In einem bevorzugten Ausführungsbeispiel wird als physikalischer Prozeß zur Rekanalisation die sogenannte Photoablation (Abb. 2) in Verbindung mit dem photohydraulischen Prozeß (Abb. 3) genutzt. Einbettmaterialien versagen und durch Einsetzen der Photoablation in diesem Matrixmaterial auf der Einkoppelseite bereits eine frühzeitige Zerstörung des Katheters bedingt ist, der beim praktischen Einsatz erhebliche Risiken für den Erfolg einer Rekana lisation mit sich bringt. Überraschenderweise hat sich dabei gezeigt, daß eine Anhebung der Zer störschwelle an der Einkoppelseite von Mehrfasersystemen für die Strahlung eines Excimer-Laser bei 308 nm und 60 nsec in Pulsbreite um mehr als 50% möglich ist, wenn physikalische bzw. anorganische Bindemittel verwendet werden. Erfindungsgemäß werden dabei lediglich sogenannten Quarz/Quarz-Fasern, die an der Fügestelle von ihrer Kunst stoffschutzummantelung befreit wurden, verwendet. Die Anordnung diese Quarz/Quarz- Fasern erfolgt in hexagonal dichtester Packung, um so ein möglichst hohen geometrischen Koppelwirkungsgrad zu erreichen. Je nach Art des Lasers (1) wird aus dem Laserresonator austretende Bündel entweder direkt oder nach Strahlquerschnittsveränderung durch ein Teleskop (20) oder einen geeigneten Prismensatz, wobei anamorphotische Optik ausdrücklich erfindungsgemäß miteinbezogen ist (für asymmetrische Strahlprofile), durch eine an geeigneter Stelle positionierte körperliche Blende (11) gelenkt, die Randstrahlen abschattet und dann nach Umlenkung durch eine se mitransparenten dielektrischen Spiegel (12) durch ein fokussierendes optisches Endglied (Linse) (18) auf die Eintrittsfläche (3) des Mehrfasersystems (15) gelenkt. Dabei wird die fokussierende Linse (18) derart angeordnet, daß sie ein reelles Bild der körperlichen Blende (11) auf der Eintrittsfläche (3) des Mehrfasersystems (15) derart erzeugt, daß alle Fasern in hexagonal dichtester Packung durch die Laserstrahlung beaufschlagt werden. Dabei entsteht die Strahltaille des Laserbündels vor der Eintrittsfläche des Faserbündels. Bei geeigneter Wahl der optisch geometrischen Daten kann dadurch eine Homogenisierung des transversalen Intensitätsprofils der Laserstrahlung, wie in Abb. 7 gezeigt, erreicht werden. Gleichzeitig bietet diese Anordnung den Vorteil, daß durch zusätzlich einschränkbare Teilblenden (A, B, C) am Ort der körperlichen Blende (11) (Abb. 8), wie oben beschrieben, Strahlung auch nur auf wohl definierte Fasergruppen (A, B, C) der hexagonal dichtesten Packung gelenkt werden kann (Abb. 8). Bei entsprechend geometrischer Sortierung des Faserbündels, ebenfalls am distalen Ende, können damit auch einzelne Kreissegmente (A, B, C) des distalen Endes gezielt anesteuert werden. In einem typischen Ausführungsbeispiel ist der Durchmesser der körperlichen Blende 10 mm und die Brennweite des abbildenden optischen Endgliedes f = 100 mm bei einem Abstand von 11 f wird ein Durchmesser der Blendenabbildung von etwa 1 mm erreicht. Im Bereich der körperlichen Blende von 10 mm Durchmesser ist die Energiedichte des Strahls noch unterhalb der Ablationsschwelle der verwendeten Blendenmaterialien, weswegen es an dieser Stelle einfach möglich ist. Diese Anordnung hat den Vorteil, daß über den gleichen Laser simultan ein Mehrfaserkatheter (5) und ein Einzelfasersystem (6) angeschlossen werden kann, wobei das Einzelfasersystem (6) aus einem nach dem Stand der Technik bekannten hohlen, jedoch flüssigkeitsdichten Führungsdraht (6), und einer in seinem Inneren angeordneten Einzelfaser (14) besteht. Dieser hohle Führungsdraht wird so bemessen, daß er duch das innere Lumen des Führungsschlauches des Mehrfaserkatheters (5) bis über das distale Ende vorgeschoben werden kann. Dadurch ist es möglich, zunächst über das Einfasersystem, das im Inneren des Mehrfaserringkatheters vorgeschoben wird, ein Führungsloch zu bohren und über dieses Einfaserbührungssystem, sodann das Mehrfasersystem zum Aufbohren eines größeren Loches nachzuschieben. Dabei sind die Außendurchmesser von Einzelfaser (14), hohlen Führungsdraht (6) und Führungsschlauch des Ringkatheters (5) so bemessen, daß jeweils zwischen Einzelfaser und flüssigkeitsdichtem hohlem Führungsdraht bzw. zwischen Führungsdraht und Führungsschlauch ein Restlumen verbleibt, durch das Spülflüssigkeit zum distalen Ende des Katheters, gegebenenfalls mit Röntgenkontrastmittel, vorgepreßt werden kann.The optical arrangement shown in Fig. 6 is selected to couple the laser radiation. In Fortführung des Erfindungsgedankens wird der semitransparente Strahlleiter (12) für die Laserstrahlung in der Art ausgelegt, daß er Strahlung im Spektralbereich <400 nm teilweise reflektiert im oben angegebenen Teilverhältnis, jedoch Strahlung im Spektralbereich zwischen 400 und 700 nm vollständig transmittiert und wiederum Strahlung im Spektralbereich zwischen 700 nm und 3 µm im angegebenen Teilverhältnis reflektiert. Dadurch wird die erfindungsgemäße Möglichkeit geschaffen, durch diesen Strahlteiler (12) hindurch, mittels eines optisch angepaßten Beleuchtungssystems (8), das Faserbündel des Ringkatheters mit sichtbarer breitbandiger Strahlung zu beaufschlagen, um damit am distalen Ende des Mehrfaserringkatheters die Beleuchtung für ein erfindungsgemäß im Führungsschlauch des Ringkatheters vorzuschiebendes bildübertragendes Element (7) zu erzeugen. Als bildübertragendes Element wird erfindungsgemäß ein miteinander verschmolzenes Faserbündel mit einem am distalen Ende befindlichen selfoc-element als optischem Abbildungselement vorgeschoben. Derartige Bildfaserbündel sind nach dem Stand der Technik mit 2000-6000 Einzelfasern bei Außendurchmessern zwischen 0,2 und 0,5 mm erhältlich. Nach dem Stand der Technik sind derartige Bildleiterbündel sterilisierbar, und die Kosten für derartige Bündel liegen derzeit bei ca. 100,-- DM pro Meter und, obwohl derartige Bildbündel für rein industrielle Zwecke einwickelt wurden, bietet sich damit an, diese für die medizinische Anwendung als Einwegartikel zu benutzen und lediglich ein sterilisierbares, wiederverwendbares Okular (9) vorzusehen. Im Vergleich zu den nach dem Stand der Technik bekannten Mehrfunktionskathetern bzw. Endoskopen, bei denen die verschiedenen Funktionen, wie Beobachtung, Laserbestrahlung und Spülung, simultan vorgesehen sind, bietet das erfindungsgemäß vorgestellte System den Vorteil, daß lediglich die jeweils zum Rekanalisationsprozeß notwendige technische Funktion bereitgestellt wird, und daß durch die äußerst geringe Abmessung der verwendeten Bauelemente das gesteckte Ziel einen auch für kleinere Gefäße, wie sie im Bereich der Unterschenkeletage und der Koronarien des Herzens auftreten, hinreichend kleine Katheterdurchmesser, sogar unter French 5 bei Beibehaltung aller Vorteile der Laserrekanalisation, erzielt werden können.In continuation of the inventive concept, the semitransparent beam guide ( 12 ) is designed for the laser radiation in such a way that it partially reflects radiation in the spectral range <400 nm in the above-mentioned partial ratio, but completely transmits radiation in the spectral range between 400 and 700 nm and again radiation in the spectral range between 700 nm and 3 µm reflected in the specified ratio. Insbesondere werden hier erfindungsgemäß die Plasmaemissionslinien des bei in dem Prozeß entstehenden Plasmas analysiert bzw. die aufgrund des Raman-Effektes verschobene Streustrahlung, die gewebespezifische Informationen beinhaltet. Grundsätzlich ist auch die im Gewebe entstehende Fluoreszenzstrahlung für eine Analyse geeignet. Eine sichere Anordnung der Einzelfasern (15) zum Saphirzylinder (17) wird durch eine Fixierung mit Hydrosilikat in einem bevorzugten Ausführungsbeispiel mit Natriumhydrosilikat (Wasserglas) erreicht. Anschließend wird wieder in üblicher Weise der äußere Schutzschlauch positioniert. Aufgrund der wesentlich besseren mechanisch elastischen Eigenschaften des Saphirs wird damit sicher eine Zerstörung der Endflächen des Mehrfaserkatheters vermieden. In Weiterführung dieses Erfindungsgedankens wird ein zweiter Kathetertyp vorgesehen, bei dem der Saphirring (17) an der inneren distalen Circumferenz mit einer kegeligen bzw. torischen Fase versehen ist, wodurch aufgrund des Rechnungsindexunterschiedes eine kegelige Aufweitung der zylindrisch austretenden Laserstrahlung erreicht wird und damit durch mehrmalige Passage eines zuvor bereits initial ring-kanalisierten Gefäßes größere Durchmesser als in dem Katheter selbst entsprechend, rekanali siert werden können, was insbesondere bei größeren Gefäßdurchmessern von Interesse ist, um mit möglichst geringen Einstichöffnungen, d. h. geringer Patientenbelastung, auch großlu mige Gefäße rekanalisieren zu können.In order to avoid damage to the fiber ring catheter ( 5 ) at the distal end due to the photohydraulic effect, which is based on the generation of shock waves by the ablation process, the end of the fiber ring catheter is designed as a sapphire hollow cylinder ( 17 ) ( 9 ) such that the wall thickness of the hollow cylinder corresponds to the catheter wall thickness. Diese segmentweise Aufdehnung erfolgt durch einen im inneren Führungsschlauch plazierten Miniaturballondilatationskatheter und hat gegenüber einer symmetrischen Aufdehnung simultan aller Faserendspitzen den Vorteil, daß die im Bereich der Fasergruppen gleichbleibende Energiedichte oberhalb bestimmter kritischer Schwellenenergiedichten zur Ablation gehalten werden kann, wohingegen die radialsymmetrische Aufspreizung oberhalb gewisser Durchmesser, dann nämlich, wenn der Abstand der Einzelfaser wesentlich größer wird als der Faserdurchmesser, die Energiedichten zur Abtragung nach dem Prinzip der Photoablation bereits wenige Zehntel Millimeter hinter den Faserendflächen unterschritten wird, bzw. das im Kontaktverfahren zu große Areale von Verschlußsubstanz nicht der Ablation zugeführt werden können. Die segmentweise Aufdehnung hat zwar grundsätzlich für den Bereich der Dehnungsfugen die gleiche Problematik, jedoch muß man aus praktischen Erwägungen feststellen, daß die Großzahl der Verschlüsse bereits asymmetrisch, also nicht homogen auf der Circumferenz des Hohlsystems verteilt ist und von daher in einem größeren Prozentsatz bereits die segmentweise Abtragung völlig ausreicht, zum anderen kann durch einfache Torsionsbewegung des Katheters und mehrmaliges oszillierendes Bewegen des Katheters in der Längsachse des Hohlsystems das Gefäß suffizient freigestellt werden.In continuation of this inventive concept it is alternatively provided according to the invention to elastically connect the individual fiber groups (A, B, C) described above at the circumference at the distal end in such a way that the distal end of the multi-fiber ring catheter is expanded in segments without the sapphire ring can.
