlichen oder tierischen, insbesondere von Rindern, Ursprungs sein und weist vorzugsweise osteoinduktive Eigenschaften auf, um den Heilungsprozess zu begünstigen.

Claims (10)
- Wirbelsäulenimplantat, bestehend aus einem Grundkörper (
10 ,20 ,30 ) aus spongiösem Knochenmaterial, in den zumindest ein Last tragendes Unterstützungselement (12 ,22 ,32 ) aus kortikalem Knochen eingebettet ist. - Wirbelsäulenimplantat nach Anspruch 1, wobei sich das Unterstützungselement (
12 ,22 ,32 ) von einer Seite zu einer gegenüber liegenden Seite des Grundkörpers (10 ,20 ,30 ) erstreckt. - Wirbelsäulenimplantat nach Anspruch 1, wobei das Unterstützungselement auf die Art eines Stiftes (
12 ,32 ) gebildet ist. - Wirbelsäulenimplantat nach Anspruch 1, wobei eine Vielzahl von parallelen Unterstützungselementen (

32 ) über die äußere Kontur des Grundkörpers (30 ) hinaus vorsteht. - Wirbelsäulenimplantat nach Anspruch 1, wobei der Grundkörper zumindest einen Hohlraum für die Aufnahme von Knochenpulver oder Knochengranulat enthält, der von seiner Außenseite aus zugänglich ist.
- Wirbelsäulenimplantat nach Anspruch 1, wobei der Grundkörper eine Oberfläche aufweist, die gewellt, gezahnt oder mit Höckern versehen ist.
- Wirbelsäulenimplantat nach Anspruch 1, wobei der Grundkörper im Wesentlichen blockförmig ist und zumindest ein durchgehender Stift in diesen Block als ein Unterstützungselement (
12 ,32 ) eingebettet ist.
