Description

Die Erfindung betrifft eine Hüftgelenk-Endoprothese nach dem Oberbegriff des An­ spruchs 1.
Stand der Technik ist der modulare Aufbau einer Hüftgelenk-Endoprothese mit einem Prothesenschaft, der im Oberschenkelknochen verankert ist, einem Kugelkopf und einer Pfanne bzw. einem Pfanneneinsatz, der im Beckenknochen verankert ist. Zur Befestigung des Kugelkopfs auf dem Prothesenschaft ist dieser mit einem konisch geformten Anschlußzapfen versehen, wobei der Kugelkopf eine hieran angepaßte konische Bohrung aufweist. Durch diese Art der Befestigung mittels konischer Klem­ mung entsteht im Belastungsfall in der konischen Bohrung des Kugelkopfes an deren Basis eine Zugspannung, was im Langzeiteinsatz bei kleinen Kugelkopfdurchmessern zum Versagen führen kann.
Der Erfindung liegt die Aufgabe zugrunde, eine Hüftgelenk-Endoprothese nach dem Oberbegriff des Anspruchs 1 so weiterzubilden, daß nahezu keine Zugspannungen im Kugelkopf auftreten. Außerdem soll ein Verfahren zur Herstellung angegeben werden.
Erfindungsgemäß wird diese Aufgabe bezüglich der Vorrichtung dadurch gelöst, daß der Kugelkopf durch eine Schrumpfpassung auf dem Anschlußzapfen befestigt ist.
Im Gegensatz zur konischen Klemmung hat die Schrumpfpassung den Vorteil, daß die Klemmspannung auch dann konstant bleibt, wenn sich die aufgebrachte Last verän­ dert. Die Kraft mit der der Kugelkopf auf dem Anschlußzapfen durch die Schrumpf­ passung fixiert ist, sollte so gewählt werden, daß eine Drehsicherheit gewährleistet ist, d. h. daß sich der Kugelkopf auf dem Anschlußzapfen nicht drehen kann.
In bevorzugter Ausführungsform ist der Anschlußzapfen zylinderförmig ausgebildet und der Kugelkopf weist eine daran angepaßte Bohrung auf, wobei der Anschlußzap­ fen die Bohrung vollständig ausfüllt. Durch diese formschlüssige Verbindung am Boh­ rungsgrund führt eine Lasteinleitung im Kugelkopf nur zu einer Druckspannung, je­ doch zu keiner Zugspannung, was eine Optimierung der Spannungsverhältnisse ge­ genüber der konischen Klemmung darstellt, bei der auch im Bereich des Domes der Bohrung Zugspannungen entstehen.
Ein bevorzugtes Verfahren zur Herstellung einer erfindungsgemäßen Hüftgelenk- Endoprothese zeichnet sich dadurch aus, daß der Kugelkopf erhitzt und/oder der An­ schlußzapfen des Prothesenschaftes gekühlt wird, so daß der Kugelkopf auf den An­ schlußzapfen aufgesetzt werden kann und sich nach dem Abkühlen des Kugelkopfes und/oder Erwärmen des Anschlußzapfens eine Schrumpfpassung ergibt.
Zweckmäßigerweise wird zur Kühlung des Anschlußzapfens flüssiger Stickstoff ver­ wendet.
Weitere Merkmale der Erfindung ergeben sich aus der Zeichnung, die nachfolgend beschrieben wird.
Mit dem Bezugszeichen 3 ist ein Prothesenschaft bezeichnet, der im Oberschenkel­ knochen verankert ist. An seinem zum Beckenknochen gewandten Ende weist der Prothesenschaft 3 einen Anschlußzapfen 2 auf, der zylinderförmig ist. Auf diesem An­ schlußzapfen 2 ist ein Kugelkopf 1 aufgesetzt, der hierzu eine Bohrung 5 aufweist Der Anschlußzapfen 2 füllt die Bohrung 5 vollständig aus. Die kraftschlüssige Verbin­ dung ist über eine Schrumpfpassung erreicht. Hierzu ist der Kugelkopf 1 erhitzt wor­ den, so daß er sich ausdehnt und/oder der Anschlußzapfen z. B. durch flüssigen Stickstoff gekühlt worden, so daß er schrumpft. Anschließend ist der Kugelkopf 1 auf den Anschlußzapfen 2 aufgesetzt worden. Wenn die Teile später wieder Raumtempe­ ratur erreicht haben, ist eine kraftschlüssige Verbindung durch eine Schrumpfpassung erreicht.
Mit dem Bezugszeichen 4 ist eine Pfanne oder ein Pfanneneinsatz bezeichnet, der im Beckenknochen verankert ist.
Der Prothesenschaft 3 und der Anschlußzapfen 2 bestehen bevorzugt aus Titan. Der Kugelkopf 1 kann aus Metall oder aus einer Keramik bestehen. Keramische Kugelköp­ fe haben sich als äußerst vorteilhaft erwiesen. Die Pfanne 4, die auch noch in eine äußere Schale eingelassen sein kann, besteht vorteilhafterweise aus einer Keramik. Sie kann aber auch aus Metall oder Polyethylen hergestellt werden.

Claims (4)

1. Hüftgelenk-Endoprothese mit einem im Oberschenkelknochen verankerten Pro­ thesenschaft (3) mit einem Anschlußzapfen (2) und einem auf dem Anschlußzap­ fen (2) aufgesetzten Kugelkopf (1), der in einer Pfanne (4) bzw. einem Pfannen­ einsatz gelagert ist, der wiederum im Beckenknochen verankert ist, dadurch gekennzeichnet, daß der Kugelkopf (1) durch eine Schrumpfpassung auf dem Anschlußzapfen (2) befestigt ist.
2. Hüftgelenk-Endoprothese nach Anspruch 1, dadurch gekennzeichnet, daß der Anschlußzapfen (2) zylinderförmig ausgebildet ist und der Kugelkopf (1) eine daran angepaßte Bohrung (5) aufweist, wobei der Anschlußzapfen (2) die Bohrung vollständig ausfüllt.
3. Verfahren zur Herstellung einer Hüftgelenk-Endoprothese nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß der Kugelkopf (1) erhitzt und/oder der Anschluß­ zapfen (2) des Prothesenschaftes (3) gekühlt wird, so daß der Kugelkopf (1) auf den Anschlußzapfen (2) aufgesetzt werden kann und sich nach dem Abkühlen des Kugelkopfes (1) und/oder Erwärmen des Anschlußzapfens (2) eine Schrumpfpassung ergibt.
4. Verfahren nach Anspruch 3, dadurch gekennzeichnet, daß zur Kühlung des Anschlußzapfens (2) flüssiger Stickstoff verwendet wird.
