Abstract

The contact lens has a lens body (22) received in a holder (37) and having a concave lens front face (29) that is formed for attaching on the eye. An annular gap (44) is formed at an edge of the lens front face between the lens body and the holder. The holder is formed in such a manner that a vacuum channel between the annular gap and the eye is not closed with the holohedral eye that is applied to the lens front face of the lens body.

Description

The The invention relates to a contact lens for ophthalmic surgery, the one in a socket accommodated lens body having a has a concave lens front surface for placing on the eye, being at the edge of the lens front surface between the lens body and the socket is an annular gap formed together with the applied eye acts as a suction channel, on the negative pressure for fastening of the contact lens can be applied to the eye.

One such contact glass is shown in WO 2005/048895 A1, which is otherwise with the attachment of the contact glass to a laser treatment device employed.

Contact glasses in the Eye surgery are examples of Adapters that use the laser processing device with an object mechanically couple. This coupling is required because the accuracy of the Positioning of the laser beam in the object usually at the Machining achieves precision. Only the exact three-dimensional positioning of the laser beam in the processing volume, for example in the cornea of the eye, allows a high-precision machining. The fixation of the to be processed Object is therefore made by an adapter, the one exactly defined position of the object, for example of the eye opposite the Guaranteed laser processing device. The adapter commonly used in eye surgery as a contact lens is designated, is thus part of the beam path. Is the outer shape of the object to be processed is not known exactly, the adapter has usually also the function, the object, as far as possible, a certain, at to give the presupposed shape of the application of a laser beam.

There the front surface the cornea of the human eye varies from patient to patient in the laser-assisted Eye surgery on a regular basis Adapter provided in the form of a contact glass. The US 2001/0021844 A1 describes a corresponding contact lens that not only the Fixation of the eye, but also accomplished a shaping of the corneal anterior surface. The US script suggests before, between the cornea and the contact lens designed as a lens negative pressure which sucked the cornea to the contact lens becomes. By so taking place between lens body and cornea Under pressure attachment, the cornea automatically takes the form the lens body front surface (front surface is on the patient). However, this type of attachment is for the Patients uncomfortable, especially if in one embodiment US 2001/0021844 A1 on the underside of the lens body version provided, barb-like knobs are used, which improved To achieve attachment of the contact lens to the eye.

Of the The invention is therefore based on the object, a contact glass of the aforementioned type so that a secure attachment of the lens body on the eye and a reliable one Shaping the cornea for the patient as gently as possible Way done.

These Task is according to the invention with a Contact glass of the type mentioned solved, in which the socket so is formed that at entire area to the lens front surface the eye is not yet attached to the suction channel between the annular gap and the eye closed is.

Damit ist der gesamte Prozeß des Anlegens und Fixierens besser kontrollierbar.in the Contrary to the procedure described in US 2001/0021844 A1 the cornea is not sucked by vacuum directly to the front surface of the lens, but applied by mechanical pressure to the lens front surface. In that the Version is designed so that at Completely to the lens front surface the eye of the suction channel between the annular gap and the eye remains open (the cornea does not yet cover the annular gap) can be found in In this state, an accurate alignment of the contact lens to the eye respectively. Only when the contact glass is pressed further on the eye, covered the cornea covers the annular gap, causing the suction canal between Annular gap and eye is closed and the vacuum fitting acts. The vacuum applied in the annular gap thus serves only for Fixation of the eye, but does not directly cause the deformation of the cornea. This is the entire process of creation and fixation more controllable.

The vacuum applied through the suction channel serves only for fixation of the eye, but does not directly cause the deformation of the cornea, there is no vacuum between the anterior surface of the lens and the cornea. The contact glass according to the invention is so special for Patients who are in the area of the cornea, attached to the Lens front surface should be applied, corneal damage, abnormalities or not at all healed cuts earlier have eye surgery. Because in this area of the cornea no Negative pressure is now a damaging effect of the contact glass attachment locked out.

On the patient side, therefore, the transition between the lens front surface and the socket (except for the annular gap) is fluid; the version forms beyond the annular gap no jump.

The The frame is geometrically designed so that the cornea in one Condition in which the eye is already fully applied to the front surface of the lens is, not yet at the edge of the frame, which limits the annular gap, is, Consequently, the ring gap is not yet closed to the suction channel. This can be achieved for example by the fact that the axially outermost outline of the socket across from a curvature surface, the the cornea, when the eye is fully in contact with the anterior surface of the lens, so does not project or opposite this even lags behind. The shape of the curvature surface depends essentially from the condition that the eye has when fully attached to the anterior surface of the lens. This condition is also referred to as "applanated" condition in eye surgery.

In a simple design can be for the curvature surface, opposite the axially foremost Outline of the Does not project or even remains behind, also with reference to the curvature the lens front surface take, so that then the axially foremost outline of Version opposite the imaginary extension the lens front surface does not project or even remains behind.

With the contact glass according to the invention can The natural Form of the cornea to be maintained when applied contact glass, which for the Patient is particularly pleasant. The eye is at a suitable radius of curvature placed with minimal deformation on the contact glass.

A Particularly simple production can be achieved when the annular gap by one or more truncated cone lateral surfaces or conical surfaces Frame and / or lens body formed. With such conical surfaces is beyond usually a further embodiment simply realized, in which the annular gap tapers away from the eye and especially at the end facing away from the eye at an acute angle ≤ 90 ° converges.

A Such design of the annular gap is further advantageous because it effectively avoids parts the cornea is sucked into the upper area of the suction channel and thereby partially block it. The introduction of the Negative pressure in the annular gap is then uncritical, since always the entire Annular gap acts as a suction channel and in particular the vacuum port not can be closed by the cornea.

The Contact glass according to the invention allows especially if the curvature surface of the lens front surface something shallower curved is, as the cornea, a particularly simple placement of the Contact glass on the cornea. When approaching the contact glass initially touched only the central area of the cornea, that is, the corneal vertex, the lens front surface. With increasing touch, the cornea is gradually at the full Lens front surface in which, even in this state, the suction channel is not yet closed and the patient is still free of the eye despite partial deformation can move. In this state is a simple orientation of the contact glass across from possible for the eye. Is the desired Adjustment reached, so the distance between the contact glass and Patient eye is still slightly reduced, whereby the annular gap of the Cornea of the eye is closed and the eye by negative pressure across from is fixed to the contact glass.

The Curvature surface, which from the lens front surface imprinted on the eye as a target shape, can be application-dependent chosen become. In particular, aspheric curvature surfaces are possible, which allow optical Aberration during introduction of treatment laser radiation to minimize. Also, the lens front surface may be spherical with a radius of 5-30 mm be. A radius slightly less than that of the human Cornea, so z. B. between 15 and 25 mm is particularly preferred. This sphericity is due to the inventive design without problems, there, unlike the prior art in the form of US 2001/0021844 A1, there is no danger that the Cornea the negative pressure suction channel so closes that only one incomplete Attachment of the contact lens is done on the eye.

It has been found that in the vacuum fixation of an ophthalmic contact lens, the suction of conjunctiva on the means of vacuum attachment should be avoided, since otherwise insufficient fixation of the cornea, on which the surgical procedure is to take place. It is therefore quite independent of the other design of the contact glass and in particular regardless of the design of the means for vacuum attachment appropriate to provide a contact lens for eye surgery that has a trained for placement on the eye lens front surface and a device for vacuum attachment of the contact lens on the eye and further characterized in that a coding element coding for a geometrical or optical parameter of the contact lens is provided on the contact lens. Conveniently, the geometric or optical parameter is the diameter of the lens front surface. The coding element is conveniently mounted on the contact lens so that a user, ie an eye surgeon, from outside the desired geometric or optical parameters, such as the desired diameter of the lens front surface can recognize. As a coding, for example, a bar code, a number or letter identifier or a geometric or color code in question. Particularly preferred is a contact glass, which carries a color marking, wherein the color marking is associated with the diameter of the lens front surface or any other geometric or optical parameter of the contact glass. If the contact glass is manufactured in two parts, ie from a lens body having the front surface of the lens and a holder holding the lens body, the coding element will be conveniently arranged on the socket. in the case of color coding, for example, the socket itself can be colored.

In order to a user can select a contact lens with the desired parameters, the coding is advantageous as optically perceptible coding trained so that a Surgeon can quickly and accurately select the desired contact lens.

The described coding element is of course particularly advantageous in a contact glass described here before or below Art used.

1 shows a treatment device for an ophthalmological procedure similar to that in the EP 1159986 A1 or the US 5549632 described. The treatment device 1 of the 1 serves to one eye 2 a patient to perform an ametropia correction according to the known femtosecond LASIK method. For this purpose, the treatment device 1 a laser 3 on, the pulsed laser radiation emits. The pulse duration is z. As in the femtosecond region, and the laser radiation acts by means of non-linear optical effects in the cornea in the manner described above. The one from the laser 3 along a optical axis A1 emitted treatment beam 4 falls on a beam splitter 5 that the treatment beam 4 on a scanning device 6 passes. The scanning device 6 has two scan mirrors 7 and 8th which are rotatable about mutually orthogonal axes, so that the scanning device 6 the treatment beam 4 deflects two-dimensionally. An adjustable projection optics 9 focuses the treatment beam 4 on or in the eye 2 , The projection optics 9 has two lenses 10 and 11 on. The treatment device 1 represents a laser processing device.

The lens 11 is a contact glass acting as an adapter 12 downstream, that via a bracket H firmly with the lens 11 and thus the beam path of the treatment device 1 connected is. The contact glass to be described later 12 lies on the cornea of the eye 2 at. The optical combination of treatment device 1 with attached contact glass 12 causes the treatment beam 4 in one in the cornea of the eye 2 located focus 13 is bundled.

The scanning device 6 will be just like the laser 3 and the projection optics 9 via (unspecified) control lines from a control unit 14 driven. The control unit 14 determines the position of the focus 13 both transverse to the optical axis A1 (through the scanning mirrors 7 and 8th ) and in the direction of the optical axis A1 (through the projection optics 9 ).

The control unit 14 continue to read a detector 15 out, the radiation backscattered from the cornea, the beam splitter 5 as re-radiation 16 happens, reads. By means of the detector 15 can the operation of the laser 3 be controlled very precisely.

The contact glass 12 ensures that the cornea of the eye 2 receives a desired target shape. The eye 2 is due to the conditioning of the cornea 17 on the contact glass 12 in a predetermined position to the contact glass 12 and thus to the associated treatment device 1 ,

This is schematically in 2 shown a section through the cornea 17 shows. To get an exact positioning of the focus 13 in the cornea 17 To reach, the curvature of the cornea must 17 be taken into account. The cornea 17 has an actual shape 18 which varies from patient to patient. The adapter 12 lies now on the cornea 17 such that it in a desired desired shape 19 deformed. The exact course of the desired shape 19 depends on the curvature of the eye 2 facing lens front surface of the contact glass 12 from. Through the Adap ter 12 are known geometrical and optical conditions for the introduction and focusing of the treatment beam 4 in the cornea 17 given. Because the cornea 17 on the contact glass 12 is applied and this in turn on the holder H relative to the beam path of the treatment device 1 is stationary, the focus may be 13 by controlling the scanning device 6 as well as the adjustable projection optics 9 three-dimensionally accurate in the cornea 17 be positioned.

Durch Andrücken des Kontaktglases 12 auf die Augenhornhaut 17 verleiht die Linsenvorderfläche 29 des in der Fassung 37 gehaltenen Linsenkörpers 22 der Augenhornhaut 17 die gewünschte Soll-Form 19.The 3 and 4 show an embodiment of the contact glass 12 in detail; 4 is a sectional view, 3 a view of the contact glass 12 from the front (in 4 from below), ie within sight of the patient. The contact glass 12 is constructed in two parts and consists of a lens body 22 in a version 37 is attached, for example, at a splice 43 is glued. The lens body is made of glass or a medically approved plastic, such as PMMA or polycarbonate. These substances are also suitable for the version, which may additionally be made of polyuretan or silicone rubber. By pressing the contact glass 12 on the cornea 17 gives the lens front surface 29 in the version 37 held lens body 22 the cornea 17 the desired target shape 19 ,

To provide the negative pressure, the socket 37 a neck 38 on, via a luer-lock connector 39 and a negative pressure supply line running in the nozzle 40 features. The supply line 40 opens to the side of the lens body 22 above a neck ring 41 the version 37 , Between one of the lens body 22 facing ring inner surface 46 of the neck ring 41 and the frusto-conical shaped lens edge in this embodiment 47 is an annular gap 44 formed with the supply line 40 communicates and acts as a suction channel, can be attached by the annular negative pressure on the cornea. By means of negative pressure on the Luer lock connection 39 thus becomes the contact glass 12 so attached to the eye that the cornea by mechanical pressure to the lens front surface 29 is created, causing the target shape 19 is reached.

The neck ring 41 is relative to the optical axis A1 with respect to the lens front surface 29 in front.

When placing the contact lens on the cornea first contact between the lens front surface 29 and the corneal vertex. With further application of the cornea to the anterior surface of the lens 29 Contact with the cornea is established over an ever larger area of the anterior surface of the lens. Is the cornea completely on the anterior surface of the lens 29 on, due to the curvature of the cornea becoming stronger in the edge region of the cornea, there is still no contact between the axially foremost outline of the attachment ring 41 and the cornea made so that through the annular gap 44 provided suction channel is not yet closed by the cornea. In this state, it is still possible, the eye in contact with the lens front surface 29 to adjust so that the optical axis A1 is exactly as desired, for example, coincides with the visual axis. Only when the contact lens and the eye are further compressed does the cornea also attach itself to the axially foremost outline 45 of the neck ring 41 on, whereby the suction channel at the annular gap 44 closed and the contact glass 12 attached to the eye.

The version 37 So it is with respect to the approach ring 41 designed so that the axially foremost contour 45 of the neck ring 41 not opposite to that of a curvature surface caused by the curvature of the eye when fully on the lens front surface 29 applied cornea is projecting. The neck ring 41 lies in one embodiment exactly in the imaginary extension of the curvature surface. In simplification may also refer to the curvature of the lens front surface 29 be taken.

By the eye tapered at an acute angle form of the annular gap 44 It reliably prevents parts of the cornea from reaching the area of the supply line 40 be sucked and thereby at least partially clog or grow. Also is a clogging of the suction channel 44 avoided, so that an annular application of the negative pressure is ensured.

The 5 to 8th schematically show various embodiments of the geometry of lens body 22 and version 37 , Elements that are already in the construction of the 3 / 4 are provided with the same reference numerals, so that in this regard also to the description of the 3 and 4 is referenced.

Again, the contact glass has 12 the lens body 22 on, in the version 37 is held. The lens body 22 is limited by an entrance area 23 and the lens front surface 29 , Preferred is the shape of a plano-concave lens, wherein the concave lens front surface 29 particularly preferably corresponds in terms of its curvature of the human cornea, or is somewhat flattened. However, other shapes are possible, in particular the front surface of the lens 29 also aspheric to minimize optical aberrations.

The version 37 encloses the lens body 22 on a part of its circumference, creating a connecting surface between lens body 22 and socket element 37 is formed substantially along a cylindrical lateral surface. Between the edge of the lens body 22 and the neck ring 41 the version 37 and the edge 47 the lens front surface 29 of the lens body 22 is the serving as a suction channel annular gap 44 educated.

To execute this annular gap can now, like the 5 to 8th show either the ring inner surface 46 of the neck ring 41 and / or the lens edge 47 be formed conically, ie be obliquely opposite to the optical axis A1. In the embodiment of the 5 is the lens edge 47 conical, the ring inner surface 46 cylindrical. It can also be seen clearly that the approach ring 41 does not protrude from the already mentioned curvature surface K, which is predetermined by the curvature of the applied cornea, but rather remains slightly behind. The construction of the 5 has the advantage that a simple, substantially tubular socket 37 can be used. Next is the diameter of the lens front surface 29 smaller than the entrance area 23 ,

Auch reicht hier der Ansatzring 41 exakt bis an die Krümmungsfläche K.The reverse case shows the 6 , here is the lens edge 47 cylindrical, the ring inner surface 46 Conversely, however, the cone is now open to the curvature surface K out. Also, the approach ring is enough here 41 exactly up to the curvature surface K.

In the construction according to 7 are both the lens body 22 on the lens edge 47 conical (tapering to the eye) as well as the ring inner surface 46 (widening towards the eye). Next remains the axially leading contour of the approach ring 41 back to the curvature surface K. This has the advantage that a complete adjustment of the eye relative to the contact glass 12 is possible and only when additionally applied mechanical pressure of the suction channel is closed by conditioning the cornea.

Vorteilhaft ist in diesem Fall der kleinere Durchmesser der Auflagefläche des Kontaktglases 12 auf dem Auge bei gleichzeitig großem Durchmesser der Eintrittsfläche 23. 8th Finally, a construction similar to that 7 , however, are both ring inner surface 46 as well as lens edge 47 Conically tapered to the eye trained. The advantage in this case is the smaller diameter of the contact surface of the contact glass 12 on the eye with a large diameter of the entrance surface 23 ,

Of course they can here described geometric embodiments also individually or in others, not expressly used combinations shown or described advantageous become.
