Abstract
本发明公开了一种治疗面神经麻痹的中药组合物、中药制剂及其制备方法,属于中医药领域,解决了中药处方药味多、用量大,以及容易引起过敏反应的技术问题。该治疗面神经麻痹的中药组合物包括附子10‑15份、肉桂6‑9份、路路通10‑15份、细辛3‑5份、干姜6‑10份、猪牙皂10‑15份。该治疗面神经麻痹的中药组合物不仅在临床治疗面神经麻痹病症方面具有疗效显著、疗程短、毒副作用小以及不复发的特点,而且以六种药味配伍组合,具有处方简单、药味少、用量小的特点,降低了面神经麻痹的治疗成本。

技术领域
本申请实施例涉及中医药技术领域,尤其涉及一种治疗面神经麻痹的中药组合物、中药制剂及其制备方法。
背景技术
面神经麻痹又叫贝尔麻痹,是因茎乳孔内面神经非特异性炎症所致的周围性面瘫,确切病因尚未完全明确。激发因素可能是风寒、病毒感染和自主神经功能不稳等引起局部的神经营养血管痉挛,导致神经缺血水肿而发病。中医学认为,面神经麻痹属于“面瘫”范畴,是因为头面部经脉空虚,外邪乘虚侵袭导致经脉阻滞不通、筋肉失于约束、弛缓不收而出现面瘫,临床表现为面部突然出现麻木、板滞感、肌肉松弛、单侧额纹消失、皱眉和鼓腮困难或漏气、口角歪斜、口角夹食,以及耳后疼痛、味觉减退等症状。
目前,中医通常认为面神经麻痹的基本病机是风痰阻络、气血痹阻,因而普遍以祛风化痰为治则,方选“牵正散加减”进行治疗,并取得了较好的治疗效果。
但是,牵正散以全蝎、蜈蚣、白附子、白僵蚕、黄芪、防风、荆芥、鸡血藤、秦艽、赤芍、川芎组成基础方,基础方的药味达11种之多,处方复杂、药味多且用量大,导致面神经麻痹的治疗成本高。同时,牵正散治疗面神经麻痹疗效好的原因是白附子辛散善治头面之风;白僵蚕可化痰驱络中之风;全蝎定风解痉,能缓经络拘急,因白附子、白僵蚕和全蝎均有一定毒性,容易引起过敏反应,例如皮肤损伤。
有鉴于此,有必要对现有治疗面神经麻痹的中药处方进行改进,以解决现有治疗面神经麻痹的中药处方药味多、用量大,以及容易引起过敏反应的问题。
发明内容
本申请实施例通过提供一种治疗面神经麻痹的中药组合物、中药制剂及其制备方法,解决了现有治疗面神经麻痹的中药处方药味多、用量大,以及容易引起过敏反应的问题。
第一方面,本申请实施例提供了一种治疗面神经麻痹的中药组合物,包括以下重量份的原料:附子10-15份、肉桂6-9份、路路通10-15份、细辛3-5份、干姜6-10份、猪牙皂10-15份。
结合第一方面,在本申请实施例的一些优选实施方式中,该治疗面神经麻痹的中药组合物包括以下重量份的原料:附子12-15份、肉桂7-9份、路路通12份、细辛4-5份、干姜7-10份、猪牙皂12-15份。
结合第一方面,在本申请实施例的一些优选实施方式中,该治疗面神经麻痹的中药组合物包括以下重量份的原料:附子12份、肉桂8份、路路通12份、细辛4份、干姜8份、猪牙皂12份。
在上述研究基础上,本申请实施例还提供了该治疗面神经麻痹的中药组合物在制备治疗面神经麻痹的药物中的应用。
第二方面,本申请实施例还提供了一种治疗面神经麻痹药物,由所述治疗面神经麻痹的中药组合物和药学上可接受的辅料制成。
结合第二方面,本申请实施例所述治疗面神经麻痹的药物的剂型包括膏剂、凝胶、涂膜剂、乳剂、贴剂。
第三方面,本申请实施例还提供了一种治疗面神经麻痹药物的制备方法,制成膏剂包括以下步骤:
对原料进行混合并粉碎成粉末状态;
对粉末状态的混合原料进行热解气化,并收集气化原料;
将气化原料密封在散热容器中,然后埋入地下凝华成固态药粉;
对固态药粉加入水或酒精并混合均匀,即得治疗面神经麻痹药物膏剂。
结合第三方面,在本申请实施例可能的优选实施方式中,密封气体原料的散热容器埋入地下深度为1-1.5m。
结合第三方面,在本申请实施例可能的优选实施方式中,密封气体原料的散热容器埋入地下100天。
结合第三方面,在本申请实施例可能的优选实施方式中,其特征在于,所述散热容器为红泥瓦罐。
与现有技术相比,本申请实施例的有益效果或优点包括:
本申请实施例提供的治疗面神经麻痹的中药组合物,通过附子、肉桂、路路通、细辛、干姜和猪牙皂进行配伍组合,其中,附子、猪牙皂为君药,以附子回阳救逆、补火助阳、散寒止痛,以猪牙皂解毒燥湿、杀虫补血;细辛为臣药以解表散寒,祛风止痛,通窍,温肺化饮;肉桂、干姜为佐药以补火助阳、引火归元、散寒止痛、温通筋脉、温中散寒、回阳通脉、温肺化饮;以及路路通为使药以祛风活络,利水,通经。以上诸药配伍共奏解表散寒、祛风止痛、通窍活血的功效,从而快速、高效地治疗面神经麻痹。临床药效实验表明,该中药组合物治疗面神经麻痹的痊愈率为91.1%,总有效率为97.1%,且在21天的疗程内的治愈率为66.2%;同时,在治疗期间没有出现局部药物刺激性皮肤损伤、局部及全身性过敏等毒副作用。因此,该中药组合物治疗面神经麻痹具有疗效显著、疗程短、毒副作用小以及不复发的特点。除此之外,该中药组合物以六种药味配伍组合,具有处方简单、药味少、用量小的特点,降低了面神经麻痹的治疗成本。
具体实施方式
为使本技术领域的人员能更好地理解本申请实施例的技术方案,下面将对本申请实施例中的技术方案进行清楚、完整地描述。显然,以下描述的实施例只是本申请的一部分实施例,而不是全部的实施例。基于本申请中的实施例,本领域普通技术人员在没有做出创造性劳动前提下所获得的所有其他实施例,都应当属于本申请保护的范围。
以下实施例描述中所涉及的中药原料均为市售药材。
本实施例提供的治疗面神经麻痹的中药组合物,通过附子10-15份、肉桂6-9份、路路通10-15份、细辛3-5份、干姜6-10份、猪牙皂10-15份制备成。上述处方中,附子、猪牙皂为君药,其中附子回阳救逆、补火助阳、散寒止痛,猪牙皂解毒燥湿、杀虫补血;细辛为臣药,以解表散寒,祛风止痛,通窍,温肺化饮;肉桂、干姜为佐药,以补火助阳、引火归元、散寒止痛、温通筋脉、温中散寒、回阳通脉、温肺化饮;路路通为使药,以祛风活络,利水,通经。以上诸药配伍共奏解表散寒、祛风止痛、通窍活血的功效,从而快速、高效地治疗面神经麻痹。
各药味的具体药理作用如下:
附子:其性大热,既具有回阳救逆、补火助阳、散寒止痛的功效,又具有扩张血管,增加血流,改善血液循环及抗炎作用;
肉桂:其性大热,既具有补火助阳、引火归元、散寒止痛、温通经脉的功效,又具有扩张动物外周血管,增加冠脉和脑血流量及抗菌作用;
细辛:既具有解表散寒、祛风止痛、通窍、温肺化饮的功效,又具有镇静、镇痛、抗感染、抑菌、抗变态反应及促进呼吸系统和循环系统的作用;
干姜:既具有温中散寒、回阳通脉、温肺化饮的功效,又具有镇痛、抗炎及明显抑制血小板聚集的作用;
猪牙皂:在体外对多种细菌具有明显的杀灭和抑制作用。
路路通:祛风活络,利水,通经。用于关节痹痛,麻木拘挛,水肿胀满。路路通中的没食子酸通过维持钙稳态及营养神经细胞发挥神经保护作用。
以上药物配合不仅具有解表散寒、祛风止痛、通窍活血的功效,而且具有扩张血管、促使机体新陈代谢的作用。
在本申请实施例的一些具体实施例中,该治疗面神经麻痹的中药组合物是由附子12份、肉桂8份、路路通12份、细辛4份、干姜8份、猪牙皂12份制备成。
在本申请实施例的一些具体实施例中,该治疗面神经麻痹的中药组合物是由附子15份、肉桂9份、路路通15份、细辛5份、干姜10份、猪牙皂15份制备成。
在本申请实施例的一些具体实施例中,该治疗面神经麻痹的中药组合物是由附子10份、肉桂6份、路路通10份、细辛3份、干姜6份、猪牙皂10份制备成。
下面结合具体实施例对本发明作进一步阐述。
实施例1
本实施例1提供一种治疗面神经麻痹药物膏剂,通过以下步骤制备成:
S101:取附子12kg、肉桂8kg、路路通12kg、细辛4kg、干姜8kg、猪牙皂12kg混合,然后通过粉碎机碾碎为粉末,备用;
S102:将药物粉末置于循环流化床气化炉中进行热解,并以红泥瓦罐收集热解生成的气态药物;
S103:将充满气态药物的红泥瓦罐进行密封之后埋入地下1m处,经100天后取出,得到固态药粉;
S104:向固态药粉中加水混合,制备成治疗面神经麻痹药物膏剂。
实施例2
本实施例2提供一种治疗面神经麻痹药物膏剂,通过以下步骤制备成:
S101:取附子15kg、肉桂9kg、路路通15kg、细辛5kg、干姜10kg、猪牙皂15kg混合,然后通过粉碎机碾碎为粉末,备用;
S102:将药物粉末置于循环流化床气化炉中进行热解,并以红泥瓦罐收集热解生成的气态药物;

S101:取附子10kg、肉桂6kg、路路通10kg、细辛3kg、干姜6kg、猪牙皂10kg混合,然后通过粉碎机碾碎为粉末,备用;

实施例4
本实施例4提供了上述实施例1-3制备成的治疗面神经麻痹药物膏剂在临床治疗面神经麻痹中的应用效果,具体包括:
一、病例资料
1.诊断标准
证见突然口眼歪斜,眼睑闭合不全或面部抽搐,颜面麻木作胀,伴有鼻唇沟变浅,口角流涎,舌质暗,苔薄腻或厚腻,脉弦滑;脑CT、MRI检查正常。
2.纳入标准
临床症状符合诊断标准;病情在发病后1-3天内达到高峰;特发性面神经麻痹未经治疗或患病后经治疗一周以上无明显疗效者。
3.排除标准
中枢性面神经麻痹或小脑桥脑角病变、脑干病变、手术损伤、肿瘤压迫、耳病、腮腺病、格林巴利综合征所致的周围性面神经麻痹者;特发性面神经麻痹在外院或应用其他方法治疗一周以上有明显疗效者;患有急慢性传染病、恶性肿瘤、肝肾功能障碍的患者,妊娠期、哺乳期妇女,生命体征不稳的患者。
4.病例资料
回顾研究2005年1月至2020年12月间住院和门诊病患资料。本次统计特发性面神经麻痹病例共计450例。
5.评价标准
治愈:症状完全消失,口眼端正,鼻唇沟对称,皱眉、闭目、露齿、鼓腮、噘嘴等均恢复正常;
好转:症状、体征改善,但遗留不同程度的面肌功能障碍,如外观面部有轻微不对称,眼睑闭合欠佳,大笑时口角稍歪斜等;
无效:症状、体征无改善。
6.治疗方法
6.1制备穴内贴:将实施例1制成的治疗面神经麻痹药物膏剂涂布于直径为1cm的穴位贴内,制成治疗面神经麻痹穴位贴,备用。其中,治疗面神经麻痹药物膏剂用量为0.2-0.3g/贴;
6.2用药方法:选取健侧翳风、听会、下关、阳白、四白、迎香、地仓、颊车、颧舅缪等穴位,并对各穴处皮肤进行酒精常规消毒或用皮肤渗透剂擦试2-3遍,然后将治疗面神经麻痹穴位贴敷贴于各穴位处,并在必要时外覆医用胶带加固。敷贴4-6h之后摘除,每24h敷贴1次,7次为1个疗程。如有皮肤不适等情况,可隔日敷贴,或休息一周后再进行第二疗程。重复治疗者按上法再次贴敷。
7.治疗结果
本实施例在治疗7个疗程之后统计治疗情况,其结果如下表1。

通过表1可以看出,该治疗面神经麻痹药物膏剂应用于临床治疗面神经麻痹,450例病例经过7个疗程治疗之后,410例痊愈,痊愈率为91.1%;好转27例,好转率为6%;无效13例,无效率为2.9%;总有效率为97.1%。而且在410例痊愈病例中,第1疗程内痊愈32例,占比为7.1%;第2疗程内痊愈126例,占比为28%;第3疗程内痊愈140例,占比为31.1%;第4疗程内痊愈36例,占比为8%;第5疗程内愈者32例,占比为7.1%;第6疗程内痊愈23例,占比为5.1%;第7疗程内痊愈21例,占比为4.7%,其中,在前3个疗程内治愈298例,占比为66.2%。由此可见,该治疗面神经麻痹药物膏剂在临床治疗面神经麻痹病症中具有疗效显著、治愈率高、治疗时间短等特点。
实施例2、实施例3制备的治疗面神经麻痹药物膏剂在临床治疗面神经麻痹病症中,治疗效果与实施例1基本相同,具有可比性。
通过以上描述,本实施例的治疗面神经麻痹的中药组合物具有如下特点:
1.配伍科学:该中药组合物由附子、细辛、肉桂、干姜、猪牙皂和路路通组成,全方诸药配伍具有解表散寒、祛风止痛、通窍活血的功效,其针对面神经麻痹病证进行外治,其中,附子气味俱厚,回阳救逆,散寒止痛,可在局部发挥猛力治疗;细辛解表散寒,祛风止痛;肉桂、干姜,温经散寒,通经走窜,芳香开窍,活血通络,并促进药物吸收;猪牙皂,解毒燥湿、杀虫补血,诸药合用外治以达药力专、吸收快、收效速的效果。
2.毒副作用小:该治疗面神经麻痹的中药组合物药味成分简单,剂量小,且处方减少了毒性药味的应用,患者依从性好,具有毒副作用小的特点。在临床实验中,未出现局部药物刺激性皮损和局部及全身性过敏等毒副作用;每次贴后局部有轻微灼热感与微弱刺痛,1-2天即可消失,不需要中断贴敷治疗或特别处理。
3.经济实用:该治疗面神经麻痹的中药组合物由六种常见药材组成,且各药材用量较小,药源丰富,治疗费用低,易于推广,具有先进性和可行性。

1.一种治疗面神经麻痹的中药组合物,其特征在于,包括以下重量份的原料:附子10-15份、肉桂6-9份、路路通10-15份、细辛3-5份、干姜6-10份、猪牙皂10-15份。
2.根据权利要求1所述的治疗面神经麻痹的中药组合物,其特征在于,包括以下重量份的原料:附子12-15份、肉桂7-9份、路路通12份、细辛4-5份、干姜7-10份、猪牙皂12-15份。
3.根据权利要求1所述的治疗面神经麻痹的中药组合物,其特征在于,包括以下重量份的原料:附子12份、肉桂8份、路路通12份、细辛4份、干姜8份、猪牙皂12份。
4.一种根据权利要求1-3任一所述的治疗面神经麻痹的中药组合物在制备治疗面神经麻痹的药物中的应用。
5.一种治疗面神经麻痹药物,其特征在于,由权利要求1-3任一所述治疗面神经麻痹的中药组合物和药学上可接受的辅料制成。
6.根据权利要求5所述的治疗面神经麻痹药物,其特征在于,所述治疗面神经麻痹的药物的剂型包括膏剂、凝胶、涂膜剂、乳剂、贴剂。
7.一种根据权利要求6所述的治疗面神经麻痹药物的制备方法,其特征在于,制成膏剂包括以下步骤:
对原料进行混合并粉碎成粉末状态;
对粉末状态的混合原料进行热解气化,并收集气化原料;
将气化原料密封在散热容器中,然后埋入地下以得到固态药粉;
对固态药粉加入水或酒精并混合均匀,即得治疗面神经麻痹药物膏剂。
8.根据权利要求7所述的治疗面神经麻痹药物的制备方法,其特征在于,密封气体原料的散热容器埋入地下深度为1-1.5m。
9.根据权利要求7所述的治疗面神经麻痹药物的制备方法,其特征在于,密封气体原料的散热容器埋入地下100天。
10.根据权利要求7-9任一所述的治疗面神经麻痹药物的制备方法,其特征在于,所述散热容器为红泥瓦罐。
