背景技术
人类的中枢神经包括脑和脊髓，是支配人体感觉、运动及意识的高级中枢。中枢神经功能损害往往有很高的致残性，如脑中风后的偏瘫，即是中枢神经功能损害后非常常见的后遗症。对于中枢神经功能损害的治疗一直是医学上的难点。现代医学对中枢神经损害后的病理生理变化此进行了大量的研究，一般认为中枢神经功能损害除了损伤当时的机械性损伤外，继发的自由基及脂质过氧化产物是导致中枢神经功能继发性损害的重要因素，亦是药物治疗的重要干预目标。但现代医学在中枢神经功能损害的治疗上并无重大进展，目前中枢神经功能损害性疾病的致残率仍非常高。中枢神经功能损害以肢体瘫痪、废用为特点，属于中医“痿症”范畴。《内经》指出“肺热叶焦”是其重要病机，并提出“治痿独取阳明”，其中“肺热”则耗伤阳气，“阳明”更为“气血之海”，故均提示气虚、阳虚在是痿证的根本病机。而督脉是人体的“阳脉之海”是人体阳气的“调度”中心，且解剖上督脉贯脊络脑，生理上与脑的关系密切。我们结合临床实践，认为痿证的病机在于督脉阳气亏虚，痰浊瘀阻经脉，治疗当以温养督脉、化浊通络为法。督脉阳气充沛，则痰浊自散，从而使瘀阻的经络复归于通畅，损伤的神经功能逐渐恢复。前期临床试验证实该方能把握中枢神经功能损害的根本环节，能有效提高膀胱肌及肢体肌肌力，明显改善患者排便及肢体活动功能，且无毒副作用，是一个较理想的治疗中枢神经功能损害的中药制剂，具有广阔的应用前景和巨大的社会经济效益。

本发明的目的是提供一种治疗中枢神经功能损害的温养督脉化浊通络中药组合物，本中药治疗中枢神经功能损害效果明显，无毒副作用。
本发明为了实现上述目的，采用的技术解决方案是：

鹿角胶3-20g 黄芪15-60g 党参10-30g 蜈蚣1-4条 土鳖虫5-20g淡附片2-10g 桂枝5-20g 白芥子5-20g 麻黄2-10g 川芎5-20g 熟地黄10-40g 淫羊藿5-20g 当归3-12g郁金5-20g 红花2-10g地龙5-20g肉桂1-6g防风 10-20 猪苓10-40g炒车前子10-40g细辛1-6g黄精10-40g莪术5-20g甘草5-10g枳壳5-20g。

治疗中枢神经功能损害的温养督脉化浊通络中药的制备方法如下：
（1）将除鹿角胶以外的诸药按照上述重量份配比将混合物与水按照重量比1:5的比例混合，在水中浸泡30分钟；
（2）将所述步骤（1）中的混合物及水煎煮90分钟后，过滤得到第一滤液及滤渣；
（3）将所述步骤（2）中所得滤渣与水按照重量比 1：4的比例混合，再煎煮60分钟，然后过滤得到第二滤液及滤渣，将第一滤液与第二滤液混合，将鹿角胶烊化入混合液，获得药物的汤剂。

将上述多味中药原料经过煎煮制备成汤剂，诸药合用，共奏温养督脉化浊通络之功。对于治疗中枢神经功能损害效果显著。
临床资料：
为表明本发明药物的治疗效果，本发明经过120例脑神经功能损害恢复期患者进行系统的临床研究，120 例病例均来源于广州中医药大学附属骨伤科医院。
治疗组：按就诊顺序根据随机数字表法选取 60 例，在原治疗基础上，停用所有治疗药物，单纯服用本发明药物治疗6个月。
对照组：按就诊顺序根据随机数字表法选取60例，口服胞二磷胆碱钠胶囊（齐鲁制药有限公司），1粒/次，3 次/日，治疗6个月。
两组在病情、性别、年龄、病程等方面经统计学处理差异无统计学意义。
服用方法 ：治疗组 ：每日早晚各一次服用本发明药物的汤剂 300 ml，疗程为1个月。对照组 ：口服胞二磷胆碱钠胶囊（齐鲁制药有限公司），1粒/次，3 次/日，早、中、晚进食后半小时200ml温开水送服，疗程为3个月。
观察指标和方法：
参照《中药新药临床研究指导原则》、《中医病证诊断疗效标准》的标准。
治疗组60例，痊愈12例，显效35例，有效10例，无效3例，总有效率为 95.00％；对照组60例，痊愈0例，显效15例，有效18例，无效13例，总有效率为55％。 两组的有效率差异有明显统计学意义，治疗组明显优于对照组。
本发明经过 120 例脊髓神经功能损害恢复期患者进行系统的临床研究，120 例病例均来源于广州中医药大学附属骨伤科医院。
治疗组：按就诊顺序根据随机数字表法选取 60 例，在原治疗基础上，停用所有治疗药物，单纯服用本发明药物治疗6个月。
对照组：按就诊顺序根据随机数字表法选取60例，口服甲钴胺片，治疗6个月。
两组在病情、性别、年龄、病程等方面经统计学处理差异无统计学意义。
服用方法 ：治疗组 ：每日早晚各一次服用本发明药物的汤剂 300 ml，疗程为1个月。对照组 ：口服甲钴胺片，每次100mg，每天2次，早、晚进食后半小时200ml温开水送服，疗程为3个月。
观察指标和方法：
参照《中药新药临床研究指导原则》、《中医病证诊断疗效标准》的标准。
治疗组60例，痊愈8例，显效36例，有效10例，无效4例，总有效率为 90.00％；对照组60例，痊愈0例，显效15例，有效17例，无效14例，总有效率为53％。 两组的有效率差异有明显统计学意义，治疗组明显优于对照组。
具体实施方式
实施例 1
一种治疗中枢神经功能损害的温养督脉化浊通络中药，由下述重量份配比的原料制成：
鹿角胶烊化3g 黄芪15g 党参10g 蜈蚣1条 土鳖虫5g淡附片先煎3g 桂枝5g 白芥子5g 麻黄2g 川芎5g 熟地黄10g 淫羊藿5g 当归3g郁金5g 红花3g地龙5g肉桂焗服3g防风 10g猪苓10g炒车前子10g细辛2g黄精10g莪术5g甘草3g枳壳5g。
治疗中枢神经功能损害的温阳化浊通脉中药的制备方法如下：
（1）将除鹿角胶以外的诸药按照上述重量份配比将混合物与水按照重量比1:5的比例混合，在水中浸泡30分钟；
（2）将所述步骤（1）中的混合物及水煎煮90分钟后，过滤得到第一滤液及滤渣；
（3）将所述步骤（2）中所得滤渣与水按照重量比 1：4的比例混合，再煎煮60分钟，然后过滤得到第二滤液及滤渣，将第一滤液与第二滤液混合，将鹿角胶烊化入混合液，获得药物的汤剂。
实施例2
一种治疗中枢神经功能损害的温养督脉化浊通络中药，由下述重量份配比的原料制成：
鹿角胶烊化6g 黄芪30g 党参15g 蜈蚣2条 土鳖虫10g淡附片先煎5g 桂枝10g 白芥子10g 麻黄5g 川芎10g 熟地黄20g 淫羊藿10g 当归6g郁金10g 红花5g地龙10g肉桂焗服3g防风 15g 猪苓20g炒车前子20g细辛3g黄精20g莪术10g甘草5g枳壳10g。
治疗中枢神经功能损害的温养督脉化浊通络中药的制备方法如下：
（1）将除鹿角胶以外的诸药按照上述重量份配比将混合物与水按照重量比1:5的比例混合，在水中浸泡30分钟；
（2）将所述步骤（1）中的混合物及水煎煮90分钟后，过滤得到第一滤液及滤渣；

实施例3
一种治疗中枢神经功能损害的温养督脉化浊通络中药，由下述重量份配比的原料制成：
鹿角胶烊化12g 黄芪60g 党参30g 蜈蚣4条 土鳖虫20g淡附片先煎10g 桂枝20g 白芥子 20g 麻黄10g 川芎20g 熟地黄40g 淫羊藿20g 当归12g郁金20g 红花10g地龙20g肉桂焗服6g防风30g 猪苓40g炒车前子40g细辛6g黄精40g莪术20g甘草10g枳壳20g。
治疗中枢神经功能损害的温养督脉化浊通络中药的制备方法如下：

典型病例
病例一：梁某，男，46岁，2015年11月07日初诊。主诉:右侧肢体活动不利，麻木不仁2个月 。病人自述2月无明显诱因出现脑血栓,致右半身瘫痪。曾在当地医院神经内科诊治，CT 检查提示:左侧基底节区脑梗死。诊断:脑血栓形成。给予胞二磷胆碱等药物对照治疗，1周后好转。现自觉现右侧半身活动不利，右侧肢体麻木乏力，言语欠利，口眼歪斜，无头晕头痛，纳眠可，二便正常，舌暗红、少苔，脉弦细。检查：右侧肢体皮肤感觉减退，肌力3级，生理反射减退，右霍夫曼征（+）。申请人认为，本病属督脉阳气亏虚，痰浊瘀阻经脉之中枢神经功能损害，治疗当以温养督脉化浊通络为法。处方：鹿角胶6g烊化 黄芪30g 党参15g 蜈蚣2条土鳖虫10g淡附片5g先煎 桂枝10g 白芥子 10g 麻黄5g 川芎10g 熟地黄20g 淫羊藿10g 当归6g郁金10g红花5g地龙10g肉桂3g焗服防风 15g 猪苓20g炒车前子20g细辛3g黄精20g莪术10g甘草5g枳壳10g，6剂，每天1剂，水煎服。11月14日复诊：右侧肢体麻木、乏力较前明显好转，仍守上方，续服18剂。12月05日复诊：右侧肢体麻木、乏力及口眼歪斜较前改善，仍守上方，续服36剂。2016年1月9日复诊，患者肢体麻木乏力继续好转。续服上方至半年，表情开朗，口眼歪斜基本恢复，能参与家事，右侧肢体麻木、乏力较前明显减轻，行走基本自如，右侧肢体近端肌力基本正常，远端肌力4+级，生理反射减退，病理征（-）。
病例二：杨某，女，47岁，2016 年 3 月6 日初诊。主诉：颈部酸楚伴走路不稳1月。1月前患者无明显诱因出现颈项疼痛酸楚，两肩重着，伴上肢乏力、手麻，胸胁裹束感强烈，下肢拘禁，步履有时欠稳，纳眠可，二便正常。外院 MRI 示：C5/6、C6/7 椎间盘突出，椎管狭窄，颈髓变性。查体：神清，双上肢肌力 4级，双下肢肌力5级，四肢肌张力正常，感觉正常，双侧霍夫曼（+），双侧膝反射（+），双侧踝反射（+），无阵挛，下肢锥体束征（-），舌暗红，苔薄白，脉弦细。诊断：颈椎病（脊髓型）。申请人认为，本病属督脉阳气亏虚，痰浊瘀阻经脉之颈部脊髓神经功能损害，治疗当以温养督脉、化浊通络为法。
处方：鹿角胶6g烊化 黄芪30g 党参15g 蜈蚣2条 土鳖虫10g淡附片5g先煎 桂枝10g白芥子 10g 麻黄5g 川芎10g 熟地黄20g 淫羊藿10g 当归6g郁金10g 红花5g地龙10g肉桂3g焗服防风 15g 猪苓20g炒车前子20g细辛3g黄精20g莪术10g甘草5g枳壳10g，6剂，每天1剂，水煎服。3月13日复诊：颈部疼痛及上肢体麻木、乏力较前明显好转，仍守上方，改鹿角胶为12g，续服18剂。4月06日复诊：颈部疼痛及上肢体麻木基本消失，双上肢乏力较前显著改善，双上肢肌力5级，双侧霍夫曼（-），双侧膝反射（±），双侧踝反射（±），下肢拘禁显著改善，行走基本自如。续服此方1个月，以巩固疗效。5月06日随访：患者自觉药后诸恙基本痊愈，上肢已觉有力，无麻木感，行走基本自如。
病例三： 郭某 , 男 , 58岁，2016 年 5 月6 日初诊。主诉：胸椎管狭窄症术后，伴双下肢瘫痪2年。 患者两年前无明显诱因出现背部疼痛伴双下肢瘫痪、麻木，二便排出困难，遂到广东省人民医院就诊，CT、MRI检查提示：胸 10 胸 11 胸 12椎管狭窄 ,以胸 11为主，诊断为胸椎管狭窄症，立刻予手术治疗。术后背部疼痛伴双下肢麻木明显好转，仍有双下肢瘫痪及二便排出困难。现自觉背部疼痛，翻身及二便排出困难，必需予保留尿道插管，伴双下肢瘫痪，无头晕头痛，纳眠可，二便正常，舌暗红、苔少，脉弦细。查体：平脐下两指痛温觉减退，深感觉消失，下腹壁反射、肛门反射基本消失，上中腹壁反射存在，双侧膝腱反射、跟腱反射亢进，双侧巴氏征 (+ )，双侧踝阵挛 (+ ) ，双下肢肌张力稍增高，双下肢肌力 1-2级。申请人认为，本病属督脉阳气亏虚，痰浊瘀阻经脉之胸部脊髓神经功能损害，治疗当以温养督脉、化浊通络为法。处方：鹿角胶6g烊化 黄芪30g 党参15g 蜈蚣2条 土鳖虫10g淡附片5g先煎 桂枝10g 白芥子 10g 麻黄5g 川芎10g 熟地黄20g 淫羊藿10g 当归6g郁金10g 红花5g地龙10g肉桂3g焗服防风 15g 猪苓20g炒车前子20g细辛3g黄精20g莪术10g甘草5g枳壳10g，6剂，每天1剂，水煎服。5月13日复诊：自觉背部疼痛明显缓解，翻身及二便排出困难较前明显好转，双下肢较前有力，仍守上方，改鹿角胶为12g，续服6剂。5月20日复诊：自觉背部疼痛消失，翻身及二便排出困难较前显著好转，可以拔除尿管，双下肢较前有力，双下肢肌力 2-3级。仍守上方续服18剂。6月6日复诊：已可拔除尿管，小便可自主排尿，大便排出困难继续较前显著好转，双下肢较前有力，双下肢肌力 3-4级，可以拄拐下地活动。续服此方1个月，以巩固疗效。

Claims (2)

1.一种治疗中枢神经功能损害的温养督脉化浊通络中药组合物，其特征在于，由下述重量份配比的原料制成：
鹿角胶3-20g、黄芪15-60g、党参10-30g、蜈蚣1-4条、土鳖虫5-20g、淡附片2-10g、桂枝5-20、白芥子5-20g、麻黄2-10g、川芎5-20g、熟地黄10-40g、淫羊藿5-20g、当归3-12g、郁金5-20g、红花2-10g、地龙5-20g、肉桂1-6g、防风10-20g、猪苓10-40g、炒车前子10-40g、细辛1-6g、黄精10-40g、莪术5-20g、甘草5-10g、枳壳5-20g。
2.根据权利要求1所述的治疗中枢神经功能损害的温养督脉化浊通络中药组合物，其特征在于：
