Description

  • Technical description:
  • The invention relates to a lumbar vertebral body replacement implant for the partial or complete replacement of vertebral bodies.
  • Corresponding vertebral body replacement implants are used in the area of the lumbar spine for tumors or fractures of the spinal column or degenerative or inflammatory diseases for the partial or complete replacement of vertebral bodies.
  • Numerous vertebral replacement implants are known in the prior art. Usually, these include an upper implant part with an upper vertebral contact surface and a lower implant part with a lower vertebral contact surface, wherein the vertebral contact surfaces are usually designed to abut against the vertebral bodies. The vertebral body replacement implants are introduced from the front or side in a starting position. The affected vertebral body can be removed in advance. After introduction of the vertebral body replacement implant in the starting position, this is spread open. In this case, usually the implant parts are pressed apart. The vertebral body replacement implant can be individually adapted in this way to the respective extent of the existing space. Then it is fixed. In addition, stabilization can be done from the rear using pedicle screws and / or from the front via plates.
  • In the spreading of the vertebral body replacement implant mechanical and / or pneumatic spreading devices are used. From the US 7,641,693 B2 , of the US 6,524,341 B2 and the US 6,176,881 B1 Vertebral replacement implants with mechanical spreading devices are known. Pneumatic spreading devices go out of the US 2010/0063510 A1 , of the US 2010/0268340 and the corresponding one US 2010/0268341 A1 out.
  • In order to achieve an optimal result during the operation, it is necessary to adapt the vertebral body replacement implant as exactly as possible to the individual situation, in particular the existing space. Furthermore, the vertebral body replacement implant in the set extent (vertical) must be secure and easy to fix. The effort to be done for this should be kept as low as possible for the surgeon. Among other things, this also applies to the tool necessary for the introduction and spreading. In order to achieve a relatively minor injury to the skin and soft tissues, the necessary mechanisms for the spreading must be easily and safely accessible.
  • The already mentioned US 7,641,693 B2 describes a vertebral body replacement implant, in which the spreading takes place by means of a spindle winch in a spindle rod and an axial ring. The axial ring is toothed and can be driven via a relatively complex designed tool, which is attached laterally to the vertebral body replacement implant.
  • Based on this prior art, it is an object of the present invention to provide a better vertebral body replacement implant, which is particularly easier to use. Furthermore, the above requirements should be met.
  • This object is achieved by the device according to claim 1.
  • The spreading of the vertebral body replacement implant is achieved by an expansion Machanismus.
  • As a result, a stepless adjustability of the distances between the upper implant part and the lower implant part is achieved. The necessary force is relatively low, so that only a very small torque must be applied. Furthermore, the expansion device according to the invention with the expansion mechanism has the advantage that it is reversible. That is, the vertebral body replacement implant can be spread open, but also made smaller again. Additional mechanisms for blocking the vertebral body replacement implant in an end position are not necessary.
  • Theoretically, the spreading force can be applied by any mechanism. Preferably, the spreading device comprises a spindle winch with a spindle rod which is held in a rotatably mounted internal thread. This is a relatively simple and robust mechanism that can be combined in a simple manner with the expansion mechanism according to the invention.
  • The spreading device may comprise a guide, in particular a longitudinal groove, for the rotationally fixed guidance of a / the spindle rod in the axial direction along a spindle longitudinal axis. The guide can be used to inhibit the rotation of one of the swirl bearing surfaces when operating the expansion mechanism. In this respect, no unnecessary rotational force is applied to the vertebral bodies.
  • The spreading device according to the invention may comprise a mechanism for converting a rotational force into a lifting force and a transmission as a translation. These two components are in active connection. In one Embodiment, as already explained, a spindle winch used to implement the rotational movement in the lifting movement.
  • The drive axle may comprise a tool holder. Preferably, this tool holder is aligned so that it is accessible from one side of the intervertebral implant. For better power transmission, the tool holder may comprise a multi-round profile. Alternatively, polygonal profiles can be used.
  • The spreading device may comprise a housing for supporting the expansion mechanism. Preferably, the housing comprises a connection device for receiving a fixing device. For example, this may be a thread into which the fixing device is screwed. In one embodiment, the fixing device is simply a rod for holding the housing and thus for holding the intervertebral implant. Theoretically, the fixing device may simultaneously comprise a tool for actuating the expansion mechanism. In this case, it is advantageous if the connection device and the tool holder are arranged on the same side of the housing.
  • The housing can support the drive axle in such a way that the longitudinal axis of the drive axle runs perpendicular to the longitudinal axis of the spindle rod (spindle longitudinal axis). The spreading device can be designed such that the longitudinal axis of the drive axis runs parallel to a connection axis for the connection device. This arrangement of the longitudinal axis and / or the connection axis ensures easy accessibility.
  • The vertebral body replacement implant may be at least partially made of a metal alloy, in particular of a titanium alloy.
  • An embodiment of the vertebral body replacement implant according to the invention is as follows:
    The vertebral body replacement implant is essentially composed of a spreading device with a housing, an upper implant part arranged on one side of the housing and an implant part arranged on the opposite side of the housing. On the side facing away from the housing sides of the upper implant part and the lower implant part are each swirl bearing surfaces.
  • At the front of the housing two openings are provided. These are tool holders for attaching a drive tool and a connection device for receiving a fixing device. The connecting device has a thread for screwing in the fixing device, wherein the thread of the connecting device extends substantially parallel to a drive axis. Accordingly, the tool holder is aligned so that the driving tool can be introduced along the drive axle.
  • The spreader of the vertebral body replacement implant comprises an expansion mechanism which is arranged in the housing such that its drive axis is perpendicular to the spindle axis. The drive shaft is screwed with a nut such that it is rotatably held in the housing.
  • The spindle rod and the gear form a spindle winch for lifting the upper implant part. For this purpose, a gear is formed as a ring and has on the inside thereof an internal thread which is formed corresponding to a spindle thread on the spindle rod. In the assembled state, the gear sits on the spindle rod. As soon as the gear, which is held in the horizontal direction by the housing and the housing fixing screw, rotates, the spindle rod lowers or rises. To prevent entrainment of the spindle rod during the rotation of the gear, the housing has a corresponding guide.
  • The rotation of the gear is initiated by the drive axle whose threads engage tooth flanks on the outside of the gear. Gear and drive shaft thus form a gear.
  • If, therefore, a rotational movement about the drive axis is initiated via the tool holder, the applied force is transmitted to the toothed wheel with a strong reduction ratio. This rotates about the spindle axis and raises or lowers the spindle rod, which in turn leads to a lifting or lowering of the upper implant part relative to the lower implant part or housing.
