Abstract
本发明公开了一种治疗颈椎病、感冒、抑郁症的组合物,其有效成分的原料药为葛根、柴胡、黄芩、熟党参、法半夏、白芍、桔梗、甘草、生姜和大枣。各药共同作用,达到宣展枢机、疏通经络、解肌祛风、调和气血之功效。该组合物不仅对颈椎病患者有效,同时,在抑郁症(肝郁气滞、脾胃不和型)及感冒(外感风寒型、虚体感冒型)的治疗上也能收获良效。

本发明涉及一种中药组合物,具体涉及一种治疗颈椎病、感冒、抑郁症的中药组合物。
背景技术
颈椎病又称颈椎综合征,是一种以退行性病理改变为基础的疾患。主要由于颈椎长期劳损、骨质增生,或椎间盘脱出、韧带增厚,致使颈椎脊髓、神经根或椎动脉受压,出现一系列功能障碍的临床综合征。现代医学把颈椎病分为6型:颈型颈椎病、神经根型颈椎病、脊髓型颈椎病、椎动脉型颈椎病、交感神经型颈椎病、食管压迫型颈椎病。
颈型颈椎病也称局部型颈椎病,具有头、肩、颈、臂的疼痛及相应的压痛点,X线片上没有椎间隙狭窄等明显的退行性改变,但可以有颈椎生理曲线的改变,椎体间不稳定及轻度骨质增生等变化。此型在临床上极为常见,是最早期的颈椎病。不少反复落枕的病人即属于此种改变。此型实际上是颈椎病的最初阶段,也是治疗最为有利的时机。而由于颈部交感神经受激惹致椎动脉受累可引起眩晕视力模糊等综合症状,称之为椎动脉型颈椎病、椎动脉压迫综合征、颈性眩晕、椎动脉缺血综合征、椎-基底动脉供血不足等。椎动脉型颈椎病较之脊髓型颈椎病略为多见,因其中大多系由于椎节不稳所致,易经非手术疗法治愈或好转,故住院及需手术者较少。本型主要引起头痛症状。脊髓型颈椎病是由于颈椎椎骨间连接结构退变,导致脊髓受压或脊髓缺血,继而出现脊髓的功能障碍,因此脊髓型颈椎病是脊髓压迫症之一,可严重致残,占全部颈椎病的10%~15%。严重影响日常工作与生活。
目前治疗颈椎病采用的西药,多为对症治疗,治标不治本,效果有限,针对疼痛剧烈者短期内可选择西药消炎镇痛,但停药后,症状有复发的可能。中药辩证治疗,重在治本,疗效持久,体现出治病求本的中医治疗思想。很多研究表明中药在治疗颈椎病(尤其颈型、椎动脉型、脊髓型颈椎病),比西药更有优势,治疗颈椎病的中药制剂市场巨大。
中医理论认为:颈椎病当属中医“痹证”范畴,乃素体虚弱,外感风寒湿邪入侵,阻滞经络,气血阻闭不通导致。病理特点为本虚标实,病机为“经络阻滞,气血阻闭”。治则以调气活血通经为法。目前很多人局限地认为:颈椎病由外感风寒湿邪,气血阻闭不通导致,属实症。治疗时,不敢稍用扶正之药,恐阻碍邪气外出或助邪。在这一理论的影响下,目前上市的治疗颈椎病的中药制剂,多为辛温之药,以活血通络、散寒止痛为主(如颈复康颗粒、颈痛片等);或以祛风除湿、活血通络为主(如小活络丹、疏风定痛丸等)。这样的中药颈椎病制剂,针对特定人群,体内正邪交争时间长,起效慢,治愈后,正气损失多,导致身体虚弱,容易复发。因此,目前市场上仍然缺乏有效且见效快的中药颈椎病制剂。
抑郁症是情感性精神障碍的一种临床类型。是以显著而持久的情绪低落为主要特征的精神障碍。其病因未明,一般认为与遗传因素、神经介质及躯体、心理和环境等因素有关。属于现代中医郁证的一个病类。由于情志不舒,气机瘀滞所致,以心情抑郁,情绪不宁,胸部满闷,胁肋胀痛,或易怒易哭,或咽中如有异物梗阻等为主要临床表现。现有治疗抑郁相关的中药组合物,主要以安神、活血通窍等为治则,未见有上市的治疗抑郁的中药组合物。
发明内容
本发明的目的在于克服现有技术的不足,提供一种治疗颈椎病、感冒、抑郁症的组合物。

本发明的有益效果是:
第一,小柴胡汤,源自《伤寒杂病论》,方中含:柴胡、黄芩、人参、半夏、甘草、生姜、大枣,其功效主要是和解少阳,和胃降逆,扶正祛邪。桂枝加葛根汤,出自《伤寒论》,即桂枝汤调整用量加葛根而成,方中含:葛根、麻黄、芍药、生姜、甘草、大枣、桂枝,其功效主要是解肌发表,生津和营。“太阳病项背强几几,反汗出恶风者,桂枝加葛根汤主之”。该方是仲景为风寒之邪客于太阳经所设。两者的功效接近,基于常规中医理论,根据证型择一化裁使用即可。因此目前临床上常单方如小柴胡汤,小柴胡加桂枝汤、葛根汤桂枝加葛根汤专治某类疾病,未见将小柴胡汤和桂枝加葛根汤结合,治疗病机相同的疾病。本发明创新性将经典方剂小柴胡汤和桂枝加葛根汤结合,共同作用,达到宣展枢机、疏通经络、解肌祛风、调和气血之功效。
第二,在此基础上,加上不可或缺的一味中草药——桔梗搭配,桔梗始载于《神农本草经》,性平、味甘苦辛,归肺、胃经。具有宣肺、散寒、祛痰、利咽、排脓等功效。现代药理学研究证实:桔梗中所含的多种化学成分,尤其是三萜皂苷类,具有抗炎、免疫调节、保肝、心血管保护、镇痛等作用。其中,桔梗皂苷D的C28甲酯衍生物(PrisMe)可通过阻止NF-kB抑制蛋白降解介导的抗炎过程,同时可通过激活去甲肾上腺素能下行神经通路和血清素信号通路来发挥镇痛作用。颈部骨质增生与周围软组织摩擦形成无菌性炎症,可用此方抗炎、镇痛。
第三,本药方适应症广泛,不仅对颈椎病患者有效,同时,在抑郁症(肝郁气滞、脾胃不和型)及感冒(外感风寒型、虚体感冒型)的治疗上也能收获良效。抑郁症是情感性精神障碍的一种临床类型。是以显著而持久的情绪低落为主要特征的精神障碍。其病因未明,一般认为与遗传因素、神经介质及躯体、心理和环境等因素有关。属于现代中医郁证的一个病类。由于情志不舒,气机瘀滞所致,以心情抑郁,情绪不宁,胸部满闷,胁肋胀痛,或易怒易哭,或咽中如有异物梗阻等为主要临床表现。本方具有疏肝理气、活血止痛之功效,尤适用于郁证中肝郁气滞、脾胃不和之证。除此之外,运用本方治疗体虚感冒,屡试不爽。临床治此证时不可发汗,恐耗散虚人之气血阴阳,虚其虚。而方中柴胡、黄芩、半夏能旋转少阳枢机以达太阳之气;熟党参、甘草、大枣、生姜补助中焦脾土以领邪外出,于稳妥平和之中,大具扶正达邪之力。为治虚人感冒佳方。
具体实施方式
发明人根据中医理论,“邪之所凑,其气必虚”,颈椎病发病有正虚的一面,张介宾曰“痹症大抵因虚者多、因寒者多,唯气不足,故风寒得以入之”。综上所述,正虚卫外不固是颈椎病发生的内在基础,感受风寒湿外邪是发生的外在条件,邪气痹阻经脉是为病机之本,病变累及肢体筋骨、肌肉、关节,甚则影响脏腑。
方中,葛根为君,柴胡、黄芩为臣,熟党参、法半夏、白芍、桂枝、桔梗、甘草、生姜及大枣为佐使。重用葛根,能升阳,解肌舒筋,缓解因经气不利、经脉失养所致的颈项强痛及头晕症状。本方能降低血流阻力,增加脑血流量,可显著改善脑内内耳-椎动脉-基底动脉供血,从而对头痛、项强、头晕等症状发挥疗效。方中柴胡散邪透表,使半表之邪得以外宣,黄芩除热清里,使半里之邪得以内彻,两药同用,和解少阳,宣展枢机;熟党参、甘草益气扶正,以助祛邪;半夏和胃降逆而止呕,并制约柴胡助呕之弊;白芍、甘草配合生津养液,缓急止痛;桂枝疏通经脉;桔梗理气行滞;生姜、大枣合用可调和营卫,宣畅气血,枢机转则闭自展,营卫调则血气安。诸药合用,共奏宣展枢机、疏通经络、解肌祛风、调和气血之功,如此,气顺血畅,眩晕得解,颈痛得除。临床常以本方治疗颈型、椎动脉型、脊髓型颈椎病因感受风寒而急性发作者,每获良效。
葛根:甘、辛、凉,解肌退热,生津,透疹,升阳止泻。用于外感发热头痛、项背强痛,口渴,消渴,麻疹不透。为豆科植物野葛Puerarialobata(Willd.)Ohwi或甘葛藤PuerariathomsoniiBenth.的干燥根。
柴胡:苦、微寒,和解表里,疏肝,升阳。用于感冒发热,寒热往来,胸胁胀痛,月经不调,子官脱垂,脱肛。为伞形科植物柴胡Bupleurumchinense DC.或狭叶柴胡BupleurumscorzonerifoliumWilld.的干燥根。现代药理学研究证实:葛根含有葛根黄酮苷,酮苷有缓解肌肉痉挛、扩张脑皮外周血管作用。
黄芩:苦、寒,清热燥湿,泻火解毒,止血,安胎。用于湿温、暑温胸闷呕恶,湿热痞满,泻痢,黄疸,肺热咳嗽,高热烦渴,血热吐衄,痈肿疮毒,胎动不安。本品为唇形科植物黄芩ScutellariabaicalensisGeorgi的干燥根。
熟党参:甘、平,补中益气,健脾益肺。用于脾肺虚弱,气短心悸,食少便溏,虚喘咳嗽,内热消渴。为桔梗科植物党参Codonopsispilosula(Franch.)Nannf.、素花党参(西党参)CodonopsispilosulaNannf.var.modesta(Nannf.)L.T.Shen或川党参CodonopsistangshenOliv.的干燥根。
法半夏:辛、温,燥湿化痰,降逆止呕,消痞散结。用于痰多咳喘,痰饮眩悸,风痰眩晕,痰厥头痛,呕吐反胃,胸脘痞闷。为天南星科植物半夏Pinelliaternata(Thunb.)Breit.的干燥块茎。
白芍:苦、酸、微寒,平肝止痛,养血调经,敛阴止汗。用于头痛眩晕,胁痛,腹痛,四肢挛痛,自汗,盗汗,为毛茛科植物芍药Paeonialactiflora Pall.的干燥根。
桂枝:辛、温、甘,发汗解肌,温经通脉。治风寒表证,肩背肢节酸疼,胸痹痰饮,经闭症瘕,为樟科植物肉桂Cinnamomum cassia Presl的干燥嫩枝。
桔梗:苦、辛、平,宣肺,利咽,祛痰,排脓。用于咳嗽痰多,胸闷不畅,咽痛音哑,肺痈吐脓,为桔梗科植物桔梗Platycodon grandiflorum(Jacq.)A.DC.的干燥根。
甘草:甘、平,补脾益气,清热解毒,祛痰止咳,缓急止痛,调和诸药,为豆科植物甘草GlycyrrhizuralensisFisch.、胀果甘草Glycyrrhizainflata Bat.或光果甘草Glycyrrhizagla-bra L.的干燥报及根茎。
生姜:辛,微温,解表散寒,温中止呕,化痰止咳。用于风寒感冒,胃寒呕吐,寒痰咳嗽,为姜科植物姜ZingiberofficinaleRosc.的新鲜根茎。
大枣:甘、温,补中益气,养血安神,为鼠李科枣属植物枣ZiziphusjujubaMill.var.inermis(Bunge)Rehd.的干燥成熟果实。
下面结合实例,进一步说明本发明。
实施例1:汤剂
有效成分的中药重量比为:葛根:2、柴胡:2、黄芩:1、熟党参:1.5、法半夏:1、白芍:1、桂枝:1、桔梗:1、甘草:0.5、生姜:0.8、大枣:1.5。
其中,一剂的汤剂组成为:葛根:20g、柴胡:20g、黄芩:10g、熟党参:15g、法半夏:10g、白芍:10g、桂枝:10g、桔梗:10g、甘草:5g、生姜:8g、大枣:15g。汤剂的具体制备方法如下:
将一剂中药放入1000ml水中,浸泡约30分钟,然后武火煎制20分钟,文火煎制20分钟,过滤去渣,煎出的药液即为所需中药。一剂共煎制2次,共煎出药液400~600ml。
实施例2:散剂
有效成分的中药重量比为:葛根:3、柴胡:1、黄芩:1.5、熟党参:2、法半夏:1、白芍:1.5、桂枝:1.5、桔梗:1.2、甘草:0.7、生姜:0.6、大枣:3。
将中药研成细粉,将上述粉末配研混匀,过筛,分装即得。
实施例3:颗粒剂
有效成分的中药重量比为:葛根:2、柴胡:1.4、黄芩:2、熟党参:2.5、法半夏:1.3、白芍:1.3、桂枝:1.8、桔梗:1.6、甘草:0.5、生姜:0.5、大枣:2。
取上述药材,采用渗漉法方法提取,以60%左右的乙醇或欲饮度数的白酒为溶剂,提取液回收乙醇后,蒸发浓缩至稠膏状,加入适宜的辅料,制软材,制颗粒,干燥,整粒,包装即得。
实施例4:蜜丸剂
有效成分的中药重量比为:葛根:1.5、柴胡:1.8、黄芩:1.4、熟党参:2、法半夏:1.5、白芍:1.5、桂枝:1.2、桔梗:2、甘草:1、生姜:0.7、大枣:2.5。
按上述药物暴晒干燥后,用打粉机打成细粉,以药粉的重量按1∶1.2-1.5的比例配好蜂蜜,把蜂蜜先文火后武火煎煮,趁热把蜂蜜倒入事先准备好的药粉中,搅拌至药蜜混合,软硬一致,放置温热后趁热分丸即得。
实施例5:胶囊剂
有效成分的中药重量比为:葛根:2.5、柴胡:1.5、黄芩:1.8、熟党参:2.4、法半夏:2、白芍:1、桂枝:2、桔梗:1.5、甘草:0.6、生姜:0.6、大枣:1.8。
将上述中药按比例制成药液,再采用压制法系明胶、甘油与水等混合溶解后制成薄厚均匀的胶带,再将药液置于两层胶带之间,用钢板模或旋转模压制,即可。
实施例6:片剂
有效成分的中药重量比为:葛根:2、柴胡:2、黄芩:1.5、熟党参:3、法半夏:1.8、白芍:2、桂枝:1.4、桔梗:1.5、甘草:0.8、生姜:0.8、大枣:2.4。
将上述中药粉碎、造粒,干燥,再用压片机制成片状,即可。
本发明适用于:1、颈椎病(颈型、椎动脉型、脊髓型);2、抑郁症(肝郁气滞、脾胃不和型);3、感冒(外感风寒型、虚体感冒型);孕妇忌服。
组合物对颈椎病治疗效果:
颈椎病患者临床症状疗效评定表参照《中医病证诊断疗效标准》而制定,根据临床症状疗效评定表统计患者临床症状和体征记分的总和,改善的百分率按下列公式评定6个月远期疗效,计算公式如下:
减分率=(治疗前总分-治疗后总分)/治疗前总分×100%
临床控制:减分率≥90%,显效:60%≤减分率<90%,有效:30%≤减分率<60%,无效:减分率<30%。
颈椎病患者根据临床分型(颈型、椎动脉型、脊髓型)进行临床随机对照研究并进行指标评价。
146例颈型颈椎病患者,观察组73例,男42例,女31例,年龄27~52(38.31±1.27)岁,病程1~13(6.73±0.18)年,经络瘀滞型30例,肝肾亏损型22例,风寒湿痹型12例,外伤型9例;对照组男38例,女34例,年龄25~51(37.62±1.35)岁,病程2~14(7.02±0.25)年,经络瘀滞型32例,肝肾亏损型22例,风寒湿痹型14例,外伤型4例。对照组给予常规西医治疗,观察组在其基础上联合使用实施例1的中药(中药煎煮,口服,日一剂,分早晚2次服用,每次服用约200-300毫升)进行治疗,两组疗程为14d,疗程结束后,对照组减分率平均值为83.33%,观察组减分率平均值为97.30%,颈部疼痛情况有效缓解。
250例椎动脉型颈椎病患者随机分为观察组和治疗组各125例,其中观察组男73例,女52例,年龄30~64岁(47.23±3.56)岁,病程5~65d(22.68±3.12);对照组男性46例,女性79例,年龄31~65岁(48.45±3.78),病程4~66d(23.37±3.28)。观察组予实施例1的中药进行治疗(中药煎煮,口服,日一剂,分早晚2次服用,每次服用约200-300毫升),对照组予颈复康颗粒治疗,治疗2周,疗程结束后,观察组的减分率为90.24%,对照组减分率为78.05%,表明实施例1的药物能有效缓解椎动脉型颈椎病症状。
120例脊髓型颈椎病随机分为观察组和对照组各60例,观察组男25例,女35例,年龄35~75岁,平均(59.32±10.71)岁,对照组男26例,女34例,年龄36~73岁,平均(59.26±10.69)岁;观察组采用实施例1的中药进行治疗(中药煎煮,口服,日一剂,分早晚2次服用,每次服用约200-300毫升),8周为1个疗程;对照组采用地塞米松,口服,1次/d,持续三周后将地塞米松用量减至1.5mg,8周为1个疗程;观察患者的ET水平,结果表明,观察组有效率为93.33%,高于对照组71.61%,临床症状得到有效缓解。
组合物对抑郁症的治疗效果
抑郁症患者临床症状疗效评定参照精神科量表制定。根据HAMD量表评分减分率进行疗效评定,计算公式如下:
减分率=[(治疗前HAMD总分-治疗后HAMD总分)÷治疗前HAMD总分]×100%。
痊愈:HAMD评分减少≥75%;显效:HAMD评分减少≥50%,且<75%;有效:HAMD评分减少≥25%,且<50%;无效:HAMD评分减少<25%。
抑郁症患者根据中医辨证(肝郁气滞、脾胃不和型)进行临床分析及指标评价。
68例肝郁气滞型抑郁症患者随机分为观察组和对照组各34例,其中观察组男20例,女14例,平均年龄(38.8±1.8)岁,平均病程(1.4±0.4)年;对照组男18例,女16例,平均年龄(38.3±1.4)岁,平均病程(1.2±0.2);观察组予实施例1的主药进行治疗(中药煎煮,口服,日一剂,分早晚2次服用,每次服用约200-300毫升),疗程2个月;对照组予常规口服氟西汀治疗,20mg/d,疗程2个月;疗程结束后,观察组有效减分率为91.20%,明显高于对照组73.50%,同时观察两组治疗前后生活质量综合评定问卷评分。
106例脾胃不和型抑郁症患者随机分为观察组60例和对照组46例,其中男49例,女57例,年龄50~82岁,平均(62.5±7.3)岁;对照组给予西酞普兰治疗,20~40mg,每天1次口服;观察组在对照组基础上给予以实施例1的主药进行治疗(中药煎煮,口服,日一剂,分早晚2次服用,每次服用约200-300毫升),共治疗2周;疗程结束后,治疗组治愈9例,显效34例,有效14例,无效3例,总有效率95.00%;对照组治愈5例,显效16例,有效18例,无效7例,总有效率84.78%。
组合物对感冒的治疗效果
感冒患者临床症状疗效评定表参照《中药新药临床研究指导原则》,根据临床症状疗效评定表统计患者临床症状和体征记分的总和,改善的百分率按下列公式评定疗效,计算公式如下:
减分率=(治疗前总分-治疗后总分)/治疗前总分×100%
痊愈:患者体温基本恢复正常,减分率≥90%;显效:患者治疗3d后体温恢复正常,60%≤减分率<90%;有效:患者治疗3d后体温降低,30%≤减分率<60%;无效:患者治疗3d后体温无明显下降,减分率<30%。
采用实施例1中冲剂(每包5g,含生药16.5g)进行临床实验,用法如下:1~3周岁,半包;3~6周岁,1包;6~9周岁,1包半;9~14周岁,2包,14周岁以上,3包,冲服,每天3次,3天为1疗程。156个病例(风寒证73例,风热证45例,暑湿证38例)中按1:1随机分为治疗组和对照组,男93例,女63例;年龄4~75岁,平均(34.29±8.34),平均病程1.97天。
观察组使用本实施例1主药进行治疗(冲剂,口服,14周岁以上3包,14周岁以下2包,日3次),根据患者治疗效果调整治疗时间(8~12d);对照组予常规治疗,抗菌治疗可服用青霉素V钾片每次1~2片,每6小时1次,疗程至退热后至少2d,共治疗5d,针对发热超过38℃患者予以尼美舒利降温治疗,每次0.05~0.1g口服,每天2次,治疗10d;观察2组患者临床疗效,治疗1、2、3d后体温情况及疾病复发率。
疗程结束后观察组临床治愈者5例(风寒证4例,暑湿证1例),显效者16例(风寒证8例,风热证3例,暑湿证5例),有效者48例(风寒证27例、风热证8例、暑湿证13例),无效者9例(风寒证2例、风热证4例、暑湿证3例),总有效率为88.46%;对照组临床治愈者3例(风寒证1例、风热证2例),显效者12例(风寒证4例、风热证4例、暑湿证4例),有效者45例(风寒证15例、风热证19例、暑湿证11例),无效者18例(风寒证9例、风热证3例、暑湿证6例),总有效率为76.92%。具体如下表所示:

1.一种中药组合物,其有效成分的原料药包括以下重量份的:葛根:1.5~3份、柴胡:1~2份、黄芩:1~2份、熟党参:1.5~3份、法半夏:1~2份、白芍:1~2份、桔梗:1~2份、甘草:0.5~1份、生姜:0.5~0.8份、大枣:1.5~3份。
2.根据权利要求1所述的中药组合物,其特征在于:其原料药重量份组成为:葛根:1.5~3份、柴胡:1~2份、黄芩:1~2份、熟党参:1.5~3份、法半夏:1~2份、白芍:1~2份、桂枝1~2份、桔梗:1~2份、甘草:0.5~1份、生姜:0.5~0.8份、大枣:1.5~3份。
3.根据权利要求2所述的中药组合物,其特征在于:其原料药重量份组成为:
葛根:2份、柴胡:1份、黄芩:1份、熟党参:1.5份、法半夏:1份、白芍:1份、桂枝:1份、桔梗:1份、甘草:0.5份、生姜:0.5份、大枣:1.5份。
4.根据权利要求1所述的中药组合物,其特征在于:所述中药组合物的剂型为口服剂或透皮吸收剂。
5.根据权利要求4所述的中药组合物,其特征在于:所述口服剂选自汤剂、冲剂、散剂、片剂、丸剂、口服液或胶囊。
6.根据权利要求1~3任一项所述的中药组合物,其特征在于:所述原料药的有效成分为其水提物或醇提物。
7.中药组合物在制备治疗疾病药物中的应用,所述中药组合物如权利要求1~6任一项所述,所述疾病由经络阻滞或气血不畅引起。
8.根据权利要求7所述的应用,其特征在于:所述疾病选自颈椎病、抑郁症或感冒。
9.根据权利要求8所述的应用,其特征在于:
所述颈椎病选自颈型、椎动脉型及脊髓型颈椎病;
所述抑郁症选自肝郁气滞、脾胃失和型抑郁症;
所述感冒选自外感风寒型、虚体型感冒。
