Anti-asphyxia device for prone position ventilation therapy
Technical Field
The invention relates to the field of medical auxiliary instruments, in particular to an anti-asphyxia device for prone position ventilation therapy.
Background
Acute respiratory distress syndrome is a clinical syndrome caused by intrapulmonary and/or extrapulmonary causes, characterized by refractory hypoxemia, and is of great interest because of high mortality. The causes of acute respiratory distress syndrome are various, and the pathogenesis of acute respiratory distress syndrome caused by different causes is different. The clinical manifestations are acute onset, respiratory distress, hypoxemia difficult to be corrected by conventional oxygen therapy, and the like; at present, the diagnosis and severity stratification of ARDS are mostly internationally defined by Berlin definition, and the differential diagnosis of ARDS with various diseases is required. The clinical examination contents relate to: diagnosis and differential diagnosis, treatment monitoring and treatment guidance, critical degree and prognosis evaluation and the like; treatment of acute respiratory distress syndrome includes both mechanical and non-mechanical ventilation therapies, and effective treatments continue to be explored.
Treatment of acute respiratory distress syndrome includes both mechanical ventilation therapy and non-mechanical ventilation therapy. Mechanical ventilation is the primary treatment for patients with acute respiratory distress syndrome. According to different mechanical ventilation modes, the method can be divided into non-invasive ventilation and invasive ventilation, the non-invasive ventilation depends on a mask for ventilation, the invasive ventilation depends on a tracheal cannula or a tracheotomy catheter for ventilation, and the two modes need to depend on specific illness conditions to determine the time; at present, the mechanical ventilation strategy for patients with acute respiratory distress syndrome mainly comprises the following: lung protective ventilation strategy: such as small tidal volume ventilation, pressure-limited ventilation, permissive hypercapnia, inverse ventilation, PEEP applications, and the like; lung patency strategy: the specific technology comprises the following steps: pulmonary atelectasis, optimal PEEP application, and selection of mechanical ventilation modes; mechanical ventilation adjuvant therapy: such as intra-airway administration, prone position ventilation, extracorporeal pulmonary oxygenation technology and the like.
Patient prone position ventilation refers to a method in which a patient takes a prone position for mechanical ventilation. The medicine is an auxiliary measure for treating acute respiratory distress syndrome, and the mechanism of the medicine utilizes the action of gravity to increase the blood flow of the front chest and the ventilation of the back and improve the gas exchange. The ventilation method is mainly suitable for patients with severe pulmonary diseases, such as acute respiratory distress syndrome, and can not maintain oxygenation required by normal human bodies through high-intensity breathing machine support. The advantage of this ventilation is that it facilitates blood-gas exchange in the lungs close to the back, improving oxygenation. Typically, two to three treatments are administered a day, each lasting about 20-30 minutes, with care being taken to observe the patient's vital signs during the treatment.
Under the condition that auxiliary equipment is incomplete, the implementation process of prone position ventilation needs at least four or more medical workers to complete together, meanwhile, the risk of certain complications is increased due to the prone position ventilation, and accidents of patients occur in the prone position state, and the documents of emergency treatment methods are few. Therefore, it is recommended that ventilation in prone position should be implemented under sufficient human and material guarantee and complete risk prevention measures and emergency plans.
The existing solution is to set up air cushions at the lower side of the face and the body of the patient when the patient takes the prone position, and set corresponding grooves at the position of the pipeline to prevent the trachea and the like from being extruded, so as to play a preventive role, but does not provide the solution when the pipeline is extruded, and the patient still needs to be closely monitored by trained medical personnel, and the complexion and the vital sign of the patient can be observed at any time, so that the patient can find abnormality in time, and the safety of the prone position ventilation process can be ensured. To this end, we propose an anti-asphyxia device for prone position ventilation therapy.
