Abstract
Description
The invention relates to a surgical instrument for Removal of bones, tissue or the like of a living essence, with a tubular outer jacket and a tubular hollow shaft extending through the outer jacket extends therethrough a cutting element at its distal end ment for removing tissue, bones and the like carries and at its proximal end by a drive is drivable; wherein the outer jacket is formed is that it has a limited section of the cutting element tes releases, and the tubular hollow shaft with means for supplying fluid and / or for suction of fluid with suspended material from the operations area is connectable.
Such a surgical instrument, which is preferred in arthroplasty under arthroscopic control or in other non-open surgical procedures such. B. can be used under laparoscopy is in DBP 32 06 782 described. It has turned out to be disadvantageous represents that due to the construction of this surgical instrument from an outer jacket and a motor-driven hollow shaft only in a substantially straight Execution can be trained.
In many cases, however, an instruct for non-open surgical interventions with various the strong curvatures to make them otherwise inaccessible make operational areas accessible. In these Previously, flexible shafts or bifilar coils had been wound Springs used instead of a hollow shaft, but that have the disadvantage that they either inside Fill the outer sheath to a large extent, and thus not enough cavity to supply fluid and esp especially for extracting fluid with suspended material release from the operating area or, for. B. in the case a bifilar wound spring with a smaller volume for Tend to hit.
A satisfactory construction that takes advantage of the surgical instrument according to DBP 32 06 782 with egg ner curved embodiment is not yet known.
The invention is therefore based on the object of a chir Urgent instrument for the specified area of application to provide that both with a hollow shaft to drive the cutting element and to feed and down sucking fluid is provided, as well as formed can be that even with a straight instrument accessible operating areas are made accessible.
According to the invention this object is achieved in that the The outer jacket is curved in at least one area det and the hollow shaft in the area of curvature of the outer casing is constructed from hollow gimbals.
The cardan elements preferably have recesses on the end face gene or projections on the interaction of two interlock adjacent gimbals.
The projections of a gimbal element sit in front of it moves with play in the corresponding recesses of the be neighboring cardan element.
The invention further proposes that the end faces the projections are substantially parallel and / or the outer surface of the gimbals towards the end faces is flattened conically.
It is with the surgical instrument according to the invention became possible for the first time, all disadvantages from the prior art described in the introduction in surprising simple and effective way to avoid. Through the Curvature of the outer jacket is in at least one area it is possible to operate in otherwise inaccessible areas to make them accessible. In contrast to also ge curved surgical instruments from the prior art Technology is provided according to the invention that the hollow hollow in the curvature area of the outer element Cardan elements is constructed so that there is no loading impairment of for flushing the operating area necessary internal cross-section comes. To be an easy one Guarantee rotation of the hollow shaft in the area of curvature, the gimbal elements arranged there are on the front Provide recesses or projections, the two be interlocking adjacent gimbals. It is particularly preferred if the projections with play in the Recesses sit to wedge or the like in Avoid operation. The conical tapering of the cardanels elements towards the end faces facilitate intermeshing grasp or buckle the cardanels against each other elements during assembly and operation.
The surgical instrument according to the invention is advantageous assembled so that initially just that Cutting element in the distal part of the outer jacket is pushed that then the outer tube in the there for the intended area is bent that the Area of curvature of the outer jacket with gimbals is filled and that finally the rest of the straight Part of the hollow shaft is introduced.
Of course it is with this construction also possible to multiple curvatures of the surgical to provide strumentes if necessary.
Dabei zeigt:Further features and advantages of the invention result from the following description, which is based on the enclosed ing drawing a preferred embodiment of the Invention explained in more detail. The essential elements of the surgical instrument according to the invention are shown in FIG. 1. The part of the instrument which is introduced during the surgical procedure consists essentially of a fixed tubular outer jacket 20 and a tubular hollow shaft 30 which is held in a rotationally movable manner in this outer jacket. This consists essentially of three sections, namely the cutting element 30 'arranged at the distal end, which extends from the curvature region 20 ' of the outer jacket through the mounting element 22 for the outer jacket through to the connecting element 32 which can be coupled into the drive unit (not shown) he stretching hollow shaft section 30 '' and the Kardanelemen th 40th The connecting element 32 is itself rotatably mounted in the support member 22 and has an opening for connecting a fluid supply line (not shown) and a coupling element 36 for connection to the drive unit in connection to the proximal end 30 '''of the hollow shaft.
The essential section of this illustration for the present invention lies in the curvature region 20 'of the outer shell 20 . There are three gimbals 40 shown as an example, both with each other and on one side with the cutting element 30 'and on the other side with the straight hollow shaft portion 30 ''are engaged and represent the hollow shaft structure in the curvature region 20 ' of the outer shell. The structure of a single gimbal element is shown in greater detail in FIG. 2.
Fig. 2a shows an exemplary embodiment of such a gimbal element in a partially sectioned Be tenansicht. Here, a substantially cylindrical central portion 40 'and the end face of this central portion 40' at both sides thereof by alternating Ausneh rules 42 and projections 44 are provided. In the embodiment of the gimbal element according to the invention, a recess 42 on one end face of the same has a slightly narrower projection 44 on the other side in the longitudinal direction. When putting together the gimbal elements engages the corresponding jump before an adjacent gimbal element or the cutting element 30 'or section 30 ''of the hollow shaft with play in the recess 42 and thus produces a cooperative engagement. In this way it has become possible for the first time to provide a curved design of the known surgical instrument without reaching the hollow shaft structure and thus the optimal internal cross section for the supply and suction of fluid.
Of course, other design options for cardan elements are possible, which serve the same purpose as that shown in Fig. 2. In addition, it is also possible to produce surgical instruments according to the invention with more than one curvature, if necessary.
The in the above description and in the claims Chen and the accompanying drawings disclosed features the invention can be used individually as well as in any Combination for realizing the invention in their different embodiments may be essential.
