Abstract

本实用新型公开了一种多球囊内阻断胸腹主动脉转流管,由多个可充、放气体或液体的密封球囊和管组成,该转流管具有一条主杆管和多个分叉管。本实用新型能在不阻断主动脉正常血流、保证腹腔各脏器正常供血的情况下,为血管性疾病特别是腹动脉瘤手术减少难度,避免了大流血,降低了手术死亡率。

Description

多球囊内阻断胸腹主动脉转流管
本实用新型涉及一种胸腹主动脉转流管,具体的说是胸腹主动脉瘤手术时,可在不阻断正常血流的情况下应用的多球囊内阻断胸腹主动脉转流管,属于A61M25/00类。
随着医疗诊断技术的进步,如血管造影和B超的应用,发现动脉瘤是一种常见的疾病,尤其是胸腹主动脉瘤是动脉瘤中最常见的发病率较高的疾病。当前手术切除为唯一有效治疗方法。在进行动脉手术时,要先用阻断钳夹紧上下两端的血管壁阻断正常的血流,才能切除动脉瘤,进行人工血管搭桥术。但这种手术危险性大,原因是腹腔内的各脏器缺血时间不宜过长,肝脏缺血不应超过十五分钟,肾脏缺血不应超过三十分钟,肠缺血不应超过三小时,否则会形成组织变性,坏死,功能丧失,甚至死亡。还有长时间用阻断钳夹紧动脉壁,会引起动脉壁损伤。如不阻断血流,会出现大出血,造成失血性休克、死亡。这种传统的手术操作,对人体损伤大,操作困难,术中异常紧张,死亡率极高,据统计一般肾动脉水平以下的腹腔动脉瘤手术死亡率为5-10%,而肾动脉(包括肾动脉)水平以上的腹主动脉瘤手术死亡率则为30%,如果腹主动脉瘤并发破裂出血时,施行急症手术,死亡率可高达90%,一般情况很少有人愿冒此风险,所以腹主动脉瘤,好似一个不定时的炸弹,随时有爆炸的危险,而且一旦发生幸存者极少。
本实用新型的目的是针对上述的缺点提供一种器械,能在不阻断主动脉正常血流,保证胸腹腔各脏器正常供血的情况下为患者进行手术。
本实用新型的目的是这样实现的:一种多球囊内阻断胸腹主动脉转流管,由多个可充放气体或液体的密封球囊和管组成,并具有一条主杆管和多个分叉管。
主杆管由胸主动脉转流管和腹主动脉下段转流管构成。
分叉管由腹腔动脉分流管、肠系膜上动脉分流管、及左、右肾动脉分流管构成。
密封式球囊,均具有导通气体或液体的导管。
由于采用上述方案,各个管头部分质地较硬,便于插入,明显的提高了胸腹主动脉瘤病人的手术成功率,减少和避免了传统的阻断血流情况下手术的并发症,降低了死亡率,而且更重要地降低了手术的难度,便于推广普及,有利于血管医学地发展。并经北京邮电医院内部试用证明效果良好。
下面结合附图及实施例对本实用新型进一步详细说明。

如图1所示,本实用新型由多个可充、放气体或液体的密封球囊1、6、8、10、13、17、和胸主动脉转流管2,腹主动脉下段转流管14、腹腔动脉分流管4、肠系膜上动脉分流管7、右肾动脉分流管11和左肾动脉分流管16组成。转流管和分流管可选用硅胶管,不宜形成血栓。
由胸主动脉转流管2和腹主动脉下段转流管14组成主杆管;由腹腔动脉分流管4、肠系膜上动脉分流管7和左肾动脉分流管16及右肾动脉分流管11构成分叉管。
所述密封式球囊1、6、8、10、13、17均具有导通气体或液体的导管3、5、9、12、15、18。
图2表本实用新型用于胸腹主动脉手术示意图,图中胸主动脉19,腹主动脉下段21以及腹腔动脉25肠系膜上动膜24、右肾动脉23、左肾动脉22均由各气囊均充满气体或液体和相应管壁阻塞密封。
手术时先在瘤子上方正常动脉壁19上切开一小孔1901,将胸主动脉球囊管放入充水阻断血流,再将腹主动脉下段21动脉瘤下方正常动脉壁上切一小孔,放入腹主动脉下段球囊管充水即可阻断血流,各分叉球囊管通过手术时切开部位插入相应的动脉管内充水阻断血流,上述各动脉管通过本器械能恢复正常血流。
试验病例:
病例一:
患者郑××,男性,66岁,住院号54560,主因腹胀双下肢乏力一年,于93年11月15日入院。入院查体:发现腹部肿物约6×7cm2,位于腹正中脐水平上方,不活动,有搏动。并于93年12月28日行腹主动脉瘤切除人工血管搭桥术,术中应用多球囊内阻断胸腹主动脉转流管,使手术非常顺利,术后恢复良好。
病例二:
患者刘××,男性,37岁,住院号为54582,主因发现腹部包块,伴腹胀4个月于93年11月17日入院。入院后行腹主动脉造影发现胸腹主动脉明显瘤样扩张,长约25cm,宽度约8.5cm,并于93年12月21日行胸腹主动脉瘤切除人工血管搭桥术,术中应用多球囊内阻断胸腹主动脉转流管,术中见动脉瘤长32cm,最宽10cm,尽管分离切除非常困难,但在此管帮助下还是很顺利地完成手术,术后病人肝肾功能很快恢复正常,体质恢复良好,痊愈出院。

1、一种多球囊内阻断胸腹主动脉转流管,由多个可充、放气体或液体的密封球囊和管组成,其特征是:该转流管具有一条主杆管和多个分叉管。
2、根据权利要求1所述多球囊内阻断胸腹主动脉转流管,其特征是:主杆管由胸主动脉转流管(2)和腹主动脉下段转流管(14)构成。
3、根据权利要求1所述多球囊内阻断胸腹主动脉转流管,其特征是:所述分叉管由腹腔动脉分流管(4)、肠系膜上动脉分流管(7),左、右肾动脉分流管(16、11)构成。
4、根据权利要求1所述多球囊内阻断胸腹主动脉转流管,其特征是:所述密封式球囊(1、6、8、10、13、17)均具有导通气体或液体的导管(3、5、9、12、15、18)。
