Description
Technical Field
The invention belongs to the technical field of auxiliary treatment devices for postoperative neck anastomosis fistula of esophageal cancer, and particularly relates to a double-negative-pressure drainage and flushing integrated device and a using method thereof.
Background
The statements herein merely provide background information related to the present disclosure and may not necessarily constitute prior art.
Anastomotic fistulas are the most common complications of esophageal cancer surgery, severely affect postoperative recovery of patients and can even lead to death, and have plagued clinicians for many years. Most of the current methods for treating the neck anastomotic fistula are to immediately remove a neck incision suture, open drainage, debride and change medicines once or for many times every day, keep the periphery of the anastomotic fistula dry, and promote granulation growth near the anastomotic fistula until the anastomotic fistula is healed. Fasting, gastrointestinal decompression, infection control, enteral parenteral nutrition support therapy are given simultaneously. Because digestive juice and necrotic tissue accumulate around the fistula, the anastomotic stoma is always immersed in the digestive juice and necrotic tissue, which can cause local infection, inflammation and corrosion of surrounding tissues, even has the risk of corroding neck blood vessels and heavy bleeding, thereby causing a series of problems of slow healing of the anastomotic stoma, large psychological pressure of patients, long hospitalization time, more dressing change times, increased hospitalization cost and the like, and simultaneously aggravating the workload of doctors.
Disclosure of Invention
Aiming at the defects in the prior art, the invention aims to provide a double-negative-pressure drainage and irrigation integrated device and a method.
In order to achieve the purpose, the invention is realized by the following technical scheme:
in a first aspect, the invention provides a dual negative pressure drainage and flushing integrated device, which comprises a first negative pressure drainage and flushing component and a second negative pressure drainage and flushing component, wherein,
the first negative pressure drainage flushing component comprises a first flushing pipe, a first drainage pipe and a first negative pressure drainage bottle connected with one end of the first drainage pipe, and the first negative pressure drainage bottle is detachably connected with the first drainage pipe;
one end of the first flushing pipe is fixedly connected with the free end of the first drainage pipe, and the other end of the first flushing pipe is used for connecting an injector;
the second negative pressure drainage flushing component comprises a second flushing pipe, a second drainage pipe and a second negative pressure drainage bottle, and the connection relation of the second negative pressure drainage flushing component is the same as that of each element of the first negative pressure drainage component.
In a second aspect, the present invention is further directed to a dual negative pressure drainage and intermittent irrigation method, wherein different irrigation methods are available according to the disease development of an anastomotic fistula, including the fistula size, tissue adhesion and partition in the fistula cavity, and the region that a doctor wants to irrigate with emphasis, such as a large amount of stomach contents and necrotic objects in the fistula cavity at the early stage of the fistula, the irrigation range is large, and the fistula cavity and the periphery of the anastomotic stoma are irrigated with emphasis, and the irrigation fluid can be alternately irrigated through a first irrigation tube and a second irrigation tube and adjusted through a buckle, and can have a plurality of different flow and irrigation directions and be respectively/simultaneously discharged from corresponding drainage/suction ports through two drainage tubes. In the late stage of fistula, local adhesion exists in the fistula cavity, the washing range is not too large, and the two washing/drainage tubes wash corresponding areas respectively. The method comprises the following specific steps:
the free ends of the first drainage tube and the first flushing tube are arranged at the fistula, and the free ends of the second drainage tube and the second flushing tube are arranged at the lowest position in the fistula cavity or the position where effusion in the fistula cavity is remained and the drainage is least easy;
the flushing liquid is injected into the fistula cavity through the first flushing pipe or/and the second flushing pipe, and can have different flushing directions by adjusting the corresponding buckles, and is respectively/simultaneously discharged from the corresponding drainage/suction ports through the first drainage pipe and/or the second drainage pipe, so that digestive juice, necrotic substances and purulent secretion in the fistula cavity are discharged.
In addition, in the double negative pressure suction process, as the fistula cavity is communicated with the alimentary canal, the negative pressure in the drainage bottle and the drainage tube is gradually reduced until the negative pressure disappears, so that the device designs an assembly and a method for generating the negative pressure in the drainage bottle. By adjusting the snaps (closing snaps 1, 3, opening snaps 2, 4), air within the drainage bottle is drawn through the first/second drainage/suction ports with the syringe, thereby creating negative pressure within the drainage bottle, after which dual negative pressure suction continues.
One or more implementations of the invention described above achieve the following benefits:
the two sets of negative pressure flushing and drainage assemblies are arranged, when the free ends of the first flushing pipe and the first drainage pipe which are one set are arranged at the fistula, the leakage liquid at the fistula can be better drained, the amount of the leakage liquid entering the neck incision cavity is reduced, the detention time of the leakage liquid in the cavity can be effectively reduced, and the infection can be prevented from further spreading. Moreover, set up the free end of first flushing pipe in fistula department, clean flush fluid can wash the fistula in the very first time to quick control infects and keeps the cleanness of fistula department through the last negative pressure drainage afterwards, and then helps the healing of fistula.
Because the fistula cavity is large, only one drainage tube is adopted to difficultly drain the leaked liquid and the flushing liquid in the fistula cavity fully, so that a second drainage tube and a second flushing tube are arranged and arranged at the lowest part of the fistula cavity or the position where the accumulated liquid is most easily accumulated. The advantage of two negative pressure drainage and washing lies in that can carry out more abundant drainage with the digestive juice in the fistula intracavity and necrotic thing etc. when the negative pressure is attracted, and can adopt two washing pipes and have multiple washing mode thereby carry out more abundant washing with the fistula intracavity when washing.
In addition, two groups of negative pressure flushing and drainage components are adopted, a large amount of exudates and inflammatory necrosis substances can be led out, the drainage is more sufficient, no dead angle is left, the infection can be controlled as soon as possible, further spread of the infection is effectively prevented, and finally the fistula can be healed as soon as possible.
When only setting up the drainage tube, the material such as gastric content, the necrotic tissue of drainage gets into in the drainage tube to partly adhere on the drainage tube inner wall, if adopt the drainage tube to inject the flush fluid into the fistula when rethreading, intraductal material can be washed backward and get into the fistula intracavity, does not accord with aseptic technique requirement, is unfavorable for the control of infection and fistula healing to a certain extent. Therefore, the device is provided with the flushing pipe independently, the flushing pipe is only used for injecting flushing liquid into the fistula cavity, the sterile environment can be kept within a certain time range, and the fistula recovery is facilitated.
The fistula cavity can be in a negative pressure state by adopting negative pressure drainage, tissue hyperplasia and wound surface adhesion are facilitated, the pus cavity is gradually reduced and finally closed, and the healing period of the fistula opening is obviously shortened. In addition, the negative pressure drainage bottle is adopted for drainage, so that the measurement of drainage liquid is convenient, and the color and the property of the drainage liquid are easy to observe.
The drainage tube and the flushing tube are thin and soft, have good tolerance, and compared with the open dressing change drainage tube, the utility model can significantly reduce the pain of patients and improve the life quality during the treatment period.
The two functions of the drain/suction port are: 1) during flushing, the drainage residual liquid is discharged by adjusting the buckle and taking the port as a water outlet; 2) since the negative pressure is slowly disappeared (sometimes 1 to 2 hours or less) during the negative pressure suction process, particularly at an early stage of fistula, the negative pressure is sucked through the drain/suction port by the syringe by adjusting the snap, and then the double negative pressure suction is continued.
