Abstract
本发明提供一种一种补肾活血通络方及其制备方法,所述补肾活血通络方为:补肾活血通络方为:补骨脂15克,骨碎补15克,红花15克,乳香15克,没药15克,制川乌18克,制草乌18克,白芍12克,威灵仙15克,透骨草30克,伸筋草30克,羌活10克,独活10克,肉桂9克。必要时加减当归15克、苏木12克,杜仲18克。本发明的补肾活血通络方定向透药法治疗肾虚血淤型慢性腰痛临床疗效可靠,能帮助患者改善症状,缓解腰痛,有效降低炎症细胞因子水平,临床上值得推广。

技术领域
本发明涉及中药技术领域,具体涉及一种补肾活血通络外用方及其制备方法。
背景技术
慢性腰痛是一种腰部疼痛综合征,涉及腰骶肌肉、筋膜、韧带、关节以及腰椎间盘等结构的病损。以下背、腰骶、臀部反复疼痛为主要表现,常伴有不同程度的腰部功能障碍。流行病学调查表明,腰痛发病率在美国仅次于上呼吸道感染而位居第二,在我国可能更高.大约80%成年人一生中至少有一次曾遭受过下背痛的困扰,文献报道上海工人腰痛大于1天的比例超过了50%。由于许多慢性腰痛患者很难在短时间内治愈。而且即使暂时治愈,复发率也相当高.致使许多人不断遭受持续或间歇性发作的下腰部疼痛的困扰。从而影响了工作和生活,严重者甚至丧失劳动能力。不仅如此,患者的心理功能、社会活动能力以及总体健康状况等也会随之下降,从而严重影响患者的生存质量。各国每年都要耗费高昂的医疗费用,其中美国每年用于LBP的费用估计在500-1000亿美元,是各类肿瘤总花费的3倍以上,在人口老龄化不断发展的今天,随年龄的增加患腰痛的可能性明显增加,慢性腰痛的发病率还将进一步提高。
《素问·痹论》提出“病久人深,荣卫之行涩,经络时疏,故不通”;《素问·调经论》提出“病在血,调之络”说明经络为气血运行的通道,疾病迁延日久,“血伤入络”,必然影响气血在经络中的正常运行而出现阻滞;络脉瘀阻,调之于络。叶天士在前人思想基础上,在治疗痛症方面提出“久病入络”、“痛久必入络”的学说,叶氏认为邪气袭人后,其传变途径“由经脉继及络脉”,又说“初为气结在经,久则血伤入络”,“经年宿病,病必在络”,指出内伤杂病随着病程的进展,病邪由经入络、由气及血的过程。
祖国医学对于腰痛认识已久。《素问·脉要精微论》指出:“腰者,肾之府,转摇不能,肾将惫矣。《素问·六元正纪大论》云:“房室劳伤,肾虚腰痛者,是阳气虚弱不能运动故也。”《三因极一病证方论》日:“夫腰痛,虽属肾虚,亦涉三因所致,在外则脏腑经络受邪,在内则忧思恐怒,以至房劳坠堕,皆能致之。”《丹溪治法心要》所治外邪腰痛多为寒湿及湿热为病,《济生方》认为,“肾脏既虚,风寒湿毒得以伤之,遂致腰痛”。腰痛不外“三因致病”,不论内、外因多以肾虚为其发病基础。清代程国彭《医学心语》说“通则不痛,痛则不通”。清代陈修园《医学三字经》说“痛不通,气血壅,通不痛,调和奉”,说明气血运行障碍,络脉失于通畅,气血瘀滞不通是疼痛发病的主要因素。王晓东等研究60-69岁老年人体质与慢性腰痛的关系,发现随着60-69岁老年人气虚质、血瘀质检出率增高,慢性腰痛的发病率也增高,两者呈正相关。
慢性疼痛后期的病机综合阐述当属叶天士,叶氏总结前人基础上提出“久病必瘀闭”。“久发频发之恙,必伤及络,络乃聚邪之所,久病必瘀闭。”所谓瘀闭是指瘀血、痰饮等病理产物痹阻络脉。“络中气血,虚实寒热,稍有留邪,皆能致痛”。络病引起的疼痛,其机理有虚实寒热四端,故临床辨证必须分清病由。一般而言,络虚之痛,痛势绵绵、动后痛剧、休息痛减、喜按喜揉,乃络中气血不足,失于润运温煦,络脉不畅所致,如叶天士《临证指南医案》所云:“初病气伤,久泻不止,营络亦伤,古为络虚则通”,“下焦空虚,络脉不宜,所谓络虚则通是也”。慢性腰痛患者疼痛性质正如上所述。慢性腰痛临床病程长,逐渐消耗人体之气血津液,随着病情的进展进而消耗人体之元阴元阳,肾中所藏之先天精气消耗殆尽,后天之精气亦无法补充,肾虚而不能履行其生理功能,故称久病及肾。经脉瘀阻不通,日久可以肾精亏耗;肾精不足又影响气血的生化。无论病邪在络,久病肾虚都导致本病迁延难愈,临床反复发作,给病人带来痛苦。
现代康复学的研究非常重视患者生存质量,然而临床康复治疗手段繁多,只要能想到的,都有人在尝试,有人例举了52种治疗手段,这充分说明了临床治疗上认识同一性的缺乏,也提示对于本病治疗上的无奈。但是其中又有得到专家推荐及临床肯定的治疗手段,例如物理因子治疗。以透药治疗为例,透药治疗的中药药剂片是统一制剂,这就明显违背了中医的辨证论治体系,不能更好的发挥中医的个体化治疗方案特色。经皮给药具有减少首过效应、可以局部治疗、顺从性好等优势,透皮吸收研究成为皮肤外用制剂研究的热点,经皮透药治疗的方法多种多样,包括物理的、化学的;经皮透药治疗的评定方法、动物模型的选择、扩散装置不能够统一,尚存在不足,但从辨证论治原则、中药的性味归经、中药外治的经络传导方面进行研究,将为中药外用药这一新领域开拓广阔的空间。
发明内容
本发明的目的是提供一种补肾活血通络方。
本发明提供的一种补肾活血通络方,其含有以下中药:补骨脂,骨碎补,红花,乳香,没药,制川乌,制草乌,白芍,威灵仙,透骨草,伸筋草,羌活,独活,肉桂。加减当归、苏木、杜仲。
具体的,所述中药组合物含有以下重量份的中药:补骨脂15克,骨碎补15克,红花15克,乳香15克,没药15克,制川乌18克,制草乌18克,白芍12克,威灵仙15克,透骨草30克,伸筋草30克,羌活10克,独活10克,肉桂9克。
在上述基础方的基础上必要时加减当归15克、苏木12克、杜仲18克。
进一步的,所述一种补肾活血通络方中各种药材的计量单位可以是μg、mg、g、kg等医药领域公知的重量单位,也可以是其倍数,如1/10、1/100、1、2、3、5、10、100倍等。
本发明还提供了一种制备上述补肾活血通络方的方法,该方法包括以下步骤:按照配比称取各中药,然后直接粉碎,混匀,即可。
进一步的,在获得上述混合物后,浓煎上清液,浸泡透药治疗棉块,敷于患处(阿是穴)。
本发明提供的一种补肾通络的中药组合物具有以下优点:
1、具体而言,本申请的补肾活血通络方的中药组成及其配伍关系具体为:
补肾活血通络方组方。方中补骨脂、骨碎补补肾壮骨为君,红花、川乌、草乌活血通络定痛为臣,乳香、没药行气止痛、白芍、威灵仙柔筋缓急止痛,羌活、独活、杜仲、肉桂,祛湿通络、温补肝肾作用为佐,透骨草、伸筋草通络止痛、引药达络为使。全方以补肾活血通络止痛立方,以补肾活血药为主药,佐以行气活血、通络、化湿药,共同扶正驱邪,以期达到驱邪不伤正,使经脉通畅,气血调和,通则不痛的目标。正如《证治准绳·腰痛》云:“大抵诸腰痛,皆起肾虚,既夹邪气,则须除其邪。如无外邪积滞而自痛,则惟补肾而已”。因此中医治疗应当遵循辨证论治的原则,邪实则祛邪,正虚则扶正。
2、适应症范围广,副作用小,使用方式灵活。
补肾活血方适用范围:急慢性腰痛(腰椎间盘突出、腰肌劳损、第三横突综合征、腰部筋膜炎),颈痛(颈椎病,颈背部筋膜炎),慢性软组织损伤,膝骨关节炎,滑膜炎,踝关节损伤后遗症,跟痛症,以及轻、中度疼痛。
副作用小:该组方已在临床应用5年,临床应用中,副作用少,以皮肤瘙痒、红为主要常见。极少患者长时间应用,会出现皮肤色素沉着。
使用方法灵活:该申请方可以浓煎汤剂,取上清液,作为透药治疗的介质,透皮应用;可粉碎后,装药袋,黄酒浸泡后,局部热敷应用;可做成巴布剂,作为膏药外敷应用;还可作为泡足方应用。根据患者要求及治疗需要灵活选择使用方法。
3、临床研究提示该方治疗慢性肾虚血淤型腰痛效果明显。
将补肾活血通络方与中医定向透药法相结合,用于治疗肾虚血淤型慢性腰痛患者,研究发现,观察组治疗后M-JOA评分及Oswestry评分明显下降,症状评分明显低于单纯口服药物治疗者,提示补肾活血通络方定向透药法能有效改善患者日常生活能力,提高患者生活质量;比较两组患者中医证候积分发现,观察组腰痛如刺、拒按、膝腿无力、面色白及肢冷畏寒等症状评分均显著降低,提示补肾活血通络方定向透药法明显改善患者症状体征,中医证候均有所好转。IL-1β增加毛细血管通透性,促进炎症反应发生,加剧疼痛;IL-6通过调节椎骨组织胶原、细胞外基质蛋白多糖等参与椎间盘退变,促进炎症反应;IL-17是由Th17细胞分泌产生,Th17细胞介导炎性反应则是导致慢性腰痛的重要途径之一。观察组采取补肾活血通络方定向透药法治疗后炎症细胞因子明显降低,提示该方法能有效减轻炎症反应,缓解疼痛。比较两组患者治疗效果发现,口服西药治疗有效率不足80%,采取补肾活血通络方定向透药法治疗有效率高达94.59%,在临床上值得推广。
4、临床透药治疗所用透药电极片,作为中药治疗的中医药治疗方法,存在管理及合理性问题。中药定向透药所用方剂无试验支持,无循证依据,不公开或无法公开;临床患者疼痛分型多种多样,透药治疗电极片单一(统一配方),完全无视中医药的辨证分型原则,忽视了中医药一证一方的优势。所申请的补肾活血通络方,尊经典,重实效,分型对症用药,能更好发挥中医药优势。
本发明所述的补肾活血通络方创新性:
1、临床多以“补肾活血、通络止痛”立方内治为主,补肾活血通络方为几代人的积累成方,将叶天士“久病入络”,“久病必瘀”,“久病及肾”的理论引入慢性腰痛患者的外治法治疗,以其为理论指导。
2、组方严格,兼顾主证、次证。以“补肾活血通络”组方,以补肾活血药为主药,佐以行气活血、祛风通络、化湿药,共同扶正驱邪,以期达到驱邪不伤正,使经脉通畅,气血调和,通则不痛的目标。
3、兼顾急、慢性疼痛,适应症范围广。从现代医学研究看其组方方中补肾壮骨药:补骨脂、骨碎补、威灵仙等具有增加骨质疏松患者骨含量作用,川乌、草乌有效成分具有直接镇痛作用,红花具有明显的改善循环的作用,全方以此为中药组方的君、臣药,标本兼顾,兼顾腰痛发病急性期和缓解期。也可应用于颈椎病、膝关节炎、肩周炎、跟痛症,筋膜炎等疼痛类疾病。
4、应用方法灵活。本方临床应用过程中,可以作为中医定向透药的透皮药剂,可以作为颈、腰、膝疼痛的外敷药,也可以制备为巴布剂应用,制备简单,应用方法灵活,方便患者。
5、不良反应小。本方作为外用剂型,局部、外部给药,避免患者长期、慢性疼痛患者服药可能带来的胃肠道血、药物依赖等严重不良反应。
6、复发率低。经研究发现,从肾虚血瘀立论组成该方,治疗慢性疼痛,可明显改善患者就诊次数及复发率。
具体实施方式
为使本发明实现的技术手段、创作特征、达成目的与功效易于明白了解,下面结合具体实施方式,进一步阐述本发明。
实施例1一种补肾活血通络方及其制备方法
取补骨脂15克,骨碎补15克,红花15克,乳香15克,没药15克,制川乌18克,制草乌18克,白芍12克,威灵仙15克,透骨草30克,伸筋草30克,羌活10克,独活10克,肉桂9克,加减当归15克、苏木12克、杜仲18克。
将药物按照上述比例混合,进行粉碎,粉碎后,久煎,沉淀,取上清液,作为中医定向透药治疗的介质药备用。
治疗时,浸泡透药棉毡大于20分钟,敷于局部穴位(阿是穴),透药治疗,每次25分钟,强度以耐受为度
实施例2一种补肾活血通络方及其制备方法
取补骨脂15克,骨碎补15克,红花15克,乳香15克,没药15克,制川乌18克,制草乌18克,白芍12克,威灵仙15克,透骨草30克,伸筋草30克,羌活10克,独活10克,肉桂9克,加减当归15克、苏木12克、杜仲18克。
将药物按照上述比例混合,进行粉碎,粉碎后,久煎,沉淀,取上清液,用毛巾浸泡后,敷于膝关节,肩关节疼痛处,热敷治疗,每次以皮肤红润为度。
实施例3一种补肾活血通络方及其制备方法
按照补肾活血方:补骨脂15克,骨碎补15克,红花15克,乳香15克,没药15克,制川乌18克,制草乌18克,白芍12克,威灵仙15克,透骨草30克,伸筋草30克,羌活10克,独活10克,肉桂9克,加减当归15克、苏木12克、杜仲18克。
将药物以3-5倍量按照上述比例混合,进行粉碎,粉碎后,装入棉质布袋,缝合;将药袋浸泡于黄酒或白酒中,酒量以淹没药袋为准,时间约2-4小时。用前药袋蒸30分钟,治疗部位覆盖干毛巾,进行烫熨治疗;药袋温度降低后撤去毛巾,继续外敷治疗。每次以皮肤红润为度。
实施例4一种补肾活血通络方及其制备方法
取补骨脂15克,骨碎补15克,红花15克,乳香15克,没药15克,制川乌18克,制草乌18克,白芍12克,威灵仙15克,透骨草30克,伸筋草30克,羌活10克,独活10克,肉桂9克,加减当归15克、苏木12克、杜仲18克。
将药物按照上述比例混合,制备成巴布剂备用。
治疗时,将巴布剂涂抹于无纺布,敷于局部穴位(阿是穴),外敷治疗,时间8-12小时,强度以耐受为度,皮肤红痒者减少外敷时间。
实施例5一种补肾活血通络方的临床试验
1.1临床资料
选取2019年1月-2019年12月我院收治的74例肾虚血淤型慢性腰痛患者,随机等分为对照组及观察组。对照组37例,男性20例,女性17例,年龄38-62岁,平均年龄(53.17±5.62)岁,病程6个月-5年,平均病程(2.17±0.62)年,其中腰肌劳损29例,腰肌退行性病变8例;观察组37例,男性22例,女性15例,年龄34-61岁,平均年龄(52.51±5.53)岁,病程8个月-4年,平均病程(2.35±0.71)年,其中腰肌劳损30例,腰肌退行性病变7例。两组患者在性别、年龄、病程及疾病类型等一般资料上无明显差异(P>0.05),具有可比性。
1.2诊断标准
1.3纳入及排除标准
纳入标准[5]:(1)符合肾虚血淤型腰痛诊断,且并病程≥3个月;(2)年龄18-65周岁;(3)1个月内未接受正规药物治疗;(4)患者对本次研究知情同意,并签署知情同意书。
排除标准[6]:(1)脊髓病变者;(2)伴其他重要脏器功能损伤者;(3)腰椎外伤史或手术史;(4)神经精神疾病者;(5)难以配合本次研究者。
1.4治疗方法
补肾活血通络方透药液制备:补骨脂15克,骨碎补15克,红花15克,乳香15克,没药15克,制川乌18克,制草乌18克,白芍12克,威灵仙15克,透骨草30克,伸筋草30克,羌活10克,独活10克,肉桂9克。上方浓煎,取上清液备用。
治疗组取补肾活血方浓煎上清液,浸泡透药治疗棉块,敷于患处(阿是穴),透药治疗,每次25分钟,强度以耐受为度,每周3次;
对照组空白棉毡透药治疗。
1.5观察指标
(1)改良下腰痛评分法(M-JOA)评分[8];包括日常生活能力、体征及症状,评分越高说明病情越严重;功能障碍指数问卷(Oswestry)评分:包括步行、提物、生活自理能力、疼痛强度及旅游等,评分越高说明病情越严重。
(3)炎症细胞因子水平:所有患者治疗前后于清晨空腹抽取静脉血,常规离心分离血清后采取酶联免疫吸附法测定白介素-1β(IL-1β)、IL-6及IL-17;
(4)治疗效果[9]:痊愈:腰痛症状完全消失,肾虚血淤证症状积分减少≥95%;显效:腰痛症状明显改善,70%≤肾虚血淤证症状积分减少<95%;有效:腰痛症状有改善,30%≤肾虚血淤证症状积分减少<70%;无效:腰痛症状无明显变化或加剧,肾虚血淤证症状积分减少<30%。治疗总有效率(%)=(痊愈+显效+有效例数)÷总例数×100%。
1.6统计学处理
结果采用SPSS 18.0统计学软件进行处理并进行统计学分析,计量资料比较采用t检验,计数资料比较采用X2检验,以P<0.05为差异具有统计学意义。
临床试验结果
2.1两组患者治疗前后M-JOA评分及Oswestry评分比较
两组患者治疗后M-JOA评分及Oswestry评分均有所下降,且观察组下降更为明显,差异具有统计学意义(P<0.05),具体见表1。

2.2两组患者治疗前后中医证候积分比较
两组患者治疗后各项中医证候积分均有所下降,且观察组下降更为明显,差异具有统计学意义(P<0.05),具体情况见表2。

2.3两组患者治疗前后炎症细胞因子比较
两组患者治疗后IL-1β、IL-6及IL-17均有所改善,且观察组改善更为明显,差异具有统计学意义(P<0.05),具体情况见表3。

2.4两组患者治疗效果比较
观察组治疗总有效率94.59%,对照组为75.68%,差异具有统计学意义(P<0.05),具体情况见表4。

由此可见,通过本申请实施例1-6所述的补肾活血通络方治疗,可以实现较好的临床效果,具体为:(1)两组患者治疗后M-JOA评分及Oswestry评分均有所下降,且观察组下降更为明显,差异具有统计学意义(P<0.05);(2)两组患者治疗后各项中医证候积分均有所下降,且观察组下降更为明显,差异具有统计学意义(P<0.05);(3)两组患者治疗后IL-1β、IL-6及IL-17均有所改善,且观察组改善更为明显,差异具有统计学意义(P<0.05);(4)观察组治疗总有效率94.59%,对照组为75.68%,差异具有统计学意义(P<0.05)。
因此,可以明确的看出本申请的补肾活血通络方定向透药法治疗肾虚血淤型慢性腰痛临床疗效可靠,能帮助患者改善症状,缓解腰痛,有效降低炎症细胞因子水平,临床上值得推广。

Claims (7)
1.一种补肾活血通络方,所述补肾活血通络方包括:补骨脂,骨碎补,红花,乳香,没药,制川乌,制草乌,白芍,威灵仙,透骨草,伸筋草,羌活,独活,肉桂。
2.如权利要求1所述的补肾活血通络方,各成分的具体份数为:补骨脂15份,骨碎补15份,红花15份,乳香15份,没药15份,制川乌18份,制草乌18份,白芍12份,威灵仙15份,透骨草30份,伸筋草30份,羌活10份,独活10份,肉桂9份。
3.如权利要求1所述的补肾活血通络方,所述补肾活血通络方还包括:当归、苏木,杜仲。
4.如权利要求3所述的补肾活血通络方,各成分的具体份数为:当归15份、苏木12份,杜仲18份。
5.一种制备权利要求1至4之一所述的肾活血通络方的方法,包括以下步骤:按照配比称取各中药,然后直接粉碎,混匀。
6.如权利要求5所述的方法,在获得上述混合物后,浓煎上清液,浸泡透药治疗棉块。
7.如权利要求1至4之一所述的补肾活血通络方在制备治疗或者减缓慢性腰痛的药物或制剂中的用途。
