In this case, other structures of the needle insertion assembly are changed correspondingly, for example, a gear slide or a connecting rod rotation mode is adopted to enable a pull rope to realize bidirectional control. In the present embodiment, as shown in fig. 5A to 5C, the retreat preventing needle assembly 14 is provided on the main casing 11 and detachably engaged with the suture needle 13 to prevent retreat of the suture needle 13. As shown in FIGS. 7A and 7B, the operating handle 6 of the present embodiment is used in conjunction with the suture instrument 1 of the present embodiment. As shown in fig. 9A and 9B, the inner circular ring assembly 62 includes an inner circular ring 621, a washer 622, and a screw 623. The inner ring 621 is inserted into the central hole 6521 of the central shaft 652, fitted with the washer 622, and screwed down with the screw 623. As shown in fig. 10A-10F, the clutch assembly 63 includes compression springs 631 and 632, a limit pin 633, a screw 634, a limit 635, and an operating handle clutch lever 636. Pulling the pull rod 834 drives the driving wheel 813 to rotate counterclockwise. When the pull rod 834 is released, the spring 832 resets the pull rod 834 to realize reciprocating pulling, and the driving wheel 813 continuously rotates anticlockwise. The anti-back teeth 822 are intended to prevent the drive wheel 813 from rotating clockwise when the pull rod 834 is released. As shown in FIGS. 15A and 15B, the external forceps tube 3, the knotting tube 5, the attachment ring 4, and the stapler 1 are attached to the endoscope 2. Specifically, the external forceps tube 3 is fixed to the flexible tube portion 21 of the endoscope 2 via the coupling ring 4, and the knotting tube 5 is fitted into an inner hole of the external forceps tube 3. Another attachment ring 4 is fitted over the distal end 22 of the endoscope 2, and the stapler 1 is fitted over the attachment ring 4 to fix the stapler 1 to the distal end 22 of the endoscope 2. The head end 31 of the external forceps tube 3 is inserted into the stapler 1. The coupling ring 4 in this embodiment can function to fix both the external forceps tube 3 and the endoscope 2, and also can increase the coupling strength of the distal end 22 of the endoscope 2, so that the suture instrument 1 can be firmly fixed to the distal end 22 of the endoscope 2. In practical use, the plurality of connecting rings can be distributed along the hose of the endoscope at multiple points, thereby dispersing the stress. In this embodiment, the treatment device may further include a hook forceps 9 (as shown in fig. 18B), the hook forceps 9 performs a tissue grasping operation or a suture hooking operation at the distal end of the endoscope, and finally may grasp both ends of the suture to complete a knotting operation of the suture. The hook forceps can enter through the forceps channel of the endoscope and can also enter through the external forceps channel of the endoscope, and no limitation is made here. In practical use, the hook clamp can simultaneously extend into two hook clamps through two clamp paths to work, and also can work by only using one hook clamp. In this embodiment, the front end of the hook forceps is in the shape of a forceps with a hook.
Through the ring lantern ring, stitching instrument and hook forceps ware, the treatment device and the treatment system that this embodiment provided can realize multiple functions such as the cutting of tissue, suture, knot. In addition, the treatment device and the treatment system provided by the embodiment can further comprise biological glue, plugging glue, a stent, an anastomosis nail and an anastomosis clip, so that the diversification of functions is realized.
Next, how to attach the treatment device of the present invention to the endoscope will be described. Optionally, the endoscope can be matched with an endoscope with the end diameter of 98mm, the diameter of the forceps channel of less than or equal to 2mm and the working length of less than or equal to 1300 mm. First, as shown in fig. 16A and 16B, the operation handle 6 is attached to the operation end of the endoscope 2, and the operation handle 6 and the endoscope 2 are firmly fixed together by the handle fastener 68. Next, as shown in FIGS. 17A and 17B, the coupling ring 4 is fitted over the distal end 22 of the endoscope 2, and the suture instrument 1 is fitted over the coupling ring 4 at the distal end 22 of the endoscope 2. The endoscope 2 is turned on, and the direction of the stapler 1 is adjusted to the C-shaped opening downward under the monitor. After the position is adjusted, the stapler 1 is tightly sleeved until the step surface of the sleeve ring of the stapler 1 is tightly pressed against the front end 22 of the endoscope 2.
As shown in fig. 18A and 18B, the hook forceps 9 is inserted from the insertion jaw of the endoscope 2, reaches the distal end of the endoscope 2 through the forceps channel, and then the first pull cord 121 of the suture instrument is grasped, and the first pull cord 121 located near the distal end is pulled into the forceps channel. The hook forceps 9 is then removed and pulled out from the insertion jaw 23, thereby bringing one end of the first cord 121 out of the insertion jaw 23 (the arrow in the figure indicates the direction of movement of the hook forceps 9). The two pull ropes are distinguished at the thread outlet on the stitching instrument 1. Preferably, the ends of the first and second drawstrings may be marked differently. The present invention is not limited to this, and in other embodiments, the first pulling rope and the second pulling rope may be identical and distinguished by the operator. In other embodiments, the first and second pulling ropes can be marked by material, color, etc. for easy distinction.
As shown in fig. 19A to 19C, the square hole 6211 of the inner circular ring 621 of the operating handle 6 is aligned with the side hole 671 of the front case. The first pulling rope 121 pulled out of the inserting jaw 23 is threaded into the square hole 6211 of the operating handle and then tightened through the two binding holes 6212 on the wing of the inner circular ring 621). As shown in fig. 20A and 20B, the hook forceps 9 is inserted again from the insertion jaw 23 of the endoscope 2, reaches the distal end of the endoscope 2 through the forceps channel, and then the second pull cord 122 of the suture instrument is grasped to pull the second pull cord 122 located near the distal end into the forceps channel. The hook forceps 9 are then removed and pulled from the insertion jaw, thereby pulling one end of the second pull cord 122 out of the insertion jaw.
As shown in fig. 21A-21C, the square hole 6531 of the outer ring of the operating handle is aligned with the side hole 664 of the rear housing. The first pulling rope 121 pulled out of the inserting jaw is threaded into the square hole of the operating handle and then tightened through the two binding holes 6532 on the wheel wing of the outer ring 653. Rotating the outer ring clockwise loosens the second pull cord 122 from the cord on the outside of the handle, wrapping into the circular groove of the outer ring 653.
In one embodiment, the endoscope 2 may be a single-channel endoscope. As shown in FIGS. 22A-22C, an external clamp tube 10 is first attached to the endoscope 2. The leading end of the external forceps tube 10 is inserted into the second channel 1112 at the edge of the first housing of the stapler until it abuts against the stepped surface 1114 of the second channel 1112. A plurality of coupling rings are distributed along the forceps channel direction, and the external forceps channel tube 10 and the hose portion of the endoscope 2 are fastened by the coupling rings. The knotter tube 20 is inserted into the external clamp tube 10.
In another embodiment, the endoscope may be a dual-channel endoscope. In this case, no external clamp tube is required to be installed. Because the endoscope has two forceps channels, the push-knot tube only needs to be inserted into the vacant forceps channels.
Next, how the suture instrument of the present invention performs the suture will be described.
First, as shown in fig. 23A, the hook forceps 9 is inserted from the insertion jaw of the endoscope, reaches the tip of the endoscope through the forceps channel, and then finds and grips the suture 131. The other grasper 9 is inserted into the knot pushing tube, reaches the distal end of the endoscope, grasps the edge of the target suture tissue 30, and is withdrawn in a direction to bring the target suture tissue 30 in the middle of the C-shaped opening of the stapler (two strings are not shown in the drawing). As shown in FIGS. 23B and 23C, the handle pull 643 is continuously pulled back to advance the needle through the tissue. The depth of the suture (full or layered) depends on the wound or non-invasive tissue treatment.
As shown in FIGS. 23D and 23E, the pulling back lever 643 is stopped after the needle 13 is rotated 180 degrees in the needle track (i.e., the first sliding groove). The knob of the inner circular ring 621 of the operating handle is then gently lifted by hand to pull out about 3mm while rotating clockwise to tighten the second cord 122. Further pulling back the handle rod 643 rotates the needle 13 180 ° in the first chute to the initial position. The knob of the inner ring 621 of the operating handle is then lifted slightly to pull out about 3mm, while rotating clockwise to tighten the second cord 122. For larger sutured objects, one side of the target tissue may be sutured first, the other side of the target tissue may be sutured, the suture may be appropriately tightened, and tied. As shown in fig. 23F, the two grapple devices 9 are released. As shown in FIG. 23G, the grasper 9 of the internal channel grasps the suture 131 at the leading end (near the needle) and pulls the suture 131 through the tissue. As shown in FIG. 23H, the grasper 9 of the other knotting tube grasps the trailing end of the suture thread 131, and adjusts the position of the stapler on the endoscope 2 so that the trailing end of the suture thread 131 is inside the C-shaped opening of the stapler and the leading end of the suture thread 131 is outside the C-shaped opening of the stapler.
As shown in FIG. 23I, the handle is continuously pulled back, and the resistance of the pull rod is increased after the monitor sees that the suture needle is inserted for about half a turn, i.e. the pull rod is stopped. The inner ring knob of the operating handle is lifted by hand to be pulled out, and simultaneously, the clockwise rotation makes the second pull rope tensioned. As shown in FIG. 23J, the handle is continuously pulled back, and the resistance to pulling back the handle is increased after the suture needle is inserted for about half a turn under the monitor, i.e., the pulling back of the handle is stopped. The inner ring knob of the operating handle is lifted by hand to be pulled out, and simultaneously, the clockwise rotation makes the second pull rope tensioned. As shown in FIG. 23K, the needle has been returned to its initial position by one rotation of the needle. The steps described in this paragraph are repeated, i.e., the needle is operated to rotate one more revolution. The above-described sewing steps are repeated to continuously perform the sewing.
As shown in FIG. 23L, the hook forceps 9 for grasping the suture thread tip is released, and after the suture needle passes over and half-knots, the needle is moved forward of the suture thread tip quickly, and the suture thread 131 is grasped again. As shown in FIG. 23M, the two loopers 9 are pulled tight while observing through the monitor, and the knot is gradually pushed tight by the knot pushing pipe 20 to perform knotting. In order to ensure that the knotted knot is firm enough, the knotting operation in the section can be repeated to complete the triple knot.
The knot pushing pipe 20 and the two hooked forceps 9 are withdrawn, the scissors for the endoscope are inserted into the empty forceps channel of the endoscope, and the suture is cut off. And after the suturing is completely finished, withdrawing the endoscope and the auxiliary tool, and detaching the suturing device.
As shown in FIGS. 25 and 26, another embodiment of the present invention provides a stapler. The stapler 1' provided in the present embodiment has substantially the same structure as the stapler 1 provided in the previous embodiment. The same elements as those of the previous embodiment are given the same reference numerals, and only the differences will be described below. The components of the stitching instrument 1' of the embodiment are connected by laser welding, so that the arrangement of screws and lugs is omitted, the number of parts of the device is reduced, the production cost is reduced, and the assembly time is saved. Specifically, the upper shell 123 'of the suturing device provided by the present embodiment does not need to be provided with two upper fixing holes 1232, the lower shell 141' does not need to be provided with two lower fixing holes, and the elastic piece 143 'does not need to be provided with fixing holes, so that the second shell 112' does not need to be provided with three corresponding screw holes.
The "hole" mentioned in the present invention is not necessarily circular, and may be square, oval, etc., and all included in the scope of the "hole" to be expressed by the present invention. The terms "first" and "second" as used herein are used merely for convenience of description for distinguishing between names, and do not necessarily mean that the first and second must exist simultaneously. Claims (10)

1. A stapler, comprising:
a main housing;
one end of the suture needle is connected with a suture line, and the other end of the suture needle is a tip end;
the needle inserting component is arranged on the main shell and forms a separable clamping relationship with the suture needle so as to control the advance of the suture needle;
and a withdrawal prevention needle unit which is provided in the main casing and which prevents withdrawal of the suture needle by forming a detachable engagement with the suture needle.

The suturing device of claim 1, wherein the main housing has a sliding slot, the needle insertion assembly includes a pull cord, a top case, a spring, a needle insertion tooth and a sliding block, the pull cord passes through the sliding block and the top case, the needle insertion tooth is mounted in the sliding block, one end of the spring abuts against the surface of the needle insertion tooth, the other end of the spring abuts against the top case, the needle insertion tooth, the sliding block and the spring are mounted in the sliding slot of the main housing, and the top case covers the sliding slot.

a stapler fixed to a distal end of an endoscope, the stapler comprising:
a main housing;

the needle inserting component is arranged on the main shell and forms a separable clamping relationship with the suture needle so as to control the advance of the suture needle;
a retreat prevention needle assembly which is arranged on the main shell and forms a separable clamping relationship with the suture needle so as to prevent the retreat of the suture needle;
and the operating handle is fixed at the operating end of the endoscope and controls the needle inserting assembly so as to control the advance of the suture needle.

7. The treatment device of claim 5 further comprising a snare loop, the snare loop performing a cutting action of tissue at a distal end of the endoscope.
8. The treatment device of claim 5, further comprising a hook forceps that performs a tissue grasping action or a suture hook pulling action at a distal end of the endoscope.
9. A therapeutic system, comprising:
an endoscope is provided with a plurality of endoscope heads,
a treatment device used together with an endoscope, the treatment device comprises a stitching instrument and an operating handle,
a stapler is fixed to a distal end of the endoscope, the stapler including:
a main housing;
one end of the suture needle is connected with a suture line, and the other end of the suture needle is a tip end;
the needle inserting component is arranged on the main shell and forms a separable clamping relationship with the suture needle so as to control the advance of the suture needle;
a retreat prevention needle assembly which is arranged on the main shell and forms a separable clamping relationship with the suture needle so as to prevent the retreat of the suture needle;

The treatment system of claim 9, wherein the treatment device further comprises an attachment ring, a snare loop, and a grasper, the attachment ring being mounted on the endoscope, the snare loop performing a tissue cutting action at a distal end of the endoscope, the grasper performing a tissue grasping action or a suture hooking action at the distal end of the endoscope.
