Etiology and pathogenesis of surgical diseases of traditional Chinese medicine
Firstly, pathogenic factors: the surgical diseases are mainly caused by six exogenous factors, special toxicity, external injury, emotional internal injury, improper diet, fatigue, asthenia, phlegm, fluid retention, blood stasis and sepsis. The above pathogenic factors can cause diseases alone or simultaneously, and internal injury and external infection are often combined.
Phlegm-fluid retention and blood stasis are pathological products of visceral dysfunction, and under certain conditions, they can act on some organs to cause new pathological changes and secondary diseases. Clinically, phlegm and stasis are both pathogenic and cause each other. The specific manifestations are different depending on the location of phlegm coagulation and the disease. For instance, phlegm-phlegm and milk-stasis can induce breast nodule and nodules.
Secondly, pathogenesis: since various pathogenic factors attack and act on the body and compete with the healthy qi of the body, the prevailing pathogens cause stagnation of qi and blood, obstruction of the meridians, disharmony of ying qi and qi, disharmony of the zang-fu organs, and yin-yang disharmony, leading to various pathological changes, and surgical diseases occur. 1. Preponderance and decline of pathogenic factors and healthy qi: surgical diseases, like any other diseases, have a fundamental contradiction between the struggle between pathogenic qi and healthy qi throughout, which not only relates to the occurrence of the disease, but also determines the characteristics of the syndrome "excess pathogenic qi and deficiency essence qi", and also directly affects the prognosis and outcome of the disease. Healthy qi exists in the interior, and pathogenic qi cannot dry out. If the healthy qi is abundant and the resistance is strong, the invasion of pathogenic factors is difficult, i.e. the disease does not occur. However, if the pathogenic qi is preponderant, the healthy qi is relatively deficient, and the pathogenic qi wins the positive qi and the negative qi, surgical diseases occur. After onset, the healthy qi is vigorous, and clinically, yang syndrome and excess syndrome are mostly manifested as smooth development and good prognosis. Deficient healthy qi is manifested as yin syndrome and deficiency syndrome, while deficient healthy qi with excessive pathogenic factors or lingering pathogenic factors is easy to reverse, resulting in poor prognosis. During surgical diseases, the changes of the preponderance and decline of pathogenic factors and healthy qi are greatly influenced by the therapeutic drugs. For instance, in the early stage of yang-syndrome sores and ulcers, large doses of cold, cool and sore-relieving medicines are orally taken in one location, which often causes internal damage of healthy qi and stagnation of qi and blood, and accumulation of toxin and retention of qi; for example, when pus is formed in sores and ulcers, it is not supposed to be done with the supporting method for yang syndrome and yin syndrome, or it is difficult to discharge after ulceration and cut open the drainage in time, so it can cause toxicity to remain in skin, tendons and bones, and even attack the zang-fu organs. After the body resistance is injured by severe or chronic diseases, yin is injured by heat-toxicity, or qi and blood are injured by pus discharge, the yang-confirmation syndrome can be transformed into yin-deficiency syndrome, which leads to the dynamic change of the essence of the relationship between body resistance and pathogens.
2. Stagnation of Qi and blood: it refers to the pathological changes of dysfunction caused by the failure of qi and blood to circulate or dysfunction of qi and blood. The onset and progression of the disease is a dynamic change, and thus the pathological process is constantly developing and changing. When local stagnation of qi and blood is caused by pathogenic factors, pain, swelling, nodules, lumps, bleeding, skin thickening, purpura, etc. may appear. For example, sufficient qi and blood can cause surgical sores and ulcers to easily initiate and break ulceration, and promote tissue regeneration and granulation to heal; if qi is deficient, the disease is difficult to initiate and break; blood deficiency makes it difficult to promote tissue regeneration and heal wound; prolapse of anus due to collapse of qi; blood deficiency failing to moisten can cause dry skin, desquamation and pruritus. It is seen that the abundance or insufficiency of qi and blood has a close relationship with the prognosis and treatment of surgical diseases.
3. Blockage of meridians: local meridian blockage is one of the general pathogenesis of surgical diseases; meanwhile, local weakness of the meridians and collaterals of the body can also be a condition for the onset of surgical diseases. For example, the blood stasis and bleeding due to trauma and stasis, whitish wind, and oily wind due to scalp trauma and hematoma, the "the most deficient part" is the place where pathogens are contained. In addition, the meridians and collaterals of the affected part are also important in connection with the occurrence and development of surgical diseases. Meridians and collaterals are the pathways for transmitting toxic pathogens, and they have the functions of circulating qi and blood and connecting internal and external tissues and organs of human body, so the external toxic pathogens can be transmitted from the outside to the inside to attack the internal organs, and the internal pathological changes of the internal organs can be expressed from the inside to the outside, all formed by the transmission of meridians and collaterals. Therefore, the meridians and collaterals are closely related to the occurrence and change of surgical diseases.
4. Disharmony of the zang-fu organs: the human body is a complete and unified organism, and surgical diseases are mostly caused on certain parts of the skin, flesh, vessels, tendons and bones of the body surface, but are closely related to the zang-fu organs. Dysfunction of the zang-fu organs can lead to surgical diseases. Therefore, there are the descriptions of the interior forms and the exterior forms and the interior forms. Thus, the occurrence of surgical diseases is associated with clear dysfunction of the viscera. The pathological changes of the zang-fu organs can be reflected on the body surface, and the transmission of the pathogenic toxins on the body surface through the meridians can also affect the zang-fu organs to cause pathological changes.
In short, from the process of occurrence, development and change of surgical diseases, it is closely related to qi and blood, viscera, meridians and healthy qi. Although the general pathogenesis of the local stagnation of qi and blood, disharmony of ying-qi and obstruction of meridians results in dysfunction of zang-fu organs, the general pathogenesis is that the imbalance or preponderance of yin and yang cannot be eliminated because the imbalance of yin and yang is the root cause of the occurrence and development of diseases, and qi and blood, zang-fu organs and meridians are all contained in yin and yang. Qi is yang and blood is yin; fu organs belong to yang and zang organs belong to yin; the meridians and collaterals are divided into yang and yin meridians, which depend on, restrict and transform each other. The disease can occur due to imbalance of yin and yang caused by various pathogenic factors destroying the relationship.
Dermatosis disease cause and cause machine
Diseases occurring in the skin, mucous membranes and skin appendages of the human body are called skin diseases. The etiology of skin diseases is complicated, but they are classified into two types, internal and external. The external causes are mainly six excesses (especially wind, dampness and heat), insects, toxins and the like; internal causes are mainly internal injuries due to seven emotions, overeating and liver and kidney deficiency. Its pathogenesis is mainly due to qi and blood disharmony, viscera disorder, pathogenic toxin accumulation, wind generation, dampness generation, dryness and deficiency, stasis, heat and yin impairment.
1. Wind: many skin diseases are closely related to wind pathogen. Wind pathogen can cause diseases alone or in combination with other pathogens. Skin diseases can occur when the skin is obstructed due to insufficient striae and skin striae, weak wei-defensive qi, invasion of wind-evil into the skin, failure of qi and blood circulation, and failure of nourishing skin. Skin diseases caused by wind pathogens generally have the following characteristics: the hair is immortal and suddenly disappears, such as urticaria and red and white wandering wind; severe itching, dry skin, desquamation, such as pruritus; it is often found in the upper part, such as the wind of swimming, the wind of white dandruff, etc. Clinically, wind pathogen is often combined with other pathogens to cause diseases, such as wind-dampness, wind-heat, wind-cold, etc.
2. Wetting: dampness is classified into internal dampness and external dampness, and skin diseases are mostly caused by dampness, but external dampness and internal dampness are combined to cause diseases. When dampness invades the skin, it stagnates and is not dispersed, and it is mixed with qi and blood, so as to cause herpes, effusion, erosion, pruritus, etc. Skin diseases caused by damp pathogen, including blister, polymorphism, erosion, invasion of liver and kidney, and watery sensation, are usually in the lower part, and the course of disease is lingering, difficult to cure and easy to develop after cure.
3. Heating: heat is yang pathogen, and fire and heat are homologous. The heat is gradually the fire, while the heat is slight and causes itching; the excessive heat may cause pain. Skin diseases can occur due to exogenous pathogenic heat or excessive heat in the zang-fu organs, which can block the skin without leaking, fumigating the skin surface. The pathogenic heat is usually found in the upper part of the body, and the skin lesions mainly include erythema, red swelling, pustule and erosion, and are conscious of itching or pain.
4. Insect killing: the skin diseases caused by insects are various, and the skin damage is different when the insects are different. The first is the direct pathogenic disease of parasitic diseases in skin, such as scabies caused by scabies, tinea (fungus) can cause tinea manuum, tinea pedis, tinea corporis, tinea etc.; one is a skin disease caused by the invasion or allergy of insect toxins, such as damage caused by the biting of mosquitoes, bed bugs, midges, lice, and dermatitis from insect bites. 5. Poison: the skin diseases caused by the poison can be divided into medicine poison, food poison, lacquer poison, insect poison and the like, the pathogenesis of the skin diseases is not dependent on the poisoning or the innate endowment, and the body is allergic to certain substances. The skin diseases caused by the toxin have a history of eating the toxic substances before the diseases are attacked, or a certain medicine is taken before the diseases are attacked, or the diseases are attacked by the toxic substances or the poisonous insects bite history, and the diseases can be attacked after a certain incubation period. 6. Blood stasis: is the important etiology and pathogenesis of skin diseases. It is caused by six excesses affecting the exterior and seven emotions affecting the interior, which can lead to disorder of qi movement, qi being the commander of blood, blood moving with qi, and qi stagnation leading to blood stasis. The blood stasis syndrome is mostly seen in chronic skin diseases, and is characterized by dark skin lesions, purplish red and blue purple, or appearing with scaly skin, pigmentation, ecchymosis, hypertrophy, nodules, lumps, scars, alopecia, purple tongue or petechia, wiry and unsmooth pulse, and the like, such as melon vine winding and blackish black spots.
7. Blood deficiency and wind dryness: also an important pathogenesis of skin diseases. Chronic skin diseases such as long-term skin pruritus, frequent sleep, restlessness, spleen deficiency, anorexia, spleen and stomach dysfunction, and yin and blood dysfunction; or stagnation of the wind-damp for a long time, heat transformation into fire due to depression, impairment of yin and blood, and yin and blood deficiency; or chronic deficiency of the original qi can cause blood deficiency and wind dryness. The skin damage is characterized by mainly dryness, hypertrophy, roughness and desquamation, little erosion, seepage, conscious pruritus and long disease period, such as psoriasis, porrigo, chronic eczema, wind pruritus, ichthyosis and other chronic skin diseases.
8. Deficiency of liver and kidney: the visceral disorders are the important etiology and pathogenesis of skin diseases, and the deficiency of liver and kidney is the most common cause. Liver and kidney deficiency mainly includes congenital essence deficiency and acquired essence-blood deficiency. For example, if the liver blood is deficient and the nails are not nourished, the nails become thick, dry and brittle; if the liver is deficient and blood is dry, the tendon and qi can fail to nourish the stomach, the wart can be produced; liver meridian fire stagnation in blood can cause blood nevus. When kidney essence is not sufficient and hair fails to nourish, hair is dry and easy to fall off; when the kidney is deficient, the natural color is developed, and the face becomes dark and gloomy. Skin diseases caused by liver and kidney deficiency are mostly in a chronic process, and skin damage is manifested by dryness, hypertrophy, roughness, desquamation, or accompanied by withered and lusterless hair, alopecia, pigmentation, nail damage, or accompanied by warts and blood nevus. Because the kidney is the innate root, certain congenital and hereditary skin diseases are also related to the liver and kidney, such as ichthyosis and alopecia.
In conclusion, the occurrence of skin diseases is not caused by a single cause, but is caused by the combined action of more than one cause. Or the internal injury and the external infection are mixed together, or the syndrome is an excess syndrome, or a deficiency syndrome, or a mixture of deficiency and excess.
Common symptoms of skin diseases
Skin diseases can produce a series of subjective symptoms and subjective symptoms in the process of pathogenesis.
(1) Subjective symptoms: i.e. the subjective feeling of the patient. The subjective symptoms of skin diseases depend on the nature of the primary disease, the degree of the disease, and individual differences among patients. The most common symptoms are itching, followed by pain, and in addition burning, numbness, mosquito sensations, and the like.
Pruritus: it can be caused by many factors, but is mainly due to wind pathogen and heat pathogen. In addition, the red and hot skin lesions caused by heat in nutrient and blood may cause severe itching, especially acute itching with papules, erythema and cellulite.
The chronic dermatosis has complicated pruritus reasons, and the pruritus can be caused by factors such as cold, dampness, phlegm, stasis, worm, blood deficiency, wind dryness, liver and kidney deficiency and the like. Except for the attack of cold pathogen, the cold syndrome pruritus can also be caused by the generation of internal cold due to the deficiency of spleen and kidney yang, and the skin rash is red and the fever symptom is not obvious, or cold nodules, ulcers and the like appear; pruritic rashes due to damp-heat can be manifested as fluid or blisters; nodules often appear in pruritus caused by phlegm pathogen; itching due to blood stasis can be manifested by purpura, pigmentation, etc.; itching is severe due to the damp-heat in the blood stasis, and the skin lesion is hard and stubborn and difficult to heal; pruritus caused by excesses of insects is manifested by pruritus caused by walking of insects or walking of ants, paroxysmal pruritus is difficult to endure, and the pruritus is infectious; itching caused by blood deficiency and wind dryness and deficiency of liver and kidney often has scab or scurf like desquamation, skin chapping, lichen morphosis and the like.
② pain: the skin diseases with pain symptoms are not many, and are generally caused by cold pathogen, heat pathogen or phlegm coagulation and blood stasis, blocking meridians and collaterals are obstructed, and the pain is not caused when the skin diseases are passed. Pain of cold syndrome is manifested as local bluish purple, which is aggravated by cold and moderate by warm; heat syndrome with pain with red swelling, fever and painful skin damage; the pain due to blood coagulation and stasis may be a nodule of phlegm core, ecchymosis or bluish purple, and the pain is usually not fixed. (2) Chronic skin diseases: most of the diseases are slow in onset, and the skin lesions are mainly secondary in appearance, such as lichenification, pigmentation, rhagadia, scale, etc., or accompanied by alopecia and toe nail changes. The main causes of this disease are blood stasis or nutrient-blood deficiency, liver and kidney deficiency, and disharmony of thoroughfare and conception vessels. It is closely related to liver and kidney. The liver stores blood, and blood deficiency can lead to wind generation, dryness generation and malnutrition of vessels to cause diseases; kidneys store essence, black pertains to kidneys, hair is the origin of kidneys, and insufficiency of kidney essence can cause skin pigment change and alopecia.
In summary, skin disorders are the manifestations of systemic diseases on the skin of the human body, and many of the systemic diseases can be reflected on the skin; while local irritation on the skin can also cause systemic lesions. Therefore, when treating skin diseases, TCM should follow the principle of treating external diseases by treating the internal diseases locally and integrally. According to the constitution of the patient, the specific etiology, pathogenesis and skin damage characteristics, the treatment is dynamically performed according to the differentiation of symptoms and signs.
Clinical application of zang-fu organ syndrome differentiation in dermatology

The close connection between qi and blood can maintain the normal physiological function and development and growth of human body.
Changes of qi and blood are related to skin diseases. The pathological changes of qi and blood are commonly seen clinically as qi deficiency, qi stagnation, blood deficiency, blood stasis, blood heat, blood dryness and qi and blood disorder, and the pathological changes of qi and blood are reflected in the aspects of skin diseases as follows:
