Description
Technical Field
The invention belongs to the technical field of traditional Chinese medicine preparations, and particularly relates to a traditional Chinese medicine for treating alopecia areata, androgenetic alopecia and leukotrichia and a preparation method of the traditional Chinese medicine.
Background
The cause of alopecia areata is unclear. The disease is caused by the infiltration of lymphocytes around hair follicles and is sometimes combined with other autoimmune diseases such as vitiligo and atopic dermatitis, so that the occurrence of the disease is considered to have a possible autoimmune pathogenesis. Genetic predisposition is also an important factor, possibly associated with HLA type ii, with a family history in 25% of cases. In addition, it may be associated with nerve trauma, psychiatric disorders, infectious disease and endocrine disorders.
Alopecia areata can occur at any age, but is common in young and strong years, and the incidence rate of both sexes is not obviously different. Skin lesions appear as round or oval non-scarring alopecia, with "exclamation mark" -like hair often visible at the margin of alopecia areata. Hair falls completely or almost completely, called alopecia totalis. All hairs (including body hair) are lost throughout the body, called alopecia universalis. Creeping alopecia can also be seen. The skin of the affected area has no other abnormalities except for hairless skin.
The pathogenesis is not well understood. The hair follicle is damaged and scars are formed due to various reasons, so that permanent baldness is generated. Alopecia areata is an autoimmune disease that has been more prevalent in recent years. Alterations in humoral immunity include non-organ specific autoantibodies and organ specific autoantibodies. Anti-smooth muscle antibodies, antinuclear antibodies (mainly of the spot type), anti-mitochondrial antibodies, anti-basement membrane antibodies, and rheumatoid factors have been reported. Organ-specific autoantibodies have been reported to be antithyroid antibodies, anti-parietal cell antibodies, anti-adrenal gland antibodies, and the like. The rate of positive autoantibodies is related to the sex, age and severity of the condition of the patient. The cellular immunity changes into a large amount of lymphocyte infiltration around blood vessels and hair bulbs of alopecia areata skin lesions. The study also found that the hair follicles in the bald area have strong HLA class I, II immunoreactivity, and adhesion molecules involved in the migration of hematopoietic cells are abnormally expressed. These changes may be caused by cytokine release resulting from certain stimuli such as trauma, neuroinflammation or infection. Cytokines can be involved in a variety of immune response processes leading to the development of an immune response against antigens unique to the hair follicle. In recent years, studies on various immune-modulation-related cytokines in alopecia areata have been spotlighted. Although the mechanisms described above are prevalent, the exact etiology is unclear and no definitive autoantigens and virulence genes have been demonstrated. The genetic type of hair loss is not known. Polygenic genetic patterns with different rates and degrees of expression appear to be reasonable but are still affected by additional hormones and immunity.
Androgenetic alopecia, also known as seborrheic alopecia, tinea pedis, premature alopecia, sexual alopecia, male alopecia, diffuse alopecia, and alopecia universalis, is associated with heredity, androgenic alopecia, and seborrhea, and is commonly used in medicine such as seborrheic alopecia. The symptoms are that the grease secretion of the scalp is excessive, and the hair has greasy feeling. The clinical manifestations are that the scalp fat of the patient overflows excessively, which causes the scalp to be greasy and moist, and the dust and the scurf are mixed, so the scalp is dirty without washing the head for a few days and gives off odor, especially when the temperature is high; scalp itching and inflammation are also sometimes caused, mainly because of scalp dampness and bacterial multiplication infection to cause seborrheic dermatitis. Alopecia generally starts from the middle of the two forehead corners, forehead and vertex and then spreads over the vertex, but the hair around the head is not so much lost. In severe cases, the bald area becomes oily and shiny, and the remaining hair becomes thin, soft, withered and yellow.
The alopecia is common in men and rare in women, and brainworkers are more than physical workers. Clinically, it is likely to be related to endocrine function of the body, mainly androgen, mental state, heredity and some drug factors. Mainly in the young male, but in recent years there has been an increasing trend in the number of female patients. Patients generally have persistent alopecia from the second decade to the third decade, and generally have basic baldness from the fourth decade, which seriously affects beauty.
White hair, which is the complete or partial whitening of the finger hair. For instance, the white hair syndrome is recorded in the white hair syndrome in the "theory of various etiological factors: the kidney governs the bone marrow and it is manifesting on the hair. If the blood-qi is strong, the kidney-qi is strong, and the bone marrow is full of the kidney-qi, so it is moist and black; when blood qi is deficient, kidney qi becomes weak, and when kidney qi is weak, bone marrow becomes exhausted, so the hair becomes white.
White hair includes senile white hair and lack of white hair, senile white hair is a physiological phenomenon, and white hair diseases can be divided into congenital and acquired types. Congenital leukotrichia is accompanied with albinism, sometimes occurs in some hereditary diseases patients, and is characterized by familial inheritance. In addition to senile poliosis, premature poliosis is common in acquired poliosis, which may be genetically related, but also is an important factor for mental trauma, emotional agitation, pessimistic depression, malnutrition, and the like.
Generally, in the patients with alopecia areata, androgenetic alopecia and poliosis, in addition to the external symptoms such as alopecia and poliosis which are obvious to the body, the mental health status of the patients also has certain damage during the disease period.
Modern medicine considers that under normal conditions, human hair papilla has abundant blood vessels, which provides sufficient nutrition for hair papilla and hair bulb, so that hair growth hormone is smoothly synthesized. Various adverse stimuli (influence of hormone level, influence of neurostimulation, etc.) cause vasospasm of supplying hair nutrition, and dysfunction of nutrition-transporting function of hair papilla and hair bulb part. When nutrient synthesis is inhibited in the formation of papilla and bulbus pili, or when ATP having a cell energy function is produced due to some factor, production of ATP is inhibited. ATP, which is a heat source, is not produced, and thus protein synthesis of hair is not performed, hair mother cells lose activity, nutrients in medullary and cortex portions of hair are reduced, keratinization begins, hair follicles begin to atrophy or necrose, hair enters a resting stage in a large amount, and hair falls off in a large amount.
According to the theory of traditional Chinese medicine: the hair has a close relationship with liver and kidney, the kidney stores essence and liver and governs blood, the essence and blood are insufficient due to liver and kidney deficiency, the hair follicle cannot obtain sufficient nutrition, the capacity of synthesizing melanin is weakened, white hair appears, and the hair follicle is atrophied or necrotized to cause alopecia. Otherwise, the liver and kidney are strong and the upper part is good at the head, so the hair is thick and dark.
The current treatment scheme of western medicine for alopecia areata is as follows:
1. general treatment: the removal of the medicine can induce the etiology, and attention is paid to the overstrain and the rest of alopecia areata can be naturally cured within 6-12 months. For patients with wide baldness range or alopecia totalis and alopecia universalis, the wig should be worn to relieve psychological burden.
2. Topical drug therapy:
1) 2% or 5% minoxidil (minoxidil) solution, 10% capsicum tincture, etc. can improve local blood circulation and promote hair growth, and hair regeneration can occur in 2-3 months.
2) The bald area can be externally used or injected with glucocorticoid to relieve perifollicular inflammation, and prednisolone suspension or compound betamethasone injection can be injected intradermally, and can take effect after 3-4 times.
3. Systemic drug therapy: oral administration of cystine, calcium pantothenate, vitamin B group can help to promote hair growth. Prednisone can be orally administered to patients with mental stress, anxiety, and insomnia by administering other sedatives to patients with rapid and extensive alopecia including alopecia totalis and alopecia universalis. Can be used in combination with compound glycyrrhizin, total glucosides of paeony, blood nourishing and hair growth promoting capsules, thin sesame slices, polygonum multiflorum slices and the like.
4. Photoelectric treatment: researches report that PUVA therapy, long-wave ultraviolet rays, narrow-spectrum medium-wave ultraviolet rays, photodynamic therapy, 308nm single-frequency excimer light and excimer laser, a pulse infrared diode (904nm), low-energy laser, lattice carbon dioxide laser and the like all have a certain treatment effect on alopecia areata.
5. And others: moderate to severe alopecia areata (bald area > 50%) can be treated with topical contact sensitizers, Diphenylcyclopropenone (DPCP), leading to hair regrowth at the lesion site, mainly by inducing the development of contact dermatitis.
The current treatment scheme of western medicine for androgenetic alopecia is as follows:
1. systemic treatment: finasteride can inhibit II-5 alpha reductase and testosterone to DHT, and can be continuously administered for 6-12 months or more, if the curative effect needs to be maintained for a longer time. Cyproterone is suitable for female patients, especially patients complicated with acne and hirsutism.
2. The external medicine comprises: minoxidil is an external hair growth promoting drug, generally 5% concentration is recommended for men, 2% concentration is recommended for women, and contact dermatitis and hirsutism are common adverse reactions.
3. Hair transplantation: hair transplantation is a method in which hair follicles (generally, the occiput) in a congenital androgen-insensitive region are separated and transplanted to a bald area. Follicular unit hair transplantation technique (FUT) is one of the commonly used methods.
4. Other patients with severe AGA who are ineffectual to medication or surgery may be covered with hair pieces, wigs, etc.
At present, the western medicine aims at the treatment of the white hair, and no clinically effective medicine and treatment scheme exist.
The western medicine has strong dependence on alopecia areata and androgenetic alopecia and easily causes repeated side effects after generating drug dependence.
The external treatment methods of traditional Chinese medicine can be divided into two categories: external treatment with drugs, or external treatment without drugs. According to the different treatment routes, the external treatment methods can be divided into four categories, including integral treatment, treatment of skin, mucous membrane of sense organs and orifices, treatment of meridians and acupoints, and other treatments. The treatment of skin and mucosa of sense organ and orifice refers to the method of absorption of medicine through skin and mucosa of sense organ and orifice into local part or circulatory system of body for therapeutic effect, such as application therapy, fumigation therapy, etc. The traditional Chinese medicine external treatment is a traditional Chinese medicine treatment method characterized by outstanding traditional Chinese medicine external treatment, has unique curative effect, rapid action, long history, and the characteristics of simplicity, convenience, cheapness and verification, and therefore has important significance for the development of the traditional Chinese medicine external treatment method for treating alopecia areata, androgenetic alopecia and leukotrichia.
