Description
Die vorliegende Erfindung bezieht sich auf Stents, welche als Teil eines Ballon-
Angioplastie-Verfahrens innerhalb eines Körperkanals eines lebenden Tieres oder
eines Menschen implantiert werden, um die Durchgängigkeit aufrecht zu erhalten.
Insbesondere bezieht sich die vorliegende Erfindung auf gegliederte intravaskula
re Stents zur Zufuhr durch oder Implantation in einem Blutgefäß mit einem ge
krümmten Abschnitt.
Intravaskulare Stents mit einem eingeschränkten Durchmesser zur Zufuhr durch
ein Blutgefäß und einem ausgedehnten Durchmesser zum Anlegen einer radial
nach außen sich erstreckenden Kraft zur Unterstützung des Blutgefäßes sind im
Stand der Technik bekannt. Gegliederte intravaskulare Stents entweder für eine
Zufuhr durch ein gekrümmtes Blutgefäß oder für ein Implantiertsein darin sind
ebenfalls im Stand der Technik bekannt. Sich selbst ausdehnende gegliederte
Stents sind z. B. in dem US-Patent Nr. 5,104,4304 mit dem Titel "Gegliederter
Stent" von Wolff beschrieben. Ballon-ausdehnbare gegliederte Stents sind kom
merziell verfügbar unter dem Markennamen Palmaz-Schatz Balloon-Expandable
Stents von Johnson & Johnson Intervention Systems Co.
Ein sich selbst ausdehnender intravaskularer Stent 10 des Standes der Technik,
welcher sich in einem gekrümmten Blutgefäß 16 entfaltet, wird nun unter Bezug
auf Fig. 1 beschrieben, welche tatsächlich Fig. 2 des oben in Bezug genommenen
US-Patentes Nr. 5,104,404 ist. Ein Stent 10 besteht aus einer Anzahl individueller
Segmente 12, welche durch Gelenke 14 gegliedert sind, die an jedem Ende mit
den Segmenten 12 verbunden sind. Der Stent 10 ist vorzugsweise aus Memory-
Form-Material hergestellt, und zwar z. B. aus Nitinol, und ist als solches selbstex
pandierbar nach Zufuhr von einem Zufuhrsystem, welches in US-Patent Nr.
4,830,003 von Wolff et al. beschrieben ist. Diese bekannten gegliederten intra
vaskularen Stents besitzen jedoch eine Reihe von Nachteilen sowohl während der
Zufuhr durch ein gekrümmtes Blutgefäß als auch, wenn sie darin implantiert sind,
wie es nun beschrieben wird.
Die Zufuhr des Stents 10 durch ein gekrümmtes Blutgefäß 16 ist komplizierter als
die Zufuhr eines nicht gegliederten Stents dahingehend, dass der Stent 10 derart
winkelmäßig ausgerichtet sein muss, dass seine Gelenke 14 in Richtung auf den
konvexen Abschnitt des Blutgefäßes 16 so angeordnet sind, dass der Stent 10
nach innen abgebogen werden kann. Bei dem vorliegenden Beispiel ist festzu
stellen, dass die Gelenke 14 an derselben Seite der Segmente 12 angeordnet sind,
da das Blutgefäß 16 lediglich eine einfache Krümmung in einer Ebene besitzt. Es
versteht sich leicht, dass die Zufuhr von Stents durch Blutgefäße, welche einen
oder mehrere gekrümmte Abschnitte haben, welche nicht in derselben Ebene lie
gen, noch komplizierter ist und im Allgemeinen speziell aufgebaute Stents erfor
dert.
Selbst wenn Stents 10 in einem gekrümmten Blutgefäß implantiert sind, zeigt es
sich, dass die Spalte zwischen den Segmenten 12 den gekrümmten Abschnitt des
Blutgefäßes 16 ohne Stütze belassen. Des weiteren sind die Spalte an dem kon
vexen Abschnitt des Blutgefäßes 16 wesentlich größer als die Spalte an deren
konkavem Abschnitt, wodurch ungleichmäßige und deshalb unerwünschte Span
nungen an dem Blutgefäß 16 erzeugt werden.
Aus dem US-Patent Nr. 5,102,417 bzw. der EP 0 606 165 A1 ist ein Stent be
kannt, der grundsätzlich den Aufbau eines Stents gemäß den Fig. 2A bis 2C auf
weist. Nachteilig ist bei diesem bekannten Stent, dass die über dem konvexen
Abschnitt eines Ballons liegenden Verbindungsglieder eine Tendenz dahingehend
aufweisen, dass sie nach innen gespannt werden, wenn der Stent gebogen wird.
Darüber hinaus weisen die Segmente des Stents einen Drehversatz relativ zu ei
nem Ballon auf, wenn der Stent ausgedehnt wird.
Die Aufgabe der Erfindung besteht darin, einen gegliederten Stent zu schaffen,
welcher keine spezielle winkelmäßige Ausrichtung benötigt, wenn er durch einen
gekrümmten Körperkanal geführt wird, und welcher eine kontinuierliche und
gleichmäßige Stütze für sowohl gerade als auch gekrümmte Abschnitte eines
Körperkanals schafft, wenn er implantiert ist. Es wäre außerdem wünschenswert,
dass die Struktur des Stents nicht von besonderen Ausrichtungen gekrümmter
Abschnitte eines Blutgefäßes abhängt.
Diese Aufgabe wird gemäß der Erfindung durch einen Stent gemäß Anspruch 1
gelöst. Der erfindungsgemäße Stent schafft eine kontinuierliche und gleichmäßi
ge Stütze für sowohl gerade als auch gekrümmte Abschnitte eines Körperkanals,
wenn er implantiert ist. Des weiteren hängen die Struktur des Stents und seine
Stütze für einen Körperkanal nicht von den Ausrichtungen der gekrümmten Ab
schnitte des Kanals ab.
Die Verbinder des erfindungsgemäßen Stents besitzen vorteilhafterweise zwi
schen 8 und 24 Verbindungsglieder, um eine kontinuierliche und gleichmäßige
Stütze für sowohl gerade als auch gekrümmte Abschnitte eines Körperkanals zu
schaffen.
Die Stents weisen eingeschnürte Durchmesser zur intraluminalen Zufuhr auf und
werden anschließend durch das Auflasen eines Ballons, welcher Teil ihres Ka
theterzufuhrsystems ist, auf ausgedehnte Durchmesser zum Anlegen radial nach
außen sich erstreckender Kräfte zum Stützen des Lumens von Körperkanälen de
formiert. Die eingeschnürten und ausgedehnten Durchmesser der Stents fallen
typischerweise in die Bereiche von 1,0 bis 3,5 mm bzw. 3,5 bis 10 mm.
Die Stents sind vorzugsweise aus mehr plastischen als elastischen biokompatiblen
Materialien mit niedriger Memory-Eigenschaft hergestellt, und zwar z. B. aus
nichtrostendem Stahl 316L, Gold, Tantal usw., was es ihnen ermöglicht, plastisch
aus ihren eingeschnürten Durchmessern in ihre ausgedehnten Durchmesser de
formiert zu werden.
Ein typischer Stent zur Implantation in einer menschlichen Koronararterie ist 9 bis
21 mm lang, weist drei bis sieben 2,2 mm lange Stentsegmente auf, welche durch
zwei bis sechs 1 mm lange Verbinder derart verbunden sind, dass die Enden des
Stents sich in einem Winkel zwischen 45° und 135° bei einem Krümmungsradius
von etwa 9 mm gegenüberliegen, wenn sie gebogen sind.
Die Erfindung wird nachfolgend lediglich beispielhaft unter Bezugnahme auf die
beigefügten Zeichnungen beschrieben, in welchen:
Fig. 1 eine vergrößerte Ansicht eines gegliederten Stents des Standes der
Technik zeigt, welcher in einem gekrümmten Blutgefäß entfaltet
ist;
Fig. 2A und 2B einen gegliederten Stent zeigen, welcher nicht Gegenstand der be
anspruchten Erfindung ist, und zwar in seinem entspannten und
seinem gebogenen Zustand vor einer plastischen Deformation;
Fig. 2C den ausgedehnten Stent gemäß Fig. 2 nach einer plastischen De
formation zeigt;
Fig. 2D den Stent gemäß Fig. 2 zeigt, welcher an einem Katheter in seinem
gebogenen Zustand montiert ist;
Fig. 2E und 2F den Stent gemäß Fig. 2 vor und nach Ausdehnung mittels eines
Ballons zeigt, welcher Teil eines Katheterzufuhrsystems ist;
Fig. 3A und 3B ein Ausführungsbeispiel eines gegliederten Stents zeigen, welcher
gemäß der Lehre der vorliegenden Erfindung aufgebaut und be
triebsfähig ist, und zwar in seinem entspannten und seinem gebo
genen Zustand vor einer plastischen Deformation; und
Fig. 3C den ausgedehnten Stent gemäß Fig. 3 nach einer plastischen De
formation zeigt.
Die vorliegende Erfindung bezieht sich auf einen gegliederten Stent zum Zufüh
ren durch einen Körperkanal, z. B. einer peripheren oder koronaren Arterie eines
lebenden Tieres oder Menschens und der Implantation darin als Teil eines Ballon-
Angioplastie-Verfahrens, um die Durchgängigkeit aufrecht zu erhalten.
Die Prinzipien und Funktion des gegliederten Stents gemäß der vorliegenden Er
findung können besser unter Bezugnahme auf die Zeichnungen und die beigefügte
Beschreibung verstanden werden.
Unter Bezugnahme nun auf die Zeichnungen zeigen Fig. 2A bis 2C einen geglie
derten Stent, welcher allgemein mit 100 bezeichnet ist und nicht Gegenstand der
beanspruchten Erfindung ist, welcher allgemein eine Anzahl von im Wesentlichen
steifen Segmenten 102 aufweist, welche durch Verbinder 110 verbunden sind.
Die Segmente 102 sind vorzugsweise so aufgebaut, dass sie eine feine diamant
förmige Masche von untereinander verbundenen diamantförmigen Zellen 108 mit
1 mm Seiten bei Ausdehnung darstellen, wie es am besten in Fig. 2C zu sehen ist.
Abhängig von dem beabsichtigten Durchmesser des Stents 100 weisen die Seg
mente 102 typischerweise zwischen 8 und 24 diamantförmige Zellen 108 auf.
Die Verbinder 110 weisen Verbindungsglieder 112 auf, welche ein vorderes Ende
104 mit einem hinteren Ende 106 benachbarter Segmente 102 verbinden. Die
Verbindungsglieder 112 erstrecken sich vorzugsweise in einer im Wesentlichen
schraubenförmigen Art zwischen den Scheitelpunkten der diamantförmigen Zel
len 108 an den vorderen und hinteren Enden 104 und 106 benachbarter Segmente
102 derart, dass die Anzahl von Verbindungsgliedern 112 gleich der Anzahl von
Zellen 108 ist. Die Verbindungsglieder 112 sind vorzugsweise gleichmäßig um
die Umfänge der Segmente 102 derart entfaltet, dass die Verbinder 110 in jede
Richtung gleich gebogen werden können, und um eine kontinuierliche und
gleichmäßige Stützte für sowohl gerade als auch gekrümmte Abschnitte eines
Körperkanals zu schaffen.
Abwechselnde Verbinder 110 an den vorderen und hinteren Enden 104 bzw. 106
eines Segments 102 weisen vorzugsweise Verbindungsglieder 112 auf, welche in
Uhrzeigerrichtung und Gegenuhrzeigerrichtung gewunden sind. Abwechselnde
sich windende Verbinder 110 gewährleisten, dass die Drehversetzung der Verbin
dungsglieder 112 und benachbarter Segmente 102 relativ zu den Wänden eines
Blutgefäßes und wichtiger des Ballons seines Zufuhrsystems minimiert wird,
wenn der Stent 100 ausgedehnt wird.
Die Verbinder 110 weisen eine im Allgemeinen zylindrische Konfiguration auf,
wenn der Stent 100 entspannt ist, wie es am besten in Fig. 2A ersichtlich ist, und
eine unterschiedlich gedehnte und zusammengedrückte gekrümmte Konfigurati
on, wenn der Stent 100 gebogen ist, wie es am besten in Fig. 2B ersichtlich ist.
Die gebogene Konfiguration wird durch zwei relativ gegenüberliegende Ver
schiebungen der Verbindungsglieder 112 herbeigeführt. Als erstes tritt das unter
schiedliche Dehnen der Verbinder 110 an deren konvexem Abschnitt auf, welcher
mit 114 bezeichnet ist, und zwar durch die Verbindungsglieder 112, welche von
einander weg versetzt werden. Als zweites tritt das unterschiedliche Zusammen
drücken der Verbinder 110 an deren konkavem Abschnitt auf, welcher mit 116
bezeichnet ist, und zwar durch die Verbindungsglieder 112, welche in Richtung
aufeinander versetzt werden.
Der Stent 100 besitzt einen eingeschnürten Durchmesser zur Zufuhr durch einen
gekrümmten Körperkanal, wie es in den Fig. 2A und 2B gezeigt ist, sowie einen
ausgedehnten Durchmesser, wie er in Fig. 2C dargestellt ist, zum Stützen eines
Körperkanals. Der Stent 100 ist vorzugsweise aus einem mehr plastischen als
elastischen biokompatiblen Material mit niedriger Memory-Eigenschaft herge
stellt, und zwar z. B. aus nichtrostendem Stahl 316L, Gold, Tantal usw., was es
ihm ermöglicht, plastisch aus seinem eingeschnürten Durchmesser in seinen aus
gedehnten Durchmesser deformiert zu werden. Der eingeschnürte und der ausge
dehnte Durchmesser des Stents 100 fallen typischerweise in die Bereiche von 1,0
bis 3,5 mm bzw. 3,5 bis 10,0 mm.
Unter Bezugnahme nun auf die Fig. 2D bis 2F ist der Stent 100 als über einem
Ballon 118 liegend gezeigt, welcher Teil seines Katheterzufuhrsystems 120 ist.
Der Stent 100 ist an seinem Katheterzufuhrsystem 120 in seinem eingeschnürten
Durchmesserzustand montiert, was in Fig. 2E gezeigt ist, zur plastischen Defor
mation durch Aufblasen des Ballons 118 in seinen ausgedehnten Durchmesser,
was in Fig. 2F gezeigt ist, zum Stützen der Wände eines Körperkanals. Ein bei
spielhafter Stent zur Implantation in einer menschlichen Koronararterie ist typi
scherweise 15 mm lang, besteht aus fünf 2,2 mm langen Segmenten 102, welche
durch vier 1 mm lange Verbinder 110 verbunden sind, und kann sich derart bie
gen, dass seine Enden in einem 90° Winkel in einem Krümmungsradius von etwa
9 mm gegenüberliegen.
Die Zufuhr eines gegliederten Stents 100 ist beträchtlich einfacher als die Zufuhr
eines gegliederten Stents 10 des Standes der Technik gemäß Fig. 1, da der Stent
100 in alle Richtungen gleich flexibel ist und er deshalb keine winkelmäßige Aus
richtung erfordert, um einen speziellen gekrümmten Abschnitt zu passieren. Die
ser Vorteil ist besonders wichtig zur Zufuhr durch Blutgefäße mit mehrfachen
gekrümmten Abschnitten. Es ist ein weiterer Vorteil des Stents 100 gegenüber
den Stents 10 des Standes der Technik gemäß Fig. 1, dass der Stent 100 eine kon
tinuierliche und gleichmäßige Stütze entlang der gesamten Länge eines Blutgefä
ßes mittels der Segmente 102 und der ungebogenen Verbinder 110 schafft, welche
deren gerade Abschnitte stützen, während die Verbinderabschnitte 114 und 116
deren konvexe bzw. konkave gekrümmte Abschnitte stützen.
Unter Bezugnahme nun auf die Fig. 3A und 3B ist ein gegliederter Stent 122 ge
mäß der vorliegenden Erfindung gezeigt, bei welchem Verbinder 124 Verbin
dungsglieder 126 aufweisen wobei jedes Verbindungsglied (126) mindestens ein
Paar benachbarter Abschnitte aufweist, zwischen
welchen eine oder mehrere Knickstellen (128) ange
ordnet sind. Der Auf
bau der Verbinder 124 gemäß der Erfindung ist vorteilhaft gegenüber dem eines
Verbinders 110, da der Stent 100 eine Tendenz dahingehend besitzen kann, den
Ballon 118 wegen zweier Gründe zu zerreißen. Als erstes besitzen die über dem
konvexen Abschnitt des Ballons 118 liegenden Verbindungsglieder 112 eine Ten
denz dahingehend, dass sie nach innen gespannt werden, wenn der Stent 100 ge
bogen wird. Zweitens stellen die Segmente 102 einen Drehversatz relativ zum
Ballon 118 dar, wenn der Stent 100 ausgedehnt wird.
In diesem Fall wird die unterschiedlich gedehnte und zusammengedrückte ge
krümmte Konfiguration des Verbinders 124 durch zwei relativ gegenüberliegende
Versätze der Verbindungsglieder 112 wie zuvor herbeigeführt, außer dass das
unterschiedliche Dehnen der Verbinder 124 an dem konvexen Abschnitt 114
durch Knickstellen 128 auftritt, welche etwas ausgestreckt werden, während das
unterschiedliche Zusammendrücken der Verbinder 124 an dem konkaven Ab
schnitt 116 durch die Knickstellen 128 auftritt, welche spitzer gebogen werden.
In einer ähnlichen Art wie bei dem Stent 100 besitzt der Stent 122 gemäß der Er
findung einen eingeschnürten Durchmesser zum Zuführen durch einen gekrümm
ten Körperkanal, wie es in den Fig. 3A und 3B gezeigt ist, und einen ausgedehn
ten Durchmesser, wie es in Fig. 3C gezeigt ist, zum Stützen eines Körperkanals,
wenn er darin implantiert ist.
Claims (4)
1. Stent (122) mit zumindestens zwei im Wesentlichen steifen Segmenten
(102) mit einer Vielzahl von verbundenen Zellen (108), die jeweils Schei
telpunkte aufweisen und die bei radialer Ausdehnung der Segmente (102)
eine im Wesentlichen zylindrische diamantförmige Masche bilden, und ei
nem flexiblen Verbinder (124), welcher eine Vielzahl von flexiblen Ver
bindungsgliedern (126) aufweist, welche jeweils Scheitelpunkte von in
Längsrichtung des Stents (122) benachbarten Zellen (108) an benachbarten
Segmenten (102) verbinden, wobei jedes Verbindungsglied (126) mindes
tens ein Paar benachbarter Abschnitte aufweist, zwischen welchen eine o
der mehrere Knickstellen (128) im nicht ausgedehnten und im ausgedehn
ten Zustand angeordnet sind.
2. Stent nach Anspruch 1, bei welchem der Verbinder (124) zwischen 8 und
24 Verbindungsglieder (126) aufweist.
3. Stent nach Anspruch 1 oder 2, welcher aus einem biokompatiblen Material
besteht, welches mehr plastisch als elastisch deformierbar ist.
4. Stent nach Anspruch 3, bei welchem das Material nichtrostender Stahl,
Gold oder Tantal ist.
