Description
Technical Field
The invention relates to the technical field of mammary gland detection equipment, in particular to an auxiliary positioning device special for mammary gland molybdenum target examination, and further relates to a positioning puncture suite matched with the auxiliary positioning device special for mammary gland molybdenum target examination.
Background
The mammary gland molybdenum target is called mammary gland molybdenum target X-ray radiographic examination, is a commonly used examination method for clinically detecting mammary gland diseases, is a simple, reliable and noninvasive examination method for diagnosing the mammary gland diseases, has the advantages of high resolution, good repeatability, capability of comparing left and right images and the like, and is particularly suitable for examining mammary gland masses which can not be touched by doctors (for example, mammary gland masses can not be touched by large-breast and fat-type breast doctors); for early-stage breast cancer which takes a little micro-calcification as the only expression, the breast molybdenum target has higher sensitivity and specificity, is an important basis for early diagnosis and identification of the breast cancer, and also provides an instructive basis for subsequent treatment.
Currently, a breast molybdenum target generally requires a patient to stick a breast on a breast molybdenum target machine, then the breast is compressed and flattened through a pair of clamping plates, a radioactive source irradiates downwards from the top end to shoot the internal structure of the breast into a plane, and the shot image is observed, wherein if calcification, fibrosis and other conditions exist in the breast, a high-density image (such as a star-shaped or radial shape) appears in the shot plane image. Because the breast is photographed as a planar image, the tissue structure in the plane can be photographed, but the lesion vertical to the plane may overlap, so the breast is usually clamped and photographed after being changed in angle (for example, moved 15 degrees to the left or right), and the missed puncture caused by the overlapping of the tumor in the vertical plane of the breast of the patient is prevented.
At present, the common mammary gland molybdenum target is only positioned on an X-ray picture but not positioned on the surface of a mammary gland body (namely a breast), and when the position of the mammary gland is changed, the shape of the mammary gland molybdenum target is changed and is inconsistent with the positioning point during shooting, so that subsequent puncture failure is easily caused. In addition, some patients' mammary glands are small or soft, and are difficult to place on the shooting clamping plate, the mammary glands cannot be well exposed and laid on the shooting plate smoothly, only one end of the mammary gland can be pulled, and the mammary gland can be flattened by hands, but when the shooting position is changed, the mammary gland retraction can greatly influence shooting.

When in use, the auxiliary positioning device is arranged on a molybdenum target mammary machine, namely, the lower clamping plate 100 is arranged on a fixed arm of the molybdenum target mammary machine, and the upper clamping plate 200 is arranged on a movable arm of the molybdenum target mammary machine; then, the patient sits in front of the molybdenum target breast machine, the breast to be detected of the patient is placed on the lower splint 100, the upper splint 200 moves downwards by operating the molybdenum target breast machine, and the breast of the patient is manually pressed by matching with medical staff, so that the upper splint 200 and the lower splint 100 clamp the breast of the patient together to be unfolded; in the process of extending the breasts, the suction ball bowl 410 can be buckled on the nipple of the patient, the suction button 700 is started to continuously suck the nipple of the patient, and the horizontal sliding rod 320 is adjusted according to the extension degree of the breasts of the patient, so that the breasts can be extended forwards, and the retraction of the breasts is avoided; then, the molybdenum target breast machine can be operated to carry out the first X-ray shooting on the extended breast, and after the shooting is finished, the first lower dyeing rack 133 and the first upper dyeing rack 223 can be controlled by the first control button 500 to carry out the first point-painting marking on the surface of the breast; the angle is changed to perform the second breast extension, the operation is the same as the above, it is noted that the suction ball bowl 410 continuously sucks the nipple of the patient in the whole process of the second breast extension, and the suction ball bowl 410 is hinged on the fixed seat 420 in a universal manner, so that the nipple of the patient cannot be twisted even if the lower splint 100 and the upper splint 200 rotate at the angle, and the pain of the patient is relieved; after the second time of X-ray shooting is finished, the second lower dyeing rack 134 and the second upper dyeing rack 224 can be controlled by the second control button 600 to perform second time of point marking on the surface of the breast body; then, medical staff can judge whether the mammary gland is diseased according to the images shot by the X-ray twice and the positioning painted points on the surface of the breast of the patient, and accurately determine the diseased part.
In addition, the invention also comprises a positioning puncture kit for positioning and puncturing the mammary gland, which is applied to the auxiliary positioning device special for the mammary gland molybdenum target examination in claim 8; specifically, as shown in fig. 2 and 3, the positioning puncture kit includes a puncture needle 810, a sheath 820 and a fixing hook 830; the puncture needle 810 is internally provided with a puncture channel consistent with the axis of the puncture channel, a puncture needle core capable of being movably pulled out is arranged in the puncture channel in an initial state, the sheathing 820 can be arranged in the puncture channel of the puncture needle 810 in a matched and penetrating manner, namely, after the puncture needle core is pulled out, the sheathing 820 can be easily penetrated, the sheathing 820 is provided with an operation channel consistent with the axis of the sheathing 820, the fixing hook 830 is arranged in the sheathing 820 in a matched and penetrating manner, the lower end of the fixing hook 830 is provided with an elastic expansion hook claw 831 for tightly hooking a breast lesion part, the fixing hook 830 can be pushed forwards, the elastic expansion hook claw 831 at the front end of the fixing hook can extend out of the sheathing 820, and therefore, the clamping hook can be outwards expanded at the breast lesion part of a patient under the elastic action. Furthermore, in order to prevent the upper end of the fixing hook 830 from extending out of the breast to a longer extent, the upper portion of the rod body 832 of the fixing hook 830 can be coaxially and fixedly connected by a plurality of sections of connecting rods 833 which are sequentially screwed end to end, and a bottom baffle 834 with a diameter larger than that of the sheathing 820 is arranged at the upper end of the uppermost connecting rod 833, so that after the fixing hook 830 extends into a clamping hook mammary gland lesion in the breast of a patient, the part of the connecting rod 833 exposed out of the breast can be detached, the fixing hook 830 can not extend out of the breast of the patient to a too long extent, and the fixing hook can be directly pasted with a sterile paste to cover the puncture part.
After the molybdenum target of the mammary gland is detected and determined to be a specific mammary gland lesion part, the puncture operation can be carried out, the mammary gland lesion part is judged according to an X-ray image shot by the molybdenum target of the mammary gland examination, the puncture through hole 230 corresponding to the mammary gland lesion part is punctured, and firstly, the puncture needle 810 with the puncture needle core penetrates into the breast of the patient until the needle tip reaches the lesion part; then the puncture needle core is pulled out, and the sheath 820 with the fixed hook 830 is penetrated into the puncture needle 810; then the fixing hook 830 is pushed forward, so that the elastic expansion hook 831 at the lower end of the fixing hook 830 extends out of the sheath 820 and the puncture needle 810, and the elastic expansion hook 831 can expand the hook outwards under the self elasticity to be located at the breast lesion of the patient; then the puncture needle 810 and the sheathing 820 are pulled out, the connecting rod 833 of the redundant part of the fixing hook 830 exposed outside the breast of the patient is detached, and the last connecting rod 833 with a bottom baffle 834 is screwed on the tail end of the connecting rod 833, so that the length of the whole fixing hook 830 exposed outside the breast of the patient is short; then the aseptic application can be directly covered to complete the puncture operation.
