Description

Die Erfindung betrifft ein Implantat zur Wirbelsäu­ lenkorrektur und/oder -stabilisierung, mit Knochen­ schrauben zum Einschrauben in die Wirbelkörper­ spongiosa und mit einer im wesentlichen quer zu den Schraubenachsen der Knochenschrauben verlaufenden Spannstange, die in an den Knochenschrauben ausge­ bildete Aufnahmen einlegbar und zur Wirbelkontrak­ tion oder -distraktion in Richtung der Spannstange jeweils beidseits gegen die Knochenschraubenverspann­ bar ist.
Bei einem aus der DE-PS 26 49 042 bekannten Implan­ tat dieser Art sind die Aufnahmen als offene Schlitze im Kopf der Knochenschrauben ausgebildet. Die Spann­ stange trägt zu ihrer Verspannung gegen die Schrauben­ köpfe ein Gewinde und beidseits jedes Schraubenkopfes eine Mutter, die mit einem gegen den Schraubenkopf vorstehenden Kragen in eine zugeordnete Ausnehmung des Schraubenkopfes greift, die im Vergleich zur Schlitzbreite radial erweitert ist, so daß die Spann­ stange durch den Eingriff der Muttern in die Ausneh­ mungen des Schraubenkopfes gegen ein postoperatives Herausspringen aus den Schraubenkopfschlitzen ge­ sichert ist. - Das bekannte Implantat dient speziell zur ventralen Derotationsspondylodese, jedoch ist es auch zur dorsalen Kompressionsspondylodese oder dergl. gut geeignet, und zwar besonders dann, wenn die Knochenschrauben transpedikulär in die Wirbel­ körper eingeschraubt werden, wobei die Möglichkeit besteht, das Implantat entweder nur einerseits oder je eines beidseits der Wirbelachse vorzusehen. In jedem Fall können mit Hilfe der Spannstange (n) komprimierende oder distrahierende Kräfte auf die Wirbelkörper übertragen werden, so daß korrigierende Wirbelsäulenverformungen und/oder Wirbelsäulenstabili­ sierungen erreicht werden können. - Ein Mangel dieser bekannten Implantate besteht allerdings darin, daß durch die Verspannung der Spannstange an den Schrau­ benköpfen die Richtung der Schraubenachse der Knochen­ schrauben nicht ausreichend und gezielt genug be­ einflußt werden kann, und daß bei beidseits der Wirbelachse angeordneten Implantaten versteifende Querverbindungen unmöglich sind, so daß auf Wirbel­ verlagerungen und -ausrichtungen allein durch Andern der Achsenrichtung der Knochenschrauben verzichtet werden muß.
Der Erfindung liegt die Aufgabe zugrunde, ein Implan­ tat der eingangs genannten Art so auszubilden, daß die Knochenschrauben auch bezüglich der Richtung ihrer Schraubenachse gegeneinander genau verstellt und diese eingestellten Richtungen fixiert werden können, und daß die gleiche Wirkung ausübende Quer­ verbindungen zwischen beidseits der Wirbelachse vor­ gesehenen Implantaten möglich werden.
Diese Aufgabe wird nach der Erfindung dadurch ge­ löst, daß zusätzlich zur Spannstange ein zumindest in Richtung der Schraubenachse gegen die Spannstange versetzter Spannstab vorgesehen ist, zu dessen An­ schluß an mindestens einer der Knochenschrauben in Richtung der Schraubenachse neben der Aufnahme für die Spannstange ein Gelenkteil angeordnet ist, das aus zwei um eine zur Schraubenachse senkrechte Ge­ lenkachse drehbar miteinander verbundenen Gelenkstücken besteht, von welchen das eine drehbar um die Schraubenachse an der Knochenschraube gelagert ist und das andere eine Aufnahme zum Einlegen des Spann­ stabes aufweist, der in seiner Längsrichtung beid­ seits gegen das Gelenkstück verspannbar ist.
Bei dem erfindungsgemäßen Implantat ist die das Gelenkteil tragende Knochenschraube sowohl von der Spannstange als auch vom Spannstab belastet, und zwar dank des Drehungen in zwei zueinander senkrech­ ten Ebenen ermöglichenden Gelenkteils im wesentlichen jeweils nur in Längsrichtung der Spannstange bzw. des Spannstabs. Da im übrigen die Spannstange und der Spannstab in Richtung der Schraubenachse im Abstand voneinander an der Knochenschraube angreifen, üben sie je nach ihrer Verspannung an der Knochenschraube Drehmomente und Scherkräfte auf die Knochenschraube aus und ermöglichen dadurch eine fein dosierbare Ein­ stellung der Richtung der Schraubenachse und die sichere Fixierung dieser Richtung. Der Spannstab kann über weitere Gelenkteile an eine, mehrere oder alle Knochenschrauben desselben Implantats oder, bei beidseits der Wirbelachse angeordneten Implantaten, an eine der Knochenschrauben des jeweils anderen Im­ plantats angeschlossen sein, wobei er in gleicher Weise die Einstellung und Fixierung der Achsrichtungen auch dieser Knochenschrauben ermöglicht, so daß im Ergebnis eine solide innere Fixation der Wirbelkör­ per, insbes. auch bei verschiedenen Wirbelfrakturen­ typen, gelingt.
Eine bevorzugte Ausführungsform ist dadurch gekenn­ zeichnet, daß bei im Schraubenkopf der Knochenschrau­ ben angeordneter Aufnahme für die Spannstange das Ge­ lenkteil an der Knochenschraube auf der Gewindeseite unter dem Schraubenkopf liegt und das an der Knochen­ schraube gelagerte Gelenkstück vom Schraubenkopf quer zur Schraubenachse übergriffen ist. Dadurch ist in einfacher Weise die axiale Lage des Gelenkteils längs der Knochenschraube fixiert. Im einzelnen empfiehlt es sich, daß das an der Knochenschraube gelagerte Ge­ lenkstück eine die Knochenschraube aufnehmende Boh­ rung und eine dazu senkrecht verlaufende zweite Boh­ rung aufweist, in der als Gelenkschale das mit der Aufnahme für den Spannstab versehene Gelenkstück mit einem Gelenkzapfen gelagert ist. Um möglichst kleine Abmessungen des Gelenkstücks zu erhalten, empfiehlt es sich, daß sich die Achsen beider Boh­ rungen kreuzen, daß der Durchmesser der die Gelenk­ schale bildenden Bohrung größer ist als der Durch­ messer der die Knochenschraube aufnehmenden Bohrung, und daß im Gelenkzapfen quer zur Zapfenachse eine die Bohrung für die Knochenschraube freigebende transversale Aussparung vorgesehen ist, deren lich­ te Weite in Richtung quer zu beiden Bohrungsachsen über den Durchmesser der die Knochenschraube auf­ nehmenden Bohrung verbreitert, in Richtung der Zapfenachse aber nur gleich diesem Durchmesser ist. Die Knochenschraube verhindert dann axiale Ver­ schiebungen des Gelenkzapfens in der die Gelenk­ schale bildenden Bohrung, während die Verbreite­ rung der Aussparung quer zu beiden Bohrungsachsen die begrenzte Verdrehung des Gelenkzapfens und da­ mit des am Spannstab verspannten Gelenkstücks er­ möglicht.
Die Aufnahme für den Spannstab ist zweckmäßig als offener Schlitz im Gelenkstück ausgebildet, der quer zur Zapfenachse und zur Achse der Aussparung ver­ läuft. Weiter empfiehlt es sich, den Spannstab im wesentlichen gleich wie die Spannstange auszubilden und den Spannstab am Gelenkstück in im wesentlichen gleicher Weise wie die Spannstange an den Knochen­ schrauben zu halten und zu verspannen. Dazu ist nach der Erfindung vorgesehen, daß der Spannstab zu seiner Verspannung ein Gewinde und beidseits des Ge­ lenkstücks eine Mutter trägt, die mit einem gegen das Gelenkstück vorstehenden Kragen in eine zuge­ ordnete Ausnehmung des Gelenkstückes greift, die im Vergleich zur Schlitzbreite radial erweitert ist, so daß der Spannstab durch den Eingriff der Muttern in die Ausnehmungen des Gelenkstücks gegen ein post­ operatives Herausspringen aus dem Gelenkstückschlitz gesichert ist.
Im folgenden wird die Erfindung an in der Zeich­ nung dargestellten Ausführungsbeispielen näher er­ läutert; es zeigen:

Die in der Zeichnung dargestellten Implantate dienen zur Fixation der lediglich in Fig. 5 angedeuteten Wirbelkörper 1 unmittelbar aufeinander folgender Wirbel, beispielsweise bei im einzelnen nicht darge­ stellten Wirbelfrakturen. Die Implantate bestehen ihrem wesentlichen Aufbau nach jeweils aus allgemein mit 2 bezeichneten Schrauben, einer Spannstange 3 und aus die Schrauben an der Spannstange fixierenden Muttern 4. Im einzelnen besitzen die Schrauben 2 einen zum Einschrauben in die Wirbelkörperspongiosa im Bereich der Wirbelbogenfüße geeigneten Schrauben­ schaft 2.1, dessen Länge und Gewindetiefe von Schraube zu Schraube variieren können. Der gegenüber dem Schrau­ benschaft 2.1 verbreiterte Schraubenkopf 2.2 besitzt einen Schlitz 2.3, in dem die mit einem Gewinde ver­ sehene Spannstange 3 eingelegt ist. Beidseits jedes Schraubenkopfes 2.2 trägt die Spannstange 3 eine der Muttern 4, die mit einem gegen den Schraubenkopf 2.2 vorstehenden Kragen 4.1 in eine zugeordnete zylindri­ sche Ausnehmung 2.4 des Schraubenkopfes 2.2 greift, die im Vergleich zur Breite des Schlitzes 2.3 radial erweitert ist. Im Ergebnis können die Schrau­ benköpfe 2.2 zwischen den ihnen jeweils zugeordneten beiden Muttern 4 eingespannt werden, wobei die Spann­ stange 3 je nach Verspannung komprimierende oder distrahierende Kräfte auf die Schraubenköpfe 2.2 und damit auf die Wirbelkörper 1 ausübt. Durch den da­ bei stattfindenden Eingriff der Muttern 4 in die Ausnehmungen 2.4 des Schraubenkopfes 2.2 wird die Spannstange 3 gegen ein postoperatives Herausspringen aus den Schraubenkopfschlitzen 2.3 gesichert. Die Muttern 4 besitzen einen an den Kragen 4.1 anschließen­ den Abschnitt zum Ansetzen eines Mutternschlüssels, im Ausführungsbeispiel den üblichen Sechskant 4.2.
Zusätzlich zur Spannstange 3 ist ein Spannstab 5 vorgesehen, der in Richtung der Schraubenachsen 2.5 gegen die Spannstange 3 versetzt ist. Zu seinem An­ schluß sind an den Knochenschrauben 2 in Richtung der Schraubenachse 2.5 neben dem Schraubenkopf 2.2 Gelenkteile 6 angeordnet, das aus zwei um eine zur Schraubenachse 2.5 senkrechte Gelenkachse 6.1 dreh­ bar miteinander verbundenen Gelenkstücken 6.2, 6.3 besteht. Das eine Gelenkstück 6.3 ist drehbar um die Schraubenachse 2.5 an der Knochenschraube 2 gela­ gert. Das andere Gelenkstück 6.2 ist mit einer Auf­ nahme 6.4 zum Einlegen des Spannstabes 5 versehen, der in seiner Längsrichtung beidseits gegen das Ge­ lenkstück 6.2 verspannbar ist, wobei je nach Ver­ spannungszustand wiederum komprimierende oder distrahierende Kräfte vom Spannstab 5 auf das Ge­ lenkstück 6.2 übertragen werden können. Der Spann­ stab 5 kann, wie in den Fig. 1 und 4, an die Knochen­ schrauben 2 desselben Implantats, oder, wie in Fig. 3 bei beidseits der Wirbelachse angeordneten Implan­ taten, an eine der Knochenschrauben 2 des jeweils anderen Implantats angeschlossen sein, wobei in der Fig. 5 der Fall dargestellt ist, daß zwei Spann­ stäbe 5 einander kreuzend jeweils zwei Knochenschrau­ ben 2 der sich zur Wirbelachse gegenüberliegenden Im­ plantate verbinden. Die Gelenkteile 6 liegen an der Knochenschraube 2 auf der Seite des Gewindeschaftes 2.1 unter dem Schraubenkopf 2.2, wobei das an der Knochenschraube 2 gelagerte Gelenkstück 6.3 vom Schraubenkopf 2.2 quer zur Schraubenachse 2.5 über­ griffen ist, so daß es bei in den Wirbelkörper ein­ geschraubter Knochenschraube 2 zwischen dem Wirbel­ körper einerseits und dem Schraubenkopf 2.2 anderer­ seits axial fixiert ist. Das jeweils an der Knochen­ schraube 2 gelagerte Gelenkstück 6.3 besitzt eine die Knochenschraube 2 aufnehmende Bohrung 6.5 und eine dazu senkrecht verlaufende zweite Bohrung 6.6, in der als Gelenkschale das mit der Aufnahme 6.4 für den Spannstab 5 versehene Gelenkstück 6.2 mit einem Gelenkzapfen 6.7 gelagert ist, wobei im Ausführungs­ beispiel die Anordnung so getroffen ist, daß sich die Achsen beider Bohrungen 6.5, 6.6 bei 7 kreuzen, die Spannstange 3 und der Spannstab 5 also in zwei zu­ einander senkrechten Ebenen gegeneinander verdreh- bzw. verschwenkbar sind. Der Durchmesser der die Gelenkschale bildenden Bohrung 6.6 ist größer als der Durchmesser der die Knochenschraube 2 aufnehmen­ den Bohrung 6.5. Im Gelenkzapfen 6.7 ist quer zur Zapfenachse 6.1 eine die Bohrung 6.5 für die Knochen­ schraube 2 freigebende transversale Aussparung 6.8 vorgesehen. Die lichte Weite dieser Aussparung 6.8 ist in Richtung quer zu beiden Bohrungsachsen, also in Fig. 9 in horizontaler Richtung zumindest in den peripheren Querschnittsbereichen über den Durch­ messer der die Knochenschraube 2 aufnehmenden Boh­ rung 6.5 verbreitert, in Richtung der Zapfenachse 6.1 aber nur gleich diesem Durchmesser. Das hat zur Folge, daß bei in der Bohrung 6.5 befindlicher Knochenschraube 2 der Gelenkzapfen 6.7 zwar gegen axiale Verschiebungen gesichert ist, aber entsprechend der Verbreiterung der Aussparung 6.8 begrenzt, d. h. in Fig. 9 über einen durch den Doppelpfeil 8 ange­ deuteten Winkelbereich von etwa 40°, verdrehbar bleibt. Die Aufnahme 6.4 für den Spannstab 5 ist als offener Schlitz im Gelenkstück 6.2 ausgebildet, der quer zur Zapfenachse 6.1 und zur Achse 6.9 der Aussparung 6.8 verläuft. Die Schlitzbreite entspricht dem Durchmesser des Spannstabs 5. Der Spannstab 5 trägt zu seiner Verspannung ebenfalls ein Gewinde und beidseits des Gelenkstücks 6.2 je eine Mutter 4 in der in Fig. 3 dargestellten Ausführungsform. Jede Mutter 4 greift mit einem gegen das Gelenkstück 6.2 vorstehenden Kragen 4.1 in eine zugeordnete Ausnehmung 6.10 des Gelenkstücks 6.2, die im Ver­ gleich zur Schlitzbreite radial erweitert ist, so daß der Spannstab 5 durch den Eingriff der Muttern 4 in die Ausnehmungen 6.10 des Gelenkstücks 6.2 gegen postoperatives Herausspringen aus der Aufnahme 6.4 gesichert ist, wie dies bereits im Zusammenhang mit dem Anschluß der Spannstange 3 in den Schlitzen 2.3 der Schraubenköpfe 2.2 der Knochenschrauben 2 beschrieben worden ist.
Werden in Fig. 1 die Gelenkstücke 6.2 am Spannstab 5 so verspannt, daß sich der Abstand der Gelenkstücke 6.2 voneinander vergrößert oder verkleinert, so er­ gibt sich eine Verstellung der Achsrichtungen der Knochenschrauben 2 in Richtung der in Fig. 4 einge­ tragenen Doppelpfeile 9, wobei in Fig. 4 der Fall einer Abstandsverkleinerung der Gelenkstücke 6.2 dar­ gestellt ist. Die Achsverstellung der Knochenschrauben 2 kann unschwer entweder unmittelbar in den die Spann­ stange 3 aufnehmenden Schlitzen 2.3 der Schrauben­ köpfe 2.2 oder durch eine geringfügige Biegeverfor­ mung der Spannstange 3 selbst aufgenommen werden. Je nach Verspannzustand der Gelenkstücke 6.2 am Spann­ stab 5 können Spannstange 3 bzw. Spannstab 5 auf Zug oder Druck beansprucht werden.

Implantat zur Wirbelsäulenkorrektur und/oder -stabilisierung, mit Knochenschrauben (2) zum Einschrauben in die Wirbelkörperspongiosa und mit einer im wesentlichen quer zu den Schrauben­ achsen (2.5) der Knochenschrauben (2) verlaufen­ den Spannstange (3), die in an den Knochenschrau­ ben (2) ausgebildete Aufnahmen (2.3) einlegbar und zur Wirbelkontraktion oder -distraktion in Richtung der Spannstange (3) jeweils beidseits gegen die Knochenschrauben (2) verspannbar ist, dadurch gekennzeichnet, daß zusätzlich zur Spann­ stange (3) ein zumindest in Richtung der Schrau­ benachsen (2.5) gegen die Spannstange (3) ver­ setzter Spannstab (5) vorgesehen ist, zu dessen Anschluß an mindestens einer der Knochenschrau­ ben (2) in Richtung der Schraubenachse (2.5) neben der Aufnahme (2.3) für die Spannstange (3) ein Gelenkteil (6) angeordnet ist, das aus zwei um eine zur Schraubenachse (2.5) senkrechte Gelenk­ achse (6.1) drehbar miteinander verbundenen Ge­ lenkstücken (6.2, 6.3) besteht, von welchen das eine drehbar um die Schraubenachse (2.5) an der Knochenschraube (2) gelagert ist und das andere eine Aufnahme (6.4) zum Einlegen des Spannstabs (5) aufweist, der in seiner Längsrichtung beid­ seits gegen das Gelenkstück (6.2) verspannbar ist.

Implantat nach Anspruch 1, dadurch gekennzeich­ net, daß bei im Schraubenkopf (2.2) der Knochen­ schrauben (2) angeordneter Aufnahme (2.3) für die Spannstange (3) das Gelenkteil (6) an der Knochenschraube (2) auf der Gewindeseite unter dem Schraubenkopf (2.2) liegt und das an der Knochenschraube (2) gelagerte Gelenkstück (6.3) vom Schraubenkopf (2.2) quer zur Schrauben­ achse (2.5) übergriffen ist.

Implantat nach Anspruch 1 oder 2, dadurch ge­ kennzeichnet, daß das an der Knochenschraube (2) gelagerte Gelenkstück (6.3) eine die Knochen­ schraube (2) aufnehmende Bohrung (6.5) und eine dazu senkrecht verlaufende zweite Bohrung (6.6) aufweist, in der als Gelenkschale das mit der Auf­ nahme (6.4) für den Spannstab (5) versehene Ge­ lenkstück (6.2) mit einem Gelenkzapfen (6.7) ge­ lagert ist.

Implantat nach Anspruch 3, dadurch gekennzeich­ net, daß sich die Achsen beider Bohrungen (6.5, 6.6) kreuzen, daß der Durchmesser der die Gelenk­ schale bildenden Bohrung (6.6) größer ist als der Durchmesser der die Knochenschraube (2) auf­ nehmenden Bohrung (6.5), und daß im Gelenkzapfen (6.7) quer zur Zapfenachse (6.1) eine die Bohrung (6.5) für die Knochenschraube (2) freigebende transversale Aussparung (6.8) vorgesehen ist, deren lichte Weite in Richtung quer zu beiden Bohrungsachsen über den Durchmesser der die Knochenschraube (2) aufnehmenden Bohrung (6.5) verbreitert, in Richtung der Zapfenachse (6.1) aber nur gleich diesem Durchmesser ist.
5. Implantat nach Anspruch 4, dadurch gekennzeich­ net, daß die Aufnahme (6.4) für den Spannstab (5) als offener Schlitz im Gelenkstück (6.2) ausgebildet ist, der quer zur Zapfenachse (6.1) und zur Achse (6.9) der Aussparung (6.8) ver­ läuft.
6. Implantat nach Anspruch 5, dadurch gekennzeich­ net, daß der Spannstab (5) zu seiner Verspannung ein Gewinde und beidseits des Gelenkstücks (6.2) eine Mutter (4) trägt, die mit einem gegen das Gelenkstück (6.2) vorstehenden Kragen (4.1) in eine zugeordnete Ausnehmung (6.10) des Gelenk­ stücks (6.2) greift, die im Vergleich zur Schlitz­ breite radial erweitert ist, so daß der Spannstab (5) durch den Eingriff der Muttern (4) in die Aus­ nehmungen (6.10) des Gelenkstücks (6.2) gegen postoperatives Herausspringen aus der Aufnahme (6.4) gesichert ist.
