技术背景:
风湿痹痛、跌打损伤、骨质增生是一种常见和多发病症,对中老年人尤为常见。此类病症患病周期长,极难治愈,只能采取缓解疼痛的办法予以解决。目前,对此类病症的治疗方法有三种,即:手术治疗、内服药物治疗、外贴膏药治疗。
1.手术治疗:主要是采取在病灶处进行切除的方法,但手术治疗对骨质增生不能彻底根除。此种治疗方法病人痛苦大,具有一定的风险性,治疗时间长,且治疗费用高。
2.内服药物治疗:一般采用的内服药物有如:跌打丸、大活络丸、骨质增生片等。内服药物治疗较为方便,能通过调理人体的内部机理进行治疗。但内服药物存在对病灶的治疗效果不显著,服药时间长,易对人体产生副作用,治疗费用高的不足。
3.外贴膏药治疗:外贴膏药的治疗主要是将药物的药效通过皮肤表面由表及里对病灶处进行直接治疗,是一种内病外治的方法,它能克服上述两种治疗方法的不足之处,且治疗方法简单,治疗费用低。传统的硬膏药剂系药物与固体或半固体的粘性基质混合,摊涂于裱背材上,供贴敷于皮肤上的外用剂型,中药硬膏剂称为膏药。硬膏药按基质可以分为三类:(1)以铅肥皂为基质,主要以高级脂肪酸铅盐为基质.如铅膏药,黑膏药、白膏药;(2)以橡胶混合为基质,如橡胶硬膏(俗称胶布);(3)以树脂为基质,系以树脂(如松香)或树脂与植物油加热熔合,再掺入药料混合而成,如松香膏等。硬膏药的种类按其作用性质可分为三类:(1)表皮硬膏,在表皮上固定敷料保护创伤的作用,如橡皮膏药(胶布)等;(2)皮内硬膏,硬膏内含有药物,在皮内产生局部止痛、收敛、镇静或刺激作用的硬膏,如伤湿止痛膏,黑膏药等;(3)皮下硬膏,硬膏中含有药物,能透过表皮,可产生深部或全身作用,如部分黑膏药等。但现有市场应用的硬膏药存在着组成复杂,各药物成份的拮抗对其药理作用,质量检查,制造技术均待科学的分析与阐明。某些硬膏药外贴尚存皮肤过敏,从皮肤上撕下硬膏药不能再敷贴患处的缺点。
发明内容:
本发明在于提供一种能克服传统硬膏药的不足,具有刺激性小,无局部过敏反应,直接贴敷,可反复撕贴,透皮吸收好,对风湿痹痛、跌打损伤、骨质增生所引起的急性和慢性疼痛止痛效果好的止痛膏。
本发明是通过如下技术方案来解决上述问题的。
该止痛膏是由下述重量百分比的原料制成的:
威灵仙1.10-1.30% 生川乌0.50-0.70% 生草乌0.50-0.70%
麻黄0.50-0.70% 元胡0.50-0.70% 鹿衔草0.50-0.70%
细辛0.20-0.40% 肉桂0.05 0.15% 蜈蚣1.10-1.30%
全蝎0.30-0.50% 毛姜0.30-0.50% 乳香0.30-0.50%
土鳖虫0.20 0.40% 没药0.30-0.50% 松香83.00-85.00%
甘油0.20-0.30% 液体石蜡0.20-0.30% 动物油7.30-7.70%
贻糖0.25-0.35%。
将上述的威灵仙、生川乌、生草乌、麻黄、元胡、鹿衔草、细辛、肉桂、蜈蚣、全蝎、毛姜、乳香、土鳖虫、没药这十四味药物粉碎成细末,过200目筛,混合均匀待用。取上述的松香用武火溶化,再以文火维持10-20分钟后,将上述十四味药物的细末掺入液态的松香基质中,搅拌均匀后,再加入甘油、液体石蜡、动物油、贻糖再次搅拌均匀后,趁热摊于牛皮纸上,特凉干成形后盖上玻璃纸,即得松奇止痛膏成品。
本发明的技术解决方案,是基于祖国医学对风湿痹痛、跌打损伤、骨质增生发病机理的研究和治疗原则,参考现代药理研究对内病外治的科研成果,按中医理论组方,打破传统用药禁忌,使其在药剂中具有消热解毒、祛风除湿、消炎解毒、通络止痛之功效。
本发明上述的原料组份中,每组份原料的药理及药效为:
威灵仙药理作用:有对抗组织胺作用,骨鲠后局部挛缩,用后即松驰,使骨松脱。
生川乌、生草乌主治用法:风湿性关节炎,类风湿性关节炎,大骨节病,半身不遂,手足拘挛,坐骨神经痛,跌打肿痛,胃腹冷痛,外用治牙痛,并作表面麻醉用,灭蝇蛆。
麻黄药理作用:兴奋中枢神经,对于大脑,中脑及延脑,呼吸与循环中枢,均有兴奋作用。
细辛药理作用:细辛挥发油对蛙的坐骨神经丛,豚鼠皮肤以及人舌粘膜均有局部麻醉作用。细辛挥发油能使麻醉动物血压下降,而煎剂则使血压上升。
肉桂药理作用:桂皮油对胃肠有缓和刺激作用,能增加消化机能,排除消化道内积气,缓解胃肠痉挛性疼痛,并有中枢性及末梢性扩张血管的作用,能增强血液循环。
蜈蚣药理作用:抗惊厥,对戊四氧,纯菸碱和硝酸土的宁碱所引起的惊厥有不同程序的对抗作用。对结核杆菌及常见的致病性皮肤真菌均有抑制作用。
元胡药理作用:镇痛作用和镇静作用。
全蝎药理作用:有一定的抗惊厥作用。
乳香主治:心腹诸痛、筋脉扬挛、跌打损伤、疮痛肿痛。
没药主治:跌打瘀血肿痛、痛疽肿痛、胸腹诸痛、外用治疮口久不收敛,本品功用与乳香似,故常相互配伍应用。
毛姜主治:胸腹胀痛、肩臂痹痛、月经不调、闭经、跌打损伤。
鹿衔草药理作用:本品浸剂蛙,狗及家有增强心脏收缩力的作用,心律不整时,可使其恢复正常。使血管显著扩张,而有降压作用。
土鳖虫主治:跌打损伤、瘀血肿痛、闭经、产后瘀血腹痛。
全方配伍后,与基质配方中的松香、甘油、液体石蜡、动物油、贻糖调和诸药,制成的膏药贴于患处,具有清热解毒、清肿止痛、祛风除湿、通络止痛的良好效果。
本发明的制剂与规格:硬膏剂,每贴净重20克。
本发明的用法与用量:外用。先将患处洗净,再将膏药揭开为度,平贴干净皮肤上,每隔3-5日更换一次。一个月为一个疗程,一般病症一个疗程即可治愈,重症者2-3个疗程即可治愈。
本发明的使用注意事项:(1)伤处皮肉破损者勿用,贴后皮肤发痒或起粟疹者暂停用。(2)本膏药不必用热温烤,贴后由体温自然软化起粘贴作用。
本发明的有益效果在于:
1.刺激性小,无局部过敏反应,避免了制备过程中高温产生的“火毒”所引起的局部刺激及过敏反应。
2.可以反重撕贴,克服了市场现用膏药贴于患处后撕下不能再敷贴的缺点,贴于患处后可将膏药撕下对患处进行某些特殊医疗处理后,再重贴于患处。
3.透皮吸收好,由于膏药基质成份的改变,使硬膏药内的药物在皮下能很好地透过表皮吸收,在皮内、皮下产生局部止痛、收敛、镇静或刺激作用及产生深部或全身作用。
为了表明本发明在治疗风湿痹痛、跌打损伤及骨质增生疼痛的治疗效果,本发明通过对508例患者进行系统临床观察,其结果如下:
1.一般资料
508例患者均为门诊病例。其中150例为风湿痹痛患者;178例为跌打损伤患者;180例为骨质增生患者。150例风湿痹痛患者中,男82例,女68例;年龄最小者为15岁,最大68岁,平均为38.2岁;病程最短者为三个月,最长者为30年;所有患者均表现为腰膝疼痛,骨节酸松,甚至肿胀,屈伸不利,每遇冷风加剧,西医疹断为风湿性关节炎、类风湿性关节炎,外伤性关节炎等。178例跌打损伤患者男为97例,女为81例;年龄最小的10岁,年龄最大的67岁,平均年龄37.6岁;病程最短的1.5小时,最长者为5年(属陈旧性外伤);所有患者均有不同程度的车祸,跌仆,闪扭等明确外史。180例骨质增生疼痛患者中,男为92例,女为98例;年龄最小31岁,最大为72岁;病程均在一年以上;所有患者均表现为不同程度的腰部胀痛,或膝关节疼痛,或足跟疼痛,或颈部疼痛不适,或兼见头晕,肢麻等症状,经西医疹断为颈椎骨质增生,增生性脊椎炎,腰椎骨质增生,跟骨骨刺等。
2.治疗方法
上述508例患者均用松奇止痛膏外贴治疼痛,先将患者患处洁净,再将松奇止痛膏贴敷于患处,5天换贴一次,三十日为一疗程。
3.治疗效果
3.(1)疗效标准:优:疼痛消退 良:疼痛基本消退 一般:疼痛有不同程度的缓解 差:疼痛无缓解。

制备方法及工艺流程
(一)基质原料的选择:
1、松香:松香是基质的主要成份,经验证明马尾松和松油所得的松香为最好,松香本身具有治痛疗疮疡、湿疹、外伤出血,烧烫伤等作用,制成的膏药色泽乌黑而有光泽,性粘;熬炼过程中泡沫少,上泛现象弱,便于操作,熬炼成膏药后损失少。
2、动物油:动物油以牛油或羊油为佳,其特点是制成的膏药易在体温下自然软化而粘贴于患处。
3、液体石蜡:液体石蜡以密度为(0.818-0.88)的轻质者较好,它调节基质的粘稠度效果好。
4、甘油:甘油的吸湿性很强,故在基质中起保温作用。
5、贻糖:以大米熬制的贻糖较好,在基质中起粘合作用。
(二)裱背材料(即膏药壳)的选择:
1.本品属于小膏药,故选用纸作为裱背材料。即用质地硬厚的牛皮纸制成圆形的状态,将基质与药物摊涂之上。
2.膏面覆盖材料:本品选用玻璃纸为膏面覆盖材料,用以避免相互粘膏及防止挥发性药物的散发。
(三)工艺流程:本品制备可实行系统的机械化生产工艺。也可用简单的机械装置(摊涂机)与手工相结合的生产方式。
(四)制备方法:
将处方中威灵仙、生川乌、生草乌、麻黄、元胡、鹿衔草、细辛、肉桂、蜈蚣、全蝎、毛姜、乳香、土鳖虫、没药这十四味药物,粉碎成细粉,过200目筛,混合均匀。取基质处方中的松香用武火溶化,再以文火维持10-20分钟后,将上述十四味药物的细末掺入液态的松香基质中,最后加入牛油、甘油、液体石蜡、贻糖再次搅拌均匀后,趁热摊于牛皮纸上,特凉干成形后盖上玻璃纸,即得松奇止痛膏成品。
(五)质量检查
1.外观性状,膏药乌黑发亮,油润细腻,老嫩适度,摊涂均匀,无飞边缺口,温烤或由体温自然软化能粘贴于皮上,且不移动。
2.本品每贴净重20克,重量差异限度在±6%。

典型病例:
病例一:张××,男,50岁,湖北省荆州市民政局干部,一九九八年开始右腿膝关节疼痛,每遇冷风加剧,痛处不能动,近日症状加重,于一九九八年七月就诊,经诊断为风湿性关节炎,用止痛膏敷贴二十天,疼痛消失,至今未见复发。
病例二:李××,女,60岁,湖北省荆州市荆北村三组农民。一九九七年十月开始腰痛,近来疼痛加剧,左下肢麻木,行走活动受限,经荆州市中心医院拍片诊断为腰椎盘骨质增生,服用骨质增生片三月,疼痛仍无明显好转,改用止痛膏贴敷二周后,疼痛明显减轻,继续贴敷三周后,疼痛基本消失。
病例三:胡××,男,31岁,荆州市荆州区太晖村二组农民,一九九八年十月左腿胫骨骨折,经沙市中医院骨伤科复位固定治疗,近二年来原伤处经常疼痛,劳累时加剧,经拍片诊断为陈旧性骨折,用止痛膏贴敷二周后,疼痛基本消失。
